Recurrent Cholesteatoma Presenting as Severe Otalgia and Dizziness.

Residual cholesteatoma is a known potential pitfall of surgical resection. While most patients with suspected residual disease undergo second-look surgery or magnetic resonance imaging (MRI) for surveillance, the very long-term consequences of subclinical residual cholesteatoma are rarely described. We present the case of a 64-year-old male who presented with severe otalgia and dizziness 45 years after his last otologic surgery. Subsequent examination and imaging revealed a giant cholesteatoma with significant intracranial extension, developed over nearly half a century.

Endoscopic assisted minimally invasive resection of petrous bone epidermoid cyst via a presigmoid burr hole approach: illustrative case.

BACKGROUND: Intracranial epidermoid cysts are rare congenital lesions accounting for approximately 1% of intracranial tumors. Petrous apex involvement is uncommon and surgically challenging because of the lesion's proximity to critical neurovascular structures. Reports describing minimally invasive endoscope-assisted approaches in this location remain limited. OBSERVATIONS: A 32-year-old man presented with progressive right-sided otalgia, pulsatile tinnitus, and hearing loss. Imaging demonstrated a destructive petrous temporal bone lesion with atypical diffusion characteristics, initially sus

Otitic Barotrauma Due to Eustachian Tube Dysfunction and Special Considerations in At-Risk Populations.

Otitic barotrauma results from failure to equalize pressure changes across gas-filled ear spaces, most commonly due to Eustachian tube dysfunction. It is highly prevalent among divers and aircrew, with middle ear barotrauma being the most frequent presentation. Symptoms include otalgia, hearing loss, tinnitus, fullness, vertigo, and tympanic membrane injury. Inner ear barotrauma is less common but potentially severe, especially after forceful equalization. Management depends on severity and includes prevention, conservative treatment, otolaryngologic assessment, and, in selected recurrent baro

Skull Base Osteomyelitis Complicated by Internal Carotid Artery Thrombosis and Sigmoid Sinus Thrombosis.

Skull base osteomyelitis (SBO) is a rare and potentially fatal deep-space infection most commonly arising from malignant external otitis in immunocompromised or diabetic patients. Vascular complications, including septic thrombosis of the internal carotid artery (ICA) and dural venous sinuses, are among the most severe and life-threatening sequelae, yet remain underreported in the radiological literature. We present the case of a 59-year-old male with type 2 diabetes mellitus who initially presented with severe, refractory left-sided otalgia, purulent otorrhea, and progressive lower cranial ne

Primary Diffuse Large B-Cell Lymphoma of the External Auditory Canal with Extensive Local Extension: A Case Report and Literature Review.

INTRODUCTION: Lymphoma is the second most frequent type of malignancy in the head and neck region, following squamous cell carcinoma. However, the external auditory canal (EAC) is an extremely rare primary site, with only 13 cases reported worldwide. CASE REPORT: We present a case of middle-aged woman presented with a four-month history of swelling at the right EAC, reduced hearing, and intermittent otalgia. Over the following two months, she developed swelling at the right preauricular and cervical regions. Radiological evaluation revealed an enhancing mass at the right EAC with extensive lo

Retropharyngeal Internal Carotid Artery Presenting as Chronic Otalgia and Throat Pain Mimicking Laryngopharyngeal Reflux: A Case Report.

Retropharyngeal displacement of the internal carotid artery (ICA) is an uncommon anatomical variant with significant procedural safety implications. The vessel deviates medially into the retropharyngeal space, approaching or contacting the posterior pharyngeal mucosa, placing it at immediate risk of injury during routine pharyngeal instrumentation. We report a 54-year-old woman with type 2 diabetes mellitus, hypertension, and hyperlipidemia who experienced a three-year diagnostic delay before this anomaly was identified. Her presentation of unilateral chronic otalgia and burning throat discomf

Primary or secondary otalgia-uncovering the mystery.

We describe two cases illustrating the diagnostic challenges of referred otalgia when otoscopic findings are normal. The first patient presented with a 5-month history of severe right ear pain unresponsive to prior treatment. Despite a normal otoscopic examination and a previously normal CT scan, MRI revealed extensive skull base inflammation. Subsequent surgical sampling confirmed Aspergillus, and the patient improved with targeted antifungal therapy. The second patient presented with acute left otalgia, dysphagia and tongue deviation. Otoscopy and CT imaging were unremarkable, but MRI demons

In vivo and In Silico Assessment of Acampe papillosa (Lindl.) Lindl. Seed Extract: A Multi-Pharmacological Approach.

Acampe papillosa (Lindl.) Lindl., an epiphytic orchid, has been traditionally utilized in ethnomedicine for the management of pyrexia, otalgia, rheumatism, and neuralgia. This work intends to figure out the in vivo neuropharmacological, antinociceptive, and antipyretic potentials of the methanolic extract of A. papillosa seeds (ME-APS) along with computational approaches. Phytochemical screening of ME-APS was analyzed through GC-MS. Elevated plus maze (EPM) and hole board (HBT), forced swim (FST), and tail suspension (TST) tests were used to evaluate anxiolytic and antidepressant effects, wher

Spontaneous hemorrhage of cavernous malformation in the region of the cochlear nucleus: A case report.

