Body mass index and waist-hip ratio - Risk factors for aortic valve disease: The atherosclerosis risk in communities (ARIC) study.

BACKGROUND: Elevated body mass index (BMI) has been associated with increased risk of aortic valve stenosis (AS) in observational and Mendelian randomization studies; however, its association, and that of waist-hip ratio (WHR), with aortic valve insufficiency (AI) remains unclear. We investigated the association of BMI and WHR with incident aortic valve disease (AVD) in the ARIC Study. METHODS: We analyzed ARIC participants with echocardiograms at Visits 5 (2011-13) and 7 (2018-19). BMI and WHR were assessed at Visit 5. Incident AVD between Visits 5 and 7 was categorized as aortic valve scler

International Age and Use Criteria for Transcatheter and Surgical Aortic Valve Replacement.

Transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) have transformed the management of severe aortic stenosis across a wide range of patient risk profiles. As populations age and indications extend to lower-risk and younger patients, determining the appropriate role of age in selecting TAVR versus SAVR has become increasingly complex. Current guidelines emphasize individualized decision-making, yet age-based referral patterns remain common in clinical practice. This review examines how age thresholds are applied in contemporary guidance from the United St

Exceptional Finding of Double Pannus in Aortic and Mitral Mechanical Prosthetic Valves Using Noninvasive Imaging: A Case Report.

Pannus formation is a rare but clinically important complication of mechanical prosthetic valves. The incidence of pannus in aortic prostheses is much higher than that in mitral prostheses, the latter being more prone to thrombosis. However, the presence of pannus in both prostheses is an exceptional finding. The authors present the case of a 61-year-old woman with a long history of rheumatic multivalvular disease. She had undergone three cardiac surgeries for mechanical valve prosthesis replacement in the aortic, mitral, and tricuspid positions. She was hospitalized for atypical angina and bl

Sodium-Glucose Cotransporter-2 Inhibitors in Valvular Heart Disease: From Mechanistic Insights to Clinical Application.

Valvular heart disease encompasses a diverse group of hemodynamic disorders that are frequently associated with heart failure, recurrent hospitalizations, and substantial morbidity despite advances in transcatheter and surgical therapies. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have emerged as a cornerstone of cardiovascular and renal disease management, demonstrating consistent benefits across a broad range of patient populations. Beyond their established effects in heart failure, diabetes mellitus, and chronic kidney disease, accumulating evidence suggests that SGLT2 inhibitors may

Single-Session Transcatheter Aortic and Tricuspid Valve-in-Valve Implantation in a Patient with Prior Multivalve Surgery and a Chronic Transvenous Pacing Lead: A Case-Based Review of Procedural Strategy.

Background: The population of patients surviving previous multivalve surgery is expanding, and structural Heart Teams increasingly encounter complex multivalvular disease requiring reintervention, for which redo surgery carries prohibitive risk. Patient selection, sequential procedural planning, prosthesis choice, and, in particular, pacing strategy and protection of pre-existing intracardiac hardware remain unstandardized. Case Presentation: We report a 73-year-old woman with a twenty-year history of rheumatic multivalvular heart disease, comprising mechanical mitral valve replacement, tricus

Natural history of aortic valve disease after rheumatic mitral valve surgery: implications for concomitant aortic intervention.

BACKGROUND: Rheumatic heart disease (RHD) frequently affects both mitral and aortic valves. While mitral valve replacement (MVR) is often prioritized, the natural history of coexisting aortic valve disease particularly when mild or moderate is less well understood. OBJECTIVE: The objective of this study is to assess the progression of aortic stenosis (AS) and aortic insufficiency (AI) in patients undergoing isolated MVR for rheumatic mitral disease. METHODS: We conducted a single-center retrospective cohort study involving 403 adult patients with rheumatic mitral valve disease who underwent

Disseminated Bartonellosis Involving the Kidney and Heart.

Blood culture-negative infective endocarditis poses considerable clinical and diagnostic challenges because of its atypical manifestations and the difficulty in identifying the responsible pathogens. The involvement of Bartonella henselae, a rare etiologic agent of blood culture-negative infective endocarditis, further complicates diagnosis and management. Dissemination of Bartonella across multiple organ systems is a recognized feature of the disease. A 73-year-old female with a one-year history of prosthetic mitral valve replacement for mitral valve stenosis secondary to rheumatic fever pres

Unicuspid unicommissural aortic valves: A surgical pathology analysis.

The unicuspid unicommisural aortic valve is an uncommon congenital malformation that often manifests as stenosis with or without regurgitation in adults in their third to fifth decades of life. This report characterizes the morphological features of surgically excised unicuspid valves in adults with clinical correlation. Among the surgically excised aortic valves over a period of 10 years, the clinical data and morphological features of unicuspid aortic valves were analyzed. The patients were grouped by the type of valvular function. Pathological features noted were the shape of the orifice, t

Attitude towards a moderate aortic valve dysfunction during rheumatic mitral valve surgery: a retrospective cohort study.

BACKGROUND: There are insufficient data regarding how to deal with moderate aortic valve (AV) dysfunction during rheumatic mitral valve (MV) surgery. In this study, the clinical outcomes of patients who underwent rheumatic MV surgery with or without concurrent AV procedures were compared. METHODS: A total of 343 patients who underwent rheumatic MV surgery with moderate AV dysfunction were enrolled between January 2015 and August 2022, and a median 40-month follow-up was conducted. The more-than-mild AV dysfunction during follow-up was the primary endpoint event, while all-cause mortality and

Polymeric Heart Valve Leaflets Tested in an Accelerated Wear Tester Revealed a Stable Craze Microstructure.

