OBJECTIVE: Malignant otitis externa (MOE) is a potentially life-threatening infection of the external auditory canal (EAC), often with skull base extension. Vascular complications are exceptionally rare, with only six reports of petrous internal carotid artery (ICA) pseudoaneurysm following MOE.
METHODS: We present the first reported case of a nasopharyngeal hematoma as a complication of MOE.
RESULTS: An 82-year-old male with poorly controlled diabetes presented with 2 months of recurrent left ear otalgia, purulent otorrhea, and facial nerve palsy. He was commenced on 6 weeks of antimicrobia

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr

Malignant external otitis (MEO), also known as necrotizing otitis externa, is an aggressive infection of the external auditory canal and skull base, typically affecting elderly diabetic or immunocompromised patients. We present a case of an 88-year-old non-diabetic woman diagnosed with MEO whose management was complicated by a severe allergic reaction to systemic antibiotic therapy. The patient initially presented with severe otalgia and purulent otorrhea of the right ear, unresponsive to conventional treatment. Otoscopic examination revealed granulation tissue at the osseocartilaginous juncti

Necrotizing otitis externa (NOE) is a severe and potentially life-threatening infection of the external auditory canal, particularly affecting elderly diabetic or immunocompromised patients. Pseudomonas aeruginosa is the most frequently isolated pathogen. Among fungi, Aspergillus and Candida spp. are the most commonly implicated. Nevertheless, Scedosporium sp. has also emerged as a notable pathogen in NOE. We report the case of a 67-year-old woman with poorly controlled diabetes who presented with persistent right-sided otorrhea and otalgia, complicated by facial paralysis. Imaging revealed bo

Acute otitis media (AOM) is a common infection, whereas peripheral facial nerve palsy is an uncommon extracranial complication in the post-antibiotic era. We report the case of a 53-year-old immunocompetent man who developed acute suppurative otitis media complicated by ipsilateral peripheral facial nerve palsy following an upper respiratory tract infection. The illness began with sore throat and cough during Hajj and progressed to severe right-sided otalgia, followed by complete right lower motor neuron facial weakness despite sequential empirical oral antibiotics. On return to Pakistan, otos

INTRODUCTION: Neuroglial heterotopia (NGH), is a displaced mass of mature central neuroepithelial tissue unconnected to the brain proper. NGH presents mostly with respiratory distress, neck mass and feeding difficulties at early age.
CASE REPORT: In our case, the nasopharyngeal NGH, a nine years old female child presented with otalgia and reduced hearing of left ear. Otological examination showed secretory otitis media. On nasal endoscopy solitary, smooth, mucosa covered mass on the eustachian tube opening of left side blocking eustachian tube orifice. Imaging showed a well-defined, non-enhan

BACKGROUND: Primary adenocarcinoma of the base of the tongue is an exceptionally rare malignancy, accounting for only a small proportion of oropharyngeal tumors. Diagnosis is challenging due to nonspecific symptoms and overlap with more common squamous cell carcinomas and salivary-type neoplasms.
CASE PRESENTATION: We report the case of a 59-year-old Moroccan woman presenting with progressive dysphagia and referred otalgia. Nasofibroscopy revealed a right-sided base of the tongue mass extending to the vallecula. Magnetic resonance imaging confirmed a 28 × 19 mm lesion with ipsilateral cervica

Tonsilloliths or tonsil stones are singular or multiple calculi found in tonsillar crypts that are located unilaterally or bilaterally. A review report determined the exact location of the tonsillolith, showing that in 69.7% of cases they are located in the tonsillar tissue, 21.2% in the tonsillar fossa, and 9% in the soft palate. Our research aims to document a case of unilateral tonsillolith that we removed under general anesthesia. His orthopantomography revealed a large radiopacity with respect to the anterior border of the left ramus and the angle area, measuring about 23.1 mm × 21.5 mm ×

Facial nerve palsy in older adults is commonly idiopathic or viral in origin, but secondary otologic and neurological causes must be carefully excluded, particularly when associated with hearing loss. We report a diagnostically challenging case of a 79-year-old woman who presented with acute right lower motor neuron facial palsy and new-onset hearing impairment, without otalgia, rash, or otorrhoea. Examination confirmed severe right-sided facial weakness, and initial imaging with computed tomography (CT) of the head and temporal bones demonstrated bilateral mastoid and middle ear opacification

BACKGROUND: Ramsay Hunt syndrome (RHS) typically presents with unilateral otalgia, herpes zoster oticus, ipsilateral peripheral facial palsy, and often ipsilateral vestibulocochlear involvement. Bilateral/contralateral vestibular dysfunction is extremely rare.
CASE PRESENTATION: A 60-year-old female with a history of right breast cancer presented to our clinic with 1 week of vertigo, followed by 2 days of right-sided facial deviation and otalgia. Physical examination revealed vesicular eruptions around the right ear and external auditory canal, right-sided peripheral facial palsy, spontaneous

