Speech, admittance and tone tests in school screening. Reconciling economics with pathology and disability perspectives.

In theory a criterion can be set on any test score to 'pass' or 'fail' a desired percentage of any population; but in practice, particular measures used in screening can have both intrinsically appropriate criteria and zones of great uncertainty in relation to the condition being screened. For example, the criteria for possible abnormality on oto-admittance measures in single-test screening for middle ear disease are intrinsically rather too sensitive, as potentially pathological signs are displayed by a high proportion of the population. In contrast, criteria on pure tone measures are intrins

Hearing loss and congenital rubella in Atlantic Canada.

There were 41 children born between 1973-1976 in Atlantic Canada who were victims of a severe rubella epidemic. This paper investigates prevalent audiometric patterns, incidence of middle ear disease, evidence regarding progressivity of hearing loss and any relationship between presence of middle ear disease and progressivity. Both surgical and audiological records were utilized in a combined retrospective-prospective study. Results indicate a typically flat audiogram for our group, with possible errors in the analysis of high frequency data accounting for differences between our center and th

Evaluation of the brain-stem function by the auditory brain-stem response and the caloric vestibular reaction in comatose patient.

The auditory brain-stem response (ABR) and the caloric vestibular reaction (CVR) were investigated in 100 patients in deep coma to evaluate the brain-stem function of these patients precisely and to predict their prognosis accurately. In the first ABR examination, 54 patients showed normal and 46 abnormal ABRs. Among the latter, five showed partial wave disappearance and 21 exhibited no ABRs. Twenty-six of the 30 patients who recovered (87%) showed normal ABRs and 21 of the 57 who died (37%) also exhibited normal latency at that time. On the other hand, all the patients who showed wave disappe

Multimodality evoked potentials and neurophysiological tests in multiple sclerosis. Effects of hyperthermia on test results.

Data on critical frequency of photic driving (CFPD), frequency following response (FFR), and visual, somatosensory (peroneal nerve), and brain-stem auditory evoked potentials (EPs) were obtained from 20 patients who had clinically definite multiple sclerosis and ten healthy normal subjects in a controlled, balanced study under normothermic and hyperthermic (+1 degrees C) conditions with a test-retest interval of one week. Normal subjects' test results showed no changes during hyperthermia. Patients' EP and CFPD data correlated well with history, clinical signs, and symptoms during both normoth

Comparison of the nephrotoxicity and auditory toxicity of tobramycin and amikacin.

A total of 157 patients were treated with tobramycin or amikacin in a controlled prospective randomized trial. Dosages were adjusted to renal function according to a nomogram. Trough and peak aminoglycoside levels were available at the end of the trial. Of the above total, 113 recipients of nine or more doses of tobramycin or six or more doses of amikacin, without other apparent cause of renal failure, were evaluated for nephrotoxicity. Thirty-six patients were evaluated for auditory toxicity. The patients in groups evaluated for either nephrotoxicity or auditory toxicity were similar with res

Clinical and audiological findings in a case of auditory agnosia.

A case is reported of severe agnosia for verbal and non-verbal sounds without associated aphasic disorder. A CT scan revealed bilateral, temporal lobe lesions from two ischaemic accidents that had occurred 9 months apart. The search for subtle deficits in the patient showed normal sensitivity to changes in the intensity and frequency of simple sounds; in contrast, his ability to discriminate sound duration and musical note sequences was severely impaired. The simultaneous recording of the whole auditory-evoked response pattern revealed no abnormality in the early components, which reflect the

Success and failure of evoked potentials in detecting clinical and subclinical lesions in multiple sclerosis patients.

The value of visual, brain stem auditory and somatosensory potentials in detecting clinical and subclinical lesions as compared to the routine neurological, ophthalmological and vestibular examinations was investigated in 100 M.S. patients. It would appear that the VEP and SEP are far superior to the routine techniques in demonstrating lesions. On the other hand, the BAEP is inferior to the clinical and vestibular test as an indicator of brain stem lesions. All clinically manifest posterior column lesions are associated with abnormal SEP. However a substantial proportion of clinically evident

Otologic referral criteria.

Otologic referral is an important component of an industrial hearing conservation program. Recent federal regulations in this area are reviewed, and suggestions for implementation of otologic referral are made. Criteria for otologic referral must be distinguished from a definition of significant threshold shift for internal program action. Test-retest variability is higher in industrial than in clinical settings, and unduly stringent definitions of significant threshold shift will result in many spuriously identified cases. The American Council of Otolaryngology--Head and Neck Surgery has prop

Speech acoustic-cue discrimination abilities of normally developing and language-impaired children.

The ability of 35 language impaired (LI) and 38 normally developing children between the ages of 5 and 8 1/2 yr to discriminate between speech stimuli, using an operantly conditioned, nonverbal motor response, was investigated. Six syllable contrasts ([ba] versus [da], [da] versus [ta], [epsilon] versus [ae], [dab] versus [daeb], [sa] versus [sta] and [sa] versus [integral a]) were selected to investigate discrimination of various temporal and spectral cues. Results demonstrated that the LI group made most errors discriminating syllables which were differentiated by consonants and fewest error

Hearing loss of Down syndrome adults.

