BACKGROUND AND OBJECTIVES: High jugular bulb (HJB) is an obstacle when performing retrosigmoid intradural temporal drilling for resection of the meatal part of vestibular schwannomas. Some studies reported the technical nuance of the HJB management; however, the indications, impact on surgical outcomes, and long-term outcomes of this challenging management have not been fully analyzed. This study aimed to investigate the clinical and imaging characteristics of patients requiring the intraoperative HJB management, its impact on surgical outcomes, and long-term postoperative follow-up results.

ObjectiveThe present study aimed to identify blood-based inflammatory biomarkers that would help investigate the underlying pathogenesis in vestibular neuritis (VN) using a proximity extension assay (PEA).MethodsIn this single-center prospective study, we enrolled both a cross-sectional (acute VN vs normal controls) and a longitudinal (acute phase vs recovery phase) cohorts. We quantified 92 plasma inflammatory proteins using the Olink PEA and identified differentially expressed proteins (DEPs) in each cohort using t-tests. The clinical utility and predictive value of each DEP were evaluated u

OBJECTIVE: Asymmetric sensorineural hearing loss (SNHL) is the principal audiometric trigger for magnetic resonance imaging (MRI) to exclude vestibular schwannoma (VS). Although multiple audiometric referral criteria exist, the population-level burden of applying them has not been quantified. To estimate how many adults in the general United States population would be flagged for MRI by each of the 5 recognized audiometric referral criteria per incident VS, and to place that burden in the context of established screening programs and of current clinically indicated imaging.
METHODS: We pooled

OBJECTIVES: The incidence of thyroid cancer has been increasing annually, and surgery remains the primary therapeutic approach. Clinically, postoperative voice disorders may occur in patients undergoing thyroidectomy, even in the absence of intraoperative injury to the laryngeal nerves. This retrospective study aimed to objectively evaluate postoperative acoustic voice alterations after thyroidectomy using Praat software in patients with confirmed preservation of recurrent laryngeal nerve (RLN) and superior laryngeal nerve (SLN) function. The study focused on changes in acoustic parameters wit

OBJECTIVE: Tinnitus is the perception of sound without an external source. More than 50 million people in the United States have reported experiencing tinnitus, resulting in an estimated prevalence of 10% to 15% in adults. The focus of this guideline is on tinnitus that is both bothersome and persistent (lasting 6 months or longer), which often negatively affects the patient's quality of life. The target audience for the guideline is any clinician, including allied health professionals, involved in managing patients with tinnitus. The target patient population is limited to adults (18 years an

OBJECTIVE: While prior literature has demonstrated that patients of lower socioeconomic groups may have larger tumors on presentation and poorer outcomes, there is limited literature on the impact of race and ethnicity on management of vestibular schwannoma (VS). The objective of this study was to determine whether race and ethnicity impacted treatment choice and postoperative management of VS.
METHODS: Retrospective review of patients diagnosed with VS (ICD9 225.1, ICD10 D33.3, and ICD 10 H93.3) who presented to a tertiary care center between January 1, 2018 and October 31, 2023 was complete

BACKGROUND: Clinical management of vestibular schwannomas (VS) encompasses three main strategies: observation ("wait-and-scan"), stereotactic radiosurgery, and microsurgery. Despite extensive literature, a consensus on the optimal modality is lacking due to the scarcity of high-level evidence.
METHODS: This narrative review explores the current understanding of the most significant clinical and radiographic outcomes following wait-and-scan, radiosurgery, and microsurgery. This is not a systematic review of the literature.
RESULTS: Observational studies indicate that approximately one-third o

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment.

Autoimmune inner ear disease (AIED) is a spectrum of diseases that may present solely as fluctuating sensorineural hearing loss triggered by unknown stimuli, or it may occur in association with other systemic rheumatic diseases. This article will review primary AIED as well as the etiologies of secondary AIED. It will also describe the epidemiology, pathophysiology, diagnosis, and management, including corticosteroids and steroid-sparing treatments.

INTRODUCTION: Vestibular schwannoma (VS) typically induces gradual sensorineural hearing impairment and vestibular dysfunction. Nevertheless, clinically applicable non-invasive candidate imaging markers capable of reflecting pathological alterations within the inner ear have not yet been well established. The present study sought to systematically investigate the correlative relationships between labyrinth signal intensity ratios acquired from multi-sequence inner ear magnetic resonance imaging (MRI) and audiovestibular functional deficits in patients diagnosed with VS.
METHODS: A total of 22

Vestibular schwannoma represents a significant clinical challenge characterized by hearing loss, vestibular dysfunction, and cranial nerve morbidity. Despite advancements in surgery and radiotherapy, pharmacological options for recurrent, bilateral, or progressive disease remain limited. This review synthesizes current evidence on the molecular landscape of vestibular schwannoma, focusing on NF2 and Merlin inactivation as the upstream driver. We highlight how Merlin loss dysregulates interconnected signaling networks, specifically ErbB, PI3K Akt mTOR, MAPK, Hippo YAP TAZ, Wnt beta catenin, and

INTRODUCTION: The availability of a large number of cancer expression profiles presents an excellent opportunity to re-investigate various biological and clinical questions. While several expression profiles have been established for different cancers, merging them may provide a more powerful platform for extensively extrapolating molecular and clinical features across multiple cohorts.
MATERIALS AND METHODS: In this study, five gastric tumor expression profiles from the Gene Expression Omnibus [GEO] and one in-house cohort comprising a total of 1,060 samples were merged. The batch effect was

IMPORTANCE: Over the past decades, the number of patients, especially in the older adult patient group, diagnosed with vestibular schwannoma (VS) has increased. Assuming that older adult patients have more comorbidities, a longer recovery period after surgery, a higher rate of surgical complications, and a higher mortality rate after VS surgery, a treatment strategy for this group of patients is warranted, based on clinical evidence on postsurgical survival.
OBJECTIVE: To evaluate the survival after diagnosis of a VS in patients 70 years and older, treated with either observation or surgery,

Video Head Impulse Test (vHIT) multidimensional parameters-namely vestibulo‑ocular reflex (VOR) gain, gain asymmetry (GA), and compensatory saccades (CS)-are core indicators for assessing peripheral vestibular dysfunction. The combined use of vHIT and caloric testing facilitates differential diagnosis of various vertigo syndromes. In this retrospective study, 310 adult patients with unilateral peripheral vestibular disorders were enrolled, comprising 170 cases of Ménière's disease (MD), 110 cases of sudden sensorineural hearing loss accompanied by vertigo (SSHL‑V), and 30 cases of vestibular n

INTRODUCTION: In rural communities, aging infrastructure and environmental challenges can contribute to mistrust in tap water, potentially influencing hydration behaviors and health outcomes. The relationship between perceived water safety and liver health remains underexplored, particularly in low-resource Appalachian settings in the US.
METHODS: This cross-sectional study examined data from 272 adult patients in a primary care clinic in rural West Virginia, US. Participants rated their home tap water safety on a seven-point scale and reported beverage consumption patterns in a survey. Clini
