Investigating the Relationship Between Mitral Leaflet Separation Index and Severity of Mitral Valve Stenosis Using Two and Three-Dimensional Measurement Methods in Trans Esophageal Echocardiography.

INTRODUCTION: Cardiologists use several echocardiographic methods to evaluate the Mitral Stenosis (MS) severity. Determination of mitral valve area (MVA) by three- dimensional transesophageal echocardiography (3D TEE) planimetry has the best correlation with anatomic area of the mitral orifice and is probably the reference method. This study was designed to examine the relationship between mitral leaflet separation index (MLSI) measured by TEE which is a simple and accessible method and MVA calculated by 3D Multiplanar reconstruction (3D-MPR) planimetry in TEE as the reference but less accessi

Severe continuous prosthetic mitral regurgitation from chordal rupture after valve-sparing replacement: a case report.

BACKGROUND: After mitral valve replacement with preservation of the subvalvular apparatus, prosthetic dysfunction resulting from retained native structures is extremely rare. When it occurs, it may directly threaten the patient's life. We report an unusual case of severe regurgitation resulting from preserved chordal rupture. The key distinction of this case is the presentation of late, severe continuous prosthetic regurgitation and reversibility. CASE PRESENTATION: A 54-year-old man had undergone double mechanical valve replacement for severe rheumatic mitral stenosis with mild regurgitation

Atrial Fibrillation in Valvular Heart Disease: Assessing the Impact of Genetics in the UK Biobank.

BACKGROUND: Atrial fibrillation (AF) in valvular heart disease (VHD) is generally attributed to pressure or volume overload with atrial remodeling, but the independent contribution of genetic predisposition remains unclear. OBJECTIVES: The authors evaluated the predictive value of a previously developed AF polygenic risk score (PRS) across VHD subtypes in the UK Biobank. METHODS: In this prospective cohort study (UK Biobank, 2000-2022), incident AF and VHD status were defined using ICD codes. VHD subtypes included mitral valve (MV) disease (MV prolapse [MVP], mitral regurgitation, mitral ste

Iliofemoral Venous Stenosis Complicating Large-Bore Access for Balloon Mitral Valvotomy, Unmasking Incidental May-Thurner Anatomy.

BACKGROUND: Large-bore transvenous access for balloon mitral valvotomy (BMV) can be complicated by unrecognized iliofemoral venous disease, occasionally unmasking a congenital variant. CASE SUMMARY: A 51-year-old woman with rheumatic mitral stenosis and NYHA functional class III-IV dyspnea, on acenocoumarol for atrial fibrillation, was scheduled for BMV. After right femoral venous access and transseptal puncture, hardware >8-F could not be advanced; venography showed a tight, recoiling right external iliac vein stenosis, not responding to intravenous nitroglycerine, treated with angioplasty a

Endoscopic secondary chordae transfer and edge-to-edge repair for the treatment of asymmetric anterior leaflet prolapse.

Anterior leaflet prolapse remains a challenging lesion in mitral valve repair, particularly in the presence of asymmetric degenerative disease. We present the case of a 59-year-old patient with severe mitral regurgitation due to asymmetric anterior leaflet prolapse in the setting of Barlow's disease, treated through a totally endoscopic right minithoracotomy. Intraoperative valve analysis demonstrated prolapse of the A2M2 and A3 segments with elongated secondary chordae. Mitral valve repair was performed using secondary chordae transfer, followed by asymmetric edge-to-edge repair at the A2-P2

Transseptal Transcatheter Mitral Valve Replacement for Mitral Annular Calcification: ENCIRCLE MAC Registry 1-Year Outcomes.

BACKGROUND: Patients with symptomatic mitral valve dysfunction (MVD) and mitral annular calcification (MAC) represent one of the most challenging populations in structural heart disease. Advanced age, multiple comorbidities, extensive annular calcification, and unfavorable anatomy often preclude surgical mitral valve replacement or transcatheter repair, leaving many patients with limited treatment options. Whether fully percutaneous transseptal transcatheter mitral valve replacement (TMVR) can provide a meaningful treatment pathway for patients with MAC-related MVD remains uncertain. OBJECTIV

Beyond Atrial Fibrillation: Clinical and Echocardiographic Predictors of Ischaemic Stroke and Systemic Thromboembolism in Rheumatic Heart Disease: A Systematic Review and Meta-Analysis.

