Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

INTRODUCTION: Cardiologists use several echocardiographic methods to evaluate the Mitral Stenosis (MS) severity. Determination of mitral valve area (MVA) by three- dimensional transesophageal echocardiography (3D TEE) planimetry has the best correlation with anatomic area of the mitral orifice and is probably the reference method. This study was designed to examine the relationship between mitral leaflet separation index (MLSI) measured by TEE which is a simple and accessible method and MVA calculated by 3D Multiplanar reconstruction (3D-MPR) planimetry in TEE as the reference but less accessi

BACKGROUND: After mitral valve replacement with preservation of the subvalvular apparatus, prosthetic dysfunction resulting from retained native structures is extremely rare. When it occurs, it may directly threaten the patient's life. We report an unusual case of severe regurgitation resulting from preserved chordal rupture. The key distinction of this case is the presentation of late, severe continuous prosthetic regurgitation and reversibility.
CASE PRESENTATION: A 54-year-old man had undergone double mechanical valve replacement for severe rheumatic mitral stenosis with mild regurgitation

Valvular heart disease is a major cause of cardiovascular morbidity and mortality worldwide. As the prevalence of most valvular heart diseases increases with age, population ageing has led to a substantial increase in disease burden. Due to advanced age or significant comorbidities, many patients are considered unsuitable for conventional surgery. Whereas medical therapy was therefore often the only treatment option in the past, the rapid evolution of transcatheter interventions has fundamentally transformed the management of valvular heart disease over the past two decades. Today, effective c

BACKGROUND: Valvular heart disease (VHD) lacks disease-modifying pharmacological therapies. Drug repurposing guided by human genetics may help identify therapeutic opportunities. We aimed to integrate human genetic associations, drug-target information, and clinical evidence to identify repurposing opportunities and prioritise potentially druggable targets across the VHD spectrum.
METHODS: We retrieved VHD-associated genetic variants from the NHGRI-EBI Genome-Wide Association Studies Catalogue and Ensembl, interrogated annotated genes in the Open Targets Platform, and evaluated linked drugs t

BACKGROUND: Transcatheter vacuum-assisted mass extraction (TVME) in the systemic circulation is increasingly being reported. However, its feasibility, effectiveness, and safety remain poorly defined.
AIMS: We sought to investigate the feasibility, effectiveness and safety of TVME in the left heart and aortic arch.
METHODS: Consecutive patients from 6 centres in the United States and Canada were included. Feasibility was defined as technically successful mass extraction without unplanned cardiac surgery or intraprocedural mortality. The effectiveness endpoint was feasibility with successful d

BACKGROUND: Atrial fibrillation (AF) in valvular heart disease (VHD) is generally attributed to pressure or volume overload with atrial remodeling, but the independent contribution of genetic predisposition remains unclear.
OBJECTIVES: The authors evaluated the predictive value of a previously developed AF polygenic risk score (PRS) across VHD subtypes in the UK Biobank.
METHODS: In this prospective cohort study (UK Biobank, 2000-2022), incident AF and VHD status were defined using ICD codes. VHD subtypes included mitral valve (MV) disease (MV prolapse [MVP], mitral regurgitation, mitral ste

Objective: To analyze the characteristics of disease burden and risk factors of acquired heart diseases in China from 1990 to 2023, predict their trends from 2024 to 2050, and provide data support for the development of prevention and control policies. Methods: Based on the Global Burden of Disease Study, we collected disability-adjusted life year (DALY) data of three types of acquired heart diseases (rheumatic heart disease, hypertensive heart disease, and ischemic heart disease) in China from 1990 to 2023. Joinpoint regression model was used to estimate the average annual percentage change (

BACKGROUND: Large-bore transvenous access for balloon mitral valvotomy (BMV) can be complicated by unrecognized iliofemoral venous disease, occasionally unmasking a congenital variant.
CASE SUMMARY: A 51-year-old woman with rheumatic mitral stenosis and NYHA functional class III-IV dyspnea, on acenocoumarol for atrial fibrillation, was scheduled for BMV. After right femoral venous access and transseptal puncture, hardware >8-F could not be advanced; venography showed a tight, recoiling right external iliac vein stenosis, not responding to intravenous nitroglycerine, treated with angioplasty a

BACKGROUND: Severe bioprosthetic valve dysfunction presents a diagnostic challenge because distinguishing reversible leaflet thrombosis from structural valve degeneration may not be possible using echocardiography alone.
CASE SUMMARY: A patient with end-stage renal disease, previous bioprosthetic mitral and aortic valve replacement, and prior valve-in-valve transcatheter aortic valve replacement presented with heart failure and cardiogenic shock. Multimodality imaging demonstrated severe bioprosthetic mitral valve dysfunction with restricted leaflet motion, raising suspicion for leaflet throm

