The standard Blalock-Taussig (B-T) shunt is preferred over other shunts in the treatment of infants with cyanotic heart disease and reduced pulmonary blood flow. The B-T shunt is not, however, without problems such as phrenic nerve injury and excessive bleeding. It is a time-consuming procedure owing to the degree of lateral mediastinal dissection required (the latter is especially important in critically hypoxic infants). An alternative procedure is described in which an intrapericardial approach is used. This approach requires minimal dissection of the subclavian artery and the anastomosis i

One hundred twenty-two congenital cardiac anomalies were diagnosed in 83 pigs (4.35%) during necropsies of 1906 pigs from one day to four years of age during an 11-month period. The incidence of cardiac malformation was higher at 29 to 56 days of age. Of the 83 pigs with cardiac anomalies, 47 (56%) were male and 36 (44%) were female, and of these, 15 (18%) were purebred and 68 (82%) were crossbred pigs. Of the 15 purebred pigs with cardiac malformation, there were seven Landrace pigs, four Large White pigs, three Duroc Jersey pigs, and one Yorkshire pig; whereas 94% of the 68 crossbred pigs we

The pathological anatomy of 109 specimens of aortic valvular atresia was reviewed for the purpose of identifying the cardiovascular anomalies associated with that condition. We found the most commonly associated anomaly to be coarctation of the aorta, which was present in 71 percent of our cases and judged to be hemodynamic significance in one-third of the involved cases. Other associated anomalies, in order of decreasing frequency, were mitral atresia, anomalous systemic and pulmonary venous connections, abnormalities of branching of the aortic arch, and ventricular septal defect. The study d

Long-term durability and the need for anticoagulation in conjunction with the use of porcine valves remain questionable. We analyzed valve dysfunction and thromboembolism in 325 adult (older than 20 years) and 31 pediatric survivors who received 407 porcine xenograft valves from June, 1974, to September, 1980 (46% of all valve replacements). Valves at risk in the adults were 216 aortic, 138 mitral, and 22 tricuspid; in children, 14 aortic, eight mitral, seven pulmonary, and two tricuspid. Mean follow-up was 38 (9 to 85) months. Twenty-seven valved conduits also were followed up for 9 to 85 (me

Previous studies have demonstrated that it is possible to measure the cardiac output by M-mode echocardiography (M-echo), using the echogram of the mitral (MV) or aortic valves. In this communication we pretend to demonstrate the possibility of calculating the right cardiac output by the echocardiographic image of the tricuspid valve (TV). We studied patients with atrial septal defects (ASD) because the anatomic and physiologic conditions allowed us to obtain a complete record of the TV. The study was done in a prospective form with 12 patients with isolated ASD, without pulmonary hypertension

Tricuspid annular calcification was observed on cardiac screening of a 50 year old woman with congenital valvular pulmonary stenosis and confirmed by angiography. At operation, the tricuspid annular calcification was palpable but there was no interference with valvular function. There was no associated disease process in this patient. Phosphate and calcium metabolism was normal. Tricuspid annular calcification is very rare: 7 other reported cases, aged 42 to 51 years old. All patients had right ventricular hypertension usually caused by pulmonary stenosis. Tricuspid valve function was normal i

Thrombosis of both valve leaflets of a St. Jude Medical tricuspid cardiac prosthesis, resulting in valvular dysfunction, was detected by systolic and diastolic tricuspid murmurs and confirmed by M-mode and two-dimensional echocardiograms. Thrombus formation at both hinge points of the bileaflet valve caused one leaflet to stick in the nearly closed position and the other to have severely impeded motion. Thrombotic involvement of both valve leaflets, not previously reported, occurred in a 17-year-old boy who has undergone six prosthetic cardiac valvular replacements in the past 2 years. Failure

