Split Hand/Foot Malformation with Acquired Middle Ear Cholesteatoma Secondary to Eustachian Tube Dysfunction: A Case Report and Literature Review.

Cholesteatoma is a rare otologic manifestation in patients with split-hand/foot malformation (SHFM), and previously reported cases have predominantly focused on congenital auricular malformations and external auditory canal atresia rather than acquired middle ear lesions. We herein describe the case of an 8-year-old girl with clinically diagnosed SHFM who presented with left otalgia and persistent purulent otorrhoea. Otoscopic examination revealed middle ear granulation tissue, and high-resolution temporal bone imaging confirmed the presence of middle ear cholesteatoma complicated by tympanic-

Clinical experience with the diagnosis and treatment of middle ear adenomatous neuroendocrine tumors.

Middle ear adenoma, also known as middle ear adenomatous neuroendocrine tumors (MEANTs), is a rare neoplasm, accounting for < 2% of all middle ear tumors. It is characterized by slow growth, non-invasive behavior, and nonspecific symptoms, which often complicate preoperative diagnosis. This review summarizes our clinical experience in the diagnosis and treatment of 11 cases of MEANT. In this cohort, four cases were preoperatively diagnosed as cholesterol granuloma, five as cholesteatoma, and two as middle ear tumors. The median age at diagnosis was 36.7 years, with all patients (100%) experien

Otologic Manifestations and Peripheral Facial Nerve Palsy As the Initial Presentation of Granulomatosis With Polyangiitis: A Case Report.

Granulomatosis with polyangiitis (GPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper and lower respiratory tracts and the kidneys. Although otolaryngological manifestations are common, isolated otologic symptoms as the initial presentation are rare and may delay diagnosis. We report the case of a 65-year-old man who initially presented with refractory bilateral otitis media associated with progressive mixed hearing loss, vestibular dysfunction, and subsequent peripheral facial nerve palsy. Initial temporal bone imaging was unrem

ANCA-associated vasculitis with thoracic spinal canal dural involvement and hypertrophic cranial pachymeningitis: A case report.

RATIONALE: Anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV) is a group of small-vessel necrotizing vasculitides with minimal immune complex deposition. Hypertrophic cranial pachymeningitis, a rare complication of AAV, is characterized by focal/diffuse dural thickening and fibrosis, causing neurological dysfunction such as headache and optic nerve injury. While AAV-associated hypertrophic cranial pachymeningitis is well documented, reports of concurrent spinal dural involvement remain scarce. This case highlights the rare co-occurrence of cranial and spinal dural involvement in

Bariatric Surgery as a Potential Risk Factor for Patulous Eustachian Tube: A Comprehensive Literature Review.

Patulous Eustachian tube (PET) is a condition characterized by abnormally persistent patency of the Eustachian tube, resulting in symptoms such as autophony, aural fullness, and otalgia, with weight loss increasingly reported as a potential risk factor. This review aimed to evaluate the association between bariatric surgery and PET. PubMed, Google Scholar, and Scopus were searched from inception to August 2026 for studies addressing PET, bariatric surgery, weight loss, and Eustachian tube dysfunction. Available evidence, predominantly from small observational studies and case reports, demonstr

Association between stigma and treatment satisfaction in patients with type 2 diabetes: a cross-sectional study.

BACKGROUND: The new rexinoid bexarotene is a retinoid X receptor antagonist and immune response modifier. Although combinations of oral bexarotene and psoralen plus UVA (PUVA) have been tried in patients with all stages of mycosis fungoides (MF), the dosage of bexarotene used in these combination regimens has been variable. OBJECTIVE: To assess the efficacy and safety of low-dose oral bexarotene and PUVA in patients with relapsed or treatment-refractory MF following monotherapy with multiple agents including PUVA, narrow-band UVB, interferon-alpha, oral bexarotene, and topical corticosteroids

Malignant Otitis Externa Presenting With Nasopharyngeal Hematoma: A Case Report of an Unusual Complication.

OBJECTIVE: Malignant otitis externa (MOE) is a potentially life-threatening infection of the external auditory canal (EAC), often with skull base extension. Vascular complications are exceptionally rare, with only six reports of petrous internal carotid artery (ICA) pseudoaneurysm following MOE. METHODS: We present the first reported case of a nasopharyngeal hematoma as a complication of MOE. RESULTS: An 82-year-old male with poorly controlled diabetes presented with 2 months of recurrent left ear otalgia, purulent otorrhea, and facial nerve palsy. He was commenced on 6 weeks of antimicrobia

Intrapetrous Internal Carotid Artery Pseudoaneurysm Secondary to Malignant Otitis Externa: A Case Report.

