Rapid Resolution of Grade VI Facial Nerve Paralysis in Ramsay Hunt Syndrome With Prompt Dual Antiviral-Corticosteroid Therapy: A Case Report.

Ramsay Hunt syndrome, caused by varicella-zoster virus reactivation, presents with facial paralysis, otalgia, and auricular vesicles. A Tanzanian woman with Grade VI facial palsy was treated with acyclovir and prednisolone, recovering to Grade II within 2 weeks. This illustrates dual therapy beyond the 72-h window may remain amenable to treatment.

Oropharyngeal Syphilis Presenting as Tongue Base Ulcer With Lymphadenopathy.

Within the aerodigestive tract, syphilis presents with varied signs and symptoms, some of which may mimic malignancy or other disease processes. We present the case of a 29-year-old man with otalgia and odynophagia who was found on physical examination and office laryngoscopy to have a 4-cm ulcerative mass of the tongue base and cervical lymphadenopathy. While planning for tissue diagnosis, the patient's serologies revealed positive treponemal antibody testing and elevated rapid plasma reagin (RPR) titers, confirming syphilitic infection, likely representing secondary syphilis. While awaiting

Disparity in Coronal Deformity Measurements: Standing Radiographs Versus Navigation in Varus Total Knee Arthroplasty.

BACKGROUND: The third generation of bisphosphonates is clinically in use for patients of osteoporosis or malignancy-linked hypercalcemia. The agents can also produce anti-tumor effects on bone metastasis of several types of tumors. We recently found that one of the agents achieved cytotoxicity to mesothelioma in vitro and in an orthotopic animal model. Mesothelioma is resistant to a number of chemotherapeutic agents, and suppression of local tumor growth is beneficial to the patients since metastasis to extra-thoracic organs is relatively infrequent until a late stage. METHODS/DESIGN: We demo

The authority and politics of epiphanic experience.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common and growing problem and a leading cause of child referrals to Child and Adult Mental Health Services (CAMHS). It is a drain on resources across nationally funded support agencies and associated with negative outcomes such as early criminality, school disruption and antisocial behaviour. Mainstream interventions (pharmacological and behavioural) demonstrate effectiveness whilst implemented, but are costly, often have unwanted side effects and do not appear to be affecting long-term outcomes.Development of a robust evidence

The fast and the studious? Ramadan observance and student performance.

INTRODUCTION: Avascular necrosis of the femoral head is common in routine orthopedic clinics. The challenge arises in managing early stages (I and II) without obvious radiological evidence. Authors explore this naïve research area by comparing surgical procedures in early AVN patients. MATERIALS AND METHODS: A prospective multicentric study was performed from November 2020 to February 2023 on 82 patients treated with surgical decompression and adjuvants, concerning the defined inclusion and exclusion criteria. Radiopacity and intraosseous edema resolution and THA conversion rates were assesse

Middle Ear Neuroendocrine Tumors: A Case Series Highlighting Diagnostic and Management Challenges.

OBJECTIVE: Middle ear neuroendocrine tumors (MeNETs) are rare lesions that may mimic more common middle ear pathology. Although typically indolent, they have potential for local invasion and delayed recurrence. This study aims to expand understanding of MeNETs by characterizing their presentation, management, histopathology, and long-term outcomes. STUDY DESIGN: Retrospective case series. SETTING: Two tertiary, university-affiliated medical centers. METHODS: Five patients with histologically confirmed MeNETs were identified through retrospective chart review. Data collected included demogra

Acute otitis media complicated by mastoiditis and otogenic transverse-sigmoid sinus thrombosis in an adult: a case report on infectious pathogenesis and multidisciplinary therapy.

BACKGROUND: Acute otitis media is usually a self-limited bacterial infection; however, in rare cases, contiguous spread to the mastoid and adjacent dural venous sinuses may result in life-threatening intracranial complications. Otogenic transverse-sigmoid sinus thrombosis is uncommon in adults and represents a diagnostic and therapeutic challenge because its early presentation may be nonspecific and evidence guiding management remains limited. CASE PRESENTATION: We report the case of a 32-year-old man with no relevant medical history who presented with a 3-week history of right-sided otorrhea

Two-handed endoscopic lateral temporal bone resection for squamous cell carcinoma of external auditory canal.

