[Heart antibodies in sudden death].

Anti-heart antibodies detected by indirect immunofluorescence were found in the blood of persons deceased suddenly of heart failure more often (36,7%) than in those who died of other causes (16,6%). Antibodies were found the most frequently in rheumatic heart failure (50%), in other heart diseases (50%), and in ischemic heart disease (35,5%). In ischemic heart disease there was a high level of antibodies in patients with chronic heart aneurysm (66,6%) as compared with low level in those with acute infarction (16,6%). Obviously, antiheart antibodies often accompany a heart disease without being

The antistreptococcal immune responses in children with acute rheumatic fever in Kuwait.

The antistreptococcal immune responses in 130 children with Acute Rheumatic Fever (ARF), seen in Kuwait, over a period of three years, are presented. The ages of children ranged between three to 13 years. One hundred and three presented in initial attacks while 27 presented in recurrences. The geometric mean of antistreptolysin 'O' (ASO) and antihyalurodinase (AH) titres for all patients were 488 Todd Units and 595 units, respectively. In children under the age of eight years, but not above this age, the geometric mean of ASO titres was significantly higher in children with carditis than in th

Coronary embolism in valvular heart disease.

Coronary embolism is considered to be rare but recent evidence suggests that it may be underdiagnosed, and implicated in acute myocardial infarction associated with angiographically normal coronary arteries. Twenty-six patients were studied. In six, coronary embolism was a primary cause of death confirmed at autopsy. In 20 patients, 23 episodes of coronary embolism were diagnosed clinically. The left coronary system was occluded in 65 per cent, transient electrocardiographic changes occurred in 30 per cent, and either no history or an atypical history of acute myocardial infarction occurred in

Diagnostic criteria of rheumatic fever in a New Zealand community.

The diagnostic criteria of acute rheumatic fever are reviewed from experience in a New Zealand community. Two-hundred-and-sixty-one first attacks labelled rheumatic fever and 209 other episodes occurred in a defined geographic population during 1962--76. The major importance of heart, joint and brain manifestations is confirmed. Criterior levels for fever, P-R interval prolongation, erythrocyte sedimentation rate, white cell count and antistreptolysin-O titre are suggested. Nodules and evidence of established rheumatic heart disease were not found to be of diagnostic significance because they

Clinical appraisal of the antideoxyribonuclease-B (ADNB) by means of streptonase-B test (Wampole).

Titers of antideoxyribonuclease-B (ADNB, by Streptonase-B test), antistreptolysin-O (ASO, by Rantz-Randall's micromethod) and Kinase test (antivaridase agglutination test) were performed on 118 sera of patients with suspected streptococcal infection and compared. Between ASO and ADNB, a fairly significant correlation was seen (r = 0.69), but many cases in which ADNB exceeded ASO were observed. Between Kinase and ADNB, no significant correlation was seen (r = 0.49). Successive determination of antibody titers were made in 14 consecutive patients with acute glomerulonephritis, anaphylactoid purp

[Echocardiographic study of left ventricular motion in cases with acute left ventricular diastolic volume overload(author's transl)].

M-mode and two-dimensional echocardiography were performed to investigate the motion of the interventricular septum (IVS) and posterior wall (PW) in cases with pure mitral (MI) or aortic (AI) insufficiency. Subjects were classified into four groups; 15 cases with chorda rupture or floppy mitral valve (acute MI), four with AI of acute onset (acute AI), 17 with chronic MI including rheumatic MI, mitral valve prolapse syndrome, and 11 with chronic AI. There wee no differences of left ventricular dimension (LVDI), stroke volume (SV), ejection fraction (EF) and cardiac output (CI) between acute and

Childhood rheumatic heart disease in Nigeria.

