BACKGROUND: Infective endocarditis remains a life-threatening condition requiring timely medical and surgical management. The COVID-19 pandemic posed unprecedented challenges to healthcare delivery, yet the outcomes of surgical versus conservative management among patients with infective endocarditis and underlying rheumatic heart disease during this period remain poorly defined.
OBJECTIVE: This study aims to compare 30-day outcomes following surgical versus conservative management among adult patients with acute and subacute infective endocarditis in the setting of rheumatic heart disease du

Rheumatic heart disease (RHD) remains a significant public health challenge in Latin America, disproportionately affecting vulnerable populations despite global declines in disease prevalence and mortality. It is known that RHD affects over 55 million individuals worldwide, accounting for more than 390,000 deaths annually. However, region-specific RHD epidemiologic data are scarce, and most clinical studies on RHD epidemiology date back to the 1990s. On the other hand, the emergence of echocardiographic screening initiatives in the last decade reinforced the burden of the disease, revealing al

Background: The population of patients surviving previous multivalve surgery is expanding, and structural Heart Teams increasingly encounter complex multivalvular disease requiring reintervention, for which redo surgery carries prohibitive risk. Patient selection, sequential procedural planning, prosthesis choice, and, in particular, pacing strategy and protection of pre-existing intracardiac hardware remain unstandardized. Case Presentation: We report a 73-year-old woman with a twenty-year history of rheumatic multivalvular heart disease, comprising mechanical mitral valve replacement, tricus

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic affected exposure to infectious agents. We aimed to analyze changes in Group A streptococcal (GAS), acute rheumatic fever (ARF) and rheumatic heart disease (RHD) prevalence during the COVID-19 pandemic and assess associated social vulnerability indices (SVI).
METHODS: Using the Epic Cosmos database, we performed a descriptive analysis of patients aged 2-21 in the United States from January 2016 through December 2023. We compared monthly GAS positivity rates and ARF/RHD diagnosis frequencies across pre-pandemic (January 2016-February

Intramuscular (IM) injection of benzathine benzylpenicillin G (BPG) forms the cornerstone of acute rheumatic fever (ARF) and rheumatic heart disease (RHD) secondary prophylaxis. BPG is available as either a low-cost powdered formulation or a costlier pre-filled suspension. Most of the global RHD burden lies in low- and middle-income countries, which rely on the powdered formulation. Prospects of achieving RHD control targets continue to be hindered by global BPG supply issues. Guided by a preliminary consultation of global stakeholders published in 2016 which outlined the desired characteristi

Household overcrowding is a major driver of acute rheumatic fever and rheumatic heart disease, along with other adverse social, cultural and health outcomes in remote Aboriginal communities. Overcrowding is compounded by poor thermal performance of current housing, energy insecurity and climate change. Despite strong evidence of the causes of rheumatic heart disease, upstream prevention through housing design remains underexplored. Wilya Janta, an Aboriginal-led organisation in Tennant Creek, has developed the Explain Home design: a culturally responsive, climate-adapted prototype designed to

INTRODUCTION: Rheumatic heart disease (RHD) is the acquired autoimmune heart valve damage resulting from untreated infection with the Streptococcus pyogenes bacterium, which affects people experiencing socioeconomic disadvantage globally. This study measured RHD-associated major adverse cardiovascular events (MACE) and the increased risk associated with pregnancy among women diagnosed with RHD.
MATERIAL AND METHODS: Population-level analysis of all births to women with RHD in four Australian jurisdictions was conducted, which covered 71% of the total population and 88% of the Aboriginal and T

Prompt diagnosis of group A streptococcal pharyngitis is essential for primary prevention of acute rheumatic fever and rheumatic heart disease, yet affordable point-of-care diagnostics remain limited in low-resource settings. We conducted a prospective study among Ugandan children 5-15 years of age presenting with sore throat, comparing smartphone-acquired oropharyngeal videos analyzed using an artificial intelligence-based image recognition model-trained and validated using combined data from Ugandan and US-based cohorts-with microbiologic culture as the reference standard. Among 82 children

This narrative review synthesizes nearly two centuries of evidence linking Streptococcus pyogenes infection to acute rheumatic fever (ARF), the prerequisite for the development of rheumatic heart disease (RHD). Using the Bradford Hill criteria for causality and the Oxford Centre for Evidence-Based Medicine levels of evidence, we critically evaluate historical and contemporary data pertaining to this relationship. Strong evidence demonstrates that untreated S. pyogenes infection increases ARF incidence, while antibiotic treatment reduces incidence across diverse populations and settings. Modera

