Timeless therapeutics: cardiac indications for vitamin K antagonists in clinical practice.

Anticoagulation is a critical component in the treatment of cardiovascular diseases. Large randomised controlled trials have demonstrated that direct oral anticoagulants (DOACs) are effective and safe for stroke prevention in patients with atrial fibrillation (AF) and treatment or prevention of venous thromboembolism. Nevertheless, for specific cardiac indications and patient characteristics, vitamin K antagonists (VKAs) rather than DOACs are either preferred, or the benefit of DOACs in comparison to VKAs remains uncertain. These include, among others, mechanical heart valves, AF associated wi

Resolution of Ortner's Syndrome Following Successful Mitral Transcatheter Edge-to-Edge Repair: The First Case Report.

Ortner's syndrome (OS), or cardiovocal syndrome, is an uncommon cause of hoarseness caused by recurrent laryngeal nerve palsy due to compression from underlying cardiovascular disease. It is classically associated with left atrial enlargement due to rheumatic mitral stenosis and, less frequently, severe mitral regurgitation. We present the case of an 82-year-old man with progressive dyspnea and hoarseness who was found to have severe mitral regurgitation and marked left atrial dilation. Following successful transcatheter edge-to-edge mitral repair, he experienced significant improvement in car

Congenital Mitral Rings: Insights From a Single-Institution Study and Review of Current Literature.

BackgroundCongenital mitral rings represent a rare subset of congenital mitral stenosis often associated with complex cardiac anomalies. Comprehensive literature on congenital mitral rings, particularly from large single-institution experiences, remains sparse. Methods: This retrospective study, conducted at All India Institute of Medical Sciences, New Delhi, analyzed data from 52 patients who underwent surgical correction for congenital mitral rings between 2014 and 2024. Data on demographics, clinical presentation, preoperative echocardiographic and cardiac catheterization parameters, associ

Contemporary Review of Outcomes in Patients Undergoing Transcatheter Aortic Valve Replacement with Concomitant Mitral Stenosis.

INTRODUCTION: Multivalvular cardiac disease has been associated with increased mortality and morbidity as compared to isolated valve disease. The treatment of choice for combined degenerative aortic and mitral stenotic disease is considered to be double valve surgery, but it is associated with poorer outcomes when compared to isolated valve surgical correction, especially in high-risk populations. There is considerable interest in utilizing transcatheter therapies in multivalvular disease. The prevalence and outcomes of patients undergoing transcatheter aortic valve replacement (TAVR) for aort

Unusual Native Valve Remnant in the Left Ventricular Outflow Tract After Valve-in-Ring Transcatheter Mitral Valve Replacement.

Background and clinical significance. Valve-in-ring (ViR) transcatheter mitral valve replacement (TMVR) is an established therapeutic option for patients with failed surgical mitral valve repair at high surgical risk. Left ventricular outflow tract (LVOT) obstruction and prosthesis-related complications are well described, but other postprocedural findings remain poorly characterized. Case presentation. We report a challenging case of a persistent LVOT mass following ViR TMVR. A 78-year-old man underwent transapical ViR TMVR with a Sapien 3 valve for mitral stenosis. Early post-procedural echo

The Impact of Mitral Valvular Etiology on Left Atrial Functional Recovery After the Maze Procedure: A Comparison Between Mitral Stenosis, Mitral Regurgitation and Non-Mitral Valve Disease.

Background: Although the concomitant Maze procedure successfully restores sinus rhythm in patients with valvular atrial fibrillation, it remains unclear whether electrical restoration translates into uniform functional recovery across different valvular etiologies. To address this issue, we compared the long-term left atrial (LA) mechanical recovery between patients with mitral stenosis (MS) and mitral regurgitation (MR) after the Maze procedure. Methods: This retrospective study included 211 patients who underwent the Maze procedure concomitant with valvular surgery and maintained sinus rhyth

Correlations between early ultrasound markers and the outcomes of staged lateral guided bone regeneration.

PURPOSE: Guided bone regeneration (GBR) is a widely used strategy for augmenting lost jawbone. The extent of bone gain varies among patients and typically remains unknown until after a healing period of 4-6 months. This study aims to demonstrate the use of novel ultrasonography techniques to provide predictive values for bone gain. METHODS: This study included 16 subjects with 20 edentulous ridges undergoing GBR for lateral bone deficiencies of 1-3 mm in the posterior mandible. Ultrasound (US) images were captured at the surgical site 10 days postoperatively. Four US-derived parameters were a

Myxoid Pleomorphic Liposarcoma Imitating Recurrent Traumatic Hemopericardium.

