Infective endocarditis (IE) is a life-threatening infection involving the cardiac valves and endocardium, most commonly associated with streptococci and enterococci in patients with underlying structural cardiac abnormalities. The bicuspid aortic valve (BAV) represents a significant predisposing valvular condition compared with patients with a tricuspid aortic valve. Culture-negative endocarditis accounts for a substantial portion of all cases, often attributed to prior antibiotic exposure, and represents a major diagnostic challenge in which histopathological examination of the surgical speci

Heart failure (HF) and valvular heart disease (VHD) often coexist and share complex pathophysiological pathways. Traditional management strategies follow a step-by-step approach, prioritizing guideline-directed medical therapy (GDMT) and reserving interventional (surgical or percutaneous) options only in case of persistent symptoms or worsening HF after GDMT. However, greater experience with the use of percutaneous procedures, even in high-risk patients, could support a more integrated approach, which exploits the synergistic effects of medical and interventional therapies to increase the tole

BACKGROUND: Knowing the prevalence of patent foramen ovale (PFO) and the increasing number of people who inject drugs (PWID) with right-sided infective endocarditis (RSIE), we aimed to better characterize the syndrome of injection drug use, RSIE, and PFO with systemic embolic events.
METHOD: A systematic search across multiple databases was independently screened using predefined criteria (from inception until September 2025). Published cases of adult PWID diagnosed with RSIE and ischemic stroke or other systemic embolic event were included, provided they had PFO assessment. Individuals were

BACKGROUND: Outcomes after isolated tricuspid valve surgery (ITVS) are strongly influenced by advanced right-sided heart failure and hepatorenal dysfunction; however, the prognostic significance of preoperative diuretic burden remains unclear. This study examined whether preoperative diuretic burden is associated with outcomes after ITVS and whether combining it with the model for end-stage liver disease-sodium (MELD-Na) score improves risk stratification.
METHODS: We retrospectively analyzed 85 patients who underwent ITVS at 16 Japanese hospitals between 2015 and 2024. Diuretic doses were co

Tricuspid transcatheter edge-to-edge repair alleviates symptoms, but survival benefit remains inconsistent. The authors conducted a Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020-compliant systematic review and meta-analysis (CRD42024600438) to identify multivariable predictors of procedural success and adverse outcomes. PubMed, Scopus, the Cochrane Library, and Google Scholar were searched (from January 2008 to March 2025) for adjusted predictors of mortality, heart failure hospitalization, major adverse cardiovascular events, and procedural success. BACKGROUND: Management of valvular heart disease (VHD) has shifted from predominantly surgical approaches to widespread use of transcatheter interventions, including transcatheter aortic valve replacement (TAVR), mitral transcatheter edge-to-edge repair (M-TEER), transcatheter mitral valve replacement (TMVR), and emerging tricuspid transcatheter valve intervention (TTVI). Although ESC/EACTS and ACC/AHA guidelines recognize frailty as a major determinant of mortality, complications, and recovery, they offer limited guidance on how to modify frailty before intervention.

BACKGROUND: Coronary artery disease (CAD) is common in patients treated for structural heart disease, but its prevalence, prognostic impact, and optimal management in patients undergoing tricuspid transcatheter edge-to-edge repair (T-TEER) remain uncertain.
OBJECTIVES: The aim of this study was to examine whether the presence of CAD, its anatomical complexity, and the extent of revascularization influence outcomes after T-TEER.
METHODS: In this observational study, 361 consecutive T-TEER patients underwent preprocedural coronary angiography with SYNTAX (Synergy Between PCI With Taxus and Car

OBJECTIVES: In the transcatheter aortic valve replacement (TAVR) era, the presence of low-flow, low-gradient aortic stenosis (LFLG AS) itself does not preclude intervention. However, chronic kidney disease (CKD) and right-sided extravalvular damage (RSED) complicate procedural risk assessment. Both their association with each other and their joint impact on post-TAVR mortality in LFLG AS and across its subtypes remain incompletely characterized.
METHODS: The authors performed a retrospective study of patients with LFLG AS who underwent TAVR between 2019 and 2022, stratified by CKD stage (from

Point-of-care ultrasound (POCUS) has emerged as a valuable extension of the bedside physical examination, enabling rapid identification of cardiac abnormalities that may not be clinically apparent. Intracardiac masses, particularly in the right atrium, represent a diagnostic challenge and carry significant clinical implications, including the risk of embolization. Early recognition is critical to guide timely management. We report the case of a patient admitted with vaso-occlusive sickle cell crisis who underwent bedside cardiac ultrasound as part of the clinical evaluation. POCUS revealed a p

We aimed to establish a rat TR model to further investigate the mechanisms of right heart remodeling and organ damage induced by TR. Intervention group rats (n = 6) underwent coronary stent system implantation via transjugular approach. The control group (n = 6) underwent the same procedure, except for coronary stent implantation. All rats survived at 8 weeks follow-up postoperatively. TR occurred immediately after the implantation of coronary stent in the intervention group. The right ventricular fractional area change (FAC) decreased by 33.9% compared with baseline from 0.59 ± 0.11% to 0.39