BACKGROUND: Acute rheumatic fever (ARF) is an acute, immune-mediated process that occurs following an untreated Group A Streptococcus (GAS) infection that can result in complications, such as valvular heart disease, myocarditis and arrhythmias. ARF is now rare in the United States which can make it difficult recognize. The standard for diagnosing ARF remains the revised Jones criteria, including serologic evidence of GAS infection. Given it is encountered so infrequently in the United States, clinicians may face uncertainty when interpreting serologic studies and pursuing cardiac imaging in su

BACKGROUND: Ortner's syndrome, or cardiovocal syndrome, is a rare form of left recurrent laryngeal nerve palsy caused by cardiovascular pathology. Historically associated with massive left atrial enlargement from rheumatic heart disease (RHD), it is now an uncommon diagnosis. However, in regions such as Nepal, where rheumatic heart disease remains endemic, it represents an important, though overlooked, complication.
CASE PRESENTATION: A 48-year-old Tamang woman, working as a farmer from Nepal, with a history of percutaneous transvenous mitral commissurotomy (PTMC) performed 12 years prior for

The M protein of Group A Streptococcus (GAS) is an essential virulence factor that promotes both superficial and invasive infections, as well as the development of immune sequela, including acute rheumatic fever and rheumatic heart disease. M protein is a prototypical sortase-anchored surface protein, yet has also been observed free of the GAS surface during human infection. This suggests a mechanism for its release, and this change in localization could influence innate immune detection, effector functions, and antigenic potential. Here, we show the protease SpeB cleaves M protein from the mi

Rheumatic heart disease (RHD) is a chronic sequel of acute rheumatic fever characterized by sustained inflammation, fibrosis, and valve degeneration; however, the underlying processes remain unclear. To identify dysregulated molecular pathways, we conducted LC-MS/MS-based proteomic profiling of RHD mitral valve tissue compared with ischemic controls and validated systemic inflammation in RHD patients through peripheral blood analysis. Proteomic analysis revealed upregulation of immune response proteins, TGFβ signaling, and extracellular matrix (ECM) regulators. Notably, proteins associated wit

Acute rheumatic fever (ARF) is an autoimmune disease that develops as a complication of group A beta-hemolytic streptococcal infections and may lead to rheumatic heart disease, particularly in children and young adults in developing countries. With an annual incidence of approximately 10-20 per 100,000, Turkey is considered a moderate-to-high risk country. Early diagnosis, treatment, and prophylaxis are crucial in the prevention of ARF and rheumatic heart disease. This study aimed to evaluate the knowledge level of first-year medical students (in the basic medical education phase) and sixth-ye

BACKGROUND: Rheumatic heart disease (RHD) remains a major public health problem in low- and middle-income countries. Long-term benzathine penicillin G (BPG) secondary prophylaxis prevents recurrent acute rheumatic fever (ARF) and progressive valve damage, but maintaining adequate uptake (i.e., ≥80% dose) remains challenging in high-burden settings. We assessed BPG uptake, factors associated with high uptake, and its association with reported recurrent ARF manifestations among people with RHD in Ethiopia.
METHODS: A multicentre facility-based cross-sectional study was conducted among 400 parti

AIM: In Aotearoa New Zealand, people with acute rheumatic fever (ARF) receive 4-weekly intramuscular benzathine penicillin G (BPG) injections for a minimum of 10 years to prevent recurrence and progression to rheumatic heart disease. Within a phase IIA trial of a higher-dose subcutaneous injection of penicillin (SCIP), which provides at least 10 weeks of coverage, we piloted immersive virtual reality (VR) distraction to assess whether it improved injection acceptability through reductions in pain and anxiety.
METHODS: Ten participants aged 9-20 years used a Meta Quest VR headset during their

Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality in elderly patients. Therefore, efforts to optimize the healthcare process for patients with CAP are warranted. An organized approach to management is likely to improve clinical results. Assessing the severity of CAP is crucial to predicting outcome, deciding the site of care, and selecting appropriate empirical therapy. Unfortunately, current prognostic scoring systems for CAP such as CURB-65 (confusion, uraemia, respiratory rate, low blood pressure and 65 years of age) or the Pneumonia Severity Index have not been

AIM: Acute rheumatic fever (ARF) is an autoimmune response to group A Streptococcus (Strep A) infection that can lead to rheumatic heart disease (RHD). To prevent Strep A infection for people with ARF or RHD, clinical guidelines recommend monthly intramuscular penicillin (Bicillin L-A®) injections for at least 10 years. However, multiple barriers to accessing healthcare affect adherence. This research aimed to improve adherence by developing a Māori and Pacific-centred model of care.
METHODS: Qualitative, Kaupapa Māori and Kakala methodology underpinned the research. Data collection from peop

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation
