Sex Disparity in Enrolment to Clinical Trials for Onchocerciasis and Trachoma.

PURPOSE: To determine whether female enrolment in randomized interventional clinical trials for onchocerciasis and trachoma reflects the real-world sex distribution of disease burden, and to evaluate sex-based differences across trial characteristics. METHODS: We performed a cross-sectional analysis of completed randomized interventional clinical trials registered on ClinicalTrials.gov evaluating onchocerciasis or trachoma and initiated between 1974 and 2020. Eligible trials used double masking or greater and reported results through ClinicalTrials.gov or peer-reviewed publication. Female rep

Comparison of Methods for Assessing Chlamydia trachomatis Transmission Intensity: A Systematic Review.

Field-graded trachomatous inflammation-follicular (TF) has limitations as the sole indicator of Chlamydia trachomatis (Ct) transmission for trachoma programmatic decision-making. To assess the relationship between field-graded TF and other methodologies for evaluating Ct transmission intensity, a systematic review was conducted. A total of 35,764 studies were screened, yielding 235 included studies from 49 countries spanning the years of data collection from 1991 to 2021 and the years of publication from 1991 to 2022. The relationship between TF and other indicators was stronger before mass dr

Ripple Effects Mapping of a gender equity and social inclusion behaviour change activity in Uganda's trachoma effort.

BACKGROUND: Uganda's trachoma program has faced persistent challenges in achieving adequate preventive medicine coverage. To address these, the Ministry of Health applied Ripple Effects Mapping (REM) to assess a gender equity and social inclusion (GESI) behaviour change activity (BCA). To our knowledge, this represents one of the first documented applications of REM to a neglected tropical disease (NTD) program. METHODS: Seven REM workshops were conducted with government officials, health staff and community members. Facilitators guided participants to identify the BCA's inputs, outputs and i

Discrepancy Between Active Trachoma and Tests of Chlamydial Infection in Alto Amazonas, Peru.

BACKGROUND: Trachoma mapping in Latin America is incomplete, especially in indigenous communities of the Amazon thought to be at highest risk. The aim of this study was to determine the prevalence of clinical, microbiological, and serologic evidence of trachoma in a remote area of the Peruvian Amazon. METHODS: A population-based sample of households from rural communities in Alto Amazonas province was selected using 2-stage cluster sampling. In children aged 1-9 years, conjunctival photographs, conjunctival swabs, and dried blood spots were collected. Photographs were graded for trachomatous

Iron limitation-induced modulation of transcription in Chlamydia trachomatis.

Chlamydia trachomatis (C. trachomatis) is an obligate intracellular bacterial pathogen responsible for trachoma and sexually transmitted infections. Like other Chlamydiota members, such as Waddlia chondrophila, C. trachomatis undergoes a biphasic developmental cycle alternating between infectious elementary bodies (EBs) and replicative reticulate bodies (RBs). Under stressful conditions, RBs differentiate into non-dividing aberrant bodies (ABs), a reversible state associated with persistence. Here, we investigate how early iron deprivation affects transcriptional regulation in C. trachomatis.

Integrated serosurveillance to assess disease elimination in coastal Ecuador: onchocerciasis, yaws, and trachoma.

BACKGROUND: Testing blood samples with multiplex bead assays can assess elimination and identify residual foci of transmission for multiple pathogens simultaneously. In Ecuador, onchocerciasis and yaws are presumed eliminated, and the status of trachoma is unknown. We assessed their elimination status by measuring IgG antibodies in children 6-24 months. METHODS: In a birth cohort of 404 children measured between 2021-2024 in Esmeraldas province, Ecuador, we tested dried blood spots at ages 6, 9, 12, 18, and 24 months using a multiplex IgG assay that included antigens to Onchocerca volvulus (O

Sexually transmitted infection prevalence following azithromycin mass drug administration for trachoma in Nauru: a before-and-after cross-sectional survey.

