Fatal bacterial CNS infections: a systematic review and a forensic case report.

Central nervous system (CNS) infections may cause sudden unexpected death when diagnosis is delayed or missed. We report a fatal case of Streptococcus pyogenes meningoencephalitis and systematically review postmortem investigations of fatal bacterial CNS infections. A 57-year-old woman was found dead at home after untreated otalgia, hearing loss, purulent otorrhea, low-grade fever, asthenia, and vomiting. Autopsy showed diffuse cerebral edema, subdural empyema involving the posterior fossa, and cerebellar tonsillar herniation. Histology demonstrated acute suppurative meningoencephalitis with v

Anthropogenic tritium as a continental-scale tracer in river-derived recharge.

BACKGROUND: Environmental hazards like floods, droughts and landslides pose serious public health and wellbeing consequences for populations, especially vulnerable communities in countries with low adaptive capacity. Empirical evidence from peer-reviewed literature and official climate change assessments indicates that, alongside high interannual climate variability, Uganda has experienced observable climate change over the past 50 years. This changing and increasingly variable climate poses challenges to population health in diverse ways, including through the transmission of infectious disea

COVID-19 is considered a respiratory disease which has many symptoms associated with the larynx and the lungs infections. COVID-19 has wide spectrum of clinical features starting from mild symptoms to severe illness. Otolaryngological symptoms as nasal obstruction, loss of smell, taste dysfunction, sore throat, sticky mucus, and dysphagia are common in COVID-19 patients. Other vocal symptoms as dysphonia and phonesthenia are common in COVID-19 patients. The aim of this study is to detect the occurrence of vocal symptoms in COVID-19 patients in Egypt and to investigate the videolaryngoscopic fi

Nasal biopsy in ANCA-associated vasculitis: a game changer or a false friend?

Sigmoid sinus and transverse sinus thrombosis may occur as intracranial complications of chronic otitis media. Central venous sinus thrombosis typically presents with picket fence fever along with otalgia, otorrhea, and altered mental status. CT and MRI are investigations of choice for diagnosis. Once diagnosed, one should be started on empiric antibiotics. The use of anticoagulants has been debatable. From a surgical point of view, the current trend is to perform a mastoidectomy with the removal of inflammatory tissue from the sinus walls.

Human otoacariasis by Amblyomma testudinarium identified with 16S rRNA gene sequencing.

Human otoacariasis caused by tick bites in the external auditory canal is rarely reported in Japan, despite the recent increase in the movement of wild animals, known hosts of ticks into human living areas due to global warming. We report a case of human otoacariasis caused by Amblyomma testudinarium, genetically identified using 16S rRNA gene sequencing. A 43-year-old man presented to an urban hospital in Tokyo with left-sided otalgia. An endoscopic examination revealed a tick anchored to the skin of the external auditory canal, which showed erythema and swelling. Initial attempts to remove t

Posterior circulation hypoperfusion plays an important role in refractory otalgia: case report.

This case explored the association among posterior circulation hypoperfusion, cryptogenic vertebral artery dissection (CVAD), and idiopathic intractable otalgia. A 67-year-old male patient, afflicted with recurrent right otalgia for over 50 years and misdiagnosed with otitis and trigeminal neuralgia, demonstrated posterior circulation hypoperfusion on cerebral CT perfusion imaging. Conventional vascular tests turned up nothing, yet dynamic contrast-enhanced CT (DCE-CT) disclosed a long-segment CVAD with endothelial flaps within the intracranial vertebral artery, precisely matching the hypoperf

Hidden Peril in the Ear: A Case of External Auditory Canal Cholesteatoma in a Teenager.

External auditory canal cholesteatoma (EACC) is a rare otologic condition characterized by the accumulation of keratinized material in the external auditory canal (EAC), leading to progressive bony erosion. While typically associated with middle ear disease, isolated EACC in young patients is uncommon and often misdiagnosed. We report the case of a 17-year-old male who presented with left-sided otalgia, purulent otorrhea, and progressive hearing loss over 3 months. Otoscopic examination revealed a firm, yellowish mass occluding the left EAC. High-resolution computed tomography demonstrated a s

[A rare cause of otitis externa : Manifestation of myeloid sarcoma in the petrous bone].

A 45-year-old male patient presented with pronounced swelling of the external auditory canal and symptoms of otitis externa which did not respond to conservative treatment. Due to concomitant mastoiditis, mastoidectomy was performed in the later course, which did not provide conclusive histological results. In the absence of improvement, biopsy of the external auditory canal and mastoid revision with repeated pathological analysis were carried out. Ultimately, a myeloid sarcoma representing a relapse of previously remitted acute myeloid leukemia (AML) was identified.

A case report of ceruminous adenocarcinoma of the external auditory canal.

Ceruminous adenocarcinoma (CAC) represents an exceedingly rare malignant glandular neoplasm originating from the ceruminous glands within the external auditory canal (EAC), constituting approximately 5%-10% of all EAC malignancies. Owing to its insidious onset and nonspecific symptoms, diagnosis is frequently delayed. This report delineates a case of right EAC CAC in a 39-year-old male, focusing on the diagnostic trajectory, multimodal therapy, and short-term outcomes. The patient presented with a one-month history of recurrent bloody purulent discharge from the right ear, intermittent otalgia

Digital Otoscopy in the Pediatric Emergency Department: Can It Limit Repeat Ear Exams?

BACKGROUND/OBJECTIVES: Otoscopy is a fundamental yet challenging skill for pediatric trainees. Otoscopy can be uncomfortable for children, particularly if repeat examinations are required. Digital otoscopy (DO) allows clinicians to record and review otoscopic images. This study evaluated whether trainees' use of DO could reduce repeat examinations by supervisors, and its perceived educational value to trainees. METHODS: We conducted a prospective observational study in a pediatric emergency department in New York City. Pediatric, emergency medicine, and pediatric nurse practitioner trainees u

A case of adenoid cystic carcinoma of the larynx: Diagnostic challenges, surgical management, and long-term surveillance.

Adenoid cystic carcinoma of the larynx is an uncommon malignant neoplasm, accounting for less than 1% of all laryngeal cancers. It arises from minor salivary glands, predominantly located in the subglottic region. ACC is characterized by slow but aggressive growth, with a notable propensity for perineural invasion and distant hematogenous dissemination, particularly to the lungs, which occurs in approximately 35%-50% of cases. Clinical presentation depends on tumor localization and may include dyspnea, dysphonia, dysphagia, and otalgia. Diagnosis is based on clinical and endoscopic examination

Greater Auricular Nerve Blocks Resulted in Reduced Ear Pain and Noise Sensation.

This case report demonstrates a patient with pain located around the posterior left auricular area and lateral upper neck, with "roaring" noise in the ear. The patient underwent a series of 4 ultrasound-guided greater auricular nerve blocks, which provided significant and lasting reduction of both pain and noise, highlighting the role of the greater auricular nerve in both otalgia and tinnitus and further supporting the use of greater auricular nerve blocks in the treatment of these symptoms.

Minimally Invasive Skull Base Approach Styloidectomy for Treatment of Eagle Syndrome.

INTRODUCTION: Eagle syndrome is an underrecognized condition caused by the elongation of the styloid process or calcification of the stylohyoid ligament, with an unknown true incidence. It is diagnosed clinically through symptoms related to compression of the surrounding neurovascular and musculoskeletal structures, along with radiographic confirmation of styloid elongation. Symptoms are variable and may include neck pain, dysphagia, tinnitus, dizziness, headaches, otalgia, and cognitive complaints. Surgical management via intraoral or transcervical approaches is the current standard of care.
