Prognostic Thresholds of Tricuspid Regurgitation in Transthyretin Amyloid Cardiomyopathy.

IMPORTANCE: In transthyretin amyloid cardiomyopathy (ATTR-CM), tricuspid regurgitation (TR) severity may be underestimated by conventional (semi-)quantitative echocardiographic criteria derived from nonamyloid populations, given the restrictive, low-flow hemodynamics characteristic of the disease. OBJECTIVES: To derive and validate disease-specific prognostic, quantitative TR risk thresholds in ATTR-CM and to compare their prognostic performance with current guideline definitions and the Tricuspid Valve Academic Research Consortium (TVARC) 5-grade extension. DESIGN, SETTING, AND PARTICIPANTS

Cone reconstruction for severe tricuspid valve dysplasia in a dog.

Tricuspid valve dysplasia (TVD) is a congenital cardiac malformation in dogs characterized by abnormal tricuspid valve morphology, leaflet tethering, impaired leaflet coaptation, and right atrial and ventricular remodelling. This report describes cone reconstruction for severe TVD in a dog. A three-year-and-10-month-old female Labrador retriever presented with a grade IV/VI systolic murmur and exercise intolerance. Echocardiography revealed severe tricuspid regurgitation, a right atrial-to-left atrial ratio of 1.57, malformed and tethered tricuspid valve leaflets, impaired leaflet coaptation,

A Small-Diameter Tricuspid Valve Prosthesis for the Correction of Congenital Heart Defects Based on an Adult Donor Mitral Valve Homograft.

BACKGROUND: Tricuspid valve replacement is the preferred method when valve repair is not feasible. Stented xenobioprostheses, which limit the growth of the fibrous ring due to their rigid titanium frame and often have a short service life, are particularly problematic in children. An alternative approach is the implantation of a mitral valve homograft in the tricuspid position, which is more resistant to calcification. However, the limited availability of small-diameter homografts from adult donors restricts their use. To address this, a new technique for downsizing an adult mitral homograft w

From Valve to Vessel: Disseminated Staphylococcus aureus Endocarditis Leading to Splenic Artery Pseudoaneurysm and Mycotic Aneurysm.

Infective endocarditis (IE) is an infection of the endocardial surface of the heart, with Staphylococcus aureus as the leading cause in the United States. Identified risk factors include advanced age, male sex, intravenous drug use (IVDU), prior IE, structural or valvular heart disease, HIV infection, and chronic hemodialysis. In IVDU, IE most commonly affects the right-sided valves, while bivalvular involvement is uncommon but associated with more severe disease and increased risk of rapid deterioration. Multiple complications arise from IE, including variable vascular phenomena and systemic

The Causal Relationship between Blood Cell Traits and Valvular Heart Disease: A Two-Sample Mendelian Randomization Study.

BACKGROUND: The pathogenesis of valvular heart disease (VHD) remains unclear, highlighting the need for further research on risk factors. OBJECTIVES: To explore the potential causal links between blood cell traits and four types of VHD: rheumatic heart disease, mitral valve disease, aortic valve disease, and tricuspid valve disease. METHODS: This study utilized summary statistics from large-scale GWAS and a two-sample Mendelian randomization approach, analyzing thirteen blood cell traits as exposures and four VHD phenotypes as outcomes. The primary MR method was inverse-variance weighted (IV

Bailout valve-in-valve therapy during transcatheter aortic valve replacement in bicuspid and tricuspid aortic stenosis: an observational single-centre study in China.

BACKGROUND: Bailout valve-in-valve (ViV) transcatheter aortic valve replacement (TAVR) is a critical rescue strategy for procedural failure, yet evidence regarding its outcomes in bicuspid aortic valve (BAV) anatomy remains limited. METHODS: This retrospective, single-centre study analysed 1597 patients (48.3% BAV) undergoing TAVR. Patients were stratified by the requirement for bailout ViV, which was conducted for significant residual aortic regurgitation (AR) or valve embolisation. Predictors were identified via multivariate logistic regression. Early- and mid-term survival outcomes were co

Innovations in transcatheter tricuspid valve interventions: what has failed, what has worked, what is coming.

Innovation in transcatheter tricuspid valve intervention has advanced by learning from what did not work and expanding what does. Early attempts, often adapted from the mitral space, exposed practical constraints and safety tradeoffs that underscored how distinct the tricuspid valve is in anatomy, imaging, and physiology. Transcatheter edge-to-edge repair has shown improvement in symptoms, health status, and functional capacity with a favorable safety profile, when anatomy is suitable. Orthotopic transcatheter tricuspid valve replacement (TTVR) reliably abolishes regurgitation across complex a

Population Prevalence of Valvular Heart Disease in the United States: The PREVUE-VALVE Study.

