The Role of A Surgeon in Restoring the Senses & in Biomedical Engineering.

This review is based on my 2025 Jepson Lecture and outlines my personal journey to restore hearing in people with a severe sensorineural hearing loss. My dream started as a 5-year-old as I told my kindergarten teacher when I grow up 'I want to fix ears' because my father is deaf. The journey resulted in a career in surgery followed by auditory neuroscience, engineering and audiology. While the key stages in the development of the multi-channel cochlear implant are highlighted, their importance in discovering the first restoration of a human sense, and in understanding brain function is present

Differential cross-modal functional plasticity in deaf individuals: A meta-analysis of neuroimaging studies.

A manifest condition of brain plasticity is cross-modal plasticity in deafness, wherein auditory-deprived cortices are recruited by intact sensory modalities. While cross-modal plasticity is well documented, the underlying principles remain debated, centering on the functional change and functional preservation principles. To quantitatively evaluate these principles, we conducted the largest coordinate-based meta-analysis to date, synthesizing 47 studies (801 deaf and 880 hearing participants) spanning linguistic and non-linguistic tasks. Deaf individuals exhibited consistent cross-modal activ

Auditory- and Visual-Speech Evoked Activation of the Superior Temporal Gyrus Contralateral to the Cochlear Implant Predicts Speech Recognition Outcomes.

OBJECTIVES: Speech recognition outcomes vary widely in cochlear implant recipients, but the reasons for this variability are not well explained by patient history factors alone. Neuroplastic reorganization after both deafness and cochlear implant switch-on can affect how new recipients adapt to electrical hearing. In this study, we evaluate the associations between speech recognition outcomes and stimulus-evoked activation of cortical language processing areas. DESIGN: Forty-three newly implanted post-lingually deaf adult cochlear implant recipients (17 female, mean age at implantation: 58.5

[Methodological implications of the minimal clinically important difference in patient-reported outcome measures in the field of cochlear implant provision. German version].

To determine the minimal clinically important difference (MCID), patient-reported outcome measures (PROMs) are presented that cover the dimensions 1. subjective hearing ability, 2. hearing-specific quality of life, and 3. generic quality of life, which are largely mentioned in corresponding guidelines and recommendations for device provision and aural rehabilitation in Germany. In the absence of recognized MCID thresholds in the field of cochlear implant (CI) provision in Germany, a methodological framework for future clinical studies was developed based on a two-stage anchor-based retrospecti

Viewpoint on Crossmodal Neural Plasticity.

Crossmodal neural plasticity, the brain's ability to adapt in response to sensory deprivation, serves as a primary model for understanding the multifaceted interplay between intrinsic neurobiological constraints and environmental experience in brain development. In this viewpoint, Olivier Collignon and Ella Striem-Amit engage in a collaborative dialogue exploring the fundamental principles of brain organization in individuals born with blindness, deafness, or without hands. The discussion evaluates current shifts in the field, including the move toward high-resolution neuroimaging, deep data a

Case Report: Streptococcus suis-associated meningoencephalitis with profound bilateral labyrinthine deafness and intra-operative cochlear fibrosis treated by cochlear implantation.

Streptococcus suis is a common zoonotic pathogen that usually causes monomicrobial meningitis. Occult bloodstream detection of Klebsiella pneumoniae, together with proven central-nervous-system S. suis infection, followed by irreversible cochlear fibrotic changes requiring cochlear implantation, appears exceptionally rare. We report a 52-year-old male with chronic heavy alcohol intake and occupational swine-exposure history who developed severe sepsis, recurrent suppurative meningoencephalitis caused by S. suis, and permanent bilateral labyrinthine-related deafness. Combined blood and cerebros

Automated Question-and-Answer-Based Speech-In-Noise Hearing Test Using a Large Language Model.

A conversational speech-in-noise test administered without human supervision is desirable to reduce the need to attend the clinic. This study aimed to create and evaluate a novel question-and-answer-based speech-in-noise test called 'CHAT', where stimuli were synthesised using text-to-speech (TTS), responses captured using automatic speech recognition (ASR), and responses evaluated using a large language model (LLM). Three tests were compared: the newly developed CHAT, the Adaptive Sentence List (ASL) and the Four Alternative Auditory Feature Test (FAAF). CHAT used synthetic speech while ASL a

Validating the Gap-Startle Paradigm for Tinnitus Detection: A Machine Learning Approach in CBA/CaJ Mice.

