Management of High Jugular Bulb in Retrosigmoid Surgery for Vestibular Schwannoma: Analysis of 590 Consecutive Cases.

BACKGROUND AND OBJECTIVES: High jugular bulb (HJB) is an obstacle when performing retrosigmoid intradural temporal drilling for resection of the meatal part of vestibular schwannomas. Some studies reported the technical nuance of the HJB management; however, the indications, impact on surgical outcomes, and long-term outcomes of this challenging management have not been fully analyzed. This study aimed to investigate the clinical and imaging characteristics of patients requiring the intraoperative HJB management, its impact on surgical outcomes, and long-term postoperative follow-up results.

Identification of blood-based inflammatory biomarkers for vestibular neuritis using a proximity extension assay.

ObjectiveThe present study aimed to identify blood-based inflammatory biomarkers that would help investigate the underlying pathogenesis in vestibular neuritis (VN) using a proximity extension assay (PEA).MethodsIn this single-center prospective study, we enrolled both a cross-sectional (acute VN vs normal controls) and a longitudinal (acute phase vs recovery phase) cohorts. We quantified 92 plasma inflammatory proteins using the Olink PEA and identified differentially expressed proteins (DEPs) in each cohort using t-tests. The clinical utility and predictive value of each DEP were evaluated u

Asymmetric Hearing Loss as a Population Screening Signal for Vestibular Schwannoma: A Number-Needed-To-Scan Analysis Using NHANES and SEER.

OBJECTIVE: Asymmetric sensorineural hearing loss (SNHL) is the principal audiometric trigger for magnetic resonance imaging (MRI) to exclude vestibular schwannoma (VS). Although multiple audiometric referral criteria exist, the population-level burden of applying them has not been quantified. To estimate how many adults in the general United States population would be flagged for MRI by each of the 5 recognized audiometric referral criteria per incident VS, and to place that burden in the context of established screening programs and of current clinically indicated imaging. METHODS: We pooled

Spontaneous Closure of an Intraosseous Jugular Tubercle Venous Complex Arteriovenous Fistula with a Single Venous Drainage Route Following Diagnostic Cerebral Angiography.

OBJECTIVE: Spontaneous closure of dural arteriovenous fistulas (AVFs) has been reported. However, complete spontaneous obliteration of a jugular tubercle venous complex (JTVC)-centered intraosseous AVF appears to be rare. We report a case of a JTVC-centered intraosseous AVF that spontaneously occluded after diagnostic cerebral angiography. CASE PRESENTATION: A 34-year-old man presented with a 6-month history of pulsatile tinnitus. MRA suggested an arteriovenous shunt in the JTVC. DSA revealed an intraosseous AVF with a shunt pouch located within the JTVC, supplied by multiple feeders. The les

Work Productivity Loss and Increased Recovery Needs in Working-Age Adults With Bilateral Severe Hearing Loss: Evidence From a Prospective Cohort Study.

OBJECTIVES: The impact of severe bilateral hearing loss on work productivity has not been fully explored. Given the communication difficulties experienced by this population, it has been hypothesized that work productivity may be affected, resulting in an economic burden. This study aimed to investigate work productivity among participants with severe acquired bilateral hearing loss. In addition, cognitive functioning was also examined to control for the possible intermediate effect on work productivity. DESIGN: This study presents observational data from 50 participants, aged between 18 and

Change in hearing loss and its association with general health in a community-based longitudinal study of middle-aged individuals: The Busselton healthy ageing study.

INTRODUCTION: With the ageing of the global population, disabling hearing loss is a major health concern. While age and sex are established determinants, the relation between general and environmental health factors and the progression of hearing loss remains incompletely understood, particularly in middle-aged adults. METHODS: We used data from the Busselton Healthy Ageing Study (BHAS), a population-based cohort in Western Australia. Adults aged 45-70 years at baseline (2010-2015) underwent audiometric testing and health assessments with a six-year follow-up. The better-ear four-frequency Pu

Auditory performance and cortical plasticity in blind individuals with sensorineural hearing loss: insights into dual sensory impairment.

