The use of computerized tomographic imaging in revision mastoid surgery for chronic otitis media.

Recurrent otorrhea, pain, and a conductive hearing deficit are common indicators of possible recurrent disease following mastoidectomy for chronic otitis media. High-resolution computed tomography (CT) provides an excellent method for examination of the surgically altered mastoid. Areas of residual infected air cells, cholesteatoma, cholesterol granuloma, and other surgical conditions within the mastoid are localized by this technique prior to reexploration. The technique provides information regarding the extent of disease as well as possible anatomic variations and potential complications th

Role and results of cortical mastoidectomy and endolymphatic sac surgery in Menière's disease.

It is almost impossible to establish the natural history of Menière's disease and, by the same token, it is difficult to confirm the efficacy of endolymphatic sac surgery. The authors doubt the logic of sac operations. Only two controlled trials of sac surgery have been found and both cast doubt upon its value. There do not appear to be any controlled trials indicating that it works. In 14 cases of incapacitating Menière's disease, where vestibular nerve section was indicated, cortical mastoidectomy was offered and accepted in the expectation that two-thirds might be spared the more major proc

Selection of cases and classification of tympanoplasty.

Meticulous mastoid surgery does not always result in a dry, stable ear. Eustachian tube malfunction has been accepted as an important factor in chronic and recurrent middle ear infection. There are many parameters of eustachian tube malfunction, which form a complicated problem for investigation and analysis. Of most importance appears to be varying degrees of malformation of the nasopharynx and palate. A gradient from mild to overt deformity appears to be related to the degree of eustachian tube function. Other factors, such as nasal infection and allergy, nasopharyngeal scar tissue and tumor

[Mastoidectomy and seromucous otitis].

Twenty-nine children presenting with chronically recurring serous otitis media, or with serous otitis media complicated by retracted eardrum, underwent either unilateral or bilateral mastoidectomy. For 23 of the children (26 surgical cases) whose follow-up extended over a sufficiently long period of time, results were assessed by otoscopic eardrum inspection, audiometry, and tympanometry. Long-standing recovery of serous otitis media, accompanied by a return to normal of the tympanic membrane, the tympanometric and audiometric parameters, was achieved in 2/3 of cases. These data compare well w

Bioactive glass ceramic in ossicular reconstruction: a preliminary report.

Current techniques for ossicular reconstruction include ossicular autografts and homografts, and plastic and ceramic prostheses. Experimental testing of ceramic materials has led to the development of a nonporous, bioactive form of glass ceramic (Ceravital) for tympanoplastic reconstruction. To determine the clinical usefulness of these implants, Ceravital ossicular reconstructions were evaluated in 37 patients treated at the University of Michigan Medical Center during an 18-month period. Thirty-two patients (86%) with chronic ear disease underwent Ceravital ossicular reconstruction with tymp

Toxic shock syndrome following septorhinoplasty. Implications for the head and neck surgeon.

Toxic shock syndrome (TSS) is an acute multisystem disease that has recently been recognized to occur in a variety of clinical settings. Postoperative TSS has been described following many different surgical procedures, though, to our knowledge, only one report has been published to date describing TSS following a head and neck procedure. We report a case of classic TSS following septorhinoplasty. The analogous role of postoperative nasal packing and the packing of other cavities (eg, in maxillectomies or mastoidectomies) to the use of vaginal tampons as a possible risk factor for TSS developm

[The treatment of chronic adhesive otitis by tympanoplasty and active self-insufflation according to valsalva (author's transl)].

More than one year after tympanoplasty 85 to 95% of the followed up middle ears are aerated. The percentage depends on the degree of the preoperative adhesive process. We see the reason of this surprising success on the one hand in some special steps during operation (consequent silastic sheeting, preservation of the posterior bony canal with a deep middle ear cleft, visualisation of the tympanic orifice of the tube, widening of the natural connection between tympanon and mastoid without opening the chorda-facial-angle, some times mastoidectomy), on the other hand in a consequent postoperative

Treatment of carcinoma of the middle ear.

31 patients with carcinoma of the middle ear were observed over a period of 15 years. Polytomography was used to determine the local extent of the disease. Of 22 patients given radical therapy, 15 were treated by mastoidectomy followed by radiation therapy. The 5-year survival rate was 40%, which is in agreement with the findings of several other authors. Patients treated by irradiation alone had a poor survival rate.

Results of conservative surgery for middle ear cholesteatoma.

A retrospective study was carried out to analyze treatment results for cholesteatoma at the University of Iowa Hospitals and Clinics. All patients undergoing primary surgical treatment from January 1, 1969 to December 31, 1973 were followed through October 1976. Treatment failures were based on the occurrence of postoperative cholesteatoma. The probability of being disease free for periods up to five years after original surgery was then estimated for each type of operation. Postoperative cholesteatoma occurred after atticotomy in 17%, intact canal wall mastoidectomy in 35%, and modified radic
