Influence of vibration to the neck, trunk and lower extremity muscles on equilibrium in normal subjects and patients with unilateral labyrinthine dysfunction.

To investigate the role of proprioceptors of different skeletal muscles in postural control, in normal subjects and patients with unilateral labyrinthine dysfunction (ULD), the effect of vibration on these muscles was studied by postulography. The subjects comprised 59 normal subjects and 12 patients with ULD due to resection of acoustic tumours. Sagittal body sway was observed during vibration to the triceps surae, tibialis anterior and upper dorsal neck muscles. No significant change in sway was observed in the frontal plane in normal subjects. Significant differences between normal subjects

[Infrared and video oculography--alternatives to electrooculography?].

With the introduction of each new technique for registering eye movements, the question arises concerning whether these provide a reliable and accurate alternative for the diagnosis of labyrinthine dysfunction when compared to the most commonly used electro-oculographic technique. To answer this question we compared mean slow-phase velocity (SPV) using three different recording techniques:electro-oculography (EOG), video-oculography (VOG) and infrared oculography (IROG) during four different types of examinations. The examinations were the eye target tracking test (ETT), horizontal and vertica

Surgical labyrinthectomy in the older patient.

OBJECTIVE: Ablation of vestibular function is a highly efficacious option in the treatment of disabling vertigo arising from unilateral labyrinthine dysfunction. Regardless of the method used to ablate vestibular function, permanent posttreatment impairment of the balance function will develop in a number of treated patients. Many physicians who are involved in the care of the older patient with episodic vertigo are reluctant to recommend or perform a vestibular ablation procedure, because this treatment may result in permanent disequilibrium, which may be more detrimental to the older patient

[Acute vertigo].

Among all patients presenting with acute vertigo, majority will be suffering from benign paroxysmal positional vertigo, vestibular neuritis or Meniere's disease. One of the most important central vestibular disorder that imitates labyrinthine dysfunction is vertebrobasilar artery disease. To differentiate this condition from peripheral vestibular lesions, particular attention should be directed to the type of nystagmus which is present. Additionally, duration of vertigo and further neurological symptoms and signs are crucial for correct localization and etiology of the underlying disorder.

Vestibular nerve section.

INTRODUCTION: Vestibular nerve section is considered to be the most effective surgical procedure for control of intractable symptoms secondary to labyrinthine and eighth nerve function. This study was developed to retrospectively evaluate the efficacy of vestibular nerve section in patients treated for disabling labyrinthine dysfunction. METHODS: A retrospective review of hospital and office records was carried out on 39 patients who underwent vestibular nerve section. All patients received a comprehensive questionnaire to subjectively evaluate efficacy. RESULTS: Questionnaires were returned

[Isolated vertigo disclosing infarction in the area of the posterior and inferior cerebellar arteries].

We report three cases of small cerebellar infarcts mimicking labyrinthine dysfunction. A sudden rotatory vertigo might be the only presenting symptom of a cerebellar infarct. In these cases, the clinical features may closely mimick an acute peripheral labyrinthine disorder. However, the absence of nystagmus or a direction changing nystagmus with different eye position and the normality of caloric responses may be suggestive of a cerebellar infarct. This syndrome may be explained by the involvement of the nodulus, part of the flocculo-nodular complex, that has primary vestibular connections. Ce

Effect of alcohol on VOR compensation after unilateral labyrinthine loss.

It has been reported that alcohol ingestion reduces cerebellar control of the vestibulo-ocular reflex (VOR). To investigate the compensatory processes following labyrinthine loss, we examined gaze function and VOR before and after alcohol ingestion in 10 patients with unilateral lesion. The subjects were rotated sinusoidally in an electrically driven chair under two different visual conditions, i.e., mental arithmetic in the dark, and gaze fixation on a target on the wall. Whereas a control group (17 normal adults) showed no significant change after alcohol ingestion, patients showed a greater

Adaptation to vestibular disturbances: some clinical implications.

A knowledge of the mechanisms used in adaptation to vestibular lesions is important for both diagnostic and therapeutic purposes. Here we discuss commonly encountered circumstances--habitual wearing of spectacles, unilateral or bilateral labyrinthine dysfunction--that call for adaptation. The context specificity of vestibular adaptation is emphasized because of its crucial role in designing programs of physical therapy. The pharmacological manipulation of plasticity, either purposeful facilitation, or inadvertent retardation, is an area of current investigation of direct clinical importance. F

[The physiopathology of vertigo].

