BACKGROUND: Transcatheter aortic valve replacement (TAVR) is contraindicated for rheumatic aortic stenosis (RAS). Evidence on the efficacy and safety of TAVR is scarce despite several cases of RAS recorded in China. This study aimed to evaluate the efficacy of TAVR in patients with RAS. METHODS: A total of 359 patients were enrolled and divided into the RAS ( n = 71) and non-RAS ( n = 288) groups according to the World Heart Federation guidelines for rheumatic heart disease combined with computed tomography imaging. Propensity score matching was performed to ensure comparability between groups

Aortic stenosis (AS) is a valvular heart disease characterised by the narrowing of the valve opening area. Calcific aortic stenosis (CAS) and rheumatic aortic stenosis (RAS) have distinctly different valve morphologies. The haemodynamic environment of generic calcific and rheumatic aortic valves (AV) of various severities is analysed through the use of 3D FSI modelling techniques. For moderate (AVA = 1-15 cm2), severe (AVA < 1 cm2) and very severe (AVA ≪ 1 cm2) cases of calcific and rheumatic AS, larger TPGs with higher velocity magnitudes are estimated in the rheumatic cases compared to the c

BACKGROUND: Transcatheter aortic valve replacement (TAVR) in patients with prior mechanical mitral valve replacement (MVR) is technically challenging because of the altered aortomitral geometry and risk of prosthesis interaction.
CASE SUMMARY: We report a single-center series of 5 patients with severe symptomatic rheumatic aortic stenosis and preexisting mechanical MVR who underwent transfemoral TAVR using balloon-expandable and self-expanding valves. Comprehensive preprocedural multidetector computed tomography was used to assess aortomitral distance and angle, annular dimensions, coronary h

BACKGROUND: Computed tomography (CT) calcium scoring is used to adjudicate paradoxical low-flow, low-gradient (LFLG) severe aortic stenosis (AS). However, its reliability may be limited in rheumatic disease.
CASE SUMMARY: An 82-year-old woman with prior mitral valve replacement for rheumatic mitral stenosis was referred for AS. Transthoracic echocardiography revealed an aortic valve area (AVA) of 0.50 cm2, mean pressure gradient of 27 mmHg, stroke volume index of 30 mL/m2, and preserved systolic function, suggesting paradoxical LFLG severe AS. The CT calcium score was 889 Agatston units, belo

Although most commonly diagnosed in the elderly population, aortic stenosis can affect younger patients, being the most frequent valvular disease requiring replacement interventions, either through surgical procedures (SAVR = surgical aortic valve replacement) or through transcatheter aortic valve implantation (TAVI). In young patients, aortic stenosis generally occurs due to congenital malformations, such as bicuspid aortic valves (BAVs), or to rheumatic valve disease, both of which present specific anatomical characteristics. There is an upward trend among young patients regarding TAVI, due

Background: Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have shown cardioprotective effects beyond glucose control. In aortic stenosis, SGLT2 expression is upregulated in myocardium and valve tissue, contributing to inflammation, oxidative stress, thrombogenicity, and calcification. SGLT2 inhibition may counteract these mechanisms, potentially reducing bioprosthetic valve failure after transcatheter aortic valve replacement (TAVR), where the diseased native valve remains in place. Objectives: This study aimed to evaluate whether SGLT2i use is associated with improved clinical outcomes,

Patients with rheumatic aortic stenosis and prior mechanical mitral valve replacement present unique challenges for transcatheter aortic valve replacement, particularly when the aortomitral distance is short. This case series examines 4 such patients, focusing on procedural complexities, imaging strategies, and outcomes. Preprocedural multidetector computed tomography and intraprocedural transesophageal echocardiography were critical in assessing anatomical constraints and optimizing valve deployment. Key considerations included preventing mitral prosthesis interference, mitigating left ventri

AIMS: Few studies have examined the incidence, prevalence, survival rate and death risk for non-rheumatic and rheumatic aortic stenosis (AS; RAS), aortic insufficiency (AR; RAR) and aortic stenosis with insufficiency (ASAR; RASAR). This study aims to identify the epidemiology of AS, AR, ASAR, RAS, RAR and RASAR.
METHODS AND RESULTS: Data were collected from newly diagnosed non-rheumatic and rheumatic aortic valve disorders (AVD, ICD-10: I35 and I06, n = 101 895, female: male = 6:4) including AS, AR, ASAR, RAS, RAR and RASAR, excluding congenital heart disease. The data were sourced from the N

We report a 75-year-old female with a history of two heart operations: aortic valve replacement (St. Jude MedicalTM 21 mm) at the age of 44 years for severe rheumatic aortic stenosis and mitral valve replacement (CarbomedicsTM 29 mm) at the age of 51 years for rheumatic mitral regurgitation. Decades later, she presented with exertional dyspnea. Echocardiography revealed aortic stenosis with an effective orifice area of 0.79 cm². Coronary angiography showed #6 75% stenosis and a limited mechanical valve opening. After a thorough discussion, the patient agreed to undergo redo surgery. The surger

