Middle Ear Cholesteatoma in a Postmortem Temporal Bone: A Case Highlighting Variation in Regional Inflammation.

Cholesteatoma is an abnormal accumulation of keratinizing squamous epithelium within the middle ear that can behave in a locally destructive manner and erode the ossicular chain. Chronic inflammation and Eustachian tube dysfunction is thought to drive this process. We present a temporal bone specimen from a 38-year-old woman with a lifelong history of right-sided otorrhea and conductive hearing loss, who died of hypoxic-ischemic brain injury after cardiac arrest. Histologic examination revealed a cholesteatoma eroding the incus, with marked regional variation in perimatrix inflammation ranging

Pediatric Jugular Bulb Anomalies: Management Insights From Two Cases.

Jugular bulb anomalies are rare but important causes of pediatric conductive hearing loss. These variants can disrupt ossicular mechanics or alter middle ear function and may persist despite tympanostomy tube placement. We report two cases: a five-year-old boy with unilateral conductive hearing loss due to high-riding bulb, managed conservatively, and an eight-year-old boy with persistent conductive hearing loss post-tympanostomy, in whom intraoperative hemorrhage occurred during middle ear exploration due to jugular bulb dehiscence.

Assessing hearing and hearing loss in mice with open-source system and auditory brainstem response (OpenSABR).

The auditory brainstem response (ABR) is a measure of the neurophysiological response to sound, widely used in clinical and research settings. Here, we describe OpenSABR ("Open-source System for ABR"), an open-source system for measuring the ABR that is more flexible and about 50 times cheaper than closed-source alternatives. The heart of our system is the commercially available Texas Instruments ADS1299 chip for multichannel differential recording. We designed auxiliary hardware and software to record and visualize the ABR. To demonstrate high-quality recording, we measured the response in mi

Otologic outcomes in infants and children with bronchopulmonary dysplasia.

OBJECTIVE: Hearing outcomes among children with bronchopulmonary dysplasia (BPD) have not been well characterized across disease severity, and the impact of neighborhood-level opportunity on otologic outcomes remains unclear. We aimed to determine the prevalence of hearing loss (HL), middle ear pathology, and surgical management among children with BPD stratified by disease severity, and to evaluate the contribution of the Childhood Opportunity Index (COI) to otologic-related disparities. METHODS: We performed a single-institution, retrospective cohort study of children aged 0-8 years admitte

Novel TCOF1 Frameshift Variant and Phenotypic Heterogeneity in a Chinese Family With Treacher Collins Syndrome.

BACKGROUND: Treacher Collins syndrome (TCS) is a congenital craniofacial disorder characterized by malar and mandibular hypoplasia, downward-slanting palpebral fissures, and conductive hearing loss. Pathogenic variants in TCOF1 account for most cases, with POLR1D, POLR1C, and POLR1B also implicated. METHODS: Whole-exome sequencing was performed in a two-generation Chinese family with TCS, followed by Sanger sequencing validation. Clinical features were systematically evaluated, and bioinformatic analyses combined with structural modeling were employed to assess the potential pathogenicity of

Maternal health, antiretroviral therapy exposure, and hearing outcomes in children born to women living with HIV: A descriptive analysis from South Africa.

BACKGROUND: South Africa's high HIV prevalence presents unique challenges, including potential impacts on hearing function in children born to women with HIV. While antiretroviral therapy (ART) has significantly reduced vertical transmission of HIV, the combined influence of maternal health conditions and in utero HIV and ART exposure on childhood hearing outcomes remains poorly understood. Evidence regarding the contribution of maternal ART exposure and comorbidities to auditory outcomes among children who are HIV-exposed uninfected remains limited, particularly in African settings. AIM: Thi

The Effect of Bisphosphonate Therapy on Hearing and Middle Ear Dysfunction in Pediatric Osteogenesis Imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a heterogeneous inherited connective tissue disorder characterized by impaired bone matrix formation. Hearing loss is observed in 28 to 65 percent of patients with OI. Most patients with OI experience conductive hearing loss (CHL); however, mixed and sensorineural (SNHL) hearing loss has also been described. PURPOSE: The primary aim of this retrospective study was to characterize the rates of middle ear dysfunction (MED) and hearing loss (HL) with pediatric OI and investigate the efficacy of bisphosphonate therapy in HL. RESEARCH DESIGN: Retrospect

Notched Cartilage With Incus Interposition Ossiculoplasty Combined With Cavity Obliteration: A Novel Approach: Analysis of Hearing Results in 50 Cases.

BACKGROUND: In chronic suppurative ear disease, conductive hearing loss with ossicular chain erosion, poses a serious challenge to the otologist. To overcome this hurdle, various ossiculoplasty techniques have been developed to date. Little modifications in the ossicular reconstruction technique, combined with cavity obliteration in selected cases, can yield predictably good results. OBJECTIVE: A) To study the hearing outcome in 50 patients of Austin group A ossicular defect, using a piece of notched autologous conchal cartilage over the stapes suprastructure, combined with incus interpositio

Clinical characteristics of asymmetric hearing loss in children from a large pediatric cohort.

BACKGROUND: Asymmetric hearing loss (HL) is a common pediatric condition, characterized by differences in HL severity between both ears. Anamnestic and audiologic profiles of affected children are sparse, hindering development of evidence-based clinical guidelines. METHODS: Clinical characteristics for children with bi- and unilateral asymmetric HL were retrospectively obtained from a register of children with permanent HL in Germany. Cohorts were stratified by laterality (uni-/bilateral) and presence of comorbidities (with/without). Between-group differences were assessed using absolute stan

Multimodal imaging in conductive hearing loss: Optimising CT, MRI and CTA for accurate diagnosis and management.

UNLABELLED: Conductive hearing loss (CHL) results from impaired mechanical transmission of sound through the external or middle ear and is commonly associated with conditions such as otosclerosis, cholesteatoma, ossicular discontinuity, congenital malformations and trauma. Less frequently, it may arise from vascular or neoplastic lesions. Imaging plays a central role in the evaluation of CHL, with high-resolution CT (HRCT) regarded as the gold standard for initial assessment because of its superior spatial resolution in detecting bony pathologies, including fenestral otosclerosis, ossicular ch