BACKGROUND: Sexually transmitted infections (STIs) are a major public health problem globally, particularly in low and middle-income countries. In the Western Pacific region, including Nauru, STIs are highly prevalent. Syndromic management misses a high proportion of infections and screen-and-treat strategies are not currently feasible at scale in these settings. Mass drug administration (MDA), widely used for control of neglected tropical diseases, may provide an alternative approach to the population control of STIs. METHODS: We used a before-and-after study design to evaluate the impact of

Influence of Chlamydia trachomatis Sequence Variation on pgp3 Antigenicity.

Chlamydia trachomatis (Ct) is a sexually transmitted infection (STI) linked to ectopic pregnancy, infertility, invasive lymphogranuloma venereum (LGV), and the blinding disease trachoma. STI, LGV, and trachoma are associated with disease-specific Ct genovars. Serology is an important surveillance tool for estimating exposure, but the impact of genome variation on the antigenicity of Ct proteins is unclear. Two-dimensional antigenicity profiling of Pgp3 antigens representing LGV, STI, and trachoma demonstrated differential antigenicity using antigen-specific murine sera, but geometric distances

Living tissues and dead objects in the eye: The first description of ocular immune privilege by J.C. van Dooremaal in Albrecht Von Graefe's Archiv für Ophthalmologie in 1873.

BACKGROUND: Immune privilege of the eye is considered to contribute significantly to the excellent survival of transplants in the anterior and posterior segment of the eye. While Sir Peter Medawar proved the existence of this phenomenon scientifically, a prior study by Dr. J.C. van Dooremaal already suggested the existence of ocular immune privilege and was published in this journal in 1873. METHODS: The original 1873 German paper by J.C. van Dooremaal in Albrecht von Graefe's Archiv für Ophthalmologie was translated into English and commented on by immunologist Dr. J. Dynamics of trachoma infection in West Africa revealed by a hidden state model.

Trachoma is estimated to be the leading infectious cause of blindness globally, predominantly affecting low-income populations with poor sanitation and hygiene. Over a decade of mass drug administration with antibiotics has led to substantial progress in control and elimination, but hotspots remain where infection persists or rebounds following mass drug administration for reasons that remain unclear. Transmission modelling is a key component of understanding these dynamics, but the complex dynamics of infection and reinfection with Chlamydia trachomatis are challenging to infer from cross-sec

Correlates of trachoma persistence and recrudescence: A global analysis of 33 countries.

OBJECTIVES: One criterion for trachoma elimination is a prevalence of trachomatous inflammation-follicular in children aged 1-9 years (TF1-9) < 5% in all formerly endemic implementation units (IUs). After years of antibiotic mass drug administration, many IUs have trachoma impact survey (TIS) or trachoma surveillance survey (TSS) results TF1-9 ≥ 5%. METHODS: We analyzed global data to explore potential correlates associated with TF1-9 ≥ 5% at the first TIS (TIS1), the second or higher TIS (TIS2+), and TSS. Univariable and multivariable logistic regression models were fitted. RESULTS: A total

Computational design of a novel chimeric multi-epitope vaccine against chlamydia trachomatis serovar a to prevent trachoma.

Chlamydia trachomatis serovar A is a re-emerging worldwide health problem that causes the ocular trachoma, the leading cause of avoidable blindness. The disease continues to spread due to reinfection and failure to develop long-lasting immunity. This study aimed to develop an in silico novel multi-epitope that can induce long-term immune protection against C. trachomatis serovar A. Researchers harvested the protein sequences of serovar A and queried for virulence-related factors. The predictions of epitopes for CTLs, HTLs, and B cells using immunoinformatics tools, and tested for its ability t

Community-level Monitoring of Antimicrobial Resistance following Mass Administration of Azithromycin to Reduce Child Mortality in Côte d'Ivoire: Results from Two Rounds of Surveillance.

