BACKGROUND: Skull base osteomyelitis (SBO) is an uncommon but life-threatening complication of necrotising otitis externa, typically affecting older adults with diabetes. Presentation is often non-specific, and diagnosis is frequently delayed.
CASE: An 84-year-old man with type 1 diabetes, vascular comorbidity and advanced frailty presented with a fall preceded by dizziness on standing. He reported a 6-month history of recurrent falls and severe postural light-headedness; persistent right-sided otalgia and offensive otorrhoea had been present for the preceding 3 months, indicating that the ea

Tympanic membrane perforation is a common condition with varying aetiology and clinical presentation. In this review, diagnosis, prognosis and management are outlined. Many perforations heal spontaneously, especially small traumatic ones without infection, while others may cause hearing loss and recurrent otorrhoea. Prognosis depends on size, aetiology and infection. Surgical treatment may be considered in cases with persistent symptoms.

Osteosarcoma of the head and neck is an uncommon malignancy with heterogeneous clinical and radiological presentations, often leading to diagnostic challenges. Its occurrence during pregnancy further complicates management due to the need to balance maternal and fetal outcomes. We report the case of a 23-year-old pregnant woman who initially presented with right-sided otalgia, managed as otitis externa. She subsequently developed a rapidly progressive, highly vascular mass protruding through the external auditory canal, associated with conductive hearing loss and vestibular symptoms. Magnetic

AIM: To determine the frequency and nature of referrals for children with acute or chronic otorrhoea from primary care to secondary care ENT services in the UK.
BACKGROUND: Middle ear infections in children are common; if the ear drum bursts discharge leaks out (otorrhoea). This causes hearing loss during a critical developmental period. Managing these children in an appropriate time frame to prevent disease repercussions is vital. There is currently no evidence demonstrating referral patterns and management strategies across primary and secondary care services.
METHOD: Children with otorrho

Cholesteatoma is a rare otologic manifestation in patients with split-hand/foot malformation (SHFM), and previously reported cases have predominantly focused on congenital auricular malformations and external auditory canal atresia rather than acquired middle ear lesions. We herein describe the case of an 8-year-old girl with clinically diagnosed SHFM who presented with left otalgia and persistent purulent otorrhoea. Otoscopic examination revealed middle ear granulation tissue, and high-resolution temporal bone imaging confirmed the presence of middle ear cholesteatoma complicated by tympanic-

Glomangioma (true glomus tumour) is a benign perivascular neoplasm that rarely arises in the head or neck. A man in his 70s presented with long-standing, unilateral, serous otorrhoea, progressive hearing loss and intermittent tinnitus. Otoscopy revealed a soft-tissue mass completely occluding the external auditory canal. Contrast-enhanced CT demonstrated an enhancing lesion filling the external auditory canal with extension into the middle ear, attic, mastoid antrum and tympanic opening of the Eustachian tube without bony erosion. The patient underwent canal-wall-down mastoidectomy with gross

Osteomas of the temporal bone are rare benign tumors that, in a small minority of cases, may give rise to secondary cholesteatoma. We report a case in which two independent pathological mechanisms appeared to coexist, resulting in a configuration not previously described in the literature. A 51-year-old woman presented with one year of right-sided otorrhoea and progressive mixed hearing loss unresponsive to antibiotic therapy. High-resolution computed tomography (HRCT) demonstrated a giant sessile osteoma arising from the anterior mastoid cortex with complete external auditory canal (EAC) obst

Otorrhagia following mandibular trauma may appear straightforward; however, underlying external auditory canal (EAC) fractures and associated condylar injuries can be easily overlooked without targeted evaluation. We present the case of an 88-year-old man who developed bilateral ear bleeding and hearing loss after falling and striking his chin. Computed tomography revealed bilateral mandibular condyle fractures with associated fractures of the anterior walls of both EACs. The canals were filled with hematoma, and hearing loss was presumed to result from mechanical obstruction. Otoscopic examin