Vertigo is the predominant symptom in labyrinthine dysfunction, and is characterized by an unpleasant modification of the sense of static or dynamic orientation. In order to arrive at a correct clinical interpretation of this symptom and to administer the appropriate treatment, both its etiology and the underlying physiopathological mechanisms need be specified. In the present paper, we shall review a few qualitative concepts, which seem interesting to us and namely concern, on the one hand, the physiopathological mechanisms of vertigo and, on the other hand, the basic principles of the functi

Unterberger stepping test: a useful indicator of peripheral vestibular dysfunction?

Deviation of gait in a stepping test has been proposed as a useful indicator of peripheral labyrinthine dysfunction. A prospective study of 26 patients suspected of having uncompensated peripheral labyrinthine dysfunction and 49 normal patients with normal labyrinthine dysfunction showed no significant difference in performance of the Unterberger stepping test between patients with electronystagmographically significant canal paresis and those with normal vestibular function.

Evaluation of the vestibulo-ocular reflex by gaze function.

The relationship between the vestibulo-ocular reflex (VOR) and gaze fixation was investigated in normal adults, normal children, patients with cerebellar lesion and patients with labyrinthine dysfunction. Large VOR gains were found in patients with cerebellar lesion and normal children, both presenting poor suppression by gaze. Pathological decline in the VOR gain with labyrinthine lesion impaired spatial gaze fixation; however, it did not affect fixation-induced suppression. Gaze effect on the VOR, whether amplifying or suppressing, decreased linearly and rapidly at higher frequencies so that

Effect of labyrinthine dysfunction upon head oscillation and gaze during stepping and running.

Head oscillations and eye movements when stepping and running were studied in normal subjects and in patients with labyrinthine dysfunctions. In normal subjects, whereas linear head oscillations in the horizontal plane were very limited during both stepping and running, a marked increase in the vertical linear displacement induced pitching motions of the head during running. Patients with unilateral and bilateral lesions complaining of oscillopsia manifested lateral and diffuse oscillations, respectively, in the horizontal plane during stepping. The ratio of eye amplitude to head amplitude in

Cervico-vestibular interaction in eye movements.

The cervico-vestibular interaction was investigated in normal subjects and patients with unilateral labyrinthine dysfunction using neck vibratory stimulation and caloric stimulation. There is a functional interaction between sensory inputs from the neck proprioceptors and the peripheral labyrinth in the patients with unilateral labyrinthine dysfunction. However, this interaction was not observed in normal subjects. This result indicates that the role of the cervical sensory inputs in normal subjects and the patients is different. That is, inputs from the neck proprioceptors have little effect

Recent development of the study on clinical significance of abnormal eye movement.

As a general trend, the diagnosis in medical clinics often depends on laboratory test results. Neurotological diagnosis, however, requires detailed neurological examinations on a patient by a neurotologist. Therefore, there are differing diagnostic skills among physicians, and there is a kind of "man-made flavor" in neurotological diagnostic procedure. In the present study, current development in the knowledge on the clinical diagnostic significance of pathological eye movement during the last 2-3 years is summarized. Acquired pendular wondering eye-movement. Fixation jerks. Spontaneous and tr

Vestibulo-ocular reflexes in peripheral labyrinthine lesions: II. Caloric testing.

In a group of patients with unilateral labyrinthine dysfunction, the maximal velocity of the slow component response to caloric stimulation was compared with that to horizontal vestibular rotatory stimulation by a low-frequency stimulus. It was found that the larger the magnitude of the caloric response, the larger were the gain and sensitivity of the vestibulo-ocular reflex induced by physiologic stimuli. Rotary stimulation of patients with only one functioning inner ear provides information equivalent to that obtained from caloric stimulation and allows more rigorous quantification of the st

Vascular involvement in relapsing polychondritis.

Review of four cases of relapsing polychondritis (RP) seen at one hospital in the 12-year period 1963 to 1974 revealed that one patient had aortic insufficiency with large artery involvement, two others had involvement of medium and large arteries and the fourth may have had mucocutaneous vasculitis. Valvular disease has occurred in 9% of all cases of RP reported in the literature and, if vasculitis beyong the aortic root is included, 25% of cases of RP manifested inflammatory vascular disease. The frequency of pseudotumour of the orbit and cochlear-labyrinthine dysfunction is also high and ma

Familial congenital monochromatism, cataracts, and sensorineural deafness.

Two sisters had diagnoses of congenital monochromatism, cataracts, bilateral nonprogressive sensorineural deafness, and hyperinsulinism in both, and labyrinthine dysfunction in one. This recessively inherited condition is added to the growing number of syndromes in which one of the features may be a disturbance of hypothalamic function.
