PURPOSE OF REVIEW: Optical coherence tomography (OCT) is a noninvasive optical imaging technique to image soft tissue and thin bony structures with high resolution, approaching that of histology. This review summarizes recent progress in the clinical application of OCT for tympanic membrane and middle ear disease.
RECENT FINDINGS: Recent reports from clinical trials show that OCT is having a meaningful impact on the diagnosis of fluid in the middle ear. In parallel with this, numerous groups are obtaining previously unattainable normative data in vivo on human middle ear structures. OCT is al

OBJECTIVES: Cochlear implants (CIs) can restore hearing to individuals with severe deafness, yet clinical outcomes vary widely because it remains difficult to predict how electrical currents interact with the unique anatomy of each cochlea. A central challenge in auditory rehabilitation is the lack of experimental systems that capture both the anatomical fidelity and the electrical properties of the human cochlea. To address this need, we developed electroanatomic twins: three-dimensional, 3D printed models of human cochleae that incorporate anatomically accurate features of the otic capsule a

INTRODUCTION: Auditory steady-state responses (ASSRs) objectively estimate hearing thresholds in individuals unable to provide behavioral responses. Bone conduction (BC) testing differentiates conductive from sensorineural hearing loss. Accurate BC ASSR threshold estimation relies on correction values, which are not yet well established. The reliability of BC ASSR thresholds to estimate hearing thresholds at 500, 1,000, 2,000, and 4,000 Hz is evaluated.
METHODS: A systematic search was conducted to identify studies involving normal-hearing (NH) and hearing-impaired (HI) participants of all ag

BACKGROUND: Early Hearing Detection and Intervention programmes support early identification and management of hearing loss. However, limited data exist on post-diagnostic pathways, amplification uptake, and continuity of care within public-sector paediatric services.
OBJECTIVES: To describe management pathways and outcomes of children diagnosed with hearing loss following newborn and infant hearing screening (NIHS) at a tertiary paediatric hospital in the Western Cape.
METHOD: A retrospective descriptive cohort study was conducted at the Red Cross War Memorial Children's Hospital. Records o

Congenital external and middle ear malformations (CEMEMs) represent a key etiology of conductive hearing loss, arising from dysplastic development of the first and second pharyngeal arches during embryogenesis. These malformations may occur in isolation or as a localized manifestation of a syndrome. This review synthesizes recent evidence-based medical findings to elaborate on advances in understanding the pathogenic mechanisms, clinical characteristics, and auditory intervention strategies for CEMEMs, thereby offering a reference framework for clinical diagnosis and management.

Isolated congenital ossicular chain anomalies (COCAs) are an uncommon cause of conductive hearing loss in patients with a normal external auditory canal and tympanic membrane, and familial clustering is rarely described. We report three siblings with non-progressive conductive hearing loss and normal otoscopic findings. Two siblings exhibited bilateral involvement, whereas the third had unilateral disease with a normal contralateral ear. High-resolution temporal bone computed tomography suggested incudostapedial (I-S) discontinuity caused by defects in the long process of the incus, without ex

Cholesteatomas are keratinizing cystic lesions of the temporal bone, most commonly involving the middle ear and mastoid. Intracranial extension is rare and usually associated with acquired disease, while giant supratentorial presentations are exceptional. We report a well-documented and neuroradiologically illustrated case of an exceptionally large congenital mastoid cholesteatoma with supratentorial intracranial extension. A 49-year-old woman presented with left parietal bone softening and recent ipsilateral facial sensory disturbance. MRI revealed a giant left temporoparietooccipital extra-a

OBJECTIVES: Acquired external auditory canal (EAC) stenosis or atresia is commonly associated with chronic inflammation and postsurgical changes, whereas cases resulting from trauma are understudied. Previous reports have included heterogeneous cases, limiting prognostic clarity. We investigated audiometric and surgical outcomes in a homogeneous cohort of patients with post-traumatic EAC stenosis.
METHODS: We retrospectively reviewed patients treated for posttraumatic EAC stenosis at a tertiary referral center between March 2012 and April 2025. All patients underwent modified meatocanaloplast

BACKGROUND: Revision middle ear surgery for persistent conductive hearing loss after previous tympanoplasty or ossiculoplasty is technically demanding because of scar tissue, anatomical distortion, and the unpredictable status of the ossicular chain. Although transcanal endoscopic ear surgery (TEES) has been increasingly adopted for primary tympanoplasty and ossiculoplasty, its feasibility as the sole surgical modality in postoperative ears has rarely been reported.
CASE PRESENTATION: We describe two adult patients (Case 1: a 63-year-old man and Case 2: a 72-year-old man) who presented with l

Acquired external auditory canal stenosis is rare, but leads to chronic otorrhea and conductive hearing loss that can become disabling. Surgery relies on canaloplasty associated to skin coverage to limit the high risk of recurrence. We describe a standardized retroauricular canaloplasty technique associated to retroauricular thin skin graft. The accompanying video (Supplementary Material) shows the key steps in fundus dissection, reaming, grafting and immobilization. This reproducible approach aims to optimize healing, limit the risk of anterior angle blunting and improve long-term functional

OBJECTIVE: Otitis media and conductive hearing loss are common in patients with primary ciliary dyskinesia (PCD). We primarily aimed to assess the impact of otologic features of individuals with PCD on health-related quality of life (QOL), and our secondary aims are characterization of the coexisting pulmonary and sinonasal findings.
METHODS: Demographic data, spirometry values, a health-related quality of life questionnaire for PCD patients (QOL-PCD), physical examination data of sinonasal and otologic systems, the sinonasal outcome test-22, and audiologic tests were recorded in this cross-s