Eighty Nigerian children (mean age = 8.8 years) with rheumatic heart disease (RHD) were studied prospectively. The initial acute rheumatic fever (ARF) was recognized in 34 of them (42%) while 18 patients (23%) had recurrent ARF. 76 patients (95%) had mitral incompetence, either alone or in combination with other lesions. Symptomatic mitral stenosis was present in 21 patients (26%), occurred early in life, and appeared to be causally related to recurrent ARF and intense host reactions. Aortic and tricuspid valve diseases were uncommon and pulmonary valve disease was rare. 94% of the patients pr

[Prevention of streptococcal infections and post-streptococcal complications].

Starting from the particularities of the streptococcal infections, and the possibilities for fighting acute forms and the immediate and late complications, multiple aspects are discussed that should be implied by the prophylaxis of this infection in the present stage. Aspects of primary prophylaxis are treated separately, that are related to the limiting of the circulation of the etiologic agent, especially in view of preventing the epidemic forms of the disease. Also aspects of secondary prophylaxis are discussed, aimed at preventing streptococcal re-infections in subjects in whom complicatio

[Progress in the therapy of acute and chronic cardiac insufficiency by means of systemic vasodilators. Studies with prazosin and nitroglycerin].

In acute and chronic left heart failure peripheral resistance is elevated due to increased sympathetic tone. This should compensate the decrease in stroke volume. In the diseased left ventricle however the augmentation of afterload leads to further reduction of stroke volume and to increase of heart size and myocardial oxygen consumption. This vitious cycle may be interrupted by vasodilators. Drugs like nitroglycerin, mainly acting on the venous system, reduce preload and thereby relieve symptoms of pulmonary congestion (backward failure). Phentholamin on the other hand primarily reduces after

Significance of measurement of antihyaluronidase activity in rheumatic fever.

In recent years, rheumatic fever seems to have decreased but rheumatic heart disease with latent onset appears to persist in similar frequency. Measurement of AH along with ASO is required in rheumatic heart disease with latent onset, for more definite diagnosis, therapy and suggestion for the mode of living. For the diagnosis of rheumatic fever in acute stage, serological reactions for hemolytic streptococcus other than ASO should be done along with ASO. In our cases, ASO titer was high in 84.5% of cases, but either ASO or AH titer was elevated indicating positive reaction for hemolytic strep

Jaccoud's arthropathy in patients with chronic rheumatic valvular heart disease.

In a prospective study over a period of 3 yr, involving all patients with rheumatic valvular heart disease seen at our hospital, we found the diagnostic signs of Jaccoud's arthropathy in 17 of 400 cases (4.2%). All 17 patients had a past history of acute arthritis involving the joints and showed deformities at the time of diagnosis. The valvular lesions were mitral and aortic in 11 cases, mitral in 5 cases and aortic in 1 case. The most frequent joint deformities were: ulnar deviation at the metacarpo-phalangeal joints (12 cases), lateral deviation at the metatarso-phalangeal joints (12 cases)

Arrhythmias and Stokes-Adams attacks in acute rheumatic fever.

In spite of general complacency about first-degree heart block in acute rheumatic fever, abnormal conduction with dysrhythmias, occasional complete heart block, and, rarely, Stokes-Adams attacks are important early signs of acute rheumatic fever and may precede other signs. Every person with episodic fainting is entitled to an ECG, and frequent ECGs are imperative in any case of rheumatic fever with signs of arrhythmias. Changing atrioventricular block necessitates continuous monitoring for dysrhythmias. A 13-year-old boy who appeared with Stokes-Adams attacks secondary to acute rheumatic feve

Aschoff bodies at necropsy in valvular heart disease. Evidence from an analysis of 543 patients over 14 years of age that rheumatic heart disease, at least anatomically, is a disease of the mitral valve.

