INTRODUCTION: Intracranial hemorrhage, most often intraparenchymal hemorrhage, is an increasingly common and fatal complication of warfarin-induced coagulopathy. Warfarin remains the anticoagulant of choice in valvular atrial fibrillation. However, warfarin has a narrow therapeutic index. Extra axial hemorrhage due to warfarin-induced coagulopathy is a rare adverse event. We present a rare case of right vestibulocochlear and lower motor neuron (LMN) facial palsy secondary to an intracranial bleed along the cerebellopontine angle.
CASE DESCRIPTION: A 36-year-old woman presented with headache,

BACKGROUND: Rheumatic mitral stenosis (MS) remains a leading cause of cardiovascular complications in pregnancy, particularly in low-resource settings. Hemodynamic changes during pregnancy can precipitate decompensation in previously asymptomatic patients, increasing the risk of maternal and fetal morbidity. Early recognition and management, including percutaneous mitral commissurotomy (PMC) in selected cases, are crucial to optimizing outcomes. CASE PRESENTATION: We present the case of a 32-year-old pregnant woman (G4P3) at 22.6 weeks of gestation who developed severe respiratory distress and

BACKGROUND: Primary cardiac tumours are exceedingly rare with intimal sarcomas being the rarest subtype of malignant cardiac sarcomas. These tumours are characterized by late presentation, haemodynamic compromise, and poor prognosis due to their rapid progression and metastatic potential. Multimodal management is essential given the lack of standardized guidelines.
CASE SUMMARY: A 64-year-old female presented with heart failure and atrial fibrillation. An echocardiogram revealed a 48 mm obstructive intra-cardiac mass in the left atrium causing severe functional mitral stenosis. Given recurren

BACKGROUND: Thrombus formation within a left atrial septal pouch is uncommon but may critically impact septostomy-based interventions.
CASE SUMMARY: A 55-year-old woman with severe rheumatic mitral stenosis and chronic atrial fibrillation on anticoagulation was evaluated for percutaneous transmitral commissurotomy. Transesophageal echocardiography revealed a large bilayered interatrial septal pouch containing a semimobile thrombus, whereas the left atrial appendage was thrombus-free. Cardiac computed tomography angiography confirmed a focal septal filling defect. Given the embolic risk associ

The mitral valve apparatus comprises the annulus, valve leaflets, chordae tendineae, and papillary muscles, forming an integrated biomechanical unit essential for unidirectional blood flow. The leaflets and chordae are primarily derived from endocardial cells, and damage to these structures results in either mitral stenosis or mitral regurgitation, depending on the underlying pathology. This review compares three major mitral valve diseases, rheumatic mitral stenosis, congenital mitral stenosis, and myxomatous mitral valve prolapse, to highlight their distinct etiologies, molecular mechanisms,

Objective.Accurate physiological assessment of cardiac function from heart sounds remains challenging due to background noise, variable heart rates, and the need for reliable cardiac-cycle segmentation. This study aimed to develop a fully E2E deep learning framework that extracts diagnostic information directly from raw heart sound recordings for cardiac abnormality detection and classification.Approach.We propose HS-MMNet, an E2E multi-modal deep learning framework designed for physiological heart sound analysis. Recordings are preprocessed (normalization and 25-400 Hz bandpass filtering) and

A mediastinal mass can be incidentally detected during transesophageal echocardiography, so clinicians performing this procedure should be aware of this possibility and able to interpret related echocardiographic findings. In evaluating a 69-year-old man with dyspnea, mitral stenosis was identified on transthoracic echocardiography. Subsequently, during transesophageal echocardiography, a mediastinal mass in the posterior mediastinum was observed. The computed tomography scan confirmed the transesophageal echocardiography findings and suggested that lymphoma and metastatic malignancy should be

BACKGROUND: The optimal surgical strategy for severe rheumatic mitral stenosis with more than mild mitral regurgitation (MR) remains unclear. We aimed to characterize mitral valve pathology in these patients and compare outcomes between valve repair and valve replacement.
METHODS: This dual-center retrospective study analyzed 870 surgically treated severe rheumatic mitral stenosis patients with complete imaging. Inverse probability weighting balanced baseline characteristics (standardized mean difference <0.1). Quantitative assessment combined echocardiography and computed tomography angiogra