UNLABELLED: Tinnitus is one of the most common hearing disorders affecting one-third of Americans and is defined as the buzzing or ringing sound one perceives in one or both ears in the absence of an acoustic stimulus. One objective method for tinnitus screening in rodents is gap prepulse inhibition of the acoustic startle reflex (GPIAS), a reduction in the abrupt motor response elicited by an intense auditory stimulus following a silent gap in noise. Reduced inhibition by gaps embedded in narrow-band noise is hypothesized to reflect the primary tinnitus pitch, as the tinnitus "fills-in" the g

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

[Understanding the Relationship Between Hearing and Cognition to Improve Neuropsychological Assessment in Adults Aged 45 Years and Older: A Population-Based Study].

Age-related cognitive changes occur alongside sensory changes, including presbycusis, which notably affects speech understanding in noisy environments. Although typically diagnosed after the age of 60, hearing difficulties may begin insidiously as early as midlife. Given the well-established association between sensorineural hearing loss and cognitive functioning, we aimed to investigate 1) the associations between hearing abilities and cognitive performance in adults aged 45 years and older from the general population; and 2) the cognitive profile of middle-aged adults (45-65 years) with impa

Endovascular management of labyrintho-vascular otic capsule dehiscence (type II): A different therapeutical approach.

INTRODUCTION: Otic capsule dehiscence (OCD) encompasses a spectrum of third mobile window disorders, including type II variants characterized by a direct interface between the membranous labyrinth and adjacent vascular structures. These labyrinthine-vascular dehiscences (type IIa: superior semicircular canal-superior petrosal sinus (SPS); type IIb: internal jugular vein-vestibular structures) may induce pulsatile tinnitus, vertigo, and hearing loss. Endovascular treatment has emerged as a minimally invasive alternative to conventional surgery, aiming to reduce vascular pulsatility transmission

Myeloid cell reprogramming drives enhanced defense against Streptococcus pneumoniae lung infection following exposure to commensal Prevotella.

Clinical data link the prevalent respiratory tract anaerobe Prevotella with reduced pneumonia mortality, but the mechanisms directing Prevotella regulation of lung immune homeostasis are unclear. Here, single-cell RNA sequencing was employed to define the transcriptional immune signatures underlying improved clearance of Streptococcus pneumoniae following lung exposure to Prevotella melaninogenica. Overall, we observed a substantial shift in myeloid cell transcriptional programming from interferon-dominant to a more antibacterial profile in S. pneumoniae-infected mice after pre-exposure to P.

Learning Perspectives in the Management of Sigmoid Sinus Diverticulum in the Absence of Pulsatile Tinnitus.

Sigmoid sinus diverticulum (SSD) is an aberrant venous dilatation commonly associated with pulsatile tinnitus. However, an SSD without pulsatile tinnitus may be difficult to anticipate. Pre-operative computed tomography should be carefully reviewed for such vascular anomalies, with meticulous anticipation at surgery. SSD may pose a problem in proper visualization of the surgical field, and may put the surgeons in a dilemma on whether drilling can be continued safely. Right judgment, careful and patient drilling, and proper repair of the sinus plate in the event of inadvertent breach are the es

Structural and Mechanistic Insights Into GSDME Pore Formation and Functional Regulation.

Gasdermin E (GSDME) is a unique member of the gasdermin family linked to deafness, cancer, and neurodegenerative diseases, and mediates chemotherapy-induced pyroptosis; however, its pore-forming mechanism remains poorly understood. Here, we present cryo-EM structures of GSDME pores with oligomeric stoichiometries ranging from 23 to 29 protomers. Using mutagenesis, liposome leakage assays, and analysis of cancer-associated mutations, we identified key residues responsible for membrane binding, shallow membrane insertion, and oligomerization through three distinct structural interfaces.
