Management of labyrinthine fistulae in Kyoto University Hospital.

CONCLUSION: In cases of labyrinthine fistulae, we performed complete removal of the cholesteatoma matrix in a one-stage procedure, resulting in a satisfactory bone conduction (BC) hearing preservation rate. Preoperative evaluation of labyrinthine fistulae using high resolution computed tomography (HRCT) detected 86% of cases, and this contributed to favorable results achieved with the surgical treatment of labyrinthine fistulae. We aimed to review cases of labyrinthine fistulae to summarize their outcomes and establish standards of management. METHODS: This was a retrospective chart review of

Surgical treatment of labyrinthine fistula in patients with cholesteatoma.

Labyrinthine fistula is one of the most common complications of chronic otitis media associated with cholesteatoma. The optimal management of labyrinthine fistula, however, remains controversial. Between 1995 and 2005, labyrinthine fistulae were detected in 31 (6 per cent) patients in our institution. The canal wall down technique was used in 27 (87 per cent) patients. The cholesteatoma matrix was completely removed in the first stage in all patients. Bone dust and/or temporalis fascia was inserted to seal the fistula in 29 (94 per cent) patients. A post-operative hearing test was undertaken i

The Lester-Jones tube and scuba diving.

Insertion of a Lester-Jones tube remains the standard treatment for epiphora secondary to canalicular obstruction. We report on a patient requiring removal of his correctly working Lester-Jones tube to allow him to continue his hobby of scuba diving. This particular complication of the Lester-Jones tube has never previously been reported in the literature. Patients with Lester-Jones tubes are unable to perform the valsalva manoeuvre. The valsalva manoeuvre involves blowing the nose against occluded nostrils resulting in a raised pressure in the nose and post-nasal space which is transmitted vi

Iatrogenic facial nerve palsy: lessons to learn.

INTRODUCTION: This study aims to review the management and discuss the outcome of patients with iatrogenic facial nerve palsy. METHODS: 11 patients with iatrogenic facial nerve palsy (FNP) were evaluated retrospectively in a tertiary centre between June 1995 and September 2008. All the cases were referred from other centres. RESULTS: Ten patients had iatrogenic immediate FNP secondary to mastoidectomy and one had FNP secondary to superficial parotidectomy. Of the ten cases, three had concomitant profound sensorineural hearing loss and one had concomitant labyrinthine fistula. Ten patients un

Complications of chronic otitis media with cholesteatoma during a 10-year period in Kosovo.

We review and discuss the results of treatments for complications of cholesteatomatous chronic otitis media (CCOM) in a tertiary health care center. In a retrospective study, the medical records of patients with complications of CCOM who had undergone surgical treatment at the ENT Clinic of the University Clinical Center of Kosovo for the period 1994-2004 were reviewed. From a total of 1,803 patients suffering from CCOM, in 91 patients, 55 (60.4%) men and 36 (39.6) women, one or two complications are recorded. The mean age of the subjects was 30 years, and the age range was from 1 to 76 years.

Incidence of fallopian canal dehiscence at surgery for chronic otitis media.

The objectives of this study were to determine the incidence and locations of dehiscence of the fallopian canal (FC) in patients undergoing surgery for different middle ear pathologies and to describe the findings that will aid in pre-operative prediction of dehiscence. Charts and operative details of the 118 ears managed with canal wall-down and 147 ears managed with canal wall-up tympanomastoidectomy performed by a single surgeon were retrospectively reviewed. The distribution of the diagnoses for ears that were operated was as follows: 118 ears cholesteatoma, 42 ears adhesive otitis, 23 ear

Dorsolateral medullary infarction: a neurogenic cause of a contralateral, large-amplitude vestibular evoked myogenic potential.

The vestibular evoked myogenic potential (VEMP) has become a useful tool to assess the saccule and inferior vestibular nerve function. Vestibulopathies involving the saccule or inferior vestibular nerve typically result in VEMP responses that are diminished or absent on the involved side. Abnormally large VEMPs are rare. Large VEMPs have been associated with superior canal dehiscence, Ménière's disease, and labyrinthine fistula. In all of these cases, the abnormally large VEMP can be explained on the basis of labyrinthine hydromechanical changes that result in excessive saccular displacement i
