BACKGROUND: Plasma calprotectin is a marker of neutrophil activity. Its relation to the clinical phenotype and autoantibodies in rheumatoid arthritis (RA) is only partly understood. We therefore aimed to investigate the association between calprotectin and clinical parameters, CRP, ESR, anti-CCP2 and other anti-modified protein autoantibodies (AMPAs) in patients with RA.
METHODS: Rheumatoid factor (RF), anti-cyclic citrullinated peptide 2 (CCP2) isotypes and plasma calprotectin were centrally measured in a cross-sectional cohort of 4521 Scandinavian RA patients using fluoroenzyme immunoassays

Combined aortic stenosis (AS) and mitral stenosis (MS) present a challenging multivalvular disease scenario. Simultaneous transcatheter aortic valve replacement (TAVR) and percutaneous balloon mitral valvuloplasty (PBMV) has emerged as a potential option for selected high-risk patients, but the optimal procedural sequence remains unclear. We describe 3 patients with severe AS and MS who underwent combined TAVR and PBMV following heart team evaluation, with a focus on procedural sequencing. Postprocedural hemodynamics improved immediately, and short-term outcomes were satisfactory. Our experien

BACKGROUND: Severe mitral regurgitation (MR) associated with isolated double-orifice mitral valve (DOMV) in adults is uncommon, and both its diagnosis and surgical management remain challenging.
CASE SUMMARY: A 60-year-old woman presented with symptomatic severe MR and was scheduled for mitral valve repair. Transthoracic echocardiography suggested abnormal subvalvular morphology and demonstrated dual left ventricular inflow jets, raising suspicion for DOMV. Three-dimensional transesophageal echocardiography confirmed a complete-bridge DOMV. Intraoperatively, 2 independent parachute-like subva

BACKGROUND: Concomitant aortic and mitral regurgitation (CAMR) is a rare valvulopathy, seen in patients with scoliosis, and rarely involves aortic regurgitation with concomitant primary mitral regurgitation associated with poor prognosis when left untreated. Current guidelines lack robust evidence to guide the management of CAMR in complex anatomies such as extreme scoliosis.
CASE SUMMARY: We report the first case of a one-stage transcatheter intervention for CAMR in a 62-year-old man with extreme scoliosis. The patient underwent one-stage transapical mitral transcatheter edge-to-edge repair

INTRODUCTION: Sydenham's chorea (SC) is the most common acquired chorea of childhood and a major neurological manifestation of acute rheumatic fever. Although often considered self-limiting, it can cause substantial motor, psychiatric, cognitive, educational, and social morbidity, with relapses in a relevant proportion of patients.
AREAS COVERED: In this critical perspective, the authors critically examined current approaches to SC management, focusing on three complementary pillars: eradication of Group A β-hemolytic streptococcal infection and long-term secondary antibiotic prophylaxis; sym

Cardiovascular diseases (CVDs) are the primary reason for mortality worldwide. Patients with CVD were at higher risk of drug-related adverse events and functional impairment and were more likely to be admitted to the hospital. This study aims to study trends for CVD hospital admission (HA) in England and Wales during the past 21 years. A secular-trend analysis study design was utilized to explore the trend of CVD HAs. Poisson regression was used to examine the trend in HAs. To determine trends in admission rates over time, we implemented Joinpoint regression analysis. The permutation test was

BACKGROUND: Congenitally corrected transposition of the great arteries (CC-TGA) can lead to severe systemic tricuspid regurgitation (TR). There is limited evidence on transcatheter tricuspid valve replacement (TTVR) in these patients.
CASE SUMMARY: A 59-year-old woman with CC-TGA presented with symptomatic severe systemic TR. She was at high risk for surgery, and transcatheter interventions were considered by our heart team. TTVR on the systemic tricuspid valve was performed using a Sapien M3 valve (Edwards Lifesciences), and the systemic TR was eliminated. There were no complications, with s

OBJECTIVE: Valvular heart diseases occur following different evitable and inevitable etiologies. Rheumatic valvular heart disease is among preventable etiologies and is almost disappeared in developed nations. In recent literature there are few low-quality evidences available about therapeutic efficacy of penicillin in maintaining and enhancing myocardial function in patients with rheumatic heart disease. In present study we aimed to investigate importance of penicillin in maintaining myocardial function indices within normal range.
METHODS: In this retrospective descriptive-analytic study we

