Not All Transverse Sinus Stenoses Behave the Same: Distinct Flow Phenotypes on Phase-Contrast MRI.

Background Transverse sinus stenosis is usually assessed by its anatomical appearance, although stenoses with similar morphology may not have the same local flow behavior. This study evaluated peak velocity changes across transverse sinus stenoses using phase-contrast magnetic resonance imaging (MRI) and explored whether distinct hemodynamic patterns could be identified. Methods We retrospectively reviewed 183 consecutive patients who underwent brain MRI, magnetic resonance venography, and venous phase-contrast MRI for headache, vertigo, and/or tinnitus between September 2025 and June 2026. Emerging Therapies for Genetic Deafness: The Rise of Precision Gene Editing.

Hearing loss is a prevalent sensory disorder with profound social and health impacts. Although hearing aids and cochlear implants can significantly restore auditory perception, they do not address the underlying genetic causes. Inherited forms of hearing loss, often monogenic, are ideal candidates for gene therapy, particularly since over 150 genes have been linked to deafness. Advances in vector engineering and genome editing technologies now enable precise targeting of auditory cells within the inner ear. Technologies such as Clustered Regularly Interspaced Short Palindromic Repeats-Cas syst

Case Report: Olfactory loss as the sentinel manifestation of type 2 chronic rhinosinusitis without polyps: response to IL-4Rα inhibition with stapokibart - a case series.

Chronic rhinosinusitis without nasal polyps (CRSsNP) is traditionally associated with type 1 inflammation, but emerging evidence reveals a subset of patients with a type 2 endotype driven by IL-4 and IL-13, often presenting with prominent olfactory dysfunction and responding poorly to conventional therapy. We report three cases of type 2 CRSsNP with comorbid allergic rhinitis (aged 29-30 years, two males, one female) who presented with debilitating olfactory loss as the sentinel symptom. All had elevated type 2 biomarkers, absence of nasal polyps, and CT evidence of sinusitis. After failure of

Dietary Zinc Glycine Supplementation Attenuates Aging-Driven Cochlear Hair Cell Oxidative Injury and Hearing Loss.

Background: Micronutrient imbalance is now recognized as a modifiable risk factor driving the progression of age-related hearing loss (ARHL). Zinc, an indispensable dietary trace nutrient, acts as a structural cofactor and transcriptional regulator of a broad panel of antioxidant enzymes to sustain systemic and cellular antioxidant defense capacity. Nevertheless, the therapeutic value of zinc nutritional supplementation and its precise molecular mechanisms against age-dependent cochlear degeneration remain largely uncharacterized. Methods: We established aging mouse models to observe the pheno

Early Acoustic Voice Changes After Thyroidectomy Without Laryngeal Nerve Injury.

OBJECTIVES: The incidence of thyroid cancer has been increasing annually, and surgery remains the primary therapeutic approach. Clinically, postoperative voice disorders may occur in patients undergoing thyroidectomy, even in the absence of intraoperative injury to the laryngeal nerves. This retrospective study aimed to objectively evaluate postoperative acoustic voice alterations after thyroidectomy using Praat software in patients with confirmed preservation of recurrent laryngeal nerve (RLN) and superior laryngeal nerve (SLN) function. The study focused on changes in acoustic parameters wit

Combined Bioinformatic Analysis and RNA Sequencing Uncover Key Tinnitus-Related Molecules in the Cingulate Gyrus.

Tinnitus arises from maladaptive plasticity and abnormal synaptic function, not only in auditory pathways but also in limbic regions. Among these, the cingulate gyrus plays a pivotal part in shaping the emotional burden and sensory filtering disturbances often seen in tinnitus patients. Yet, the specific molecular players operating within this region remain poorly understood, and most existing bioinformatic predictions have not been tested in living systems. Here, we combined multi-database mining with in vivo transcriptomic profiling to identify and validate key proteins and pathways linked t

Epithelial-mesenchymal Wnt crosstalk directs planar cell polarity in the developing cochlea.

