Patterns of atrial fibrillation at a regional hospital in Saudi Arabia.

The objective of this retrospective study was to report on the clinical presentation, etiology, and laboratory tests of both chronic and acute atrial fibrillation (AF) admitted to the cardiology unit of a teaching hospital in southern Saudi Arabia. We studied 219 records; 132 (60.3%) and 87 (39.7%) had documented chronic AF (group 1), and acute AF (group 2) respectively. The mean age (SD) was significantly higher in group 1 (64.6 [SD 19.4] vs 52.9 [SD 15.6]) (P<0.001). Palpitation, dizziness and syncope were the most frequent symptoms in acute AF, while dyspnea was the most common presentation

Mitral valve replacement with glutaraldehyde preserved aortic allografts.

OBJECTIVE: To present long-term results after mitral valve replacement with stent mounted glutaraldehyde preserved aortic allografts in patients older than 15 years. The clinical support for this study was to combine the glutaraldehyde technique of biological tissue preservation with the advantages of allografts when compared to xenografts. This was demonstrated in previous studies using other methods of tissue processing. METHODS: Between September 1984 and November 1994, 70 patients aged 16-77 years (mean 35.4 years) underwent mitral valve replacement with this preserved and mounted allogra

Use of bovine pericardial tissue for aortic valve and aortic root replacement: long-term results.

BACKGROUND AND AIMS OF THE STUDY: The study aimed to determine the clinical performance of bovine pericardial aldehyde-treated products alone or in combination with aortic leaflets of porcine origin. These included a composite porcine stentless aortic valve attached to a scalloped pericardial tube (BSAV), and valved and non-valved bovine pericardial conduits for use in left-sided heart lesions (BPG). METHODS: For BSAV grafts, between January 1990 and August 1996, 163 patients (119 males) had their aortic valves replaced by SJM Biocor BASV. Mean age was 37.9 +/- 17.6 years (range: 1 to 76 year

Group A beta-hemolytic streptococcal infections.

GABHS is the most common bacterial cause of tonsillopharyngitis, but this organism also produces acute otitis media; pneumonia; skin and soft-tissue infections; cardiovascular, musculoskeletal, and lymphatic infections; bacteremia; and meningitis. Most children and adolescents who develop a sore throat do not have GABHS as the cause; their infection is viral in etiology. Other bacterial pathogens produce sore throat infrequently (e.g., Chlamydia pneumoniae and Mycoplasma pneumoniae), and when they do, other concomitant clinical illness is present. Classic streptococcal tonsillopharyngitis has

Cytokines in acute rheumatic fever.

UNLABELLED: Plasma concentrations of inflammatory cytokines (IL-1 alpha, IL-1 beta, IL-6, IL-8 and TNF alpha) were determined by ELISA in 27 patients with acute rheumatic fever (RF), 12 with only arthritis (RFA) and 15 with rheumatic heart disease (RHD), before, during and after treatment. Altogether, significant increases in TNF alpha, IL-8 and IL-6 levels were observed in the acute phase as compared to the data found during and after treatment. No significant differences were observed for the other cytokines. Elevations of one or more of the inflammatory cytokines were observed in 9 of 12 pa

Inflammatory valvular prolapse produced by acute rheumatic carditis: echocardiographic analysis of 66 cases of acute rheumatic carditis.

Sixty-six cases of rheumatic fever (RF) associated with acute rheumatic carditis (ARC) diagnosed on the basis of the revised Jones criteria (1982) were studied retrospectively (Group I) and prospectively (Group II). Echocardiography in Group I revealed anterior mitral valve prolapse (AMVP) in 28/55 cases (51%), posterior mitral valve prolapse (PMVP) in four (7%), combined anterior and posterior mitral valve prolapse (BMVP) in five (9%), aortic valve prolapse (AVP) in eight (15%), and in Group II, AMVP in nine (82%), BMVP in one (9%), AVP in six (55%). Among the prolapsed valves 40% of the mitr

[Heart valve diseases specific in rheumatoid polyarthritis. Apropos of 2 cases].

Rheumatoid nodules represent a rare cardiac valvular involvement in rheumatoid arthritis. Patients are usually asymptomatic. We report two cases of such involvement: one presented as a tumour implanted on the mitral valve, with systemic embolisation; the other presented as aortic regurgitation with acute heart failure. Surgical treatment was performed in both cases. Histological examination revealed typical rheumatoid nodules. The authors discuss valvular involvement in rheumatoid arthritis.

Etiology of valvular heart disease.

The incidence of congenital valvular heart disease has not significantly altered in recent decades. Major factors contributing to altered profiles of acquired valvular heart disease in the past few decades include an increased elderly segment of the population and increasing recognition of nonrheumatic forms of valvular heart disease. Mitral valve prolapse, and similar involvement of other valves, together with senile calcific aortic stenosis have emerged as the most common forms of valvular heart disease in developed countries. Body leanness and hypertension are additional etiological factors

Serum alpha-1-antitrypsin in ischemia and rheumatic heart diseases.

Tissue damage, inflammation and necrosis are hallmarks of myocardial infarction. In the present study significant elevations of serum alpha-1-antitrypsin were noted in coronary artery disease and angina cases. Interestingly chronic rheumatic heart disease which is also characterized by tissue injury. Inflammation revealed normal levels of serum alpha-1-antitrypsin. The level in chronic rheumatic heart disease was 3.37 +/- 0.57 mumol/mt/ml (control level was 3.37 +/- 0.54 mumol/mt/ml). The corollary of these observations is that in heart diseases acute phase response in terms of enhanced levels

[Microbiocenotic changes in the large intestine of patients with various diseases].

