Using Model-Based Geostatistics to Refine Population-Based Estimates of Trachoma Prevalence: Update from a Technical Consultation.

To explore how model-based geostatistics (MBG) could support trachoma elimination efforts, a technical consultation was held on March 4 and 5, 2024 by the Centre for Health Informatics, Computing, and Statistics at Lancaster University, United Kingdom, a WHO Collaborating Centre on Geostatistical Methods for Neglected Tropical Disease Research. The meeting aimed to foster collaboration for sharing insights on using MBG for decision-making; showcase its applications in assessing trachoma elimination status; address challenges, such as setting the probability threshold for elimination and resolv

The changing neglected tropical disease landscape in Africa: implications for policy, practice, and strengthening health systems.

INTRODUCTION: Mass drug administration (MDA) has reached up to 1 billion people annually to combat neglected tropical diseases (NTDs). This study explores the evolving NTD landscape in Africa amid shifting disease patterns, funding cuts, and universal health coverage goals. METHODS: We analyzed data from ESPEN and Sightsavers databases to assess historical trends and forecast future treatment needs. RESULTS: By 2027, few or no MDAs for lymphatic filariasis and trachoma will be required and MDAs for onchocerciasis dropped sharply after 2024. In contrast, schistosomiasis and soil-transmitted h

Mass drug administration for trachoma elimination in a pastoral conflict area in Baringo County, Kenya: A study on barriers to coverage and compliance.

Trachoma, a neglected tropical disease (NTD), is the leading infectious cause of blindness worldwide. It is caused by repeated ocular infections with the bacteria Chlamydia trachomatis. Treatment coverage achieved in Baringo County, Kenya in 2020 and 2021 was 80% and 79% respectively, further investigation at the lower levels known as wards shows coverages ranging between 48% to 57% which are far below the WHO recommended threshold of at least 80% of the target population should be reached with MDA The objective of this study was to identify barriers of community participation and access to ma

Endoplasmic reticulum: the target of chlamydial manipulation.

Chlamydia, as an obligate intracellular pathogen, causes significant human diseases such as trachoma, sexually transmitted infections, respiratory illnesses, and atherosclerosis. Understanding its unique survival strategies within host cells is crucial for developing effective treatments. The endoplasmic reticulum (ER) is a key target for intracellular pathogens due to its roles in fundamental cellular functions. Chlamydia forms membrane contact sites (MCSs) with the ER. This physical connection allows Chlamydia to obtain sphingomyelin and regulate calcium ion concentrations via the ER, thereb

Assessing the effectiveness of the community participation approaches to improve access to mass drug administration for trachoma elimination in a pastoral conflict area of Baringo County, Kenya.

BACKGROUND: Trachoma remains a significant public health issue in many regions, including Baringo County, Kenya. Despite the ongoing mass drug administration (MDA) campaigns in Baringo, the achievement of optimal treatment coverage has been hindered by factors such as conflicts and the nomadic lifestyle dominating this region. To address these challenges, innovative strategies are needed to improve community engagement and enhance MDA uptake. This study evaluated the effectiveness of community participation approaches in improving MDA access among residents of Baringo County. METHODS: The stu

Prevalence and spatial distributions of trachomatous inflammation-follicular among children aged 1-9 years in rural areas of Yilmana Densa and Gonji Kolela districts, Northwestern Ethiopia.

BACKGROUND: Trachoma is the world's major infectious cause of blindness, responsible for blinding 1.9 million people, including 1.2 million irreversibly. It is still endemic predominantly in sub-Saharan Africa, including Ethiopia. Five or more follicles in the upper tarsal conjunctiva measuring at least 0.5 mm indicate trachomatous inflammation-follicular (TF) disease. No previous study determined the prevalence of TF, and it had not been determined for the study area to satisfy adequate geospatial representation/spatial distribution of TF among children 1-9 years old. These study findings can

An Investigation of Historical Injustices in the South Dakota Medical Journal 1912-1930.

Historians' recent exploration of the role of the New England Journal of Medicine in perpetuating historical biases and injustices in medicine prompted us to examine the journal of the South Dakota Medical Association for similar themes. Known as the Journal-Lancet in the years 1912-1930 covered by our study, the journal's content was searched using key words in four categories: Native American, Blacks, the feeble-minded and eugenics. Relevant articles containing key words were archived in databases (DEVONthink and Claris FileMaker) structured to permit systematic examination of the material.