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr

Malignant External Otitis Complicated by Severe Allergic Reaction to Antibiotic Therapy.

Malignant external otitis (MEO), also known as necrotizing otitis externa, is an aggressive infection of the external auditory canal and skull base, typically affecting elderly diabetic or immunocompromised patients. We present a case of an 88-year-old non-diabetic woman diagnosed with MEO whose management was complicated by a severe allergic reaction to systemic antibiotic therapy. The patient initially presented with severe otalgia and purulent otorrhea of the right ear, unresponsive to conventional treatment. Otoscopic examination revealed granulation tissue at the osseocartilaginous juncti

Necrotizing Otitis Externa Due to Scedosporium sp.: A Case Report.

Necrotizing otitis externa (NOE) is a severe and potentially life-threatening infection of the external auditory canal, particularly affecting elderly diabetic or immunocompromised patients. Pseudomonas aeruginosa is the most frequently isolated pathogen. Among fungi, Aspergillus and Candida spp. are the most commonly implicated. Nevertheless, Scedosporium sp. has also emerged as a notable pathogen in NOE. We report the case of a 67-year-old woman with poorly controlled diabetes who presented with persistent right-sided otorrhea and otalgia, complicated by facial paralysis. Imaging revealed bo

Acute otitis media complicated by peripheral facial nerve palsy in an adult: A case report.

Acute otitis media (AOM) is a common infection, whereas peripheral facial nerve palsy is an uncommon extracranial complication in the post-antibiotic era. We report the case of a 53-year-old immunocompetent man who developed acute suppurative otitis media complicated by ipsilateral peripheral facial nerve palsy following an upper respiratory tract infection. The illness began with sore throat and cough during Hajj and progressed to severe right-sided otalgia, followed by complete right lower motor neuron facial weakness despite sequential empirical oral antibiotics. On return to Pakistan, otos

Primary adenocarcinoma of the base of tongue: a case report and review of the literature.

BACKGROUND: Primary adenocarcinoma of the base of the tongue is an exceptionally rare malignancy, accounting for only a small proportion of oropharyngeal tumors. Diagnosis is challenging due to nonspecific symptoms and overlap with more common squamous cell carcinomas and salivary-type neoplasms. CASE PRESENTATION: We report the case of a 59-year-old Moroccan woman presenting with progressive dysphagia and referred otalgia. Nasofibroscopy revealed a right-sided base of the tongue mass extending to the vallecula. Magnetic resonance imaging confirmed a 28 × 19 mm lesion with ipsilateral cervica

A large unilateral tonsillolith.

Tonsilloliths or tonsil stones are singular or multiple calculi found in tonsillar crypts that are located unilaterally or bilaterally. A review report determined the exact location of the tonsillolith, showing that in 69.7% of cases they are located in the tonsillar tissue, 21.2% in the tonsillar fossa, and 9% in the soft palate. Our research aims to document a case of unilateral tonsillolith that we removed under general anesthesia. His orthopantomography revealed a large radiopacity with respect to the anterior border of the left ramus and the angle area, measuring about 23.1 mm × 21.5 mm ×

A Diagnostic Challenge of Facial Nerve Palsy and Hearing Loss With Bilateral Otomastoiditis in an Elderly Patient.

Facial nerve palsy in older adults is commonly idiopathic or viral in origin, but secondary otologic and neurological causes must be carefully excluded, particularly when associated with hearing loss. We report a diagnostically challenging case of a 79-year-old woman who presented with acute right lower motor neuron facial palsy and new-onset hearing impairment, without otalgia, rash, or otorrhoea. Examination confirmed severe right-sided facial weakness, and initial imaging with computed tomography (CT) of the head and temporal bones demonstrated bilateral mastoid and middle ear opacification

When Pain Relief Backfires: Ramsay Hunt Syndrome after Intra-articular Steroid Injection - A Rare Complication with 6-month Follow-up.

INTRODUCTION: Ramsay Hunt syndrome (RHS) results from reactivation of latent varicella-zoster virus in the geniculate ganglion, typically presenting with otalgia, auricular vesicles, and ipsilateral facial palsy. While systemic immunosuppression is a recognized trigger, RHS following local corticosteroid injection is exceedingly rare. CASE REPORT: We report a 55-year-old woman who developed classical RHS 4 days after ultrasound-guided bilateral intra-articular triamcinolone injections for knee osteoarthritis. She received early oral acyclovir and achieved full recovery within 6 months. CONCL
