Ectopic overexpression and CRISPRi-based knockdown of Chlamydia trachomatis ObgE inhibits RB replication and EB reformation.

Chlamydia trachomatis is an obligate intracellular bacterium of major clinical significance. While untreated sexually transmitted infections can result in pelvic inflammatory disease and infertility, ocular infections can cause the blinding disease trachoma. During infection of host cells, C. trachomatis transitions between the non-replicative, infectious elementary body (EB) and the replicative, non-infectious reticulate body (RB). ObgE is a GTPase that can promote morphological differentiation in some bacteria. In C. trachomatis, obgE is maximally expressed from 16 to 24hpi, a timeframe that

Integrated serological surveillance for neglected tropical diseases, vaccine-preventable diseases, and arboviruses in Samoa, 2018.

Multiplex bead immunoassays (MBA) can detect antibody responses to multiple antigens. Using MBA data from the Surveillance and Monitoring to Eliminate Lymphatic Filariasis (LF) and Scabies from Samoa (SaMELFS) 2018, we aim to estimate national seroprevalence of neglected tropical diseases (NTDs), vaccine-preventable diseases (VPDs), and arboviruses in Samoa. A community-based serosurvey of 3851 participants aged ≥ 5 years in 35 primary sampling units (PSUs). Using MBA, dried blood spots were assayed for antibodies (Ab) from 10 pathogens: LF, trachoma, yaws, tetanus, diphtheria, rubella, measle

Unlocking the blueprint to eliminating neglected tropical diseases: A review of efforts in 50 countries that have eliminated at least 1 NTD.

BACKGROUND: Neglected tropical diseases (NTDs) are a group of 21 diseases affecting approximately 1.5 billion people globally. Significant progress has been made in their control: by March 2024, 50 countries had eliminated at least one NTD, with 13 of these countries eliminating at least two. Togo achieved the highest milestone, having eliminated four. The eight NTDs eliminated in at least one country are Guinea worm disease, human African trypanosomiasis, lymphatic filariasis, onchocerciasis, rabies, trachoma, visceral leishmaniasis and yaws. We reviewed elimination efforts of these 50 countr

A possible trachoma cluster unveiling the mist of Le Rôdeur: probes in favor and against an alternative cause of the crew blindness.

In 1819, a puzzling outbreak of infectious blindness afflicted nearly all crew members and African slaves aboard the French vessel Le Rôdeur during the transatlantic slave trade. This paper investigates the etiology of this unique event, comparing different pathogens that cause blindness: Chlamydia trachomatis, Neisseria gonorrhoeae, and Onchocerca volvulus; endemic in the African Continent. Reviewing naval medicine since the seventeenth century and eye infection knowledge in 1800, alongside historical records, we explore the singular case of Le Rôdeur's outbreak. While some clinical features

Design of a Multi-Epitope Vaccine using β-barrel Outer Membrane Proteins Identified in Chlamydia trachomatis.

Chlamydia trachomatis is an obligate intracellular Gram-negative pathogen that causes sexually transmitted infections (STIs) and trachoma. Current interventions are limited due to the widespread nature of asymptomatic infections, and the absence of a licensed vaccine exacerbates the challenge. In this study, we predicted outer membrane β-barrel (OMBB) proteins and designed a multi-epitope vaccine (MEV) construct using identified proteins. We employed a consensus-based computational framework on the C. trachomatis D/UW-3/CX proteome and identified 17 OMBB proteins, including well-known Pmp fami

Adoptive NK cell transfer confers neuroprotection by attenuating neuroinflammation and alpha-synuclein pathology in a mouse model of synucleinopathy.

UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

The Outcome and Survival of Locally Advanced Gastric Cancer with Gastric Outlet Obstruction After Upfront Radical Surgery.

Management of hypertension in the elderly should take into account, in particular, the possible negative impact of antihypertensive drugs on the patient's quality of life, the deterioration of which may result in a loss of independence and reduced treatment compliance. Quality of life is recognised as a multifactorial variable and can be subdivided into different domains (symptomatic well-being, emotional, physical, work-social, cognitive and life satisfaction), which are generally explored by means of specific questionnaires or scales. When evaluating elderly patients with hypertension, it is

Spatial and network mapping of comorbidity with trachoma and visual-impairing NCDs in a pastoralist community in Kenya: implications for SDGs and UHC.

BACKGROUND: We explore the spatial distribution of comorbidity with trachoma and potentially visual-impairing non-communicable diseases (NCDs) and their risk factors among Kenyan pastoralists. METHODS: Using a cross-sectional study design, we recruited and clinically examined 262 study participants for NCDs (hypertension or diabetes) and trachoma using the World Health Organization grading system. Network models estimated interactions and risks linked with trachoma and NCDs while the Poisson point process determined their spatial distribution. RESULTS: Of the 262 participants, 140 (53%) had

Impact of refresher training on the outcomes of trachomatous trichiasis surgery.

BACKGROUND/AIMS: Trachomatous trichiasis (TT) is a severe consequence of chronic inflammation/conjunctival scarring resulting from trachoma, the leading infectious cause of blindness worldwide. Our prospective cohort study evaluated the effectiveness of refresher training (RT) for experienced surgeons (1-22 years) on the outcomes of upper lid (UL) TT surgery in rural Ethiopia. METHODS: Patients undergoing UL TT surgery in at least one eye by a participating surgeon were included. Patients were split into two cohorts: patients enrolled prior to (C1) and after (C2) RT. RT consisted of a 1-week

Accuracy, acceptability and feasibility of photography for use in trachoma surveys: a mixed methods study in Tanzania.

BACKGROUND: Photography could be used to train individuals to diagnose trachomatous inflammation-follicular (TF) as trachoma prevalence decreases and to ensure accurate field TF grading in trachoma prevalence surveys. We compared photograph and field TF grading and determined the acceptability and feasibility of eyelid photography to community members and trachoma survey trainers. METHODS: A total of 100 children ages 1-9 y were examined for TF in two Maasai villages in Tanzania. Two images of the right everted superior tarsal conjunctiva of each child were taken with a smartphone and a digit

Eliminating Trachoma in Africa: The Importance of Environmental Interventions.

Subsequent to the failure of the World Health Organisation (WHO) of achieving their target to eliminate trachoma by the year 2020, the most effective strategy in eliminating trachoma must be re-examined to accomplish the new target of eradication by the year 2030. Whilst antibiotic therapy is a core foundation of this elimination strategy, another important factor is the state of the environmental conditions in trachoma endemic countries. This manuscript aimed to identify the impact of environmental improvement strategies on the prevalence of trachoma and the significance of environmental impr

Gender differences in trachomatous scarring prevalence in a formerly trachoma hyperendemic district in Tanzania.

BACKGROUND: Trachoma is a chronic conjunctivitis caused by the bacterium Chlamydia trachomatis. Repeated infections lead to trachomatous conjunctival scarring which can progress to potentially blinding trachomatous trichiasis (TT). In trachoma hyperendemic conditions, women compared to men have an increased risk of scarring and TT, which can progress to blinding corneal opacification. This study determined if there were gender differences in scarring prevalence and severity when trachoma prevalence approaches elimination, in a formerly trachoma hyperendemic region. METHODOLOGY/PRINCIPAL FINDI
