[Hearing rehabilitation in vestibular and inner ear schwannomas - Hearing aids, bone conduction Implants and CI treatment].

Patients with vestibular schwannoma (VS) in the internal auditory canal and/or cerebellopontine angle often experience hearing loss in the affected ear, which is attributable to the disease or its treatment. If conventional hearing aids do not provide sufficient speech comprehension, cochlear implantation (CI) can be an effective means of hearing rehabilitation after microsurgical tumour resection, radiotherapy or observation, if patients are carefully selected. However, the audiological results are more variable compared to the more common CI indications. A prerequisite for good hearing resul

Development of a Placenta-Based Microsurgical Training Model for Vestibular Schwannoma Resection.

BACKGROUND: Vestibular schwannoma resection is among the most technically demanding procedures in neurosurgery. Reduced operative exposure during residency has increased the need for realistic and accessible simulation models. METHODS: A training model was created by combining human placenta with a 3D-printed retrosigmoid craniotomy to simulate vestibular schwannoma resection. Placental and amniotic tissues were configured to reproduce the tumor capsule, facial nerve, lower cranial nerves, cisterna magna, and dura mater. The simulator reproduced dural opening and cerebrospinal fluid drainage,

Parallel Postauricular Incisions in Transmastoid CSF Leak Repair: Wound Outcomes.

BACKGROUND: Cerebrospinal fluid (CSF) leak after retrosigmoid vestibular schwannoma resection remains a challenging complication. Historically, surgeons reopened the prior retrosigmoid incision for transmastoid repair because the safety of creating a separate postauricular incision was uncertain, with concerns regarding wound breakdown and infection. However, limited data support this practice, and a focused mastoid incision may offer improved access and patient tolerance. This study evaluated wound healing outcomes following transmastoid repair using a new postauricular incision for persisten

Comparative Evaluation of Deep Learning Models for 3D Segmentation and Volumetry of Vestibular Schwannomas Using Large Heterogeneous Data Sets with External Validation.

BACKGROUND AND PURPOSE: 3D segmentation and volumetry of vestibular schwannomas (VSs) is a more accurate method to determine tumor growth on serial imaging, but manual annotation is time-consuming to implement in routine clinical practice. We evaluated and compared 5 deep learning-based segmentation models (nnUNet [Base, ResEncL], U-Mamba, UNETR, and MedSAM) for 3D VS segmentation and volumetry, and we examined the robustness to acquisition heterogeneity and generalization on an external cohort. MATERIALS AND METHODS: Our refined internal data set consisted of T1-contrast-enhanced images, inc

Radiation Therapy for Benign Diseases and Premalignant Conditions.

Radiation therapy (RT), traditionally reserved for malignant conditions, has emerged as a valuable tool in the management of select benign tumors and proliferative disorders. In clinical scenarios where surgery is not feasible, incomplete, or associated with significant morbidity, RT offers a noninvasive and effective alternative, often yielding excellent local control and long-term symptom relief. For instance, although surgery is relatively safe and effective, radiation is the preferred treatment for many patients with acoustic neuroma. Many benign entities have demonstrated favorable respon

Tumor Control After Radiosurgery in Sporadic and Neurofibromatosis Type 2 Vestibular Schwannomas.

INTRODUCTION: The tumor control rate after stereotactic radiosurgery (SRS) for neurofibromatosis type 2-associated vestibular schwannomas (NF2-VSs) compared to sporadic vestibular schwannomas (S-VSs) remains unclear. This nationwide, multicenter, retrospective study (KGKRS-21-001) aimed to clarify this issue. METHODS: A total of 4718 patients treated with SRS for vestibular schwannomas were analyzed from 13 nationwide institutions in Korea. NF2-VS cases were propensity score-matched with S-VS cases at a ratio of 1:1, based on age, tumor volume, and marginal dose, resulting in 122 cases in eac

Computer-aided prediction of growth in vestibular schwannomas based on both structural and dynamic contrast-enhanced MR imaging.

BACKGROUND: Vestibular schwannomas (VS) are benign intracranial tumors in the cerebellopontine angle. For most small-sized and medium-sized tumors, a wait-and-scan (W&S) approach is employed, since active treatment does not necessarily lead to symptom alleviation and up to 60% of VS tumors remain stable or regress during their natural course. At diagnosis, it is currently not possible to reliably predict tumor behavior and whether the tumor will grow or not, although some studies employing dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI) or artificial intelligence (AI) suggest

Long-term hearing and facial nerve outcomes after hearing preservation surgery for vestibular schwannoma: a systematic review and meta-analysis.

Hearing preservation remains a key goal in the management of vestibular schwannoma. Here, we aimed to evaluate long-term hearing and facial nerve outcomes following hearing preservation surgery for VS. A systematic review and meta-analysis were conducted following PRISMA guidelines. PubMed and Embase were searched for studies reporting long-term hearing outcomes after VS resection via the retrosigmoid or middle fossa approach. Studies with at least a 5-year follow-up were included. The primary outcome was long-term serviceable hearing, defined as American Academy of Otolaryngology-Head and Nec

Analysis of corneal wavefront aberrations and corneal densitometry in eyes with epithelial basement membrane dystrophy.