INTRODUCTION: Ramsay Hunt syndrome (RHS) results from reactivation of latent varicella-zoster virus in the geniculate ganglion, typically presenting with otalgia, auricular vesicles, and ipsilateral facial palsy. While systemic immunosuppression is a recognized trigger, RHS following local corticosteroid injection is exceedingly rare.
CASE REPORT: We report a 55-year-old woman who developed classical RHS 4 days after ultrasound-guided bilateral intra-articular triamcinolone injections for knee osteoarthritis. She received early oral acyclovir and achieved full recovery within 6 months.

We report the first documented case of facial-stapedial synkinesis (FSS) secondary to Ramsay Hunt syndrome (RHS) by describing the clinical manifestations and definitive surgical treatment outcome of this rare condition. A 35-year-old female presented with initial symptoms of severe right-sided facial weakness, otalgia, hyperacusis, ear fullness, and erythematous vesicles in the external auditory canal and tympanic membrane, suggestive of RHS. She later developed an intermittent "quivering" or "buzzing" sensation in her right ear initiated by facial movements such as closing her eyes and smili

Ramsay Hunt syndrome, caused by varicella-zoster virus reactivation, presents with facial paralysis, otalgia, and auricular vesicles. A Tanzanian woman with Grade VI facial palsy was treated with acyclovir and prednisolone, recovering to Grade II within 2 weeks. This illustrates dual therapy beyond the 72-h window may remain amenable to treatment.

Within the aerodigestive tract, syphilis presents with varied signs and symptoms, some of which may mimic malignancy or other disease processes. We present the case of a 29-year-old man with otalgia and odynophagia who was found on physical examination and office laryngoscopy to have a 4-cm ulcerative mass of the tongue base and cervical lymphadenopathy. While planning for tissue diagnosis, the patient's serologies revealed positive treponemal antibody testing and elevated rapid plasma reagin (RPR) titers, confirming syphilitic infection, likely representing secondary syphilis. While awaiting

BACKGROUND: The third generation of bisphosphonates is clinically in use for patients of osteoporosis or malignancy-linked hypercalcemia. The agents can also produce anti-tumor effects on bone metastasis of several types of tumors. We recently found that one of the agents achieved cytotoxicity to mesothelioma in vitro and in an orthotopic animal model. Mesothelioma is resistant to a number of chemotherapeutic agents, and suppression of local tumor growth is beneficial to the patients since metastasis to extra-thoracic organs is relatively infrequent until a late stage.
METHODS/DESIGN: We demo

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common and growing problem and a leading cause of child referrals to Child and Adult Mental Health Services (CAMHS). It is a drain on resources across nationally funded support agencies and associated with negative outcomes such as early criminality, school disruption and antisocial behaviour. Mainstream interventions (pharmacological and behavioural) demonstrate effectiveness whilst implemented, but are costly, often have unwanted side effects and do not appear to be affecting long-term outcomes.Development of a robust evidence

UNLABELLED: Primary CNS lymphoma (PCNSL) is a rare subtype of non-Hodgkin lymphoma with the worst outcomes amongst all extranodal lymphomas. There is a scarcity of data on real-world outcomes of primary CNS lymphoma (PCNSL) owing to the rarity of the disease. This study analyzed the demographic patterns, risk stratification, treatment regimens used, & outcomes of patients treated at Tata Memorial Center Mumbai, India. This is a retrospective analysis of newly diagnosed primary CNS lymphoma patients treated at our centre over seven years from January 2013 to December 2019. A total of 142 patien

INTRODUCTION: Avascular necrosis of the femoral head is common in routine orthopedic clinics. The challenge arises in managing early stages (I and II) without obvious radiological evidence. Authors explore this naïve research area by comparing surgical procedures in early AVN patients.
MATERIALS AND METHODS: A prospective multicentric study was performed from November 2020 to February 2023 on 82 patients treated with surgical decompression and adjuvants, concerning the defined inclusion and exclusion criteria. Radiopacity and intraosseous edema resolution and THA conversion rates were assesse

OBJECTIVE: Middle ear neuroendocrine tumors (MeNETs) are rare lesions that may mimic more common middle ear pathology. Although typically indolent, they have potential for local invasion and delayed recurrence. This study aims to expand understanding of MeNETs by characterizing their presentation, management, histopathology, and long-term outcomes.
STUDY DESIGN: Retrospective case series.
SETTING: Two tertiary, university-affiliated medical centers.
METHODS: Five patients with histologically confirmed MeNETs were identified through retrospective chart review. Data collected included demogra
