Use of Envelope Following Response Normative Ranges for Diagnosing Cochlear Deafferentation.

PURPOSE: The lack of a means to diagnose cochlear synaptopathy, a type of cochlear deafferentation, prevents clinicians from identifying patients with this auditory deficit and providing them with appropriate treatments. The envelope following response (EFR) has potential as a diagnostic indicator of deafferentation. However, it is not clear what constitutes an abnormal EFR. The objectives of this study were to establish normative ranges for EFR magnitude in a population at low risk for cochlear synaptopathy and then compare EFRs from a population at high risk for synaptopathy to those normati

Cochlear deafferentation impairs perception of acoustic onset cues in kainate-exposed budgerigars.

Synaptopathy is a common cochlear pathology involving damage, dysfunction, and loss of auditory-nerve synapses with sensory hair cells. Cochlear synaptopathy can occur following sound overexposure due to glutamate excitotoxicity. Synaptopathy is undetectable with the clinical audiogram and is widely hypothesized to cause "hidden hearing loss," i.e., impaired perception of speech and other complex sounds under challenging listening conditions. However, evidence that cochlear synaptopathy causes hidden hearing loss is mixed, and the specific aspects of auditory perception impacted by synaptopath

Association between metabolic syndrome and hearing loss: The mediating role of retinol - A cross-sectional analysis of NHANES 2007 to 2018 (excluding 2013-2014).

Emerging evidence suggests associations between metabolic syndrome (MetS) and hearing loss, though population heterogeneity remains understudied. Given the high susceptibility of the inner ear to oxidative damage, fat-soluble antioxidants such as vitamins A, E, and carotenoids may exert protective effects. This study aimed to examine the MetS-hearing loss association among the U.S. adults and assess the potential mediating roles of these antioxidants. This cross-sectional study analyzed data from the U.S. National Health and Nutrition Examination Survey (2007-2018, excluding 2013-2014). Weight

A Fungal-Derived Bioactive Resource for Cochlear Protection: Sanghuangporus sanghuang Extract Mitigates Acoustic Trauma through Nrf2/HO-1 Antioxidant Axis.

Noise-induced hearing loss (NIHL) is a major form of sensorineural hearing impairment driven by oxidative stress-mediated cochlear injury. Sanghuangporus sanghuang (SS), a medicinal fungus extensively studied in microbiology and biotechnology, is known to produce bioactive metabolites with antioxidant properties; however, its functional role in the auditory system has not been established. This study investigated the otoprotective potential of SS extract against oxidative and acoustic stress using complementary in vitro, ex vivo, and in vivo models. In H2O2-treated UB-OC1 auditory cells, SS (2

Interplay of age, speech perception in noise, standard and extended high-frequency hearing, and cochlear synaptopathy in individuals with normal audiograms.

Older adults often complain about having difficulty understanding speech in noise, despite clinically normal hearing (D-SiN-NH). Two otopathologies known to decline with age and that are not reflected in the conventional audiogram are extended high-frequency (EHF) hearing impairment and cochlear synaptopathy (CS), both hypothesized to be responsible for D-SiN-NH. Nevertheless, the influence of EHF deficits on Auditory Brainstem Response (ABR) wave I amplitude, a commonly used proxy measure for CS, remains equivocal. Furthermore, the combined effects of age-related changes in peripheral auditor

Collective dynamics and synchronization of coupled hair-bundle models with elastic and viscous interactions.

Despite extensive studies of single hair-bundle dynamics, comparatively little attention has been paid to the collective dynamics arising in coupled hair-bundle systems. In this work, we investigate the collective dynamics of coupled hair bundles using a simplified dimensionless mechanical model that captures the essential nonlinear features of hair-bundle activity. We focus on two physically motivated interaction mechanisms: elastic and viscous coupling between neighboring bundles. The stability of synchronized motion is first analyzed using the master stability function framework and complem

Functional and histological assessment of 3D-printed helmet and hearing protection devices in preventing blast-induced auditory injury in Chinchillas.

Blast-induced auditory injury remains a persistent concern in many high-risk occupational environments, especially military service members and veterans. Although helmets and hearing protection devices (HPDs) have independently demonstrated efficacy in mitigating acoustic injuries, the synergistic protective effect of their combined use against blast overpressure (BOP) has not been comprehensively evaluated. The present study investigated the auditory protective performance of a custom-engineered 3D-printed helmet, both independently and in conjunction with HPDs, in a chinchilla model subjecte

When "noise" isn't simply noise: deterministic postural drive during noisy galvanic vestibular stimulation.

Age- and disease-related vestibular decline contributes to dizziness and postural instability, motivating interventions, such as noisy galvanic vestibular stimulation (nGVS). nGVS is often applied at imperceptible amplitudes and interpreted through a stochastic resonance (SR) mechanism, but because galvanic stimulation directly modulates vestibular afferents, even imperceptible currents may deterministically influence balance. The present study tested whether low nGVS stimulus amplitudes (<1 mA), commonly used in stochastic resonance paradigms, directly drive postural behavior. Twenty healthy

Neuroprotective potential of a TrkB-FL-derived cell-penetrating peptide in cochlear synaptopathy and noise-induced hearing loss.