Background Mitral stenosis (MS) is a common valvular heart disease that is frequently complicated by arrhythmias, heart failure, pulmonary hypertension, and thromboembolic events. This study aimed to determine the clinical and echocardiographic factors of complications in patients with MS. Methodology This retrospective cross-sectional study was conducted at the Cardiology Department of Northwest General Hospital and Research Centre, Peshawar, including 121 patients diagnosed with MS using echocardiography. Patients of either gender, aged ≥18 years, and diagnosed with MS confirmed by transthor

Scimitar syndrome (SS) is a rare congenital condition marked by partial anomalous pulmonary venous connection, dextroposition of the heart, right lung hypoplasia, and systemic arterial supply to the right lung. Also known as congenital veno-lobar syndrome and other names, it has been classified into three types by Lupius et al. A significant number of cases are associated with congenital heart anomalies, while association with acquired cardiac disease is rare. We present the rare case of a 39-year-old woman with exertional dyspnoea during pregnancy, initially diagnosed with mild mitral stenosi

We report a rare case of idiopathic hypereosinophilic syndrome presenting with isolated cardiac involvement in the form of sub-mitral intracardiac thrombus leading to severe functional mitral stenosis and severe eccentric mitral regurgitation. This case highlights the role of multi-modality imaging in the evaluation of intra-cardiac thrombi at unusual locations, especially in hypereosinophilic syndromes and the need of prompt and aggressive management in patients with severe valvular involvement for better clinical outcomes.

BACKGROUND: Critical care echocardiography is increasingly used for bedside hemodynamic assessment, but the prognostic relevance of large-scale, early diastolic phenotyping in unselected ICU populations remains uncertain. We evaluated whether an early echocardiography-derived elevated filling-pressure phenotype, operationally defined from structured ASE/EACVI-aligned variables, was associated with in-hospital mortality in critically ill adults with preserved left ventricular ejection fraction (LVEF).
METHODS: We performed a retrospective cohort study using the MIMIC-IV database linked to the

Atrial fibrillation (AF) is highly prevalent in internal medicine and often requires timely echocardiographic assessment. Limited availability of standard studies may delay management, particularly in acute or resource-limited settings. We conducted a multicenter, cross-sectional study across 35 Spanish hospitals, enrolling adults with new-onset or chronic AF without echocardiography in the previous 12 months, comparing internist-performed point-of-care ultrasound (POCUS) with blinded cardiologist-performed transthoracic or transesophageal echocardiography (TTE/TEE) as the reference standard.

BACKGROUND: The lifetime risk of stroke among non-anticoagulated patients with atrial fibrillation (AF) approximates 1 in 3. Oral anticoagulation (OAC) reduces stroke and systemic embolism by at least two thirds and mortality by one fourth.
METHODS: The present narrative review summarizes current evidence and practical recommendations on OAC in AF and highlights recent advances and remaining gaps in knowledge.
RESULTS: The threshold for net clinical benefit of OAC is met when stroke risk exceeds 1%-2% per year, typically corresponding to ≥ 1 traditional stroke risk factor (CHA2DS2-VA ≥ 1), w

When the risk of cardiopulmonary arrest during intubation is high, "awake" extracorporeal membrane oxygenation (ECMO) cannulation with sedation and a natural airway may provide hemodynamic stability and gas exchange to mitigate the hazards of intubation. This retrospective case series characterizes patient selection, clinical periprocedural management, and frequency of "awake" pediatric ECMO cannulations in nonintubated patients at a quaternary pediatric hospital. Between 2014 and 2024, 58 patients (7-18 years) with nonpostoperative cardiac indications underwent ECMO support, with six (10%) "a

OBJECTIVE: Severe calcific mitral stenosis remains challenging to treat. In high-risk or inoperable patients without transcatheter valve replacement options, percutaneous balloon mitral valvuloplasty (PBMV) may be considered, but it carries risks of valvular injury, worsening regurgitation, and embolic events. PBMV facilitated by intravascular lithotripsy (IVL) has emerged as a novel method to improve valve compliance and enable safer balloon dilation. We detail the step-by-step technique of IVL-facilitated PBMV.
KEY STEPS: Bilateral Sentinel devices were deployed. Transseptal puncture was pe