Rheumatic heart disease [RHD] is the most prevalent cause of valvular heart disease in the world, outstripping degenerative aortic stenosis numbers fourfold. Despite this, global resources are firmly aimed at improving the management of degenerative disease. Reasons remain complex and include lack of resources, expertise, and overall access to valve interventions in developing nations, where RHD is most prevalent. Is it time to consider less invasive alternatives to conventional valve surgery? Several anatomical and pathological differences exist between degenerative and rheumatic valves, incl

Non-rheumatic aortic stenosis (AS) is among the most common valvular diseases in the developed world. Current guidelines support aortic valve replacement (AVR) for severe symptomatic AS, which carries high morbidity and mortality when left untreated. In contrast, moderate AS has historically been thought to be a benign diagnosis for which the potential benefits of AVR are outweighed by the procedural risks. However, emerging data demonstrating the substantial mortality risk in untreated moderate AS and substantial improvements in periprocedural and perioperative mortality with AVR have challen

BACKGROUND: Rheumatic heart disease (RHD) accounts for the highest number of deaths from valvular heart disease globally. Yet, rheumatic aortic stenosis (AS) was excluded from landmark studies investigating the safety and efficacy of transcatheter aortic valve implantation (TAVI). We aimed to describe the clinical and anatomical characteristics of patients with rheumatic AS undergoing TAVI, and to compare procedural and clinical outcomes with patients undergoing TAVI for degenerative AS.
METHODS: In a prospective TAVI registry, patients with rheumatic AS were identified based on International

Aortic valve stenosis (AS) is the third most frequent cardiovascular abnormality after coronary artery disease and hypertension. A bicuspid aortic valve is the most common cause for AS until seventh decade and calcific valve degeneration is responsible thereafter. In symptomatic patients, The risk of death increases from ≤1%/year to 2%/month. An echo valve area ≤1 cm2, peak transaortic velocity ≥4 m/s, mean valve gradient ≥40 mmHg and/or computerized tomography valve calcium score >2000 Agatston units (AU) for males or more than 1200 AU for females indicate severe AS. AS stages and management

BACKGROUND: Patients with rheumatic aortic stenosis (AS) were excluded from transcatheter aortic valve replacement (TAVR) trials.
OBJECTIVES: The authors sought to examine outcomes with TAVR versus surgical aortic valve replacement (SAVR) in patients with rheumatic AS, and versus TAVR in nonrheumatic AS.
METHODS: The authors identified Medicare beneficiaries who underwent TAVR or SAVR from October 2015 to December 2017, and then identified patients with rheumatic AS using prior validated International Classification of Diseases, Version 10 codes. Overlap propensity score weighting analysis w

Rheumatic heart disease (RHD) mainly affects people in developing, low-income countries. However, due to globalization and migration, developed countries are now seeing more cases of RHD. In RHD patients who develop severe symptomatic aortic stenosis, surgical aortic valve replacement remains the treatment of choice. In the past decade, there has been an extension of transcatheter aortic valve replacement (TAVR) to intermediate-risk and lower-risk patients with aortic stenosis. This review suggests the possible utility of TAVR for the treatment of rheumatic aortic stenosis. Rheumatic aortic st

Unicuspid aortic valve (UAV) stenosis is a rare condition accounting for 5% of non-rheumatic aortic stenosis. The diagnosis can be difficult to make prior to surgical intervention and transesophageal echocardiography has been demonstrated to be more accurate in making the diagnosis compared to transthoracic echocardiography. The presence of a posteriorly located aortic orifice on the short-axis views, with one or two visible raphe anteriorly; the absence of commissures (acommissural); or the presence of a lone commissure (unicommissural) between the left and noncoronary, or the left and right

Transcatheter aortic valve implantation (TAVI) for patients with rheumatic aortic stenosis (AS) is not well-known. We herein report a case of TAVI in rheumatic AS without significant calcification and prior mitral valve replacement. An 80-year-old woman underwent TAVI for severe AS. Preoperative computed tomography revealed tricuspid aortic valve leaflets with commissural fusion, minimal calcification, and a minimal distance between the aortic annulus and mechanical mitral valve. TAVI was performed through a transfemoral approach under general anesthesia. After predilatation of the aortic valv

The authors describe a case of transposition of great arteries with a large atrial septal defect with fusion of tricuspid valve leaflets and severe aortic stenosis. The latter two were likely rheumatic in etiology. The patient's condition improved after atrial switch and aortic valve replacement surgery. (Level of Difficulty: Intermediate.).

Background: Trends in mortality from aortic stenosis across European countries are not well-understood, especially given the significant growth in transcatheter aortic valve implantation (TAVI) in the last 10 years. Methods: Age-standardised death rates were extracted from the World Health Organisation Mortality Database, using the International Classification of Diseases 10th edition code for non-rheumatic aortic stenosis for those aged > 45 years between 2000 and 2017. The UK and countries from the European Union with at least 1,000,000 inhabitants and at least 50% available datapoints over
