[Rupture of the mitral chordae tendinae].

Twenty-seven cases of ruptured chordae tendineae have been discovered during surgery for mitral regurgitation (9,3 %) : the highest incidence of ruptured chordae tendineae has been found among pure mitral insufficiency (36 %). In thirteen cases, the rupture was isolated, without any other valvular lesion. The syndrome described as characteristic of rupture was present in one third of our patients : isolated cases do not differ clinically from the others but for a more frequent acute evolution. In pure or predominant mitral regurgitation, surgery seems needed when clinical aggravation, acute or

Echocardiographic demonstration of early mitral valve closure in severe aortic insufficiency. Its clinical implications.

Severe aortic insufficiency may accelerate mitral valve closure. We noted this echocardiographic finding in several patients with the acute onset of severe aortic insufficiency. Accordingly, we examined our total echocardiographic series retrospectively for early closure of the mitral valve (ECMV) in the setting of aortic insufficiency and found it in 11 of 53 patients with confirmed aortic insufficiency. During our study ECMV was fortuitously found in two other patients without aortic insufficiency, ECMV occurred in late diastole following the echocardiographic "A" wave, often associated with

[HL-A B27 associated rheumatic disease].

Report on a HL-A B27 positive female patient with the typical cardiac lesion occasionally found inankylosing spondylitis, peripheral arthritis, and acute anterior uveitis but without clinical or radiological evidence of spine or sacroiliac joint involvement. The concept of "HL-A B27 associated disease", including ankylosing spondylitis as well as Reiter's disease or other forms of seronegative rheumatic diseases, is suggested.

Rheumatic fever in adult patients.

A study of rheumatic fever in adult patients (diagnosed according to the Jones revised criteria; Circulation 36: 664, 1965) was performed by examining 26 patients during the acute phase of the disease, and by re-examining 22 of them from 6 months to 64 months later (mean 33 months). Polyarthritis and carditis were the most common major manifestations. 23 patients had polyarthritis and 3 monoarthritis. 18 patients had signs of acute cardiac involvement. Carditis appeared to be rather benign: at the acute stage no patient developed congestive heart failure or any other serious complication. Five

An outbreak of streptococcal sore throat and rheumatic fever in a Royal Air Force training camp; significance of serum antibody to M-associated protein.

A large outbreak of streptococcal sore throat in a Royal Air Force Training Camp resulted in five cases of rheumatic fever among the 16- to 18-year-old apprentices, and one case in a 33-year-old airman. The most prevalent type of group A streptococcus isolated from throat swabs was M-type 5 and there was serological evidence that at least four of the rheumatic fever (R.F.) cases were due to this type.Among the patients with uncomplicated throat infection the anti-streptolysin O (ASO) and anti-deoxyribonuclease B (anti-DNAase B) responses were in general rather low, even where there was evidenc

Carditis in acute rheumatic fever in Varanasi, India.

In Varanasi carditis is the most important major manifestation of an acute attack of rheumatic fever and is the only one which results in sequelae. In our country carditis constitutes the largest single manifestation of rheumatic fever in its index attack. The overall picture is most distressing with its high incidence (78%), and severity as evidenced by high mortality (70%) in the initial phase. It is necessary to provide institutional treatment for those seriously infected to ensure them the fairest chance of recovery from their carditis without permanent damage to the heart. The diagnosis o

Heart-reactive antibody associated with rheumatic fever: characterization and diagnostic significance.

The sera of patients with acute rheumatic fever, uncomplicated streptococcal infections and post-streptococcal glomerulonephritis contain a heart-reactive antibody which binds to the sarcolemmal membrane of myofibrils of heart muscle as well as to other mammalian muscle tissue. The sera of patients with acute rheumatic fever have about four times as much of this antibody as is found in sera of patients convalescent from uncomplicated streptococcal infections or acute post-streptococcal glomerulonephritis. This antibody is not present in a number of other diseases, including idiopathic myocardi

Persistence of streptococcal group A antibody in patients with rheumatic valvular disease.