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) is an emerging therapy for severe tricuspid regurgitation with an evolving complication profile. Fatal prosthetic valve endocarditis (PVE) following orthotopic TTVR appears exceptionally rare, with detailed pathologic description through specialist cardiovascular autopsy remaining limited.
CASE SUMMARY: A 65-year-old man with severe functional tricuspid regurgitation and persistent atrial fibrillation underwent TTVR. The following day, he developed complete heart block requiring pacemaker implantation. He remained well at 1 and 3 mo

BACKGROUND: The assessment of the right heart (RH) plays a central role in the diagnosis and management of cardiovascular and pulmonary diseases. Although current guidelines have improved standardization, there is anecdotal evidence suggesting that there is significant variability in clinical practice regarding acquisition and reporting persist. OBJECTIVES: This international survey evaluated the extent of variability in echocardiographic assessment of the RH. A simple method for the standardization of RH measurements is proposed. METHODS: The international anonymous survey consisted of 68 que

BACKGROUND: Secondary tricuspid regurgitation (STR) is frequently observed in patients with pulmonary hypertension (PH). Whether it independently confers mortality risk beyond the associated right ventricular (RV) dysfunction remains unknown.
OBJECTIVES: The aim of this study was to evaluate the prognostic significance of STR in patients with PH other than related to left-sided valve disease or reduced ejection fraction and across the individual WHO (World Health Organization) groups of PH.
METHODS: Patients with RV systolic pressures ≥50 mm Hg on echocardiography between 2010 and 2023 and m

Tricuspid regurgitation (TR) is a valvular heart disease mostly associated with structural heart disease or pulmonary hypertension (PH). Rare cases of TR secondary to exposure to specific medications, such as dopamine agonists, have been described. A 50-year-old woman presented with a pituitary microprolactinoma (without surgical indication) and a history of pulmonary embolism (PE) in 2021 (risk factors included bromocriptine, which was switched to cabergoline). An echocardiogram performed one year after the PE showed no significant abnormalities. She was maintained on rivaroxaban 10 mg due to

BACKGROUND: Rheumatic Heart Disease (RHD) remains a major cause of valvular morbidity in endemic regions despite declining incidence in high-income countries. While early disease manifests as acute rheumatic fever, contemporary presentations increasingly reflect advanced structural sequelae. Congestive hepatopathy and cardiac cirrhosis secondary to chronic right-sided failure remain under-recognized manifestations of advanced rheumatic valvular disease.
OBJECTIVES: To describe three cases of advanced rheumatic multivalvular disease presenting predominantly with ascites and hepatic congestion

BACKGROUND: Rheumatic heart disease (RHD) is a leading cause of valvular heart disease worldwide, particularly in developing countries. The mitral and aortic valves are most often affected; however, tricuspid valve involvement has been observed in advanced cases.
CASE SUMMARY: We present a case of a patient with RHD with tricuspid valve involvement who successfully underwent transcatheter tricuspid valve replacement (TTVR).
DISCUSSION: TTVR is a promising, less-invasive alternative treatment for patients with severe symptomatic tricuspid regurgitation deemed unsuitable for surgery. Despite t

Pulmonary hypertension (PH) is a clinically significant and increasingly recognized complication of myxomatous mitral valve disease (MMVD) in dogs, contributing to worsening morbidity and mortality. The development of PH in the context of MMVD is associated with increased symptom burden, including exercise intolerance, syncope, and respiratory distress, and has been correlated with reduced survival times. Echocardiography is the primary modality for detection and monitoring, relying on Doppler estimation of tricuspid regurgitation velocity, but limitations in sensitivity and accuracy remain. T

BACKGROUND: Interventional treatment of tricuspid regurgitation (TR) is a rapidly evolving field, but selecting patients most likely to benefit remains challenging. Currently, no dedicated or widely accepted risk score exists for transcatheter tricuspid valve interventions (TTVIs). In clinical practice, established scores such as Model for End-stage Liver Disease excluding INR (MELD-XI), European System for Cardiac Operative Risk Evaluation II (EuroSCORE II), GLIDE (Gap, Location, Image quality, Density, and En-face tricuspid regurgitation morphology), Tricuspid Regurgitation Impact Score (TRI

Tricuspid valve infective endocarditis (TV-IE) in people who inject drugs (PWID) has become an increasingly important clinical and public health concern, driven largely by the expanding opioid epidemic. Although PWID with TV-IE are typically young and often lack major comorbidities, they experience disproportionately high rates of mortality, reinfection, and recurrent hospitalizations. Repeated intravenous injection introduces contaminated particulate matter and pathogens into the venous circulation, promoting microbial adherence to the tricuspid valve, with Staphylococcus aureus including met
