Closed versus open technique in the management of labyrinthine fistulae.

A labyrinthine fistula is the most common complication of cholesteatomatous chronic ear disease. Its treatment remains a controversial subject. The present paper reports our approach to the management of this complication. Operations were performed on 1,226 cases of chronic otitis media with cholesteatoma between January 1971 and December 1985. A labyrinthine fistula was detected in 158 cases. We favor intact canal wall tympanoplasty even in the presence of medium or large fistulas: in the latter case, the matrix is not removed but is trimmed to cover only the bony defect and it is left in pla

Surgical management of labyrinthine fistula with cholesteatoma.

From 684 cases of ear surgery for cholesteatoma performed by one surgeon, 35 had labyrinthine fistulae (incidence 5.1 per cent). Of these fistulae, 79 per cent involved the lateral semicircular canal only; the other sites involved were the other semicircular canals and the cochlea. The fistula test was positive in 54 per cent of cases overall, but in 80 per cent with an extended site fistula (ESF). Three surgical approaches were employed sequentially--staged combined approach tympanoplasty (CAT), open cavity tympanoplasty and attico-antrotomy. Surgically-induced deafness occurred in 3.3 per ce

Cholesteatoma surgery in children.

We recently reviewed the records of 1024 operations for aural cholesteatoma, of which 181 were in children. Our surgical management was the same as for adults. Complications of the disease (labyrinthine fistula, facial paralysis, total sensorineural impairment, and meningeal complications) were less common in patients under 16 years of age. This appears to be related to duration of disease rather than to age. The disease in children tends to be more difficult to eradicate. Staging the operation is required more frequently and there is a higher incidence of residual disease at planned second-st

Pneumocephalus following treatment of a stapes gusher.

A case history is presented of a patient who underwent stapedectomy complicated by a cerebrospinal fluid (CSF) gusher. A persistent postoperative CSF leak was controlled by elevation of the head and spinal drainage. Retrograde passage of air through a labyrinthine fistula, however, resulted in pneumocephalus and transient neurologic deficit. This is a rare but serious complication of treatment of a stapes gusher which demonstrates the potential for entry of air intracranially in the presence of a perilymph fistula.

Management of the labyrinthine fistula in cholesteatoma surgery.

The presence of a labyrinthine fistula has remained one of the major problems in cholesteatoma surgery. Confronted with this problem, the surgeon may ultimately base his choice of procedure on four basic conditions: the size of the fistula, its location in the ear, the condition of the other ear, and the cochlear function. Our attitude has been changing, and currently we prefer to perform a staged closed tympanoplasty. When a closed technique is performed, we either remove the cholesteatoma matrix and then cover the fistula immediately or we leave the matrix in situ and re-explore the mastoid

Residual and recurrent cholesteatoma in closed tympanoplasty.

There are actually two principal approaches to the surgical treatment of middle ear cholesteatoma, a subject very much debated. First, the cholesteatoma can be exteriorized and treated with an open technique (classic or modified radical mastoidectomy, open tympanoplasty); and second, the radical removal of the cholesteatoma is also possible in the majority of cases by closed tympanoplasty. In this article we examine 283 patients (forty-four children and 239 adults) operated on by the closed technique to review and discuss the postsurgical cholesteatoma problem (residual and recurrent). Total i

Labyrinthine fistula in chronic mastoiditis.

A series of 79 otic capsule fistulae occurring in 792 consecutive cases of chronic mastoiditis undergoing mastoidectomy was analyzed. The incidence (10%) is the same as in other large series, the primary difference being the location of the fistula. In all series cited, 90% of fistulae were in the lateral semicircular canal whereas in this series the comparable incidence was 75%. The 20 (25%) fistulae located in areas other than the lateral semicircular canal enabled us to document a method of "reading" the fistula test preoperatively to establish its precise location. Eye responses to the tes

Experiments on cochlear cryosurgery in the guinea pig.

A method to induce experimental cochlear lesions with cryosurgery is described. Using temperatures on the cochlear surface ranging from 0 degrees C-50 degrees C, lesions were made from the apical turn and down to 8 mm from the round window area. There were no signs of labyrinthine fistula and the remaining parts of the organ of Corti were normal in appearance. The correlation of intracochlear temperature with the spread of anatomical damage is described. It is suggested that this method can be used to achieve non-invasive selective cochlear lesions. With this method it is probably possible to

Complications of otitis media.

