Benign, persistent, and invasive: mechanistic and translational approaches to middle‑ear cholesteatoma.

Acquired middle-ear cholesteatoma is a histologically benign keratinizing squamous epithelial lesion that paradoxically exhibits locally destructive, recurrent, and invasive behavior, often resulting in ossicular erosion, hearing loss, labyrinthine fistula, and, rarely, intracranial complications. Surgical excision remains the primary management strategy; however, recurrence is common due to persistent microenvironmental drivers. Recent mechanistic studies-including single-cell transcriptomics, spatial proteomics, and epigenetic profiling-reveal a multifactorial pathogenesis orchestrated by ch

Hearing Preservation After Segmental Semicircular Canal Destruction: A Report of Two Rare Cases.

Semicircular canal fistulas are a complication of cholesteatomatous otitis media. In extremely rare cases, extensive destruction of the bony labyrinth may result in loss of approximately one-third of the semicircular canal arc, often with wide exposure and partial injury to the membranous labyrinth, thereby increasing the risk of profound sensorineural hearing loss. Complete eradication of the cholesteatoma while preserving inner ear function remains challenging. Here, we report two rare cases of segmental loss of the semicircular canal arc in which useful hearing was retained. Case 1 involved

An unusual case of neglected CSOM presenting with perineural spread along seventh/eighth cranial nerves to form cerebellar peduncle abscess: A case report.

Chronic suppurative otitis media (CSOM) is a common otologic disorder that may occasionally result in intracranial complications. However, extension of infection into the posterior cranial fossa through the facial (VII) and vestibulocochlear (VIII) nerve complex is exceedingly rare. We report the case of an 18-year-old male with a 7-year history of neglected bilateral ear discharge, hearing impairment, and left-sided lower motor neuron facial nerve palsy who presented with severe headache, vomiting, and seizures. Contrast-enhanced magnetic resonance imaging (MRI) demonstrated findings consiste

Characteristics and Mechanisms of Aperiodic Alternating Nystagmus.

BACKGROUND AND PURPOSE: Aperiodic alternating nystagmus (AAN) is defined as spontaneous alternating nystagmus in which the direction of nystagmus reverses repeatedly but the reversals do not occur at a consistent cycle interval and the cycle durations and/or peak slow-phase velocities are irregular. The etiologic distribution of vestibular disorders associated with AAN and the underlying mechanisms remain unclear. This study aimed to identify the clinical characteristics and potential underlying mechanisms of AAN. METHODS: We analyzed quantitative eye-movement recordings, audiovestibular test

Using Behavioral Skill Training to Extend Discrete-Trial Teaching Skills Across Instructional Programs.

INTRODUCTION BACKGROUND TO METABOLOMICS: Metabolomics is the comprehensive study of the metabolome, the repertoire of biochemicals (or small molecules) present in cells, tissues, and body fluids. The study of metabolism at the global or "-omics" level is a rapidly growing field that has the potential to have a profound impact upon medical practice. At the center of metabolomics, is the concept that a person's metabolic state provides a close representation of that individual's overall health status. This metabolic state reflects what has been encoded by the genome, and modified by diet, enviro