The specific features of the components of human large bowel microbiocenosis were studied in healthy persons and patients with various pyo-inflammatory and other diseases in relation to the physiological status. The presence of a pathological process was demonstrated to be a powerful factor of intestinal microbiocenotic variations. No specific changes in the large bowel microflora were found in the abnormalities in question.

[Echocardiography and Jones criteria in the diagnosis of rheumatic heart diseases: 138 cases reviewed at the Cardiology Institute of Abidjan].

The files of 138 patients suffering from rheumatic heart disease were studied in order to confirm the value of modified Jones' criteria and heart damage and to evaluate the contribution of echocardiography findings. Only files containing complete clinical and laboratory findings were studied. Of the 138 cases reviewed there were 51 cases of carditis (36.6 percent) and 87 cases of heart damage (63.4 percent). Carditis was associated with arthritis in 35.3 percent of cases. Antistreptolysin O levels were elevated in 74.5% percent of cases. Regarding Jones' criteria, the presence of a minor clini

Etiology and management of chronic valve disease in antiphospholipid antibody syndrome and systemic lupus erythematosus.

Unlike the Libman-Sacks vegetations of acute systemic lupus erythematosus (SLE), which are usually asymptomatic, valve involvement in chronic SLE and primary antiphospholipid antibody syndrome (APLAS) is similar to that of chronic rheumatic disease. Typical findings include valve thickening and nodularity, poor coaptation, and regurgitation. Elevated levels of antiphospholipid antibodies have been associated with the development of these valvular abnormalities in some but not all reported cases, and there are undoubtedly other etiologic cofactors. When cardiac valvular replacement is required,

Poststreptococcal reactive arthritis in children: a potential predecessor of rheumatic heart disease.

OBJECTIVE: To report several cases of arthritis seen in children after infection with Group A beta-hemolytic Streptococcus (GABHS) which were not associated with carditis or other major manifestations of the Jones Criteria for acute rheumatic fever (ARF); and to analyze the literature to determine these patients' potential risks for the subsequent development of rheumatic heart disease. METHODS: A retrospective chart review was performed of all patients seen in a pediatric rheumatology clinic from January, 1990 to December, 1992. RESULTS: Four patients were identified with poststreptococcal

Prevention of myocardial infarction by vitamin B6.

Vitamin B6 is effective in the treatment of carpal tunnel syndrome and related disorders in patients with vitamin B6 deficiency. Hyperhomocysteinemia, a risk factor for atherosclerosis, is associated with deficiencies of vitamin B6, folate, and cobalamin. Patients who were given vitamin B6 for carpal tunnel syndrome and other degenerative diseases were found to have 27% of the risk of developing acute cardiac chest pain or myocardial infarction, compared with patients who had not taken vitamin B6. Among elderly patients of the author (JE) expiring at home, the average age at death from myocard

Treatment of acute streptococcal pharyngitis and prevention of rheumatic fever: a statement for health professionals. Committee on Rheumatic Fever, Endocarditis, and Kawasaki Disease of the Council on Cardiovascular Disease in the Young, the American Heart Association.

Primary prevention of acute rheumatic fever is accomplished by proper identification and adequate antibiotic treatment of group A beta-hemolytic streptococcal (GAS) tonsillopharyngitis. Diagnosis of GAS pharyngitis is best accomplished by a throat culture. Penicillin (either oral penicillin V or injectable benzathine penicillin) remains the treatment of choice, because it is cost effective, has a narrow spectrum of activity, has long-standing proven efficacy, and GAS resistant to penicillin have not been documented. Various macrolides, oral cephalosporins, and other beta-lactam agents are acce

[An approach to the diagnosis of recurrent attack of rheumatic fever in patients with rheumatic heart disease].

In order to evaluate recurrent attack of rheumatic fever (RARF) in patients with rheumatic heart disease (RHD), 97 patients with diagnosis of RHD for two years and more were included in this study. Progression of the lesions of cardiac valves was found in every case by comparing the echocardiographic and Doppler findings at present and two years ago. The history of the initial attack of acute rheumatic fever or the appearance of RHD were reviewed. The present complaints at admission and necessary laboratory procedures were studied. The data showed that 75 of the 97 patients had history of init

Partial mitral homograft: a new technique for mitral valve repair.

Partial mitral homograft was used in 22 patients with localized lesions contraindicating a conventional valve repair. The etiologies of valve disease were: calcified rheumatic stenosis (n=14) and acute bacterial endocarditis (n=8). One patient died three months after surgery from cancer. Another patient required reoperation for residual stenosis 14 months postoperatively. All other patients had an excellent functional result with 19 patients in sinus rhythm. In this series, partial homograft replacement of the mitral valve significantly extended the possibilities of reconstructive surgery.

Cloning and sequence analysis of a gene encoding a 67-kilodalton myosin-cross-reactive antigen of Streptococcus pyogenes reveals its similarity with class II major histocompatibility antigens.

The group A streptococcal sequela acute rheumatic fever (ARF) has been associated with immunological cross-reactivity between streptococcal and heart proteins. To identify Streptococcus pyogenes genes that encode a myosin cross-reactive antigen(s) recognized by ARF sera, a genomic library from an emm deletion strain (T28/51/4) was screened with a single ARF serum. A positively identified lambda EMBL3 clone (T.2.18) produced a protein which reacted with myosin-specific antibodies affinity purified from individual ARF sera. The recombinant protein was initially estimated to be 60 kDa in size by
