An aneurysm of the petrous internal carotid artery.

Internal carotid artery aneurysms are a rare cause of pulsatile tinnitus and conductive hearing loss but should be borne in mind when there is a suspected diagnosis of glomus jugulare or high-riding jugular bulb. Most cases are congenital. We present a case of otorrhagia which was initially thought to be a glomus jugulare, the diagnosis of internal carotid artery aneurysm was made at angiography and treated by balloon embolization.

Management of head-injured patients in the emergency department: a practical protocol.

BACKGROUND: The management of head-injured patients admitted to emergency departments is not standardized. METHODS: The authors performed a retrospective analysis of 10,000 head-injured patients admitted to the Emergency Department of our hospital in a 21-month period and, on the basis of a statistical correlation between each clinical parameter (symptoms and signs upon arrival at the hospital or risk factors) and the presence of intracranial lesions, they propose a practical protocol in an attempt to avoid the overuse or radiologic examinations and yet identify patients with possible life-th

[Vascular anomalies presenting as middle ear mass].

We study several cases of patients who presented clinical symptoms indicating the presence of a middle ear mass (tinnitus, hearing loss, otorrhea, otorrhagia...). We had to rule out the presence of an expanding tumour (either in the form of a cholesteatoma or a glomus tumor) and the existence of vascular anomalies of the middle ear (enlarged jugular bulb or jugular megabulb deformity). Due to the possibility of a different treatment in each particular case (radiotherapy, radical surgery, jugular vein ligation or even no treatment at all), the relative value of various imaging techniques and/or

[A case of mycotic ICA petrous portion aneurysm treated with endovascular surgery].

A case is reported of mycotic aneurysm at the petrous portion of the internal carotid artery (ICA) treated with an endovascular procedure. A sixty-seven-year-old male who had suffered from chronic otitis media bilaterally for the last five years showed a sudden onset of massive left otorrhagia. Cerebral angiogram at the eighth day of otorrhagia disclosed a big aneurysm at the petrous portion of the left ICA. The aneurysm was round in shape but its wall was irregular. The angiogram also showed that a stenotic finding of the left ICA just at the proximal portion of the aneurysm and the neck of t

Spontaneous bilateral epistaxis and bilateral otorrhagia in a five year old with tympanostomy tubes.

Epistaxis and otorrhagia is seen in a variety of settings in the emergency department. We present a case of spontaneous bilateral epistaxis and bilateral otorrhagia in a five-year-old patient with tympanostomy tubes.

CT evaluation of otorrhagia associated with condylar fractures.

To assess a simplified CT evaluation of otorrhagia due to condylar fracture, 16 patients with post-traumatic otorrhagia underwent axial CT of the petrous bones and coronal CT of the temporo-mandibular joints (TMJs). Bilateral high condylar fractures were demonstrated in seven patients. Unilateral subcondylar fracture dislocations with contralateral high condylar fractures were demonstrated in the other nine patients. The otorrhagia was always ipsilateral to a high condylar fracture. Otorrhagia due to condylar fractures occurred when the fracture was bilateral and ipsilateral to a high condylar

Aneurysm arising from the petrous portion of the internal carotid artery: case report.

A 21-year-old man with hearing loss and recurrent otorrhagia was shown angiographically to have an aneurysm arising from the intrapetrous internal carotid artery (ICA). Endovascular balloon occlusion of the aneurysm with preservation of the parent ICA was attempted twice, but each time the balloon deflated and the aneurysm reappeared two weeks and three months, respectively, following the endovascular procedures. The patient was then successfully treated with a surgical trapping procedure combined with superficial temporal artery-middle cerebral artery bypass.

Ruptured petrous carotid aneurysm presenting with otorrhagia and epistaxis.

Aneurysm formation and rupture within the petrous internal carotid artery (ICA) is an extremely rare occurrence with approximately 10 such cases in the literature. Etiologies of petrous ICA aneurysms include atherosclerosis, closed head trauma, iatrogenic injury during mastoid surgery, chronic middle ear infections, and congenital causes. Therapeutic options include carotid artery ligation, aneurysm resection with or without reconstruction, and radiographically controlled vessel occlusion. The case of a patient who presented with otorrhagia, epistaxis, and transient focal neurologic signs due

Bilateral otorrhagia associated with continuous positive airway pressure.

A patient had bilateral tympanic membrane rupture and otorrhagia, an unusual complication of continuous positive airway pressure (CPAP). CPAP, applied by a bag/mask system using disposable spring valves, was used to treat acute pulmonary edema during volume resuscitation and vasopressin therapy for bleeding from esophageal varices.

Aneurysm of the ICA petrous segment treated by balloon entrapment after EC-IC bypass. Case report.

A 44-year-old man experienced the sudden onset of horizontal diplopia and hemifacial numbness. Arteriography demonstrated a left intrapetrous carotid artery aneurysm. The patient was successfully treated with a left superficial temporal artery to middle cerebral artery bypass followed by balloon entrapment of the aneurysm. There have been at least 40 previously reported cases of aneurysms of the petrous portion of the carotid artery. These aneurysms can be mycotic, traumatic, or developmental in origin. They can present with massive otorrhagia or epistaxis from acute rupture or with decreased

Pseudo aneurysm of the external carotid artery: report of a case.

The authors report a case of left otorrhagia in a 1-year-old male infant in the presence of a mass involving the parotideal and upper cervical regions, which had appeared after an infection of the upper airways. Non-invasive techniques, such as echography and CT scan, provided useful but contradicting information. Surgery allowed us to define the diagnosis of mycotic aneurysm of the external carotid artery. The authors, after pointing out the extreme rarity of such a pathology, discuss the ethiopathogenetic theories, the clinical features, the diagnosis and the surgical and medical treatment o

Bilateral otorrhagia and orbital hematoma complicating subgaleal hematoma.

A patient with bilateral otorrhagia and orbital hematoma following massive subgaleal hematoma is reported for the first time. The pathogenesis of the extensive subgaleal hematoma as well as the production of these unusual complications is discussed. This case would suggest that the weakest points in the attachment of the epicranial aponeurosis to the base of the calvarium are its attachments to the squamous part of the temporal bone and to the frontal bone under the eyebrow.

Recurrent massive otorrhagia caused by a petrous carotid aneurysm.

Aneurysms of the petrous portion of the internal carotid artery are quite rare lesions. The case of a 60-year-old man who showed recurrent spontaneous otorrhagia caused by a petrous carotid aneurysm of considerable size is reported here. Mycotic origin of the lesion is presumed. Common carotid ligation was effective in resolving bleeding and did not cause any permanent neurological deficit. Other surgical procedures are acceptable.

Acute spontaneous otorrhagia resulting from a ruptured petrous carotid aneurysm. Case report.

A 35-year-old man experienced sudden spontaneous arterial bleeding from his left ear. Arteriography demonstrated a left petrous carotid aneurysm. The bleeding was initially controlled with packing and ultimately by left common carotid ligation and transection.

'Otorrhagia'. A symptom of posterior auricular artery aneurysm.

There are many reports regarding aneurysms of the external carotid artery and its branches, but the case presented in this paper, of an aneurysm of the posterior auricular artery, is a unique one.
