Strengthening non-communicable disease health systems through sustainable health partnerships in the Horn of Africa using lessons from the Wolverhampton-Somaliland Global Health Partnership.

BACKGROUND: The purpose of our study was to assess the effect of hypolipidemiant drugs on serum markers of vascular inflammation (E-Selectin, VCAM-1 and MCP-1) in dyslipidemic men without cardiovascular disease. MATERIAL/METHODS: 84 dyslipidemic men were consecutively recruited from the Lipid Unit of a tertiary hospital. The patients were placed on statins (n=44) or fibrates (n=22), depending on the lipid profile, for 4 months. In the control group (n=18), a hypolipidemiant diet alone was indicated. RESULTS: Baseline levels of VCAM-1 and MCP-1 were not correlated with the lipid profile. By c

Malignant peripheral nerve sheath tumor presenting as mitral stenosis: case report and literature review.

Primary cardiac neurogenic tumors are rare, accounting for 5%-10% of all malignant soft tissue tumors. Among these, malignant peripheral nerve sheath tumors (MPNST) are particularly uncommon as primary cardiac tumors. This case involves a 55-year-old female with a history of type II diabetes mellitus and hypertension, who presented with dyspnea on exertion and episodes of presyncope. Initial evaluations, including chest X-ray, electrocardiography, and laboratory tests, showed no significant abnormalities. However, a transthoracic echocardiogram revealed a large mass attached to the mitral valv

Successful double valve replacement with extensive annular enlargement using the Commando procedure in a patient with small aortic and mitral annuli after previous double valve surgery.

We report a 70-year-old woman (body surface area 1.42 m2) undergoing redo double valve replacement with extensive annular enlargement using the Commando procedure. Thirteen years earlier, she received aortic valve replacement (St. Jude Medical 19 mm), mitral repair (Physio Ring 24 mm), and tricuspid annuloplasty. She presented with progressive heart failure; echocardiography showed severe mitral stenosis (mitral valve area 0.96 cm2) and moderate aortic stenosis with moderate paravalvular leak. To avert patient-prosthesis mismatch (PPM), both annuli were enlarged with bovine pericardium, permit

Orthotopic heart transplantation for Fontan failure: experience and treatment strategy-a case report.

BACKGROUND: Orthotopic heart transplantation after the Fontan operation presents technical surgical challenges due to the connection of systemic veins to pulmonary arteries and well-developed systemic-to-pulmonary collateral arteries. The altered anatomy and hemodynamics necessitate extensive vascular reconstruction. We report a successful orthotopic heart transplantation with three years of ventricular assist device (VAD) support in a child who had undergone the Fontan operation. CASE PRESENTATION: A 10-year-old boy had undergone extracardiac total cavopulmonary connection (18 mm expanded po

The Incremental Role of Stress Echocardiography in Valvular Heart Disease: A Narrative Review.

Background/Objectives: The diagnosis and risk stratification of valvular heart disease have traditionally relied on resting echocardiography. However, in a significant portion of patients, resting findings do not fully reflect the hemodynamic severity of the condition, particularly in asymptomatic individuals with severe valvular disease or those with nonspecific symptoms. In this context, stress echocardiography emerges as a vital imaging modality, providing a dynamic assessment of valvular, ventricular, and pulmonary function under hemodynamic load (from physical exercise or pharmacological

Adult Presentation of Congenital Mitral Stenosis: The Challenges of a True Parachute Mitral Valve.

BACKGROUND: Parachute mitral valve (PMV) is a rare congenital anomaly where all chordae tendineae insert into a single papillary muscle, causing stenosis or regurgitation. Adult presentations are uncommon and often underdiagnosed. CASE DESCRIPTION: A 35-year-old male presented with exertional dyspnea. Echocardiography demonstrated severe mitral stenosis, reduced ejection fraction, a bicuspid aortic valve, and pulmonary hypertension. Intraoperative findings confirmed true PMV. Due to anatomical complexity, a 27-mm mechanical prosthesis replacement was performed successfully, with rapid postope

Focused cardiac ultrasound conducted by neurologists in patients with stroke: A validation study.

INTRODUCTION: Focused cardiac ultrasound (FoCUS) has a high diagnostic yield and a rapid theoretical learning curve. FoCUS can be applied in stroke assessments performed by stroke neurologists when a cardioembolic stroke is suspected. PATIENTS AND METHODS: An international multicenter, prospective validation study was conducted to assess neurologists' ability to perform FoCUS. The FoCUS examination was defined as a simplified 2D transthoracic echocardiography. Neurologists and cardiologists performed the FoCUS independently and blinded. A twenty-question test evaluated neurologists' ability t

Mitral valve prosthesis endocarditis unveiling metastatic colorectal cancer as the primary infection source: a case study.