BACKGROUND: Systemic inflammation in psoriasis accelerates atherosclerosis and confers excess cardiovascular risk that is not fully captured by traditional clinical predictors. Metabolomic profiling may identify disease-specific metabolic signatures associated with cardiovascular events and enhance risk prediction beyond conventional factors.
OBJECTIVES: To determine whether plasma metabolomic profiling improves cardiovascular risk prediction in patients with psoriasis and to develop and externally validate an artificial intelligence-driven metabolomic risk score (MetaPsoAI) for incident card

Dextrocardia with situs solitus poses significant surgical challenges for left ventricular assist device (LVAD) implantation, particularly in patients with prior sternotomy and complex valvular disease. We report on a 60 year old man with dextrocardia, advanced heart failure, prior mechanical mitral valve replacement, and bicuspid aortic valve disease who underwent successful Corheart 6 LVAD implantation during redo sternotomy with concomitant bioprosthetic aortic and mitral valve replacement. Intraoperative findings included extensive pannus/thrombus on the mechanical mitral prosthesis (causi

BACKGROUND: Mitral valve prolapse (MVP) is generally a benign valvular heart disease, but a subset of patients is at particular risk of malignant ventricular arrhythmias (AMVP).
OBJECTIVES: The authors evaluated the electromechanical window (EMW) as an arrhythmia-risk marker in (A)MVP (the sum of all MVP and AMVP patients).
METHODS: Healthy controls, patients with MVP without ventricular arrhythmia (MVP/-VA), and patients with AMVP with nonsustained (AMVP/nsVA) or sustained ventricular arrhythmia (AMVP/VA) (ventricular arrhythmia >30 seconds) were retrospectively included from 3 centers (der

BACKGROUND: Percutaneous debulking of left-sided endocarditis and thrombus has previously relied on cerebral-only embolic protection devices. The Emboliner is an investigational nitinol-mesh filter that provides whole-body embolic protection and has been studied in transcatheter aortic valve replacement.
SUMMARY: We describe its first reported use as an adjunct to AngioVac-based debulking in 2 patients: 1 with aortic valve endocarditis and 1 with combined tricuspid and mitral endocarditis.
DISCUSSION: The Emboliner was successfully deployed in both cases, with over 90% of vegetation removed

Group A streptococcus (GAS) infection is usually initiated by colonization of mucosal sites such as the pharynx, followed by invasion and systemic dissemination. Development of a vaccine that can prevent colonization is enticing as it would prevent subsequent infections and all associated autoimmune sequalae such as rheumatic heart disease (RHD). In this study we investigated the mucosal adjuvanticity of GMMA when used as a carrier protein for GAS carbohydrate (GAC) conjugate vaccine (GAC-GMMA). Mice were immunized sublingually or intranasally with GAC-GMMA or GAC-CRM197, in the presence or ab

BACKGROUND: Late clinical decline after heart transplantation is commonly attributed to rejection, cardiac allograft vasculopathy, or ventricular dysfunction. Progressive structural valvular disease is less frequently emphasized when graft systolic function is preserved.
CASE SUMMARY: A 22-year-old woman underwent orthotopic heart transplantation at age 13 for familial restrictive cardiomyopathy. Nearly 10 years later, she developed progressive exertional fatigue, reduced exercise tolerance, and dizziness despite preserved graft systolic function and no significant cardiac allograft vasculopa

BACKGROUND: Rheumatic pulmonary valve disease accounts for<1% of cases of rheumatic heart disease. Because of its rarity, this valvular disease is often overlooked and underdiagnosed.
AIM: To conduct a systematic review of reported cases of rheumatic pulmonary valve disease worldwide.
METHODS: Studies reporting confirmed cases of rheumatic pulmonary valve disease, and providing sufficient clinical, diagnostic, therapeutic or follow-up data were included. Duplicate checking, selection, data extraction and quality assessment of the included studies were carried out by two reviewers, independen

BACKGROUND: Contemporary data comparing double mechanical prostheses (DMP) vs. double biological prostheses (DBP) for concomitant aortic and mitral valve replacement are scarce, and the cumulative hemodynamic burden of simultaneous prosthesis-patient mismatch (PPM) at both positions has never been systematically assessed.
RESULTS: DMP recipients had lower in-hospital mortality (1.9% vs. 15.2%; P = 0.010) and superior 7-year survival (91.3% vs. 67.7%; P = 0.003), but significantly higher stroke rates (32.2% vs. 5.0%; P = 0.023). Combined PPM occurred in 64.6% of DBP vs. 14.8% of DMP (P < 0.001
