Prevalence and prognostic implications of different aetiologies of low flow aortic stenosis.

BACKGROUND: The prevalence and prognostic implications of low-flow (LF) aetiologies in aortic stenosis (AS) are unknown. This study aimed to characterize the specific causes of LF, their prevalence, and prognostic significance. METHODS: In 408 patients with severe LFAS included, the aetiology of LF was identified. The primary endpoint was the composite of all-cause death and hospitalizations for heart failure (HF) up to 36 months. Secondary endpoints included individual components of the primary endpoint and cardiovascular mortality. RESULTS: The most common LF cause was reduced LVEF (n = 22

Worse survival in transcatheter aortic valve replacement with untreated tricuspid regurgitation: Implications for surgical intervention.

OBJECTIVE: Tricuspid regurgitation (TR) is frequently seen in patients with aortic stenosis; however, there is currently no guidance regarding the management of patients with aortic stenosis with TR being considered for transcatheter aortic valve replacement (TAVR) versus surgical aortic valve replacement. This study evaluates long-term outcomes of functional TR in patients with TAVR. METHODS: All patients undergoing TAVR at a single institution from 2011 to 2022 with complete pre- and post-TAVR echocardiogram follow-up were stratified by preprocedure TR grade (none, mild, moderate, or severe

Impact of dual atrioventricular valve disease on outcomes in patients undergoing transcatheter aortic valve replacement.

AIMS: Current guidelines provide no recommendations for the management of patients with dual atrioventricular valve regurgitation (DAVR) undergoing transcatheter aortic valve replacement (TAVR), and existing data on its clinical impact remain absent. Therefore, we aimed to provide detailed insights into the impact of DAVR on outcomes after TAVR, with the potential to refine clinical risk stratification tools and influence future management guidelines. METHODS AND RESULTS: We retrospectively analysed 3491 consecutive patients who underwent TAVR between 2013 and 2021. DAVR was defined as the pr

Evolution of Untreated Moderate Mitral Regurgitation After Transcatheter Aortic Valve Implantation.

Background and Objectives: Associated mitral regurgitation (MR) is frequently observed during transcatheter aortic valve implantation (TAVI). The progression of moderate MR remains undetermined, given uncertain clinical significance and natural history. This study aims to assess the evolution of moderate MR following TAVI. Materials and Methods: Between 2018 and 2023, 1476 patients underwent TAVI. We excluded those with previous aortic or mitral valve interventions, endocarditis, concomitant percutaneous coronary intervention, or emergent procedures. Patients with severe aortic or tricuspid re

Impact of hypoplastic left heart syndrome anatomic subtype on mortality, ventricular function, and atrioventricular valve function in the current era.

OBJECTIVE: The aortic atresia/mitral stenosis (AA/MS) subtype in hypoplastic left heart syndrome (HLHS) has been associated with poor single-ventricle palliation outcomes. We evaluated whether HLHS anatomic subtype influences transplant and Fontan-takedown-free survival and right ventricular (RV) and tricuspid valve (TV) function in the current era. METHODS: Retrospective chart review was done for 219 consecutive neonates with HLHS who received a Norwood at our center (2006-2023). Primary outcomes were freedom from mortality/transplant/Fontan takedown post-Norwood (n = 219) and Fontan (n = 13

Prevalence and Management of Multiple Valvular Heart Disease in Patients Undergoing Transcatheter Aortic Valve Replacement: A Multicenter Study on the Impact of Staged Valvular Interventions on Outcomes.

BACKGROUND: Multiple valvular heart disease correlates with poor outcomes following transcatheter aortic valve replacement. Previous studies have focused on mitral regurgitation (MR) or tricuspid regurgitation (TR) individually, without comparing their long-term effects. The impact of staged transcatheter edge-to-edge repair (TEER) remains unclear. We aimed to assess the prevalence and effects of severe multiple valvular heart disease (sMVHD) and evaluate the impact of staged TEER on outcomes. METHODS: Patients were recruited from 4 transcatheter aortic valve replacement centers. The primary

Cardiovascular images: parachute mitral valve in a cat.

