Progress towards the elimination of trachoma as a public health problem in four counties of Eastern Equatoria State, Republic of South Sudan.

BACKGROUND: Trachoma is targeted by the World Health Organization (WHO) for elimination as a public health problem by 2030. Trachoma impact surveys using standardised methodology are recommended to monitor progress towards elimination and to determine eligibility for continued surgery, antibiotics, facial cleanliness, and environmental improvement (SAFE) interventions. From 2007 to 2015, four counties of Eastern Equatoria State, South Sudan, received three to five rounds of mass drug administration with antibiotics. A trachoma impact survey in 2015 indicated all four counties had trachomatous-

Distribution and viability of ocular and non-ocular Chlamydia trachomatis in households in a trachoma-endemic community in Oromia, Ethiopia.

BACKGROUND: We aimed to determine the household distribution and viability of Chlamydia trachomatis (Ct) from the eyes, face, and hands during the initial two visits of a year-long fortnightly cohort study in geographically defined adjacent households. METHODS/FINDINGS: We enrolled 298 individuals from 68 neighbouring households in Shashemene Woreda, Oromia, Ethiopia. All individuals above 2 years of age residing in these households were examined for signs of trachoma. Swab samples were taken from the conjunctiva, faces, and hands and analysed for the presence and viability of Ct. Ct viabilit

Australian Trachoma Surveillance Report update: 2014-2022.

Australia is the only high-income country where trachoma has been endemic, defined as an overall trachoma prevalence in Aboriginal and Torres Strait Islander children aged 5-9 years of 5% or more. The Australian Government funds the National Trachoma Surveillance and Reporting Unit to collate and analyse trachoma prevalence data and control strategies annually. This report presents data submitted from 2014 to 2022. In 2022, there were 87 remote communities considered at-risk of endemic trachoma, a decline of 51% since 2014 when 177 communities were considered at-risk. World Health Organization

Development and Validation of Chlamydia muridarum Mouse Models for Studying Genital Tract Infection Pathogenesis.

Animal infection models play significant roles in the study of bacterial pathogenic mechanisms and host-pathogen interactions, as well as in evaluating drug and vaccine efficacies. Chlamydia trachomatis is responsible for infections in various mucosal tissues, including the eyes and urogenital, respiratory, and gastrointestinal tracts. Chronic infections can result in severe consequences such as trachoma-induced blindness, ectopic pregnancy, and infertility. While intravaginal inoculation of C. muridarum mimics the natural route of sexual transmission between individuals, transcervical inocula

Improving neglected tropical disease services and integration into primary healthcare in Southern Nations, Nationalities and People's Region, Ethiopia: Results from a mixed methods evaluation of feasibility, acceptability and cost effectiveness.

BACKGROUND: Ethiopia is one of the countries with the highest burden of neglected tropical diseases (NTDs), with 16 of 20 recognised NTDs considered a public health problem, twelve of which have been identified as public health priorities by the Ethiopian Federal Ministry of Health. However, until recently NTDs have not received adequate attention at national and subnational levels in the country. This study assessed feasibility, acceptability, and cost-effectiveness of an NTD intervention when integrated into the primary health care system in Ethiopia. METHODS: This study was conducted in Da

Identifying Borderline Trachoma Grades Using a Three-Latent Class Model.

The WHO has a simplified grading system for assessing trachoma. However, even for experts, it can be difficult to classify certain cases as strictly positive or negative for a given grade. Given the absence of a true gold standard, we performed a Latent Class Analysis (LCA) on a set of 200 graded photos of the superior tarsal conjunctiva. Ten trained graders assessed the presence of two trachoma grades: trachomatous inflammation-follicular (TF) and trachomatous inflammation-intense (TI). The LCA was modeled in two different ways: first with two classes (presence/absence), and then with three c

Prevalence of Ocular Chlamydia trachomatis and Active Trachoma among Children in Merhabete District, Amhara, Ethiopia.

There have been significant reductions in the burden of trachoma worldwide. However, some districts have experienced persistently high trachoma prevalence despite many years of intervention. Here, we report the epidemiology of trachoma in Merhabete, Ethiopia, a district in the Amhara Region that has been receiving azithromycin mass drug administration (MDA) since 2009. Data were obtained from the baseline survey of a cluster randomized trial evaluating targeted treatment strategies for trachoma elimination. An enumerative census was conducted in February 2022 to generate lists of children aged

Prevalence of ocular Chlamydia trachomatis infection and antibodies within districts persistently endemic for trachoma, Amhara, Ethiopia.

BACKGROUND: Persistent trachoma is increasingly recognized as a serious concern for the global trachoma program. Persistent trachoma is defined as those districts that have had two or more trachoma impact surveys in which the trachomatous inflammation-follicular (TF) prevalence has never been <5%, the elimination threshold for TF. Enhanced tools such as infection and serological monitoring elucidate long-term transmission patterns within persistent districts. This study aimed to clarify trachoma intensity via both traditional indicators and Chlamydia trachomatis (Ct) infection and serologic ma

Assessment of compliance with tetracycline eye ointment treatment to accelerate the elimination of trachoma in Yobe State, Nigeria.

BACKGROUND: Trachoma, a neglected tropical disease, remains a significant public health concern in many regions, particularly in sub-Saharan Africa and in Yobe State, Nigeria. One approach for elimination involves administering tetracycline eye ointment (TEO) to children <6 months of age as part of annual mass drug administration (MDA), aligning with the World Health Organization's 'A' component of the SAFE (Surgery, Antibiotics, Facial hygiene and Environmental sanitation) strategy for elimination of trachoma as a public health problem. However, suboptimal compliance rates in affected populat

Priority knowledge gaps for schistosomiasis research and development in the World Health Organization Africa Region.

Schistosomiasis, also known as bilharzia, is a widespread neglected tropical disease (NTD) in Africa, with more significant research and development (R&D) challenges and gaps compared to other preventive chemotherapy NTDs (PC-NTDs) like onchocerciasis, lymphatic filariasis, and trachoma. In response to this challenge, some global initiatives have advocated for bridging this gap, focusing on coordinated engagement with research donors. In this opinion article we highlight key R&D priorities for combating schistosomiasis in the WHO Africa region. These include defining morbidity indicators, expa

Telemedicine in the Clinical Care of Neglected Tropical Diseases: A Scoping Review.

BACKGROUND: The Neglected Tropical Diseases (NTDs) represent a global public health problem. Telemedicine uses telecommunications to deliver remote healthcare services when patients and providers are separated by distance. Using digital health technologies to offer medical care remotely to NTDs can be an important strategy for reducing inequalities in access but is challenging in low-and middle-income settings. The objective of the current review was to identify and summarize international evidence on the use of telemedicine for clinical care of patients with NTDs around the world based on a s
