Determinants of Speech Perception Outcomes After Hearing Aid Fitting in Conductive and Sensorineural Hearing Loss: A Prospective Longitudinal Observational Study.

Background/Objectives: Hearing loss is a leading cause of disability worldwide, with speech perception representing a key functional outcome of auditory rehabilitation. While hearing aids improve audibility, outcomes vary substantially across clinical subgroups. This study aimed to compare speech perception outcomes after hearing aid fitting in adults with conductive and sensorineural hearing loss and to identify determinants of variability in rehabilitation outcomes. Methods: This prospective longitudinal observational study included 250 adults with clinically confirmed bilateral conductive o

Exploring the therapeutic response to sodium-glucose cotransporter-2 inhibition in INSR-related insulin resistance monogenic diabetes: case-based analysis and narrative review.

BACKGROUND: INSR-related insulin resistance is a form of monogenic diabetes caused by pathogenic variants in the INSR gene, resulting in impaired insulin receptor signalling and a spectrum of insulin resistance. Conventional diabetes management strategies relying on insulin sensitisation or augmentation are often less effective and evidence to guide treatment in those with heterozygous INSR mutations remains limited. INDEX CASE ANALYSIS: A 63-year-old male diagnosed with type 2 diabetes aged 40 presented with a relatively stable fasting glucose and disproportionate postprandial hyperglycaemia

A Diagnostic Challenge of Facial Nerve Palsy and Hearing Loss With Bilateral Otomastoiditis in an Elderly Patient.

Facial nerve palsy in older adults is commonly idiopathic or viral in origin, but secondary otologic and neurological causes must be carefully excluded, particularly when associated with hearing loss. We report a diagnostically challenging case of a 79-year-old woman who presented with acute right lower motor neuron facial palsy and new-onset hearing impairment, without otalgia, rash, or otorrhoea. Examination confirmed severe right-sided facial weakness, and initial imaging with computed tomography (CT) of the head and temporal bones demonstrated bilateral mastoid and middle ear opacification

Investigations on Directional Hearing With One-Sided Fitting of an Active Middle Ear Implant or Bone Conduction Hearing Implant.

OBJECTIVE: In patients with conductive or combined unilateral hearing loss, implantable hearing systems can be a treatment option. Due to the overlapping indications of hearing implants, a systematic evaluation of audiologic differences in terms of speech intelligibility and binaural hearing abilities is necessary. Because of the unilateral cochlear stimulation in patients implanted with an active middle ear implant, we expect superior binaural hearing performance compared with patients implanted with a bone-conducting implant that causes bilateral cochlear stimulation. This study focuses espe

Vibrant Soundbridge Round Window Vibroplasty: Safety, Coupling Efficiency, and Speech Outcome of the Most Common Coupling Modalities.

UNLABELLED: <p>Introduction: The floating mass transducer (FMT) of the Vibrant Soundbridge (VSB) is successfully used for reverse stimulation of the round window membrane (RWM) since 2005, enabling the treatment of conductive and mixed hearing loss. The FMT can be placed directly to the RWM or indirectly by using an interposed material such as fascia and Tutopatch, or a coupler, e.g., the round window coupler (RWC), the round window soft (RWS) coupler, or the custom-made Hannover coupler version 2 (HCV2). The aim of the study was to retrospectively evaluate the long-term outcome of the most co

Audiological assessment using click auditory brainstem response and chirp auditory steady-state response in pediatric patients with congenital microtia under 36 months: A single-center experience.

OBJECTIVE: To investigate the audiological characteristics of congenital microtia (CM), and explores the effectiveness of click auditory brainstem responses (c-ABR) and chirp auditory steady-state responses (chirp-ASSR) in assessing hearing loss in affected children. METHODS: Audiological assessments were performed using c-ABR and chirp-ASSR in infants and toddlers under 36 months with CM who were unable to cooperate for behavioral testing. We used c-ABR to assess the nature of hearing loss. Spearman coefficients were used to evaluate correlations, and differences were assessed by subtracting

Audiometric Trends Among Children With Congenital Aural Atresia or External Auditory Canal Stenosis.

OBJECTIVES: In external auditory canal stenosis associated with microtia, it is unknown if the degree of conductive hearing loss changes over time and how this compares to children with complete aural atresia. The objectives of this study are to (1) compare audiometric patterns between children with external auditory canal stenosis and congenital aural atresia and (2) describe the natural progression of conductive hearing loss in children with external auditory canal stenosis. We hypothesize that stenosis patients will experience hearing improvement over time and will have unique audiometric p

"An Unusual Case of Bilateral Sudden Mixed Hearing Loss with Complete Remission": A Case Report and Pathophysiological Considerations.

Background: Sudden-onset bilateral mixed hearing loss in adults is an extremely rare condition but challenging to diagnose and treat. Conductive hearing loss is associated with otitis media, while the simultaneous presence of a sensorineural component requires supplementary investigation for possible shared pathophysiological mechanisms. Case Presentation: We report the case of a 41-year-old male who was admitted to our hospital with a 48 h history of bilateral, fast progressive hearing loss following a viral illness. The audiologic testing revealed bilateral severe mixed hearing loss. Tympano

A Novel Clinical Feature in NOG Gene Mutation-Associated Syndrome.

Introduction: Noggin encoding (NOG) gene plays a critical role in early embryogenesis and development of bones, joints, cartilage, eyes, and neural tissue. The NOG gene encodes the noggin protein. Noggin is the only secreted inhibitor of bone morphogenetic protein (BMP) that is associated with abnormal phenotypes in humans. The most commonly observed manifestations of NOG gene mutations include bilateral conductive hearing loss, proximal symphalangism, broad thumbs, hyperopia, and a distinct facial appearance. This genetic disorder was first reported in 1990 by Teunissen and Cremers. Since the
