Heterozygous Nonsense Mutation in the Nuclear Transport Factor KPNA7, a Maternal Factor Active in Embryonic Tissues, Causes Autosomal Dominant Otosclerosis.

Otosclerosis is a common cause of conductive hearing loss thought to result from dysregulated bone remodeling in the embryonic tissues of the globuli interossei. Both familial and sporadic cases have been reported. To date, 10 published OTSC loci and four genes (FOXL1 (OTSC11), SMARCA4 (OTSC12), MEPE, SERPINF1) have been identified in autosomal dominant families. Using a combined genetic and genomics approach in five affected siblings, we identified a nonsense mutation in Karyopherin subunit α7 (KPNA7, c.49C>T, p.R17X), the newest of the importin-α family of nuclear transporters. KPNA7 is a ke

Conductive hearing loss and normal CT: a clinical perspective.

PURPOSE: This article presents clinical scenarios in which patients with conductive hearing loss may not have an explanatory finding on temporal bone CT. METHODS: A literature review regarding patients with conductive hearing loss and no attributable imaging findings was performed. Illustrative examples correlating audiological exams and photon counting detector CT scans are provided. Common explanations of conductive hearing loss, e.g.: otosclerosis, are reviewed. CONCLUSION: Patients with conductive hearing loss are assumed to have attributable imaging findings on temporal bone CT. While t

Endoscopic stapes surgery for otosclerosis: clinical, radiological, and audiological outcomes in a retrospective series of 40 patients.

BACKGROUND: Otosclerosis is a primary osteodystrophy of the otic capsule, characterized by abnormal bone remodeling, leading to stapes fixation and progressive conductive or mixed hearing loss. Although microscopic stapes surgery remains the historical standard, endoscopic ear surgery has gained increasing interest due to improved visualization of middle ear anatomy and its minimally invasive transcanal approach. This study aimed to evaluate the clinical, radiological, and audiological outcomes of exclusive endoscopic stapes surgery in a retrospective cohort of patients with otosclerosis. MET

A case report of ceruminous adenocarcinoma of the external auditory canal.

Ceruminous adenocarcinoma (CAC) represents an exceedingly rare malignant glandular neoplasm originating from the ceruminous glands within the external auditory canal (EAC), constituting approximately 5%-10% of all EAC malignancies. Owing to its insidious onset and nonspecific symptoms, diagnosis is frequently delayed. This report delineates a case of right EAC CAC in a 39-year-old male, focusing on the diagnostic trajectory, multimodal therapy, and short-term outcomes. The patient presented with a one-month history of recurrent bloody purulent discharge from the right ear, intermittent otalgia

Primary external auditory canal cholesteatoma in a young adult: Diagnostic challenges and surgical management of a rare otologic entity.

External auditory canal cholesteatoma is a rare and potentially destructive otologic condition characterized by the accumulation of keratinizing squamous epithelium within the external auditory canal, leading to progressive bony erosion. Despite being described in the 19th century, EACC remains an uncommon and frequently underrecognized entity, often misdiagnosed as keratosis obturans or chronic otitis externa. Its pathogenesis is multifactorial and incompletely understood, involving impaired epithelial migration, chronic microtrauma, local inflammation, and ischemic changes of the canal skin.

The Efficacy of Acupuncture and Bee Venom Injections in Improving Hearing in a Patient with Idiopathic Conductive Hearing Loss: a case report.

Conductive hearing loss (CHL) is commonly caused by tympanic membrane perforation or otitis media. Congenital ossicular anomalies and otosclerosis are the primary considerations in patients with normal tympanic membranes and external auditory canals, whereas stapedius tendon abnormalities are less frequently observed. Acupuncture and bee venom (BV) injections are frequently used for treating otological disorders; however, their effectiveness in treating CHL remains unexplored. Therefore, this case report describes a 22-year-old man with idiopathic left-sided CHL that persisted for 101 days. Wh