BACKGROUND: This case report is significant due to its illustration of how infectious endocarditis on a prosthetic mitral valve revealed an underlying metastatic colorectal cancer. Although uncommon, associations between cardiac infection and advanced malignancy, particularly with malignancy serving as the source of infection, have been documented in the literature, making this case clinically significant and complex. CASE PRESENTATION: A 54-year-old postmenopausal woman with insulin-dependent diabetes and poorly managed hypertension, who had previously undergone mitral valve replacement with

Net Atrioventricular Compliance: A Determinant of Pulmonary Artery Pressure in Patients Undergoing Mitral Valve Replacement for Severe Mitral Stenosis.

. This single-center prospective observational study aimed to assess the correlation of net atrioventricular compliance (Cn) with pulmonary artery pressure in 26 adult patients with severe mitral stenosis (MS) undergoing surgical mitral valve replacement (MVR). Cn was estimated by doppler echocardiography, and pulmonary artery pressures measured by pulmonary artery catheter. Early outcomes including duration of intensive care unit (ICU) stay, mechanical ventilation duration, and vasoactive inotropic scores (VIS) were recorded. Cn showed moderate to strong negative correlation with systolic, di

"Flying Saucer"-Like Morphology in Infective Endocarditis Arising on a Stenotic Mitral Valve.

BACKGROUND: Infective endocarditis arising on a stenotic mitral valve is uncommon, and it may present with atypical morphology. CASE SUMMARY: We report a case of infective endocarditis in a patient with mild to moderate degenerative mitral stenosis who developed an extensive, broad-based vegetation nearly occluding the mitral valve, with an overlying contiguous aneurysm-like cystic structure. Transthoracic echocardiography demonstrated an elevated transmitral gradient, while transesophageal echocardiography revealed hemodynamic communication between the cystic structure and the left ventricle

The Importance of Lung Ultrasonography in the Assessment of Severity of Mitral Stenosis.

BACKGROUND: This study investigated the relationship between mitral stenosis (MS) severity and B-line counts on lung ultrasonography (US) and their predictive value for PMBV requirement. METHODS: A total of 105 patients (70 MS, 35 controls) underwent transthoracic echocardiography (ECHO) and lung US using a 28-point protocol. Patients performed an exercise test to 85% of their predicted heart rate, followed by repeat imaging. B-line changes were analyzed by MS severity and symptoms. RESULTS: MS patients had significantly more B-lines than controls (p < 0.001). No significant difference was f

First-in-Human Use of Intravascular Lithotripsy to Facilitate Mitral Balloon Commissurotomy After Prior Annuloplasty Band Repair.

BACKGROUND: Patients with severe mitral stenosis (MS) after surgical repair often face limited therapeutic options owing to high surgical risk and complex valve anatomy. CASE SUMMARY: A 79-year-old woman with prior surgical mitral valve repair presented with severe MS. Given her high surgical risk and extensive valvular calcification, she was not a candidate for redo surgical mitral valve, percutaneous mitral balloon commissurotomy (PMBC), or transcatheter mitral valve-in-ring. Intravascular lithotripsy (IVL)-facilitated PMBC was performed, with postprocedural improvement in gradients and sym

Vasopressin in cardiogenic shock: pathophysiology, clinical evidence, and therapeutic perspectives.

Vasopressin is a non-adrenergic vasopressor, commonly used in distributive shock, that may offer therapeutic benefits in selected phenotypes of cardiogenic shock. This review presents a comprehensive synthesis of its pharmacologic mechanisms, haemodynamic effects, clinical applications, and potential adverse outcomes in this complex setting. We delineate clinical scenarios in which vasopressin may be advantageous, critically appraise the current evidence, and support a physiology-guided, individualized approach to treatment. Patient profiles that may particularly benefit include advanced or mi

A Very Late Mitral Valve Hypoattenuated Leaflet Thickening With Significant Warfarin Response.

BACKGROUND: Hypoattenuated leaflet thickening (HALT) represents subclinical leaflet thrombosis in bioprosthetic valves with risk of occurrence peaking at 12 to 24 months after implantation. However, late presentations can occur. CASE SUMMARY: A 77-year-old woman developed severe mitral stenosis 17 years after receiving a bioprosthetic mitral valve. Cardiac CT revealed extensive hypoattenuated leaflet thrombosis affecting all leaflets. After 3 months of therapeutic warfarin, repeat imaging demonstrated significant HALT resolution, avoiding valve-in-valve intervention. DISCUSSION: This represe