PURPOSE: Atrial fibrillation (AF) is the best-established thromboembolic risk factor in rheumatic heart disease (RHD), but stroke and thromboembolism also occur without documented AF. We systematically synthesised associations between non-AF clinical/echocardiographic predictors and thromboembolism in RHD, comparing AF-adjusted with unadjusted estimates. METHODS: PubMed/MEDLINE, Embase, Scopus, and Web of Science were searched, supplemented by Google Scholar and citation screening. Eligible studies evaluated RHD patients with ≥1 non-AF predictor of stroke or thromboembolism. Two reviewers per

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Three-Point Bedside Congestion Score (IJV Collapsibility + IVC Collapsibility + Central Venous Pressure) in Elective Mitral Valve Surgery: A Prospective Observational Study.

BACKGROUND: Systemic venous congestion and right-ventricular (RV) dysfunction are key drivers of postoperative hemodynamic instability, prolonged ventilation, and acute kidney injury (AKI) after mitral valve surgery. Comprehensive echocardiography is informative but may be impractical in high-throughput pre-anesthesia workflows. We evaluated a pragmatic three-point bedside congestion score combining internal jugular vein (IJV) collapsibility, inferior vena cava (IVC) collapsibility, and invasively measured central venous pressure (CVP) for preoperative risk stratification. METHODS: In this pr

Mitral Valve Repair in Patients With Class I Obesity: A Propensity-Matched Analysis.

BACKGROUND: Obesity may increase the technical complexity of robotic mitral valve (MV) repair, but comparative outcomes with sternotomy remain poorly defined. OBJECTIVES: This study aimed to compare perioperative and long-term outcomes of robotic vs sternotomy MV repair in patients with class I obesity. METHODS: A total of 880 patients with class I obesity (body mass index 30-34.9 kg/m2) underwent MV repair at our clinic between 2008 and 2025. After excluding those with mitral stenosis, active endocarditis, prior sternotomy, or concomitant procedures, 429 patients remained. Propensity score

Patient-Specific Analysis of Ventricular Vortex Formation and Flow Energetics in Rheumatic and Calcific Mitral Stenosis.

PURPOSE: Mitral stenosis (MS) accounts for 12% of cardiac patients with single valve disease. There are two main causes: rheumatic disease of the mitral valve (MV) and mitral annular calcification (MAC). While both result in increased transmitral pressure gradients, each has unique effects on valve morphology and pathophysiology. We aimed to study differences in transmitral vortex formation and stroke energetics between the two valve types. We created an in silico experimental system using an immersed finite element-finite difference fluid-structure interaction (FSI) scheme and performed three

[Mitral valve disease: diagnosis and management revisited].

Mitral valve disease ranks among the most frequent valvular conditions and carries significant morbidity and mortality if left untreated. This review addresses anatomy, the Carpentier classification, and echocardiographic severity grading, as well as current management of primary and secondary mitral regurgitation (MR) and mitral stenosis. Transcatheter therapies, including edge-to-edge repair and transcatheter mitral valve replacement, receive particular attention and are discussed in the context of four clinical cases from a specialized valve center.

Clinical Failure After Mitral Transcatheter Edge-to-Edge Repair Mechanisms, Outcomes, and Management.

Transcatheter repair with edge-to-edge therapy (M-TEER) has been a revolutionary technology for patients with mitral regurgitation (MR). M-TEER is an established safe and effective treatment that alleviates MR, improves quality of life, and, in some patients, improves survival. Nonetheless, a significant number of M-TEER may not have procedural nor clinical success due to a variety of factors, with residual MR and/or mitral stenosis. The clinical manifestation of M-TEER failures may be early or late, with varying degrees of patient impact, ranging from an asymptomatic state to heart failure an

Computed tomography angiography of congenital left-sided obstructive heart lesions: a pictorial essay.

Congenital left-sided obstructive heart lesions (LSOLs) are a group of rare but serious congenital heart diseases. They encompass a spectrum of structural abnormalities that can obstruct blood flow at any point from the left atrium to the aortic arch. They are broadly divided into two main categories. The first is left ventricular inflow obstruction, including cor triatriatum sinister and congenital mitral stenosis. The second is left ventricular outflow obstruction, including subvalvular aortic stenosis, bicuspid aortic valve, supravalvular aortic stenosis, coarctation of the aorta, aortic ar

Procedural Sequence in Simultaneous TAVR and PBMV: A 3-Patient Case Series.

Combined aortic stenosis (AS) and mitral stenosis (MS) present a challenging multivalvular disease scenario. Simultaneous transcatheter aortic valve replacement (TAVR) and percutaneous balloon mitral valvuloplasty (PBMV) has emerged as a potential option for selected high-risk patients, but the optimal procedural sequence remains unclear. We describe 3 patients with severe AS and MS who underwent combined TAVR and PBMV following heart team evaluation, with a focus on procedural sequencing. Postprocedural hemodynamics improved immediately, and short-term outcomes were satisfactory. Our experien
