BACKGROUND: Patients with thrombotic or infective masses on native or biological mitral valve have no established treatment if deemed ineligible for surgery. This case series summarizes our initial experience with a transseptal interventional bailout approach.
CASE SUMMARY: Seven high-risk patients (aged 74.0 ± 4.87 years; EuroSCORE II median 14.05%) underwent transseptal debulking under cerebral protection. Six procedures used the INARI system (FlowTriever; INARI Medical) and the TriClip system (Abbott). Debulking was technically successful in all patients, without stroke, valve injury, or r

BACKGROUND: Hypoattenuated leaflet thickening (HALT) represents subclinical leaflet thrombosis in bioprosthetic valves with risk of occurrence peaking at 12 to 24 months after implantation. However, late presentations can occur.
CASE SUMMARY: A 77-year-old woman developed severe mitral stenosis 17 years after receiving a bioprosthetic mitral valve. Cardiac CT revealed extensive hypoattenuated leaflet thrombosis affecting all leaflets. After 3 months of therapeutic warfarin, repeat imaging demonstrated significant HALT resolution, avoiding valve-in-valve intervention.
DISCUSSION: This represe

BACKGROUND: Shone complex is a rare congenital cardiovascular anomaly characterized by multiple left-sided obstructive lesions. Adult presentation is uncommon and may result in atypical clinical manifestations, particularly when complicated by infective endocarditis. Coarctation of the aorta-associated infective endocarditis is especially rare and carries significant clinical risk.
CASE SUMMARY: We report a 25-year-old male with partial Shone complex consisting of a parachute mitral valve, mitral stenosis, and coarctation of the aorta. The patient presented with persistent high-grade fever an

AIM: The aim of this study is to share our experience of the combined double-orifice and single-patch technique for the correction of partial atrioventricular septal defect (pAVSD) and its associated tricuspid valve dysplasia (TVD).
METHODS: Between January 2014 and May 2024, 99 patients (age ≥ 18 years) who underwent repair of pAVSD and its associated TVD using the aforementioned strategy were retrospectively analyzed.
RESULTS: The mean aortic cross-clamp time was 48.5 ± 12.6 min, and the mean cardiopulmonary bypass time was 75.8 ± 13.0 min. Follow-up was 100% at a mean of 5.2 years (3-8 ye

Anticoagulation is a critical component in the treatment of cardiovascular diseases. Large randomised controlled trials have demonstrated that direct oral anticoagulants (DOACs) are effective and safe for stroke prevention in patients with atrial fibrillation (AF) and treatment or prevention of venous thromboembolism. Nevertheless, for specific cardiac indications and patient characteristics, vitamin K antagonists (VKAs) rather than DOACs are either preferred, or the benefit of DOACs in comparison to VKAs remains uncertain. These include, among others, mechanical heart valves, AF associated wi

Ortner's syndrome (OS), or cardiovocal syndrome, is an uncommon cause of hoarseness caused by recurrent laryngeal nerve palsy due to compression from underlying cardiovascular disease. It is classically associated with left atrial enlargement due to rheumatic mitral stenosis and, less frequently, severe mitral regurgitation. We present the case of an 82-year-old man with progressive dyspnea and hoarseness who was found to have severe mitral regurgitation and marked left atrial dilation. Following successful transcatheter edge-to-edge mitral repair, he experienced significant improvement in car

BackgroundCongenital mitral rings represent a rare subset of congenital mitral stenosis often associated with complex cardiac anomalies. Comprehensive literature on congenital mitral rings, particularly from large single-institution experiences, remains sparse. Methods: This retrospective study, conducted at All India Institute of Medical Sciences, New Delhi, analyzed data from 52 patients who underwent surgical correction for congenital mitral rings between 2014 and 2024. Data on demographics, clinical presentation, preoperative echocardiographic and cardiac catheterization parameters, associ

INTRODUCTION: Multivalvular cardiac disease has been associated with increased mortality and morbidity as compared to isolated valve disease. The treatment of choice for combined degenerative aortic and mitral stenotic disease is considered to be double valve surgery, but it is associated with poorer outcomes when compared to isolated valve surgical correction, especially in high-risk populations. There is considerable interest in utilizing transcatheter therapies in multivalvular disease. The prevalence and outcomes of patients undergoing transcatheter aortic valve replacement (TAVR) for aort

Background and clinical significance. Valve-in-ring (ViR) transcatheter mitral valve replacement (TMVR) is an established therapeutic option for patients with failed surgical mitral valve repair at high surgical risk. Left ventricular outflow tract (LVOT) obstruction and prosthesis-related complications are well described, but other postprocedural findings remain poorly characterized. Case presentation. We report a challenging case of a persistent LVOT mass following ViR TMVR. A 78-year-old man underwent transapical ViR TMVR with a Sapien 3 valve for mitral stenosis. Early post-procedural echo

Background: Although the concomitant Maze procedure successfully restores sinus rhythm in patients with valvular atrial fibrillation, it remains unclear whether electrical restoration translates into uniform functional recovery across different valvular etiologies. To address this issue, we compared the long-term left atrial (LA) mechanical recovery between patients with mitral stenosis (MS) and mitral regurgitation (MR) after the Maze procedure. Methods: This retrospective study included 211 patients who underwent the Maze procedure concomitant with valvular surgery and maintained sinus rhyth