BACKGROUND: Noise-induced hearing loss (NIHL) is the second leading cause of deafness globally. However, effective pharmacological treatments remain unavailable. Excitotoxicity, an early NIHL event, is a central mechanism in degeneration of cochlear synapses established between inner hair cells (IHCs) and spiral ganglion neurons (SGNs). This excitotoxic damage can trigger the degradation of the neurotrophic TrkB receptor, thereby inhibiting brain-derived neurotrophic factor (BDNF) signaling and challenging neurotrophin-based therapies for hearing loss. METHODS: We employed an ex vivo model of

Cochlear aging after synaptopathic noise: age-noise interactions in hair cell loss and axonal degeneration.

Despite many studies of cochlear synaptic loss and/or recovery after acoustic injury, none has followed degeneration of auditory-nerve fiber (ANF) peripheral axons - a critical knowledge gap in the context of regenerative strategies. We exposed CBA/CaJ mice to a synaptopathic 8-16 kHz noise stimulus producing an immediate 50% loss of inner hair cell synapses, with minimal post-exposure recovery. Noise-exposed and unexposed animals were allowed to survive for 1 month, 6 months, 1 year or 2 years before tissue harvest. Using immunostaining and machine-learning pipelines, we quantified hair cells

Resveratrol Mitigates Noise-Induced Cochlear Damage and Delays Hearing Loss in Wistar Rats.

INTRODUCTION: This study was aimed at evaluating the potential otoprotective effects of resveratrol (Changsha Huir Biological-Tech) on noise-induced hearing loss (NIHL) in a Wistar rat model. METHODS: The study employed a randomized controlled design. The noise used for stimulation was white noise, centered at a frequency of 4000 Hz (3564-4490 Hz), with an intensity of 100 dB SPL, for a period of 10 consecutive days at 8 h/day. Thirty rats were randomly assigned to three groups: a control nonexposed-to-noise group (n = 10) that received a vehicle solution of DMSO (dimethyl sulfoxide) (Sigma-A

Vestibular Function Drives Gaze Stability in Locomoting Macaques.

Stable gaze is essential for effective navigation, yet during locomotion each stride generates phasic head movements that can destabilize the visual scene. The vestibular system is well positioned to counter these disturbances by sensing head motion and rapidly generating reflexive compensatory eye movements, but its specific contribution to gaze stabilization during locomotion is not well understood. To directly test the vestibular system's contribution during walking, we compared gait and gaze kinematics in male and female rhesus macaques with normal vestibular function and those with chroni

Underwater noise exposure induces auditory threshold shifts and metabolomic alterations in the inner ear endolymph of European sea bass.

Anthropogenic underwater noise is a pervasive pollutant in marine and aquaculture environments. However, its physiological and biochemical impacts on fish auditory systems remain poorly understood, particularly in species of ecological and aquaculture importance such as the European sea bass (Dicentrarchus labrax). Here, we combined auditory evoked potentials (AEPs) with untargeted metabolomics of inner ear endolymph to quantify noise-induced auditory threshold shifts and to identify molecular pathways affected by broadband noise exposure. A total of 114 sea bass were first assessed for baseli

Molecularly defined auditory neuron subtypes show different vulnerabilities to noise- and age-related synaptopathy in mice.

Neuronal subtype-specific synaptopathy is a hallmark of many forms of neurodegeneration. We examined the cellular basis for synaptic vulnerability in the auditory system, where three subtypes of spiral ganglion neurons (SGNs)-Ia, Ib, and Ic-carry acoustic information from the cochlea to the brain. In response to noise and aging, a subset of synapses between inner hair cells and SGNs are lost, but it is unclear how this loss varies across SGN subtypes. Using genetic labeling, we showed that Ia SGNs have larger post-synaptic densities (PSDs) than Ib and Ic SGNs and are the most resilient subtype

The role of magnesium in otology - A narrative review.

Magnesium is believed to have a positive and protective effect on inner ear disorders. We conducted a systematic literature search on the topic. The mesh terms "magnesium", "tinnitus", "sudden hearing loss", "vestibular disorders", "ototoxicity", and "otoprotection" were used for the Medline search. Based on this approach, a narrative review with a structured literature search was conducted. There is good evidence that supplementation with >200 mg magnesium daily protects against the development of age-related hearing loss. A similar protective effect can be assumed for protection against nois

Comparing physiological signatures of synaptopathic noise exposure and carboplatin-induced inner-hair-cell dysfunction in chinchillas.

Aging and noise over-exposure lead to complex mixtures of cochlear degradation that impair the structure and function of outer hair cells (OHCs), inner hair cells (IHCs), and the cochlear nerve. However, IHC damage and cochlear synaptopathy (CS) remain pathologies "hidden" from the audiogram. This study aimed to compare and contrast the physiological signatures of these two popular and distinct exposure paradigms using promising non-invasive assays: Envelope Following Responses (EFRs), Auditory Brainstem Response (ABRs), Wideband Middle-Ear Muscle Reflexes (WB-MEMRs), and Distortion Product Ot

Perilymphatic ATP plays a critical role in modulating cochlear function to protect from loud sound induced hearing loss.

BACKGROUND: Extracellular adenosine triphosphate (ATP) signalling via purinergic receptors plays a key role in cochlear adaptation to loud sound. Traditionally, ATP activation of purinergic receptor P2X receptors has been proposed to induce a cation shunt, reducing the endolymphatic potential and the driving force for sound transduction. However, direct evidence for this protective mechanism remains limited. Here, we provide direct experimental evidence identifying a distinct, compartment-specific ATP signalling pathway that modulates cochlear function. METHODS: In mature Dunkin-Hartley guine