Rheumatic and calcified aortic heart valve disease is a global health concern, impacting millions of individuals across various age groups. The gold standard medical treatments recommend replacing the sick heart valves with bioprosthetic valves that are chemically fixed using glutaraldehyde, commonly sourced from bovines or porcines. Clinical investigations over more than two decades have revealed that fixed tissues are prone to premature calcification and tearing, thereby limiting their durability. An innovative alternative approach involves biomolecule-enhanced polyethylene-based linear low-

Mutational landscape and impact of clonal hematopoiesis of indeterminate potential in severe aortic valve stenosis.

BACKGROUND: Clonal hematopoiesis of indeterminate potential (CHIP) has been progressively established as a risk factor for cardiovascular disease and associated with worsened outcomes in patients with aortic valve stenosis (AVS). This cohort study aimed to evaluate the mutational landscape of CHIP and its' influence on clinical outcomes. METHODS: 194 patients with AVS undergoing transcatheter aortic valve replacement (TAVR) were sequenced using a capture panel for multiple CH driver mutations and follow up conducted for three years. RESULTS: We found high prevalences (77.8%) of a broad spect

High afterload rather than myocardial fibrosis predicts reduced ejection fraction in severe aortic stenosis with afterload mismatch.

BACKGROUND: Afterload mismatch (AM) refers to high-gradient (mean gradient ≥40 mm Hg) severe aortic stenosis (AS) with reduced left ventricular ejection fraction (LVEF <50%) that is hypothesised to arise from mechanisms other than true contractile impairment. The extent, pattern and functional impact of myocardial fibrosis (MF), which is associated with systolic impairment, is poorly understood in the context of AM. METHODS: High-gradient severe AS patients with (n=25; low ejection fraction high-gradient, LEF-HG) and without (n=33; normal ejection fraction high-gradient (NEF-HG)) reduced LVEF

Concomitant Transcatheter Aortic and Mitral Valve Replacement.

BACKGROUND: Multivalvular heart disease is a highly prevalent condition that causes significant morbidity and mortality. CASE SUMMARY: A 75-year-old man presented with worsening functional class and asthenia. A transthoracic echocardiogram showed severe aortic stenosis and severe mitral stenosis. The patient underwent transcatheter aortic and mitral valve replacement in the same procedure. DISCUSSION: There is a lack of solid evidence regarding the management of multivalvular disease. Severe mitral stenosis can influence aortic stenosis, potentially leading to a low-flow, low-gradient profil

Society of cardiothoracic surgery in great Britain and Ireland guidance for adult mitral valve disease and interventions.

The Society of Cardiothoracic Surgery (SCTS) in Great Britain and Ireland provides comprehensive guidelines for the management of adult mitral valve disease, addressing both surgical and transcatheter interventions. These guidelines serve as a resource for healthcare professionals, supplementing existing clinical guidelines. They emphasize the evolving complexity of mitral valve disease management, influenced by the development of new technologies, ongoing research, and randomized trials. The guidelines detail a thorough methodology, incorporating an extensive literature review and evidence as

A tale of two valves: where rheumatic mitral disease meets aortic papillary fibroelastoma - a narrative review.

Rheumatic heart disease (RHD) and papillary fibroelastoma (PFE) are distinct cardiovascular disorders that can rarely occur together. RHD is a chronic condition caused by untreated streptococcal infections, primarily affecting the mitral valve and resulting in severe valvular damage. On the other hand, PFE is a benign heart tumor, usually affecting the aortic valve, and is typically asymptomatic, often discovered incidentally through imaging. The simultaneous presence of RHD and PFE, especially involving the mitral and aortic valves, has only been reported in two case studies. This review exam

Oral anticoagulation in patients with atrial fibrillation and heart valve prosthesis: A substudy from the Left Atrial Appendage Occlusion Study (LAAOS) III trial.

OBJECTIVE: Societal guidelines recommend vitamin K antagonists (VKAs) for patients with atrial fibrillation with recent biological valve implantation, but the safety and efficacy of direct oral anticoagulants (DOACs) in this setting remain uncertain, especially in the early postoperative period. This substudy of the Left Atrial Appendage Occlusion Study (LAAOS) III trial aimed to compare thromboembolic and bleeding outcomes in patients discharged on VKAs versus DOACs after bioprosthesis implantation or mitral valve repair. METHODS: A total of 2645 patients were included, with 461 discharged o

Aortic regurgitation as a cause of sudden cardiac death with aortic and left ventricular remodelling - the role of the bicuspid valve.

BACKGROUND AND OBJECTIVE: Aortic regurgitation (AR) results in blood flow from the aorta back into the left ventricle, which leads to left ventricular hypertrophy and dilation which, in a clinical setting, leads to heart failure and death. However, it is not a well-recognised cause of sudden cardiac death (SCD). METHODS: We identified 10 cases of AR with no other cause of death from our database of 8,551 cases of SCD. All these cases had a full autopsy with negative toxicology. Diagnosis of AR was based upon the presence of prominent ridges on the edge of the aortic valve (AV) cusps with aort

How to Manage Double Obstructions: Stories of Aortic and Mitral Stenosis.

Multivalvular disease presents unique clinical challenges compared with isolated valve pathologies because they often involve altered hemodynamics and physiology. A common combination is aortic stenosis and mitral stenosis, which frequently coexist owing to rheumatic or degenerative etiologies. This case series explores 3 patients with concurrent severe symptomatic mitral stenosis and aortic stenosis, illustrating how management strategies can vary significantly based on the patient's surgical risk, symptom burden, and anatomical features. These cases illustrate the experiences at our institut
