Acute Rheumatic Fever and Rheumatic Heart Disease: Public Health Insights from Coroners' Inquests.

This editorial reviews the state of clinical knowledge about acute rheumatic fever (ARF) and rheumatic heart disease (RHD). It uses the lenses of public health and coronial law to consider the pathology which is significantly over-represented in the First Nations Peoples of Australia, Aotearoa New Zealand and the Pacific. Noting that it is comparatively rare for deaths from natural illness to be scrutinised by coroners, it identifies the importance of a series of inquests, heard in New South Wales, the Northern Territory, Western Australia and Queensland between 2012 and 2022, that scrutinised

Rheumatic heart valve disease: navigating the challenges of an overlooked autoimmune disorder.

Despite being a leading cause of morbidity and mortality among young people, affecting predominantly women, rheumatic heart disease (RHD) remains neglected and understudied. This autoimmune condition arises from a complex continuum that begins with repeated Group A Streptococcal (GAS) pharyngitis, leading to acute rheumatic fever (ARF) that eventually results in damage to the heart, mainly affecting the mitral valve. While RHD has been nearly eradicated in high-income countries, it continues to be a significant and active health issue in low- and middle-income countries. The resolution of this

Perioperative management of a pregnant patient with a mechanical heart valve undergoing appendectomy: a case report.

INTRODUCTION AND IMPORTANCE: Managing pregnant patients with mechanical heart valves who require non-cardiac surgery presents unique challenges, particularly in balancing anticoagulation to prevent thromboembolic complications while minimizing bleeding risks. This case report discusses the perioperative management of a pregnant patient with a mechanical mitral valve undergoing emergency appendectomy. CASE PRESENTATION: A 28-year-old pregnant woman at 22 weeks gestation with a history of rheumatic heart disease and a mechanical mitral valve presented with acute appendicitis. Anticoagulation th

The epidemiology of repeatedly positive bacterial skin infections in Auckland children, New Zealand.

OBJECTIVE: Māori and Pacific New Zealanders are disproportionately affected by Acute rheumatic fever (ARF) and rheumatic heart disease (RHD). Repeated S. pyogenes skin infections in childhood are thought to increase the risk of developing ARF/RHD. This study investigated the epidemiology of repeated bacterial skin infections within Auckland, New Zealand. METHOD: Microbiology results from wound swabs collected in primary care between 2010 and 2020 were used to estimate the rate of repeatedly positive infections, along with rate ratios and 95% CIs, across demographic groups. RESULTS: S. aureus

Identification of Coronary Morphological Damage in Patients with Chronic Inflammatory Rheumatic Diseases.

Objective: Patients with chronic inflammatory rheumatic diseases (CIRDs) have a higher incidence of coronary artery disease (CAD) due to accelerated atherogenesis. This study aimed to assess the extent and location of CAD lesions in CIRD patients compared to non-CIRD patients. Methods: A retrospective study was conducted on CIRD patients (rheumatoid arthritis, ankylosing spondylitis, and psoriatic arthritis) who underwent coronary angiography at Hospital Fundación Jiménez Díaz (Madrid, Spain) between 2018 and 2022. For each CIRD patient, at least two frequency-matched controls were selected ba

A mini review of the pathogenesis of acute rheumatic fever and rheumatic heart disease.

Acute rheumatic fever (ARF) is an autoimmune disease caused by group A streptococcal infection. Recurrent episodes of ARF can lead to rheumatic heart disease (RHD), which is the leading cause of cardiovascular mortality in children worldwide, especially in low- and middle-income countries. Investigations into the etiology of ARF and RHD constitute a crucial milestone in the advancement of both preventive measures and therapeutic interventions. The purpose of this mini review is to delineate the etiology and pathophysiological mechanisms underlying ARF and RHD. Selective searches were conducted

The Incidence of Scabies in Far North Queensland, Tropical Australia: Implications for Local Clinical Practice and Public Health Strategies.

The recognition and treatment of scabies has been incorporated into Australian guidelines for the prevention of acute rheumatic fever (ARF) and rheumatic heart disease (RHD). The incidence of both diagnosed ARF and RHD is increasing in Far North Queensland (FNQ) in northeast tropical Australia, but the local burden of scabies is incompletely defined. We reviewed the results of every skin scraping collected in FNQ's public health system between 2000 and 2023; 121/4345 (2.8%) scrapings were positive, including 19/1071 (1.8%) in the last 5 years of the study; the proportion of scrapings that were

Ocular markers of microangiopathy and their possible association with cardiovascular risk in patients with systemic inflammatory rheumatic diseases: a systematic review.