A six-month-old intact female domestic shorthair cat was presented to the Ohio State University Veterinary Medical Center for further evaluation of a heart murmur. On echocardiography, a large, left-to-right shunting ventricular septal defect was identified. There was evidence of severe left heart volume overload, increased pulmonary artery flow and tricuspid regurgitation velocities, mild mitral regurgitation, and left-sided congestive heart failure. The presence of relative pulmonary stenosis, pulmonary hypertension, and a mitral valve malformation were suspected, but further echocardiograph

Surgical tricuspid edge-to-edge repair: double-orifice repair versus clover repair.

OBJECTIVES: Surgical edge-to-edge repair has been proposed to treat tricuspid regurgitation with various aetiologies. Two techniques can be used for this repair: the 'double-orifice' and the 'clover' repairs. This study compares the clinical outcomes of these 2 techniques. METHODS: The study enrolled 258 patients who underwent tricuspid edge-to-edge repair out of 2257 tricuspid valve repairs between January 2001 and December 2021. Patients were categorized into 2 groups (double-orifice and clover repairs) and analysed using propensity score matching. RESULTS: The mean age of the 258 patients

Mixed valvular heart disease: diagnosis and management.

Mixed valvular diseases (MVDs) are common but have received little attention in the literature, especially regarding the mitral valve (MV) and the right-sided cardiac valves. Whereas echocardiography plays a pivotal diagnostic role, the diagnosis is made difficult due to haemodynamic interactions that may invalidate common indices of severity used in isolated stenosis or regurgitation. The diagnostic strategy should aim at initially separately assessing stenosis and regurgitation, taking into account the diagnostic pitfalls, with complementary use of multimodality imaging in cases of persistin

Long-term outcomes after transcatheter tricuspid valve-in-valve replacement.

OBJECTIVES: Transcatheter tricuspid valve-in-valve (ViV) replacement has emerged as a less invasive alternative to redo surgery in patients with failing bioprosthetic tricuspid valves. While short- and mid-term outcomes have been reported, data on long-term follow-up remain limited. METHODS: The authors conducted a single-center observational study including 12 consecutive patients who underwent transcatheter tricuspid ViV replacement. Clinical, echocardiographic, and procedural data were prospectively collected. Patients were followed at 1 and 12 months, and yearly thereafter; follow-up incl

Transcatheter Implantation of Balloon-Expandable Valve in a Repaired Native Tricuspid Valve Without Annuloplasty Ring.

Late complications of repaired congenital heart disease are increasingly common, including tricuspid stenosis and regurgitation. We present a case of a 36-year-old female with tetralogy of Fallot, 1.5 ventricle repair, and prior tricuspid repair using homograft tissue without an annuloplasty ring, who developed worsening congestive heart failure. Transesophageal echocardiogram revealed severe tricuspid stenosis and regurgitation. We performed transcatheter tricuspid valve replacement with a SAPIEN S3 Ultra RESILIA valve within a CP-covered stent. To our knowledge, this is the first implantatio

Long-Term Impact of Multivalvular Heart Disease in Patients Undergoing Transcatheter Aortic Valve Replacement.

BACKGROUND: Multivalvular heart disease (MVHD) is frequently encountered in patients undergoing transcatheter aortic valve replacement (TAVR). However, the long-term prognostic impact of MVHD in this population remains poorly characterised. The aim of this study was to evaluate the long-term clinical outcomes and echocardiographic evolution of MVHD after TAVR. METHODS: A total of 1918 consecutive patients with severe symptomatic aortic stenosis (AS) undergoing TAVR were included. MVHD was defined as the presence of moderate or greater mitral (MR) or tricuspid (TR) regurgitation before the pro

Total endoscopic valve surgery with the Ram device in combined mitral and tricuspid valve disease.

Total endoscopic valve surgery is considered safe and efficient, providing multiple advantages and significantly increasing patient satisfaction. However, prolonged surgical times due to high technical complexity may remain. Devices for automated suturing may address this problem. The following case report demonstrates our step-by-step approach for combined total endoscopic mitral valve replacement using the RAM technology and tricuspid valve repair in a patient with severe rheumatic mitral valve stenosis.