PURPOSE: To investigate wavefront aberrations, as well as corneal optical densitometry (COD), in eyes with epithelial basement membrane dystrophy (EBMD) and the influence on visual acuity. METHODS: In this cross-sectional study, 70 eyes of 70 patients (mean age 55.9 ± 14.0 years) with the central cornea involving EBMD were compared to 50 healthy eyes of 50 patients (mean age 58.8 ± 14.1 years) serving as controls. Wavefront aberrations of the anterior corneal surface and the total cornea were measured with the Pentacam AXL (Oculus Optikgeräte GmbH, Wetzlar, Germany), and calculated for the 6

Molecular and genetic insights into vestibular schwannoma.

BACKGROUND: Vestibular Schwannomas (VS) are benign nerve sheath tumors arising from Schwann cells of the vestibulocochlear nerve, occurring sporadically or in association with neurofibromatosis type 2 (NF2). Clinical management of VS is challenging due to the tumor's location adjacent to critical neural and vascular structures. The primary morbidities are hearing loss and facial nerve dysfunction, which significantly affect quality of life. There are no FDA-approved medical therapies, and treatment typically consists of surgery or radiation, with treatment decisions centered on optimizing hear

Fully endoscopic retrosigmoid keyhole approach versus traditional microscopic microsurgery for small-to-medium vestibular schwannomas: a retrospective matched-cohort study.

BACKGROUND: The surgical management of vestibular schwannoma (VS) has shifted focus from mortality reduction to functional preservation and quality of life. While the traditional microscopic retrosigmoid approach is the gold standard, it suffers from line-of-sight limitations, particularly when visualizing the fundus of the internal acoustic canal (IAC). The fully endoscopic sigmoid sinus keyhole approach (FEE) offers superior visualization and reduced invasiveness; however, current literature lacks rigorous, matched-cohort studies comparing its efficacy and safety against the established micr

Aprepitant and Postoperative Nausea and Vomiting in Patients Undergoing Acoustic Neuroma Surgery: A Retrospective Database Analysis.

BACKGROUND: Postoperative nausea and vomiting (PONV) are common complications, leading to prolonged hospital stays and reduced patient satisfaction. Acoustic neuroma (AN) resections are associated with a higher risk of PONV than other craniotomies. We aimed to detect if preoperative aprepitant is associated with less PONV following AN surgery. METHODS: Perioperative data were collected from the electronic medical record for patients undergoing AN resection between December 19, 2017 and April 26, 2022. Variables were compared between a cohort that received aprepitant and a matched cohort. Univ

Defining clinically significant tumor size in sporadic vestibular schwannoma to inform timing of radiosurgery during wait-and-scan management: further evidence supporting size threshold surveillance.

OBJECTIVE: Wait-and-scan surveillance is now commonly employed for initial management of small- and medium-sized vestibular schwannomas. Although small differences in tumor size are unlikely to impact outcomes significantly, treatment with either radiosurgery or microsurgery is usually recommended following radiological detection of tumor growth. The objective of the current study was to identify potential inflection points in vestibular schwannoma tumor size, where the risks of treatment with single-fraction stereotactic radiosurgery (SRS) accelerate to inform timing of intervention. METHODS

Minimal important differences to assess cross-sectional differences among groups and minimal important changes to assess longitudinal within-patient changes for the Mayo Clinic Vestibular Schwannoma Quality of Life Index.

OBJECTIVE: The minimal important difference (MID) and minimal important change (MIC) are two metrics that bridge the gap between statistical significance and clinical relevance and are critical to managing clinical decisions and conducting clinical research. The objective of this study was to define MIDs to evaluate cross-sectional differences among groups and to define MICs to evaluate longitudinal within-patient changes for the Mayo Clinic Vestibular Schwannoma Quality of Life (VSQOL) Index. METHODS: Anchor-based methods were used to define MIDs and MICs for the VSQOL Index domain scores, w

Inhibition of focal adhesion kinase impairs tumor formation and preserves hearing in a murine model of NF2-related schwannomatosis.

NF2 (neurofibromatosis type 2)-related schwannomatosis (NF2-SWN) is a cancer predisposition syndrome characterized by the development of bilateral vestibular (VS) and spinal schwannomas. While benign, these tumors can cause substantial morbidity, and effective pharmacological treatments remain limited. Here, we demonstrate that genetic ablation of focal adhesion kinase (Fak/Ptk2) impairs tumor formation and preserves hearing in a murine model of NF2. Mechanistically, we show that Fak deletion decreases macrophage infiltration, attenuates nucleotide-binding oligomerization domain-containing pro