Individuals with autoimmune rheumatic diseases (ARDs) are at a higher cardiovascular (CV) risk due to systemic inflammation, which contributes to endothelial dysfunction, atherosclerosis, and structural changes in the vessel walls. Along with traditional CV risk factors like dyslipidaemia, arterial hypertension, obesity, and impaired glucose metabolism, these patients have a severe prognosis with higher CV morbidity and mortality rates. To date, there is limited data on the optimal CV screening methods for individuals with ARDs, as conventional risk algorithms may underestimate the influence o

Severe adverse reactions to benzathine penicillin G in rheumatic heart disease: A systematic review and meta-analysis.

BACKGROUND: Fear of severe adverse reaction (SAR) and reluctance of health care providers to administer intramuscular injections are major contributing factors to poor adherence of benzathine penicillin G (BPG) in the management of rheumatic heart disease (RHD). However, data on the risk of SARs following BPG injections for RHD are relatively limited and inconclusive. Our systematic review and meta-analysis aimed to evaluate the incidence of SARs associated with BPG injections used for secondary prophylaxis of RHD. METHODS: A systematic literature search of PubMed, Scopus and Web of Science d

Acute rheumatic fever and Post-streptococcal reactive arthritis.

Acute rheumatic fever (ARF) and poststreptococcal reactive arthritis (PSRA) represent two distinct but related poststreptococcal conditions, both arising from an autoimmune response following Group A streptococcal (GAS) infection, typically pharyngitis. Although both illnesses have a shared trigger, their clinical presentations and long-term consequences differ markedly. ARF, a systemic inflammatory disorder, can impact several organs, particularly the heart, potentially resulting in chronic rheumatic heart disease with irreversible valve damage. The cornerstone of ARF treatment includes Strep

Increased angiogenesis and lymphangiogenesis in rheumatic mitral valves.

Acute rheumatic fever (ARF) and rheumatic heart disease (RHD) create a global burden in the field of cardiovascular medicine due to significant morbidity and mortality, particularly in low- and middle-income countries and the underprivileged population. Sequential surgical specimens of mitral valves excised from 19 patients with moderate to marked rheumatic mitral stenosis were included in this study and compared to eight sequential surgical myxomatous degenerative valves (control). The study group population had a mean age of 43.8 (± standard deviation (SD) 13.3) years and comprised 10 female

Acute rheumatic fever.

Acute rheumatic fever (ARF) is an autoimmune disorder resulting from Group A Streptococcus (GAS) pharyngitis or impetigo in children and adolescents, which may evolve to rheumatic heart disease (RHD) with persistent cardiac valve damage. RHD causes substantial mortality and morbidity globally, predominantly among socioeconomically disadvantaged populations, with an interplay of social determinants of health and genetic factors determining overall risk. ARF diagnosis is based on a constellation of clinical and laboratory features as defined by the 2015 Jones Criteria, although advances in molec

Juvenile Mitral Stenosis Presenting as Acute Pulmonary Edema Treated with Balloon Mitral Valvotomy.

Juvenile mitral stenosis is defined by poor left atrial emptying due to limited opening of the mitral-valve orifice in patients under the age of 20 years. Balloon mitral valvotomy is the current treatment of choice for juvenile mitral stenosis. Here we report a case of a 14-year-old female presenting with signs of acute pulmonary oedema. Later on, underlying mitral stenosis was revealed on echocardiography. After evaluation, she was successfully managed by percutaneous balloon mitral valvotomy. Screening young patients with pulmonary oedema by two-dimensional echocardiography is necessary to e

Addressing the challenge of rheumatic heart disease in Pakistan: A call to action.

Despite its high prevalence, Acute rheumatic fever (ARF) and Rheumatic heart disease (RHD) is underrepresented in Pakistan's national and federal health plans. To address this gap, we gathered multi-geographic and specialty perspectives from frontline clinicians in Pakistan'. Major challenges in RHD diagnosis and care include regional healthcare disparities, diagnostic limitations, scarcity of Benzathine Penicillin G (BPG) for prophylaxis, and lack of multidisciplinary RHD teams. Our practitioner-informed recommendations emphasize community outreach, targeted screening, and surveillance, and c