Thrombocytopenia developing after apparent resolution of infective endocarditis (IE) is uncommon. We report the case of a 61-year-old woman with severe rheumatic mitral stenosis and native mitral valve IE caused by Lactococcus garvieae. Her platelet count remained normal during antibiotic therapy but declined to 46-84×103/μL during routine outpatient follow-up after discharge. Follow-up echocardiography showed resolution of the vegetation, and inflammatory markers had normalized; however, corticosteroid treatment did not improve the platelet count. Elective surgery was performed for severe mit

BACKGROUND: Studies in adults with chronic rheumatic heart disease (RHD) suggest persistence of elevated inflammatory markers, indicating a potential role of ongoing inflammation in disease chronicity. However, similar data in the pediatric population are lacking.
METHODS: A cross-sectional observational study was conducted in the Division of Pediatric Cardiology at a tertiary care hospital in North India from August 2022 to February 2024. Forty-three patients with RHD <18 years were enrolled. Comprehensive history taking and clinical examinations were conducted. Blood samples were collected

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and

Pregnancy imposes significant hemodynamic changes that can unmask previously undiagnosed cardiovascular disease and complicate management when concurrent obstetric conditions are present. We present the case of a 32-year-old immigrant woman at 21 weeks' gestation with a limited medical history due to a significant language barrier, who presented with hypertensive crisis and was diagnosed with preeclampsia with severe features. During her evaluation, she developed worsening dyspnea and acute respiratory decompensation. Further cardiac assessment revealed critical rheumatic mitral stenosis, acco

Rheumatic heart disease remains a leading cause of preventable cardiovascular morbidity and premature mortality among children and young adults in Sub-Saharan Africa. Advances in portable echocardiography, standardised World Heart Federation diagnostic criteria, and task-sharing strategies have substantially expanded the detection of latent rheumatic heart disease. However, increased diagnostic capacity has not consistently translated into improved long-term clinical outcomes. This viewpoint argues that the principal challenge in rheumatic heart disease control has shifted from underdiagnosis

BACKGROUND: Acute rheumatic fever (ARF) is an immune-mediated sequela of group A streptococcal infection that can progress to rheumatic heart disease (RHD), defined by permanent rheumatic valvular damage. Although uncommon in the United States, delayed recognition may lead to advanced valve disease and arrhythmias.
CASE SUMMARY: A 10-year-old African-American male presented after syncope following 4 months of unexplained bilateral ankle arthritis. He was found to be in atrial flutter. Echocardiography showed a flail mitral valve with severe regurgitation, moderate tricuspid regurgitation, and

Over 300,000 people die of sudden cardiac death (SCD) in the US annually. Implantable cardioverter-defibrillators (ICDs) have been shown to be more effective than antiarrhythmic drugs for the prevention of SCD in specific susceptible populations. Many patients in whom ICDs have been implanted receive concomitant therapy with antiarrhythmic drugs, for the purpose of reducing the frequency of appropriate and inappropriate defibrillation shocks. Drugs may influence defibrillation capacity and therefore influence the function of ICDs. The objective of this article is to review and update the liter

Balloon-expandable Edwards SAPIEN 3 (23 mm) and MyVal OCTACOR (21.5 mm) valves were tested in 3D-printed silicone tubes using a low-pressure, water-perfused simulator with ultrasound and video monitoring. Balloons were inflated in 2 mm increments from 18 to 30 mm. Both valves were easily inflated from 18 to 24 mm with 2-3 atm inflation pressures and required 5.5 atm and 7 atm, respectively, to reach an inner diameter of 26 mm at the outflow level for the SAPIEN 3 and OCTACOR valves. Further expansion was not achievable. At 18 mm, leaflet opening was mildly asymmetric, but laminar flow was pres

Melatonin is a product of the amino acid tryptophan in the pineal gland. Once synthesized, the specific mechanisms governing the release of melatonin from the pineal gland and its functions are largely unknown. Besides its regulatory role in circadian rhythms in mammals, because of its widespread subcellular distribution, melatonin contributes to the reduction of oxidative damage in both the lipid and the aqueous environments of the cell. This postulate is widely supported by the experimental observations showing that melatonin protects lipids in membranes, proteins in the cytosol, and DNA in

Background Patients with mechanical heart valves require lifelong anticoagulation with vitamin K antagonists (VKAs), such as acenocoumarol. However, acenocoumarol has a narrow therapeutic index, and genetic variants in CYP2C9, VKORC1, and CYP4F2 significantly influence individual dose requirements and the risk of adverse events. Limited data exist on the prevalence of these clinically important variants in Central Indian populations. Objectives To determine the frequency of clinically significant genetic variants, alleles, and metabolizer phenotypes of the CYP2C9, VKORC1, and CYP4F2 genes, and
