Determinants of auditory performance after cochlear implantation in post-lingual adults: beyond the duration of hearing loss.

PURPOSE: Prolonged auditory deprivation progressively compromises central auditory pathway integrity, yet its prognostic impact in post-lingual adults undergoing cochlear implantation remains poorly defined. This study evaluates the influence of hearing loss duration - expressed in absolute terms, dichotomised by the conventional 10-year threshold, and as a proportion of lifetime with hearing loss - on auditory performance, and identifies the clinical determinants associated with the immediate activation effect and the post-implantation trajectory. METHODS: Single-centre, retrospective, obser

Inflammatory and immune pathways in Ménière's disease: a multi-database bibliometric and visualized analysis of research trends.

BACKGROUND: Ménière's disease (MD) is a chronic inner ear disorder characterized by recurrent episodes of vertigo, fluctuating sensorineural hearing loss, tinnitus, and aural fullness. Growing evidence indicates that aberrant immune regulation, inflammation, and allergic reactions are involved in the development of MD. However, a systematic synthesis of the current research landscape and evolving hotspots in this field remains lacking. METHODS: Publications concerning immune-inflammatory mechanisms in MD published between 1 January 2006 and 31 December 2025 were retrieved from the Web of Scie

Unexpected Identification of Two Medialized Tympanostomy Tubes in an Adult: A Case Report.

Tympanostomy tube placement is one of the most commonly performed otologic procedures and is widely used in the management of chronic otitis media with effusion (OME), recurrent acute otitis media (AOM), and Eustachian tube dysfunction (ETD). Although generally safe, complications may occur, and medialization of a tympanostomy tube into the middle ear space is a rare but recognized complication that may be associated with surgical removal due to the potential for chronic inflammation, conductive hearing loss, or middle ear injury. We present a case of two medialized tympanostomy tubes identifi

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

The impact of hearing loss on tinnitus-related brain reorganization: evidence from multimodal resting-state functional magnetic resonance imaging.

BACKGROUND: Whether hearing loss fundamentally alters tinnitus-related brain reorganization remains unclear. To address this issue, this study compared features from multimodal resting-state functional magnetic resonance imaging (rs-fMRI) between tinnitus patients with hearing loss (TIN + HL) and those without hearing loss (TIN - HL). METHODS: A total of 38 patients with tinnitus (23 TIN + HL and 15 TIN - HL) and 28 healthy controls (HCs) underwent rs-fMRI. Amplitude of low-frequency fluctuations (ALFF), regional homogeneity (ReHo), and bilateral auditory cortex (AC)-based functional connecti

A dual behavioral and electrophysiological evaluation of the sodium salicylate induced tinnitus model in rats: Comparing click and chirp stimuli in auditory brainstem responses.

Animal models are essential for investigating tinnitus physiology and therapeutic interventions. This study aimed to evaluate a salicylate-induced tinnitus rat model using a dual behavioral-electrophysiological approach and to compare the sensitivity of click and chirp stimuli in auditory brainstem responses (ABRs). Following sound-silence conditioning in a water T-maze (WTM), the salicylate group received sodium salicylate (300 mg/kg, i.p.) for six days, while the sham group received 0.9% saline. Tinnitus presence was behaviorally assessed via the water T maze swimming test, followed by ABR r

Transcriptomics Implicates the G3BP1-SG Axis in Cochlear Hair-Cell Pyroptosis During Age-Related Hearing Loss.

Age-related hearing loss (ARHL) is a degenerative change in the auditory system, but its underlying mechanism remains unclear. In this study, using RNA sequencing technology, we analysed the expression profiles of mRNAs in the cochlear tissues of ARHL-susceptible C57BL/6J (C57) mice and relatively resistant CBA/CaJ (CBA) mice. We found that the mRNA related to stress granules (SGs) was significantly increased in the cochlear tissues of ARHL mice. Combined with current research on the pyroptosis of cochlear hair cells, we constructed an aging model of House Ear Institute-Organ of Corti 1 (HEI-O

Scalp Topography of Event-Related Potentials Reveals Differences in Audiovisual Speech Processing in Autistic and Non-Autistic Children.

PURPOSE: In our prior work, conventional event-related potential (ERP) responses to audiovisual versus auditory-only speech were found to explain variance in language abilities in autistic and non-autistic children but failed to differentiate autistic children from non-autistic children. This study extends previous ERP analyses of responses to audiovisual versus auditory-only speech in autistic and non-autistic school-aged children to investigate group and condition differences in the strength and topography of the ERP based on data-driven, reference-independent metrics that overcome limitatio

DUOXA2-mediated thyroid hormone synthesis is indispensable for postnatal maturation of auditory function.

More than 500 genes associated with hearing loss have been identified, yet the pathogenic mechanisms underlying most deafness-causing mutations remain to be clarified, hindering the development of viable clinical therapeutic strategies. Clinical studies indicated that patients with DUOXA2 mutations present mucosal immune dysregulation and hypothyroidism. In addition, genetics screening in mouse models linked Duoxa2 deficiency to severe deafness, yet its pathogenic mechanism remains elusive. In this study, we demonstrated that Duoxa2 is essential for normal auditory function and clarified its u

Otoferlin Gene Therapy: Narrative Review for Ear and Hearing Special Issue: Gene Therapy for Hearing Loss.

In the last decade, gene therapy has shown promise in treating many genetic disorders. Hereditary hearing impairment represents one of the next promising frontiers. Many initial development efforts have focused on hearing impairment caused by mutations in the otoferlin gene (OTOF), as these mutations cause profound deafness in the setting of preserved auditory outer hair cells, which often remain for many years after birth. Animal models have shown that the target cells for gene delivery, the inner hair cells, are also present and viable, and they reside in a structurally intact organ that is

DNAJC17 regulates RNA splicing in auditory hair cells and is required for hearing.

RNA splicing is essential for gene regulation, but its role in maintaining cochlear hair cell function remains incompletely understood. Here, we identify a heterozygous missense variant (c.401A>C, p.E134A) in DNAJC17, which encodes a spliceosome-associated protein, that segregates with adult-onset progressive hearing loss in a multigenerational family. Hair cell-specific deletion of Dnajc17 in mice results in profound deafness with progressive hair cell degeneration, establishing its essential role in cochlear maintenance. Hair cells lacking DNAJC17 exhibit marked stereocilia disorganization a