Cochlear hair cells are precisely oriented by planar cell polarity. Wnt proteins direct planar cell polarity in vertebrates, but their roles in regulating cochlear planar cell polarity remain unclear. Here, we inhibit Wnt secretion by ablating epithelial Wntless and find shortened cochlea, mild hair cell misorientation and mislocalization of core planar cell polarity proteins, Fzd6 and Dvl2. Computational analysis predicts that epithelial Wnts (Wnt5a, Wnt7a, Wnt7b) and Wnt5a from surrounding periotic mesenchyme act on developing hair cells. Deletion of epithelial Wnt5a, Wnt7a, and Wnt7b additi

Differential mechanisms of continuous theta burst stimulation and low-frequency repetitive transcranial magnetic stimulation for chronic tinnitus: preliminary efficacy dissociation by local focalization vs. network dispersion.

OBJECTIVE: This study aimed to preliminarily compare clinical efficacy and neural mechanisms of continuous theta-burst stimulation (cTBS) and low-frequency repetitive transcranial magnetic stimulation (rTMS) targeting the auditory cortex in chronic subjective tinnitus. Differential effects on tinnitus loudness and distress, as well as the moderating role of disease duration, were explored. METHODS: Twenty-nine patients were randomly assigned to cTBS (n = 16) or rTMS (n = 13) groups. Neuronavigation-guided stimulation was applied to the individualized auditory cortex peak activation site. Outc

Effects of individualized phase-amplitude-coupled transcranial alternating current stimulation on tinnitus and cognition.

INTRODUCTION: Chronic tinnitus is a prevalent and disabling condition that is frequently accompanied by domain-specific cognitive difficulties. Abnormal neural oscillations and disrupted cross-frequency coupling (CFC) have been implicated in its pathophysiology. This study investigated whether individualized phase-amplitude-coupled transcranial alternating current stimulation (tACS), tailored to each participant's individual alpha frequency (IAF), was associated with changes in tinnitus symptoms, cognition, and neurophysiological measures. METHODS: In this prospective single-arm interventiona

Adjunctive Acorus tatarinowii essential-oil ear-canal packing for idiopathic sudden sensorineural hearing loss: study protocol for a single-center, randomized, double-blind, placebo-controlled clinical trial.

BACKGROUND: Idiopathic sudden sensorineural hearing loss (ISSNHL) is an acute otologic emergency, and hearing recovery remains highly variable and frequently unsatisfactory. Acorus tatarinowii Schott has long been used in traditional Chinese medicine to treat hearing impairment and related auditory disorders. Its essential oil (SCP-oil) has demonstrated anti-inflammatory, antioxidant, and neuroprotective activities in preclinical studies. Advances in local otologic drug-delivery strategies suggest that administration through the external auditory canal may offer a practical, noninvasive approa

The Relationship among Obesity, Intracranial Hypertension, and Cerebrospinal Fluid Leak.

Idiopathic intracranial hypertension (IIH) is a disorder characterized by elevated intracranial pressure without identifiable lesion or secondary cause. Patients can often present with otolaryngologic manifestations of IIH, such as pulsatile tinnitus, aural fullness, and cerebrospinal fluid (CSF) leak. Obesity plays an important role as a risk factor for both IIH and CSF leak as similar pathophysiological processes underly both presentations. Understanding the nuanced presentation of this condition is important for the otolaryngologist who routinely evaluates patients with these symptoms and h

Semantic working memory supports phrasal planning: Evidence from acute stroke.

This study examined the contribution of semantic and phonological working memory (WM) to lexical-semantic planning during language production for individuals tested acutely after left-hemisphere stroke. Participants completed tasks assessing phrase and sentence production, single-word retrieval, semantic and phonological WM, and single-word input processing. Using a constrained production paradigm that manipulated structure (phrase vs. sentence) and number of adjectival modifiers (one vs. two), we found that semantic WM capacity, indexed by performance on a category probe task, predicted accur

Determinants of auditory performance after cochlear implantation in post-lingual adults: beyond the duration of hearing loss.

PURPOSE: Prolonged auditory deprivation progressively compromises central auditory pathway integrity, yet its prognostic impact in post-lingual adults undergoing cochlear implantation remains poorly defined. This study evaluates the influence of hearing loss duration - expressed in absolute terms, dichotomised by the conventional 10-year threshold, and as a proportion of lifetime with hearing loss - on auditory performance, and identifies the clinical determinants associated with the immediate activation effect and the post-implantation trajectory. METHODS: Single-centre, retrospective, obser