Cerebral cavernous malformations (CMs) are low-flow, thin-walled vascular lesions affecting approximately 0.5%-0.9% of the population; they rarely arise at the skull base, and involvement of the cerebellopontine angle (CPA) is exceptionally uncommon, with only 19-21 cases reported and no prior description of cochlear nucleus involvement. We report a previously healthy 33-year-old woman who presented acutely with right-sided temporal headache, severe vertigo, sensorineural hearing loss, otalgia, and facial weakness. Multisequential MRI, including susceptibility-weighted imaging (SWI), demonstra

Laryngeal Alveolar Rhabdomyosarcoma: An Exceedingly Rare Entity.

Alveolar rhabdomyosarcoma (ARMS) of the larynx is an exceedingly rare malignancy with only 3 cases previously documented. This fourth case was a 54-year-old female patient who presented with progressive dyspnea, dysphonia, dysphagia, and otalgia. Positron emission tomography/computed tomography imaging revealed a 2.8 × 2.3 × 1.9 cm fluorodeoxyglucose-avid supraglottic mass centered in the left aryepiglottic fold with mild regional lymph node uptake. Histopathologic examination demonstrated nests of small round tumor cells with alveolar-like spaces, staining positive for myogenin, desmin, smoot

External auditory canal cholesteatoma presenting with facial nerve palsy: rare clinical entity.

INTRODUCTION AND IMPORTANCE: External auditory canal cholesteatoma (EACC) is a rare, non-neoplastic lesion representing 0.1%-0.5% of otologic cases. Despite its rarity, EACC can be highly destructive, similar to middle ear cholesteatoma. Owing to its proximity to vital structures such as the facial nerve, severe complications, including facial paralysis, may occur, although such presentations remain exceptionally uncommon and scarcely reported. CASE PRESENTATION: We report the case of a 45-year-old male patient who presented to our ENT department with right-sided facial paralysis. Otoscopic e

Eleven-Year Diagnostic Delay in Episodic Cluster Headache With a Characteristic International Classification of Headache Disorders, 3rd Edition (ICHD-3) Clinical Pattern: A Case of Migraine Misdiagnosis and Otologic Confusion.

Cluster headache is a primary headache disorder characterized by recurrent attacks of severe unilateral headache accompanied by ipsilateral cranial autonomic symptoms. Despite well-established diagnostic criteria, it remains one of the most frequently misdiagnosed primary headache disorders, resulting in substantial diagnostic delay. We report a diagnostic dilemma involving a 31-year-old man with episodic cluster headache who remained undiagnosed for 11 years despite a clinical presentation highly consistent with the International Classification of Headache Disorders, 3rd edition (ICHD-3) crit

Ear Pain: Diagnosing Common and Uncommon Causes.

Otalgia, or ear pain, is a common reason for primary care visits. It is categorized as primary (ie, originating from the ear) or secondary (ie, referred from nonotologic sources). A systematic approach guides appropriate management and identifies red flags. Acute otitis media, otitis externa, and eustachian tube dysfunction are common causes of primary otalgia. Temporomandibular joint disorders, dental pathology, and cervical spine pathology are common causes of secondary otalgia. Evaluation should include a focused history and physical examination, with further assessment for concerning or pe

Bilateral tympanic membrane perforation during bronchoscopy in an intensive care unit patient: a case report.

BACKGROUND: Bronchoscopy is a fundamental and generally safe interventional procedure in respiratory medicine, with recognized complications being predominantly confined to the airways. Traumatic tympanic membrane perforation, while a common form of barotrauma, is exceptionally rarely associated with bronchoscopy. This report describes a case of bilateral tympanic membrane perforation complicating bronchoscopy, aiming to enhance clinicians'awareness of this uncommon iatrogenic injury. CASE PRESENTATION: A 77-year-old man presented with cough and shortness of breath. Emergency investigations r

Metastatic Pancreatic Cancer Diagnosed From a Lesion in the External Auditory Canal.

Metastases to the external auditory canal (EAC) are extremely rare and most often arise from breast, renal or lung carcinoma. To our knowledge, this is the first case of metastatic pancreatic adenocarcinoma to the EAC. A 34-year-old male patient, a smoker, presented with a 15-month history of right otalgia, otorrhoea, and hearing loss. Examination revealed a large, fleshy, and friable EAC lesion suspicious for neoplastic aetiology. Biopsy reported poorly differentiated adenocarcinoma. Subsequent imaging revealed a large pancreatic head mass, confirming a diagnosis of metastatic pancreatic aden

Patent foramen of Huschke causing persistent otorrhoea.

Patent foramen of Huschke is an uncommon condition that may lead to otological symptoms. A woman in her 60s presented with left-sided clear watery otorrhoea while eating. Despite multiple consultations, diagnosis was delayed. Examination in our clinic revealed a bead of clear discharge emerging from the anterior canal wall with jaw movement. A temporal bone scan confirmed a lesion bridging the canal with the left temporomandibular joint and bulging through a dehiscence in the anterior canal wall. She was diagnosed with patent foramen of Huschke. We report the surgical management of this case,