Although intracranial otogenic complications occur less frequently than in the preantibiotic period, their mortality rate is still relatively high. On the basis of some personal clinical observations made during the last decade attention has been drawn to the most common intra- and extracranial otogenic complications (posterior fossa syndrome, labyrinthine fistula, labyrinthitis, facial palsy) mostly due to cholesteatoma, and a less frequent intracranial complication occurring mainly in children (otitic hydrocephalus) caused by chronic otitis media. It may therefore be concluded that otitis me

Experiments on cochlear cryosurgery in the guinea pig. Light and surface microscopy.

The present investigation indicates that with cryosurgery it is possible to produce well defined cochlear lesions. The point of complete ablation can be chosen at will. A great advantage also is the fact that the bony capsule of the labyrinth is left intact. The remarkably slight scarring caused as well as the morphologically intact walls of the cochlear duct indicate an effective healing process, already completed after 6 days. These findings explain the unsuccessful clinical experiences when trying to ablate the labyrinth completely or to induce labyrinthine fistula with the help of cryosurg

Cholesteatoma surgery: management of the labyrinthine fistula--a report of 97 cases.

Preoperative, operative and postoperative findings in 97 cases of labyrinthine fistula are presented. Most of these patients had had symptoms of chronic otitis media for 20 years or more and manifested some degree of sensorineural hearing impairment. Two-thirds had experienced dizziness. The fistula was limited to the lateral semicircular canal in 83 cases and involved the labyrinth more extensively in 14 instances. The intact canal wall technique was used in less than 60% and an open cavity technique in 25% of the cases. Severe or total sensorineural hearing impairment developed postoperative

Traumatic labyrinthine fistulas.

During the last 10 years, the rupture of the inner ear windows, as an entity, has been introduced in the differential diagnosis of the acute vestibulocochlear syndrome. Head trauma, Valsalva-like maneuvers and barotrauma due to diving accidents are now described as etiologic factors in the pathogenesis of labyrinthine fistula. Four cases were explored and repaired in the last four years in our department. Ataxia was the prominent symptom in three, while positional nystagmus and vertigo were consistent findings in all four cases. The definitive evidence of the fistula can be proved only by expl

Voluntary nystagmus masquerading as Tullio's Phenomenon.

Nystagmoid eye movements elicited by humming loudly was initially misdiagnosed as Tullio's phenomenon (abnormal acoustical stimulation of the vestibular apparatus owing to labyrinthine fistula). Further workup lead to a final diagnosis of a form of voluntary nystagmus characterized in some reports as "hysterical mystagmus". Some of the characteristics of this example of voluntary nystagmus differed from generally accepted descriptions.

[Rupture of the round window membrane (author's transl)].

Labyrinthine fistula of the round window should be considered in the differential diagnosis of vertigo and sudden hearing loss occurring in patients, who are not only divers, during physical stress or exertion. Surgical exploration and closure of the fistulae are the only means of diagnosis and treatment of this condition. Since these fistulae may heal spontaneously in most instances early surgery is recommended after a short interval of conservative therapy. Five patients, of whom 4 underwent surgery and one was seen for an expert opinion are presented.

Surgical treatment of long-term sensorineural hearing loss due to labyrinthine fistula.

Perilymphatic fistula, usually in the round or oval window, causes a variety of symptoms, including sensorineural hearing loss. Surgical repair of these fistulas has resulted in restoration of hearing in some cases. It has been suggested that surgery must be performed within 2 months of the trauma if improvement in hearing is to occur. This paper presents the case of a patient whose fistula and resulting hearing loss had persisted ten years before surgical repair. Restoration of normal hearing and discrimination of speech occurred in the ear which had previously presented a severe sensorineura

Ataxia and hearing loss secondary to perilymphatic fistula.

Ataxia is rarely attributed to lesions of the peripheral vestibular system. In 1973, the first case of ataxia and hearing loss secondary to a labyrinthine fistula was reported. Until now, this syndrome has not been reported in patients under the age of 10 years. A case is presented of a 5-year-old boy with symptoms of ataxia and hearing loss as well as vertigo and tinnitus after head trauma. Three physical findings appear to be most characteristic of patients with perilymphatic fistulas: a positive fistula response, positive positional testing with the involved ear down, and evidence of vestib

The incidence, site, treatment and fate of labyrinthine fistula.

Although the incidence of labyrinthine fistula appears to be similar in most reported series, the number of patients reported to have a positive response to the fistula test do not. This may be due to a lack of uniformity in the performance of the test. The method of the author, the direction of eye movement and its diagnostic significance are described. The occurrence and mechanism of severe sensorineural hearing loss in 5 out of 20 patients in whom the matrix was removed accidently from the fistula during mastoidectomy is discussed.