BACKGROUND Extracorporeal membrane oxygenation (ECMO) serves as a life-sustaining intervention for pediatric patients with severe, reversible respiratory or circulatory failure refractory to conventional therapies, providing temporary cardiopulmonary support to maintain oxygenation, perfusion, and hemodynamic stability. CASE REPORT Following a family argument, a 9-year-old boy fell from the seventh floor, with partial obstruction by a canopy. He presented with immediate loss of consciousness, absent voluntary motor activity, Kussmaul respirations, otorrhagia, and epistaxis. Initial computed to

Traumatic intrusion of the mandibular condyle into the middle cranial fossa is an extremely rare and life-threatening condition. It often lacks specific clinical manifestations, particularly in patients with polytrauma, which typically leads to oversight or misdiagnosis. Although many individuals present without obvious neurological deficits, associated craniofacial injuries such as concussion, intracranial hemorrhage, cerebrospinal fluid leakage, otorrhagia, hearing impairment, and facial nerve dysfunction are commonly observed. Most published cases have involved condylar fractures; however,

Middle ear adenoma, also known as middle ear adenomatous neuroendocrine tumors (MEANTs), is a rare neoplasm, accounting for < 2% of all middle ear tumors. It is characterized by slow growth, non-invasive behavior, and nonspecific symptoms, which often complicate preoperative diagnosis. This review summarizes our clinical experience in the diagnosis and treatment of 11 cases of MEANT. In this cohort, four cases were preoperatively diagnosed as cholesterol granuloma, five as cholesteatoma, and two as middle ear tumors. The median age at diagnosis was 36.7 years, with all patients (100%) experien

Perioperative otorrhagia is a rare complication of laparoscopic and robotic pelvic surgery performed in Trendelenburg position with pneumoperitoneum. We report two cases occurring during robotic colorectal resection performed in 25-degree Trendelenburg with 12 mmHg pneumoperitoneum. A 74 year old woman undergoing robotic sigmoid resection developed left sided otorrhagia intraoperatively, with computed tomography and otolaryngologic examination demonstrating blood in the external auditory canal and tympanic membrane injury. A 63 year old man undergoing robotic low anterior resection developed b

Internal carotid artery rupture in the middle ear is very rare and can be life-threatening. We report the case of a 78-year-old male with a history of chronic otitis media who presented with spontaneous, high-volume otorrhagia. Computed tomography imaging revealed a dissection of the petrous segment of the internal carotid artery with an associated pseudoaneurysm. As the rupture of the pseudoaneurysm was considered the cause of otorrhagia, endovascular stenting was performed. After hospital admission for monitoring and antibiotic treatment, the patient underwent subtotal petrosectomy, with no

Granulomatosis with polyangiitis (GPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper and lower respiratory tracts and the kidneys. Although otolaryngological manifestations are common, isolated otologic symptoms as the initial presentation are rare and may delay diagnosis. We report the case of a 65-year-old man who initially presented with refractory bilateral otitis media associated with progressive mixed hearing loss, vestibular dysfunction, and subsequent peripheral facial nerve palsy. Initial temporal bone imaging was unrem

RATIONALE: Anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV) is a group of small-vessel necrotizing vasculitides with minimal immune complex deposition. Hypertrophic cranial pachymeningitis, a rare complication of AAV, is characterized by focal/diffuse dural thickening and fibrosis, causing neurological dysfunction such as headache and optic nerve injury. While AAV-associated hypertrophic cranial pachymeningitis is well documented, reports of concurrent spinal dural involvement remain scarce. This case highlights the rare co-occurrence of cranial and spinal dural involvement in

Patulous Eustachian tube (PET) is a condition characterized by abnormally persistent patency of the Eustachian tube, resulting in symptoms such as autophony, aural fullness, and otalgia, with weight loss increasingly reported as a potential risk factor. This review aimed to evaluate the association between bariatric surgery and PET. PubMed, Google Scholar, and Scopus were searched from inception to August 2026 for studies addressing PET, bariatric surgery, weight loss, and Eustachian tube dysfunction. Available evidence, predominantly from small observational studies and case reports, demonstr