BACKGROUND: Primary pericardial malignancies are rare, and myxoid tumors may mimic hemorrhagic pericardial effusions on imaging, leading to diagnostic delay.
CASE SUMMARY: A 56-year-old man presented with pleuritic chest pain and was diagnosed with acute pericarditis and a presumed traumatic hemorrhagic pericardial effusion. Despite anti-inflammatory therapy and pericardiocentesis yielding bloody, lymphocyte-predominant fluid with negative cytology, he experienced rapid clinical deterioration. Repeat imaging revealed an encasing pericardial mass causing severe left atrial compression and func

OBJECTIVES: Severe mitral annular calcification (MAC) carries an increased risk of perioperative complications in patients undergoing mitral valve surgery and may be considered a prohibitive surgical risk. Cavitron ultrasonic surgical aspiration (CUSA) has been reported as a method for controlled debridement of severe MAC during mitral valve surgery; however, experience in the literature is limited.
METHODS: We assessed 30-day and intermediate-term outcomes in 67 consecutive patients (mean age: 72 [SD 9] years; female sex: 65.7% [44/67]) with severe MAC who underwent mitral valve repair or re

BACKGROUND: This study explores whether preprocedural left atrial (LA) strain predicts outcomes in individuals with rheumatic, severe mitral stenosis undergoing mitral valve (MV) interventions.
METHODS: Data were from the MASTER (Multicenter Mitral Stenosis with Rheumatic Etiology) registry, including patients with severe mitral stenosis who underwent percutaneous mitral valvuloplasty or MV replacement. Participants with moderate or greater dysfunction of other valves or missing strain data were excluded. The primary outcome was a composite of all-cause mortality and heart failure hospitaliza

BACKGROUND: Rheumatic heart disease remains a major cause of maternal morbidity and mortality in low- and middle-income countries. The coexistence of severe mitral stenosis (MS) and antiphospholipid syndrome presents an exceptionally high-risk clinical scenario requiring multidisciplinary management.
CASE SUMMARY: A 31-year-old woman with severe rheumatic MS and antiphospholipid syndrome presented at 20 weeks of gestation with shortness of breath and palpitations. Echocardiography confirmed severe MS, with a valve area of 0.8 to 0.9 cm2. Despite optimal medical therapy, she remained symptomat

BACKGROUND: Mitral annular calcification (MAC) is common and associated with increased cardiovascular risk and, when severe, mitral stenosis (MS). MAC-related MS differs anatomically and hemodynamically from rheumatic MS (RMS), challenging standard diagnostic methods. This study compares structural and flow characteristics, including kinetic energy losses, across MAC-related MS, RMS, and normal mitral valves, and evaluates the applicability of conventional diagnostic metrics in MAC.
METHODS: Three-dimensional transesophageal echocardiographic data sets from 70 patients (22 normal mitral valv

Limited data exist on outcomes for patients with mitral stenosis (MS) who have previously undergone percutaneous mitral commissurotomy (PMC) and later require surgical mitral valve replacement (MVR). This study evaluates the impact of prior PMC on clinical outcomes in rheumatic MS patients undergoing surgical MVR. We retrospectively compared rheumatic MS patients with and without a history of PMC who underwent surgical MVR between 2010 and 2020. The primary outcome was 5-year all-cause mortality, while secondary outcomes included mitral reintervention, rehospitalization for heart failure (HHF)

BACKGROUND: Rheumatic heart disease (RHD) is a key contributor to maternal cardiovascular morbidity and mortality in sub-Saharan Africa (SSA). Though low- and middle-income countries (LMICs), particularly those in SSA, face a greater burden of RHD, existing systematic reviews have not specifically focused on cardiac and obstetric complications among affected women. We aimed to study cardiac and obstetric complications in pregnant and postpartum women with RHD in SSA and to evaluate the rate of valvular interventions in pregnant or postpartum women with severe disease.

BackgroundEffective myocardial protection is essential for successful outcomes in open-heart surgery. Although both del Nido and St. Thomas cardioplegia solutions are widely used, comparative evidence in adult rheumatic mitral valve replacement remains limited. This study aimed to compare their myocardial protective efficacy and clinical outcomes.MethodsA prospective comparative study was conducted between May 2022 and October 2024. Total 50 adult patients undergoing mitral valve replacement-20 with severe mitral regurgitation (MR) and 30 with severe mitral stenosis (MS) were included. Patient

Antithrombotic therapy is highly effective for stroke prevention in patients with atrial fibrillation (AF) but increases bleeding. Direct oral anticoagulants are preferred over vitamin K antagonists for many patients with AF because they provide similar stroke protection with less intracranial bleeding, and greater convenience, yet important knowledge gaps remain. In this review, we examine the limitations of current AF risk scores for predicting stroke, bleeding, and net benefit; identify AF populations in whom vitamin K antagonists remain preferred over direct oral anticoagulants, including