BACKGROUND: Mass Drug Administration (MDA) of azithromycin for the elimination of trachoma and, more recently, for the reduction of child mortality, are public health strategies shown, with mixed results, to contribute to the development of azithromycin resistance in treated communities. The objective of this study was to monitor bacterial resistance to azithromycin among children targeted by bi-annual MDA-azithromycin campaigns to improve child survival in eight high-mortality regions of Côte d'Ivoire. METHODS: Two rounds of cross-sectional surveys were carried out in targeted health distric

Training Data Recorders with a Manual to Take High-Quality Trachoma Photographs in South Sudan.

When a country has reached a low trachoma prevalence, it can be difficult to reliably train survey graders to accurately grade trachoma signs. Conjunctival photography provides an opportunity to capture images for grading. Recently, a training manual targeting survey data recorders was published. In July 2023, this manual was used during a photography training, after which certified trainees served as data recorders and photographers during the Enhancing the 'A' in SAFE (ETAS) study in South Sudan. Five of 10 trainees passed the training and became certified. These individuals took 7,064 smart

Genome-wide association studies of C. trachomatis identify novel tissue tropism-predicting loci and sequences that discriminate among major genome sequence clades.

Chlamydia trachomatis infections cause chronic and debilitating diseases of the eye and genital tract. Recent data suggest that infections are common at other tissue sites, including the lower intestinal tract of both men and women. Our previous work demonstrated tropism-predicting sequences in male rectal isolates, with modification in the PmpE sequence and predicted structure being highly correlated with male rectal infection. An expanded collection of strains from five different tissue sites was used to identify novel tropism-predicting markers for the different tissues. These data demonstr

Vision at risk: assessing women's knowledge of trachoma in Jordan: A cross-sectional survey study.

Females are at higher risk of being affected with trachoma compared to males as they are the primary caregivers for their children who are one of the main reservoirs for trachoma-related infections. No previous research has assessed women's knowledge of trachoma in Jordan, which was the purpose of this study. Therefore, the aim of this study was to examine the females' knowledge of trachoma and its management in Jordan. This was a survey-based online cross-sectional study that was conducted in Jordan between December 27, 2025 and February 1, 2026. The study population for this research compris

Australian Trachoma Surveillance Report 2024.

Trachoma is the world's leading infectious cause of preventable blindness and is linked to poor living conditions. In Australia, trachoma has remained a public health problem in remote Indigenous communities in northern, central, and western regions. To seek formal validation by the World Health Organization (WHO) of the elimination of trachoma as a public health problem, Australia has needed to maintain levels of overall trachoma prevalence below 5% in children for at least two years in each formerly endemic state or territory. Australia must also demonstrate that the prevalence of previously

Chlamydia trachomatis ompA Genotype and Clinical Signs of Trachoma in a Longitudinal Tanzanian Cohort.

Trachoma, caused by Chlamydia trachomatis (Ct), persists as a major cause of preventable blindness despite the global SAFE strategy. Understanding how Ct genovars and genovariants influence infection dynamics and clinical outcomes is crucial for sustaining elimination efforts and informing vaccine development. A four-year longitudinal study was conducted in a trachoma-endemic region of Tanzania across multiple rounds of mass drug administration (MDA) with azithromycin. Ct infections were genotyped by ompA sequencing to identify genovars and genovariants. Associations between genetic variants,

Impact of two rounds of azithromycin mass drug administration on active trachoma and ocular Chlamydia trachomatis infection in Naoero.

BACKGROUND: Trachoma, caused by ocular infection with the bacterium Chlamydia trachomatis, is the primary infectious cause of blindness globally. To contribute to the elimination of trachoma as a public health problem, Naoero undertook two rounds of antibiotic mass drug administration (MDA) using azithromycin, with population-level coverage estimated at 87% and 93%. This study aimed to estimate the post-MDA prevalence of trachoma, and infection with C. trachomatis, in children aged 1-9 years. METHODS: A two-stage cluster sampled survey was conducted 12 months after the second MDA round. All r