Audiological hearing-test data were obtained for 51 Down syndrome adults. Depending upon hearing-loss criteria, from 51 to 74 percent of the subjects had some degree of hearing impairment. Correlation coefficients between these hearing-acuity measurements and receptive hearing vocabulary scores suggest a moderate relationship. These data indicate that Down syndrome adults are prone to a wide variety of hearing difficulties that may reflect a variety of ear pathologies. These data and other studies have shown that both Down syndrome adults and children need frequent hearing screening and, when

[Forward and backward Békésy audiometry in neurosensorial hearing loss (author's transl)].

Békésy audiometry was utilized to show that an abnormal separation of forward and reverse frequency tracing is to be referred to an abnormal fatigability in neurosensorial hearing loss (presbycusis and noise-induced hearing loss). The ascending and descending threshold with continuous tone was compared to fixed frequency per-stimulatory threshold that has been determinated using interrupted and continuous tone. During the study has been evaluated the gap between the continuous and interrupted tone and the excursion amplitude in interrupted and continuous tone. The results are than an abnormal

Brain stem auditory evoked potentials are related to interaural time discrimination in patients with multiple sclerosis.

In a combined psychophysical-electrophysiological study on 29 patients with multiple sclerosis (MS), a compromised ability to make interaural time discriminations was nearly always found to be associated with 'abnormal' brain stem potentials evoked by clicks to at least one ear. However, no obvious relationships were found between evoked brain stem potentials and several other auditory behavioral measures such as interaural intensity discrimination, pure-tone thresholds and speech discrimination.

[Disseminated sclerosis: diagnostic value of neurophysiological investigations (author's transl)].

Demyelinisation lesions of disseminated sclerosis actually present within the white matter are much more numerous than clinical manifestations would suggest that the majority of these lesions are clinically latent. Ophtalmologists were the first to seek a functional deficit reflecting the demyelinisation which so often affects the optic pathways. In practice, the recording of evoked visual potentials has proved to be the most reliable method. The brain stem is another region where demyelinisation lesions are frequent and quite often latent. Electronystagmography offers a highly sensitive means

Peripheral auditory processing in normal and abnormal ears: physiological considerations for attempts to compensate for auditory deficits by acoustic and electrical prostheses.

The processing of complex acoustic signals, such as speech, by the normal peripheral auditory system is described in terms of the properties of individual cochlear nerve fibres and the effects of lateral inhibition in the cochlear nucleus. The implications of the peripheral filtering of signals is indicated in psychophysical terms. In cochlear pathology, these filtering characteristics deteriorate, and the implications arising from the consequent deterioration in auditory frequency selectivity are outlined for the coding of loudness and for the analysis of speech. These considerations suggest

The use of intravenous lignocaine in the diagnosis and treatment of tinnitus.

The effect of intravenous lignocaine upon the symptom of tinnitus has been assessed in seventy-eight patients and the results correlated with associated hearing loss. Lignocaine appears to be highly effective in suppressing tinnitus in patients with presumed damage or degeneration of the Organ of Corti and is less effective in other groups. It is postulated that tinnitus in patients with Organ of Corti damage is the result of abnormal hyperactivity in the auditory pathway following deafferentation.

Etiological differences in patterns of psycholinguistic development of children of IQ 30 to 60.

We compared the psycholinguistic abilities of 131 mentally retarded children (IQs 30 to 60) of different etiological classifications (Down's syndrome, biologically brain damaged, environmentally caused retardation, unknown cause) and characterized each etiology according to the different patterns of psycholinguistic skills exhibited by the children. Level of skills was determined by the Illinois Test of Psycholinguistic Abilities, which was administered to each subject. Down's syndrome children exhibited significantly lower verbal-auditory skills than visual-motor skills. The environmentally c

A comparison of two response-reinforcement methods in pure-tone testing of the retarded.

The purpose of this study was to determine whether a visual reward method or play audiometry is a better motivator in eliciting the lowest possible pure-tone thresholds from a group of Down's syndrome adults. Thresholds were obtained at 250, 500, 2000, 4000, and 6000 Hz for both procedures using a modified ascending method. The method of testing was constructed such that each of the two procedures served as a comparison for validity of the other. The level of significant difference between the play audiometry and the visual reward method was determined using the Sign Test. In each case, there

On the comprehension of questions by nonspeaking children.

The question form presents both a semantic and linguistic problem to deaf and other language impaired children. The wh question requires a pronominal reference system to be well-established before the wh question can be comprehended and produced. The present study describes the language competence of a deaf, partially sighted girl who acquired language by reading it. This child neither hears nor speaks. Communication with her family was accomplished by writing in her hand, gestures, and pictures. The present study explores pronominal reference in relation to wh questions. A method of teaching