BACKGROUND: Valvular heart disease (VHD) is associated with substantial morbidity, mortality, and health care costs, yet its contemporary prevalence among older adults in the United States is unknown. OBJECTIVES: We performed a decentralized study of older adults (PREVUE-VALVE) to determine the population prevalence of VHD among older Americans. METHODS: Individuals 65-85 years old who previously filled a prescription at CVS or Walgreens pharmacies were randomly selected; contacted via e-mail, direct mail, or text messaging; and invited to participate. Enrolled participants completed study p

A decade of cardiac surgery after transcatheter aortic valve replacement: Short-term clinical outcomes at a high-volume center.

BACKGROUND: The growing adoption of transcatheter aortic valve replacement (TAVR) in younger and lower-risk patients has increased the number of patients with other cardiac diseases requiring surgical intervention. Such surgeries pose unique technical challenges due to the presence of the TAVR, often requiring explantation. Despite increasing clinical relevance, outcomes of these operations remain poorly characterized. We sought to assess the incidence, indications, and short-term results of cardiac surgery following TAVR at a high-volume institution. METHODS: This was a retrospective single-

Early Degeneration of a Next-Generation Mitral Bioprosthesis in the Tricuspid Position Treated With Valve-in-Valve.

BACKGROUND: Bioprosthetic tricuspid valve replacement is increasingly performed, but long-term durability data are limited. The Edwards Mitris Resilia valve, designed for the mitral position, has been used off-label in the tricuspid position, with scarce evidence of outcomes. CASE SUMMARY: We report 2 patients who developed early (<2-year) structural valve degeneration of the Mitris Resilia bioprosthesis in the tricuspid position. Both presented with symptomatic severe intraprosthetic tricuspid regurgitation. Given the prohibitive surgical risk of redo sternotomy, we opted for valve-in-valve

Transatrial Balloon-Expandable Valve Implantation for Severe Mitral Annular Calcification With Native Leaflet Sealing.

BACKGROUND: Severe mitral annular calcification (MAC) limits mitral intervention options. Transseptal valve-in-MAC (ViMAC) transcatheter mitral valve replacement (TMVR) risks neo-left ventricular outflow tract (neo-LVOT) obstruction and paravalvular leak, while surgical replacement requires extensive annular reconstruction. CASE SUMMARY: We report 3 high-risk patients with severe MAC (computed tomography [CT]-based MAC scores 7-10) who underwent open transatrial valve-in-MAC TMVR via a balloon-expandable valve after CT-based simulation and multidisciplinary heart review identified increased r

Current Trends and Future Challenges in Transcatheter Aortic Valve Implantation (TAVI): A Narrative Review.

The advent of transcatheter aortic valve implantation (TAVI) has redefined the treatment of aortic stenosis over the last two decades, evolving from a therapy reserved for patients that were deemed to be of prohibitive surgical risk to the standard of care for a large group of patients presenting with symptomatic disease. With improvements in technology, operator and institutional experience and longer-term outcome data, recent guidelines have supported the broadening of indications to low-risk and asymptomatic patients in addition to other pathologies including the management of failed surgic

Acute rheumatic fever or rheumatoid arthritis? The role of hydroxychloroquine: a case report.

BACKGROUND: Rheumatic fever is endemic in our part of the world. Arthritis is the most common clinical presentation and is characteristically migratory in nature. Rheumatoid-like arthritis in acute rheumatic fever is not described in literature. Although arthritis is typically treated with nonsteroidal anti-inflammatory drugs, the role of hydroxychloroquine remains less well studied. CASE PRESENTATION: A 24-year-old Pakistani female individual presented with symmetrical polyarthritis involving the small joints of the hands along with morning stiffness. She was diagnosed with rheumatic heart d

Constrictive Pericarditis After Minimally Invasive Triple Valve Replacement: Insights From a Fatal Outcome.

Triple valve surgery is performed in only a small minority of valvular procedures and carries substantial perioperative risk and a demanding postoperative course. Pericardial complications after cardiac surgery are well recognized, but progression to constrictive physiology following a minimally invasive triple valve operation is exceptionally uncommon and poorly characterized. We describe an elderly woman with longstanding hypertension, dyslipidemia, and paroxysmal atrial fibrillation who underwent minimally invasive replacement of the aortic, mitral, and tricuspid valves for combined multiva

Trends and Projections in Valvular Heart Disease Mortality Among Older Adults in the United States, 1999 to 2040.

Valvular heart disease (VHD) contributes substantially to cardiovascular mortality. We evaluated long-term VHD mortality trends among older adults to inform healthcare planning. CDC WONDER mortality data (1999 to 2023) were analyzed for adults aged ≥65 years to calculate age-adjusted mortality rates (AAMRs). Joinpoint regression identified temporal trends, alongside a subtype analysis stratifying AAMRs by nonrheumatic aortic (ICD-10 I35), mitral (ICD-10 I34), tricuspid (ICD-10 I36), and chronic rheumatic (ICD-10 I05 to I09) valve disorders. Age-period-cohort modeling assessed generational effe