BACKGROUND: Primary pericardial malignancies are rare, and myxoid tumors may mimic hemorrhagic pericardial effusions on imaging, leading to diagnostic delay. CASE SUMMARY: A 56-year-old man presented with pleuritic chest pain and was diagnosed with acute pericarditis and a presumed traumatic hemorrhagic pericardial effusion. Despite anti-inflammatory therapy and pericardiocentesis yielding bloody, lymphocyte-predominant fluid with negative cytology, he experienced rapid clinical deterioration. Repeat imaging revealed an encasing pericardial mass causing severe left atrial compression and func

High diagnostic yield of family echocardiographic screening of children with bicuspid aortic valve.

AIMS: Bicuspid aortic valve (BAV) is present in 0.8% of newborns, with associated aortopathy in 33%. While the prevalence of BAV and associated aortopathy in first-degree relatives of symptomatic BAV patients ranges from 5 to 10% and 5 to 32%, data from unselected probands are limited. Uniform family screening guidelines are lacking. We aimed to determine the prevalence of BAV and aortopathy in first-degree relatives of children diagnosed neonatally with BAV. METHODS AND RESULTS: In this case-control family study based on the Copenhagen Baby Heart Study (n > 25 000 newborns enrolled 2016-18),

Early and Intermediate Outcomes of Cavitron Ultrasonic Surgical Aspiration to Treat Mitral Annular Calcification.

OBJECTIVES: Severe mitral annular calcification (MAC) carries an increased risk of perioperative complications in patients undergoing mitral valve surgery and may be considered a prohibitive surgical risk. Cavitron ultrasonic surgical aspiration (CUSA) has been reported as a method for controlled debridement of severe MAC during mitral valve surgery; however, experience in the literature is limited. METHODS: We assessed 30-day and intermediate-term outcomes in 67 consecutive patients (mean age: 72 [SD 9] years; female sex: 65.7% [44/67]) with severe MAC who underwent mitral valve repair or re

Serum proteomics mirrors the histopathological changes underlying different etiologies of primary mitral valve disease.

Mitral valve disease (MVD) is the most common valvulopathy and a frequent cause of heart failure and death. However, data regarding the molecular basis are scarce. We aimed to thoroughly explore and compare the circulating, molecular, and histopathological profiles in the main subtypes of primary chronic MVD. In total, 300 patients with chronic primary MVD undergoing mitral valve (MV) replacement were enrolled and classified in the four main etiologic subtypes: calcific mitral valve disease (CMVD, n = 81), rheumatic disease (RHVD, n = 114), Barlow's disease (BD, n = 70), and fibroelastic defic

Stakeholders' perspectives on the management and prevention of non-communicable diseases in rural Tanzania: SWOC analysis prior to PEN Plus implementation.

Non-Communicable Diseases (NCDs), such as hypertension, diabetes, sickle cell disease, and rheumatic heart disease, pose significant health burdens in low-income countries, not the least rural Tanzania, exacerbated by limited healthcare infrastructure, human and financial resources. This paper reports the perspectives of key stakeholders on the strengths, weaknesses, opportunities, and challenges (SWOC) in managing and preventing non-communicable diseases (NCDs) in rural districts of Tanzania, prior to the implementation of the WHO PEN Plus model. Using qualitative methods through in-depth int

Preprocedural Left Atrial Strain as a Predictor of Long-Term Outcomes Following Mitral Valve Interventions in Rheumatic Severe Mitral Stenosis.

BACKGROUND: This study explores whether preprocedural left atrial (LA) strain predicts outcomes in individuals with rheumatic, severe mitral stenosis undergoing mitral valve (MV) interventions. METHODS: Data were from the MASTER (Multicenter Mitral Stenosis with Rheumatic Etiology) registry, including patients with severe mitral stenosis who underwent percutaneous mitral valvuloplasty or MV replacement. Participants with moderate or greater dysfunction of other valves or missing strain data were excluded. The primary outcome was a composite of all-cause mortality and heart failure hospitaliza

Impacts of atrial fibrillation and surgical ablation on rheumatic and degenerative mitral surgeries.

OBJECTIVE: To evaluate whether the impacts of atrial fibrillation (AF) ablation differ between rheumatic and degenerative mitral valve (MV) surgeries, given concerns that atrial fibrosis in rheumatic disease may reduce ablation efficacy. METHODS: Consecutive patients undergoing rheumatic or degenerative MV surgery between 2000 and 2022 were retrospectively examined. The primary end point was death, and the secondary the end point was composite of death, readmission attributable to heart failure, and stroke. In patients with AF, outcomes between ablation and no-ablation groups were compared us