Factors associated with health worker adoption of facial and environmental hygiene promotion in the 'SAFE strategy' for trachoma elimination in Western Province, Zambia.

BACKGROUND: Trachoma is responsible for the blindness or visual impairment of about 1.9 million people and causes about 1.4% of all blindness worldwide. In Zambia, trachoma is endemic and Western Province is one of the most affected provinces. The SAFE (surgery, antibiotics, facial cleanliness and environmental improvement) strategy is recommended for elimination of trachoma. In many settings, interventions particularly for facial cleanliness and environmental improvement are sub-optimally adopted due to lack of prioritization and inadequate funding of intervention activities. This study sough

The prevalence of trachoma, ocular Chlamydia trachomatis infection and anti-Pgp3 antibodies in Choiseul Province, Solomon Islands.

Trachoma, caused by repeated ocular infection with Chlamydia trachomatis, remains a leading infectious cause of blindness globally, with significant implications for public health. The World Health Organization and partners aim to eliminate trachoma as a public health problem by 2030, targeting specific prevalence thresholds for trachomatous trichiasis (TT) and trachomatous inflammation-follicular (TF). Diagnosis is primarily clinical. Studies have shown discrepancies between prevalence estimates of TF and C. trachomatis infection. This study, undertaken in Choiseul, Solomon Islands, evaluated

Design considerations for incorporating serological monitoring into trachoma prevalence surveys.

Serology is increasingly used to monitor disease transmission and elimination. Embedding dried blood spot collection in trachoma surveys allows transmission intensity inference through population-level seroconversion rates (SCR) estimation, but there is no formal assessment of the required sample size. Using data from 40 prevalence surveys, we estimated intra-cluster correlation coefficient, a key design parameter, and assessed survey design considerations for incorporating serological monitoring. Design scenarios focused on recent proposed operational SCR thresholds (2.2 [no action needed] an

Are Children Least Engaged with Health Care More Likely to Have Trachoma?

Distribution of azithromycin to children ages 0-9 years old is an established strategy for treating and preventing trachoma. Our study aimed to determine whether the order in which children show up for trachoma screening is correlated with their infection status. We used baseline visit data from the study Kebele Elimination of Trachoma for Ocular Health in Ethiopia. All children ages 0-9 years old in 20 randomly selected villages were tested for ocular Chlamydia trachomatis with polymerase chain reaction. We used mixed effects logistic regression to estimate the odds ratio (OR) of trachoma pos

Factors associated with low coverage in mass drug administration for schistosomiasis in mobile populations in mali: a cross-sectional study.

BACKGROUND: Neglected tropical diseases (NTDs) affect over a billion people globally. From 2020 to 2021, when this study was conducted, Mali remained endemic for multiple NTDs, including schistosomiasis and trachoma. At the time, significant efforts were underway to scale up control and elimination programs, although challenges persisted, particularly in reaching mobile populations such as nomads, migrants, and internally displaced persons (IDPs), with mass drug administration (MDA). These groups were often missed during campaigns, contributing to gaps in coverage and sustained transmission in

Lessons from the elimination of trachoma in Senegal: implications for high-burden countries in sub-Saharan Africa.

Senegal's achievement in eliminating trachoma as a public health problem offers a powerful model for countries still burdened by the disease. This success was driven by a holistic and context-sensitive implementation of the SAFE strategy. The country invested in training health workers to deliver trichiasis surgeries in hard-to-reach areas, ensured high coverage of azithromycin through consistent mass drug administration (MDA), and embedded hygiene promotion in schools and community structures. Importantly, Senegal tailored interventions to regional cultural and epidemiological contexts, enhan

Surveys to substantiate elimination of trachoma as a public health problem in the Republic of the Union of Myanmar.

BACKGROUND: To determine whether trachoma elimination thresholds have been met in Myanmar, surveys were undertaken in 2019 to estimate the prevalence of trachomatous inflammation-follicular (TF) in children ages 1-9 y and the prevalence of trachomatous trichiasis (TT) unknown to the health system in adults ≥15 y of age. METHODS: Surveys were conducted in 14 townships (evaluation units [EUs]) that were considered formerly endemic for trachoma. A two-stage cluster survey design was applied, whereby 30 villages and 30 households per village were sampled. Consenting eligible participants ≥1 y of