BACKGROUND: Blind individuals with normal hearing frequently exhibit enhanced auditory abilities driven by compensatory cross-modal neuroplasticity. However, it remains unclear whether these compensatory advantages persist in individuals with dual sensory impairment. This study investigated auditory processing and cortical plasticity in early-blind, late-blind, and sighted individuals, all presenting with concurrent sensorineural hearing loss. METHOD: Participants completed auditory behavioral tests, including working memory, spectral ripple discrimination, temporal modulation detection, and

Clinical practice guidance for imaging selection in pulsatile tinnitus: an evidence-based expert consensus.

Pulsatile tinnitus (PT) is a rhythmic sound synchronized with the patient's heartbeat that represents a complex diagnostic challenge with a broad spectrum of vascular and non-vascular etiologies. Persistent symptoms can severely impact mental health, yet a specific causative abnormality can be identified via targeted radiological imaging in more than 75% of cases. Improper imaging selection frequently leads to misdiagnosis, reduced therapeutic efficacy, and increased healthcare costs. The choice of imaging protocol must be guided by a thorough initial clinical evaluation, including detailed me

Not All Transverse Sinus Stenoses Behave the Same: Distinct Flow Phenotypes on Phase-Contrast MRI.

Background Transverse sinus stenosis is usually assessed by its anatomical appearance, although stenoses with similar morphology may not have the same local flow behavior. This study evaluated peak velocity changes across transverse sinus stenoses using phase-contrast magnetic resonance imaging (MRI) and explored whether distinct hemodynamic patterns could be identified. Methods We retrospectively reviewed 183 consecutive patients who underwent brain MRI, magnetic resonance venography, and venous phase-contrast MRI for headache, vertigo, and/or tinnitus between September 2025 and June 2026. Combined Bioinformatic Analysis and RNA Sequencing Uncover Key Tinnitus-Related Molecules in the Cingulate Gyrus.

Tinnitus arises from maladaptive plasticity and abnormal synaptic function, not only in auditory pathways but also in limbic regions. Among these, the cingulate gyrus plays a pivotal part in shaping the emotional burden and sensory filtering disturbances often seen in tinnitus patients. Yet, the specific molecular players operating within this region remain poorly understood, and most existing bioinformatic predictions have not been tested in living systems. Here, we combined multi-database mining with in vivo transcriptomic profiling to identify and validate key proteins and pathways linked t

Effects of individualized phase-amplitude-coupled transcranial alternating current stimulation on tinnitus and cognition.

INTRODUCTION: Chronic tinnitus is a prevalent and disabling condition that is frequently accompanied by domain-specific cognitive difficulties. Abnormal neural oscillations and disrupted cross-frequency coupling (CFC) have been implicated in its pathophysiology. This study investigated whether individualized phase-amplitude-coupled transcranial alternating current stimulation (tACS), tailored to each participant's individual alpha frequency (IAF), was associated with changes in tinnitus symptoms, cognition, and neurophysiological measures. METHODS: In this prospective single-arm interventiona

Adjunctive Acorus tatarinowii essential-oil ear-canal packing for idiopathic sudden sensorineural hearing loss: study protocol for a single-center, randomized, double-blind, placebo-controlled clinical trial.

BACKGROUND: Idiopathic sudden sensorineural hearing loss (ISSNHL) is an acute otologic emergency, and hearing recovery remains highly variable and frequently unsatisfactory. Acorus tatarinowii Schott has long been used in traditional Chinese medicine to treat hearing impairment and related auditory disorders. Its essential oil (SCP-oil) has demonstrated anti-inflammatory, antioxidant, and neuroprotective activities in preclinical studies. Advances in local otologic drug-delivery strategies suggest that administration through the external auditory canal may offer a practical, noninvasive approa

The Relationship among Obesity, Intracranial Hypertension, and Cerebrospinal Fluid Leak.

Idiopathic intracranial hypertension (IIH) is a disorder characterized by elevated intracranial pressure without identifiable lesion or secondary cause. Patients can often present with otolaryngologic manifestations of IIH, such as pulsatile tinnitus, aural fullness, and cerebrospinal fluid (CSF) leak. Obesity plays an important role as a risk factor for both IIH and CSF leak as similar pathophysiological processes underly both presentations. Understanding the nuanced presentation of this condition is important for the otolaryngologist who routinely evaluates patients with these symptoms and h