Among 543 necropsy patients over age 14 years with severe chronic valvular heart disease, Aschoff bodies were found in 11 patients (2%). The ages of the 11 patients ranged from 18 to 68 years (avg. 38), and nine had had a history of acute rhematic fever earlier in life. Clinically, nine of the 11 patients had mitral stenosis with or without dysfunction of one or more other cardiac valves, one had isolated aortic regurgitation, and one had both mitral and aortic regurgitation. All 11 patients had diffuse fibrous thickening of the mitral valve leaflets, and all but one had diffuse anatomic lesio

HLA phenotypes in patients with rheumatic fever, rheumatic heart disease, and Yersinia arthritis.

HLA phenotypes were determined in 109 patients with rheumatic fever (RF), 48 patients with Yersinia arthritis (YA), 86 patients with chronic rheumatic heart disease (RHD), and 326 controls. There was an increased frequency of Bw35 in RF as compared to controls (Pc less than 0.01), while B18 was more common in patients with acute carditis than in those without (P less than 0.02). HLA frequencies in RHD did not differ significantly from those in controls. A significant correlation between B27 and YA was observed (Pc less than 0.001). Carditis or iritis occurred in 10 of 31 B27 positive YA patien

[Cardiovascular signs in drepanocytosis. Apropos of 12 cases].

Sickle cell disease is the commonest of the haemoglobinopathies. The pathophysiology consists on the one hand of chronic haemolytic anaemia whose course is punctuated by acute crises with pain, fever and severe haemolysis, and on the other of a tendency to microthrombotic accidents. Of all the very varied clinical features of the disease, cardiac signs are both common and variable. The authors report a series of 92 cases, and compare their findings with those in the literature.

Group A streptococcal antigens cross-reactive with myocardium. Purification of heart-reactive antibody and isolation and characterization of the streptococcal antigen.

Heart-reactive antibody (HRA) appears in the sera of experimental animals inoculated with group A streptococci as well as patients with acute rheumatic fever. Adsorption of either serum with group A streptococcal membranes will remove the HRA. Blocking experiments between these two types of HRAs have demonstrated that the antibodies are directed towards different antigenic determinants on either the same or different molecules. To isolate and purify the antigen from the group A streptococcus cross-reactive with sarcolemmal sheaths of cardiac myofibers, it became necessary to purify the HRA fro

Pathology of chronic rheumatic heart disease with particular reference to tricuspid value involvement.

This study is based on a retrospective analysis of 144 autopsied cases of rheumatic heart disease. Majority of cases (79%) were chronic and 21% were acute in nature. The mortality was high and was maximum below the age of 30 years. The males predominated over females. A high incidence of organic tricuspid valve involvement was observed (45.8%). Tricuspid stenosis was observed in 3 cases. Latter was associated with aortic and mitral valve involvement. Mitral stenosis was severe in all the three cases. Criteria, both gross and microscopic for the involvement of the tricuspid valve have been desc

Anti-heart autoantibodies in ischaemic heart disease patients.

One hundred and ninety-nine ischaemic heart disease (IHD) patients were studied with regard to the prevalence of anti-heart autoantibodies (AHA). The incidence of AHA in IHD patients was 1%: one out of 102 patients who suffered acute myocardial infarction (AMI), one out of seventy-two patients who suffered from acute coronary insufficiency (ACI), and none out of twenty-five patients with other signs and symptoms of IHD, had AHA in their sera. An additional 2% of patients who suffered from AMI developed detectable antibody levels during a follow-up period of 15 days. In comparison,, 53% of pati

Epidemiology of diseases in adult males with leukemia.

In the Tri-State Leukemia Survey, the history of diseases in 605 adult male leukemia cases 15 years and older and in 668 adult male population controls was examined. These diseases occurred at least 1 year before leukemia was diagnosed. The data were based on respondents' answers that the disease was diagnosed by a physician; the respondent was either the subject or his spouse. Of 30 diseases studied, 7 showed an excess among the patients with leukemia: infectious hepatitis, eczema, psoriasis, diabetes, arthritis and rheumatism, heart disease, and ankylosing spondylitis. Mumps had a lower repo