Antibody levels to streptococcal Group A and A-variant carbohydrates were determined using a radioactive immune precipitation technique on patients with rheumatic fever, with and without valvular disease, on patients with post-streptococcal acute glomerulonephritis, and on age-matched controls. During the acute phase of the above illness, the means of the antibody levels to both carbohydrate antigens were equally elevated and were significantly higher than the normal controls. When Group A antibody levels were determined on sera obtained at intervals of 5-12 months and 1-5 yr after the acute i

IMMUNOLOGIC RELATION OF STREPTOCOCCAL AND TISSUE ANTIGENS. III. PRESENCE IN HUMAN SERA OF STREPTOCOCCAL ANTIBODY CROSS-REACTIVE WITH HEART TISSUE. ASSOCIATION WITH STREPTOCOCCAL INFECTION, RHEUMATIC FEVER, AND GLOMERULONEPHRITIS.

Sera from patients with recent streptococcal infection or non-suppurative sequelae exhibit with variable frequency a precipitin reaction in agar gel with a partially purified streptococcal antigen which has been shown previously to be immunologically related to human heart tissue. This precipitin could be absorbed from sera with normal human heart tissue homogenates but not with homogenates of other organs. Demonstration of this cross-reaction by heart absorption was found dependent both upon the serologic properties of individual sera and the nature or state of purification of the streptococc

Immunologic studies of heart tissue. IV. Serologic reactions with human heart tissue as revealed by immunofluorescent methods: isoimmune, Wassermann, and autoimmune reactions.

Reactions of normal and pathologic sera with heart tissue have been investigated by the immunofluorescent method, with particular reference to presumptive autoantibodies to heart and their differentiation from blood group isoantibodies and Wassermann antibody. In the heart, blood group substances A and B were found distributed in capillary walls, vascular endothelium, and interstitial connective tissue. In surveys of randomly selected sera, isoimmune reactions against tissue blood group substances A and B were noted infrequently. This finding was considered related to the limited sensitivity o

STUDIES ON THE IMMUNE RESPONSE OF THE RHEUMATIC SUBJECT AND ITS RELATIONSHIP TO ACTIVITY OF THE RHEUMATIC PROCESS : III. OBSERVATIONS ON THE REACTIONS OF A RHEUMATIC GROUP TO AN EPIDEMIC INFECTION WITH HEMOLYTIC STREPTOCOCCUS OF A SINGLE TYPE.

This study of an isolated colony showed that of seven children who escaped the epidemic streptococcus infection none developed rheumatic symptoms; and that of seventeen children who contracted the epidemic streptococcus infection, fourteen developed acute rheumatism and three showed no recognizable rheumatic manifestations. The seven children who failed to contract infection with Streptococcus hemolyticus showed clearly that susceptible individuals may live in dose association with an epidemic of acute rheumatism, develop no rise in antistreptolysin titer and maintain excellent health. The pat

STUDIES ON THE RELATIONSHIP OF STREPTOCOCCUS HEMOLYTICUS TO THE RHEUMATIC PROCESS : I. OBSERVATIONS ON THE ECOLOGY OF HEMOLYTIC STREPTOCOCCUS IN RELATION TO THE EPIDEMIOLOGY OF RHEUMATIC FEVER.

Certain factors of climate are favorable to streptococcus respiratory diseases. In those tropical environments where hemolytic streptococcus is unusual in the throat flora, scarlet fever is unknown and rheumatic fever rare. In New York City, however, following epidemic waves of pharyngitis with hemolytic streptococcus the incidence of rheumatic fever rises precipitously. The correlation between the geographical distribution of hemolytic streptococcus and rheumatic fever is a definite one. Furthermore, in New York City during the seasons of the year in which hemolytic streptococcus is seldom re