A woman in her 20s presented with a 3-month history of progressive right-sided otalgia. Otoendoscopic examination revealed a greyish mass occluding more than 75% of the external auditory canal. Biopsy confirmed well-differentiated squamous cell carcinoma. High-resolution CT demonstrated cortical erosions of the anterior and posterior bony external auditory canal with mastoid involvement. The patient underwent two-handed endoscopic lateral temporal bone resection using the Justtach ASM2 endoscope holder, combined with superficial parotidectomy and selective neck dissection. Histopathological ex

Diagnosis and Management of Middle Ear Neuroendocrine Tumour (MeNET).

Middle ear neuroendocrine tumours (MeNETs) are rare epithelial neoplasms with neuroendocrine differentiation that pose significant diagnostic and therapeutic challenges. The clinical presentation of MeNETs is often nonspecific and can mimic other middle ear pathologies, such as chronic otitis media, cholesteatoma, or paraganglioma Common symptoms include conductive hearing loss, otalgia, intermittent or persistent tinnitus, ear fullness, and dizziness. We present five patients who underwent surgery in our University Otolaryngology Centre between 2019 and 2025, in whom histopathological examina

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Analysis of Short-term Outcomes of Pancreatic Resections from a Low Volume Centre in a Tier II City in India.

BACKGROUND AND OBJECTIVE: Advanced age, co-morbid diseases, functional dependence and frailty are associated with polypharmacy and overall high health expenditures. Polypharmacy is commonly defined as the concomitant ingestion of four or more medications, particularly in community-dwelling patients, but the number of drugs being taken by institutionalised and hospitalised patients may be as high as ten. The aim of this study was to compare drug use in newly admitted patients (AP) to a geriatric medical centre for long-term care (LTC) with that of institutionalised patients (IP) at the centre.

Hypothetical criteria and types for cochlear migraine.

Cochlear migraine (CM) and cochleovestibular migraine were first reported in 2018. However, the diagnostic criteria and types of CM were still undefined. We proposed a hypothetical criteria for CM as below: A. At least 2 episodes with cochlear symptoms with or without recovery. B. Current or previous history of migraine with or without aura according to the International Classification of Headache Disorders-3. C. One or more migraine features with at least 50% of the cochlear episodes. D. No current or previous history of vestibular symptoms. E. Not better accounted for by another cochlear or

Unusual Presentation of a Rare Disease: A Case Report of Fungal Necrotizing Otitis Media and Mastoiditis.

Acute necrotizing otitis media is a severe middle ear infection which causes necrosis of the tympanic cavity. A 54-year-old female was presented who suffered from diabetes mellitus and end-stage renal disease presenting with severe otalgia, initially thought to be necrotizing otitis externa. She rapidly progressed to total necrosis of the tympanic membrane. Further assessments confirmed a fungal disease caused by Candida tropicalis and Aspergillus flavus. Immediate antifungal therapy with amphotericin B and subsequent radical mastoidectomy managed to control the disease with no sign of necrosi

Skull Base Metastasis of Uterine Carcinosarcoma Mimicking Primary Carcinoma of the Middle Ear.

Uterine carcinosarcoma is a rare type of endometrial carcinoma with poor prognosis. A bone metastasis to the skull base is extremely scarce in uterine carcinosarcoma. A 54-year-old woman with uterine carcinosarcoma complained of right ear otorrhea and otalgia. 18 F-FDG PET/CT presented an increased FDG uptake at the right middle ear involving the mastoid process and external acoustic meatus. Primary carcinoma of the middle ear was considered according to clinical symptoms and PET manifestation. However, the skull base metastasis from uterine carcinosarcoma was pathologically confirmed. The rar

Determinants of Acute Otitis Media in Children: A Case-Control Study in West Java, Indonesia.

Background and Objectives: Acute Otitis Media (AOM) is a leading cause of morbidity in children, characterized by fever, otalgia, and hearing loss. If untreated, AOM may progress to chronic complications requiring surgical management. Globally, factors such as allergies, environmental tobacco smoke, and nutritional deficiencies are well-established risk factors, but in Indonesia, particularly rural areas like Bandung Regency, limited awareness and data exacerbate the burden of disease. Smoking prevalence and low immunization rates further increase risks for AOM. Materials and Methods: This cas

Nervus intermedius sectioning for the treatment of geniculate neuralgia.

OBJECTIVE: Geniculate neuralgia (GN) is a rare facial pain syndrome characterized by severe stabbing pain in the ear canal. The exact cause of GN remains unclear; however, it is thought that the nervus intermedius (NI) is the primary implicated structure. When medical management fails, surgical interventions are offered to treat the pain. This study aimed to present a consecutive series of patients with GN who underwent craniotomy with NI sectioning, to offer insights into the safety and efficacy of the surgical approach. METHODS: A retrospective review of consecutive institutional medical re