Intracardiac phonocardiograms were obtained from the right atrium in order to study the relation between the clinical signs of tricuspid regurgitation, intracardiac murmurs and the degree of regurgitation demonstrated on right ventriculography with use of a preshaped catheter. In five patients with no heart disease, right ventriculograms showed no evidence of tricuspid regurgitation and intracardiac phonocardiograms in the right atrium demonstrated no murmur. Among 35 patients with valvular heart disease, a Carvallo sign (increased intensity of systolic murmur during inspiration) was present i

The rapid development of new methods of diagnosis and treatment of children with congenital heart disease is revealed. Because of decreasing death risk open heart surgery is now available already in infancy. Early corrective surgery has advantages to two-step repair. As an example of new ways of treatment a reconstructive method for tricuspid atresia is presented and the follow-up results of 41 total repairs of complete AV canal are reviewed. Double-outlet right ventricle and truncus communis are two other malformations that are accessible for successful reconstructive surgery.

This article discusses structural congenital heart disease in the newborn. Emphasis is given to the clinical and laboratory diagnosis including the indication for cardiac catheterization and angiography. The discussion includes the group of cyanotic, noncyanotic and other structural congenital heart diseases, as well as the group of neonatal cardiopulmonary distress without structural heart disease. An outline of the general management of these entities is included in the presentation.

To investigate the normal configuration of the tricuspid valve and the normal size of the tricuspid valve ring, 17 normals, 20 patients with atrial septal defect, 4 patients with tricuspid valve prolapse, and 41 patients with various diseased conditions were studied by cross-sectional echocardiography. The tricuspid valve ring diameter in atrial septal defect was judged to be increased in comparison with that of normals. All the patients with tricuspid valve prolapse exhibited an excessive systolic ballooning of the leaflets toward the right atrium. However, a systolic ballooning of the leafle

A series of 50 children aged between 1 month and 15 years old, with ventricular septal defects (VSD) were investigated by two-dimensional sector scanning and cineangiography. Fifty other children of the same age group with congenital heart disease without VSD were also investigated by two-dimensional echocardiography and angiography. No VSDs were diagnosed by echocardiography in children with angiographically intact inter ventricular septa. In the group with VSD, two-dimensional echo localised with precision: --36 membranous VSDs (36/36) --5 infundibular VSDs (5/6) --2 isolated muscular VSDs (

The tactical decision in patients with decompensated valvular disease associated with a severe stenosis of the aortic isthmus is always difficult. One stage surgical repair using two separate approaches is a long and high risk procedure. It would seem more logical and safer to treat the lesions in two stages a few weeks apart, the severest lesion being managed first. In the two cases reported. The isthmic stenoses and valvular lesions were of the same severity and made both classical techniques impracticable. Therefore the patients underwent a single stage procedure by a median approach associ

Cardiac surgery to treat severe heart failure was of benefit to two patients with endomyocardial disease and hypereosinophilia-eosinophilic endomyocardial disease. Both patients had severe biventricular fibrosis with mitral and tricuspid regurgitation. One had predominant right ventricular disease and was treated by right ventricular endocardectomy with tricuspid and mitral xenograft valve replacement. The second patient's main haemodynamic problem was considered to be mitral regurgitation. His mitral valve was replaced by a Starr-Edwards prosthesis; endocardectomy was not performed. Though bo

The long term follow-up of 59 Kuwait citizens who underwent valve replacement is presented. There was an early mortality of 12% and late mortality of another 12% with actuarial survival of 76% at five years. These values compare well with earlier Western series. The mean age of 26 is about half that reported in the Western literature. The ratio of mitral valve involvement (pure or mixed) to aortic valve involvement is 2.5:1 indicating that rheumatic heart disease is still prevalent. However, no case was operated during rheumatic activity. Anticoagulation therapy was well tolerated among the Be

171 children aged 2-17 years underwent cardiac valve replacement between 1969 and December 1979. Valve lesions were of rheumatic origin in 160 and congenital in 11 cases. Single valve replacement of the mitral valve was performed in 116 cases, of the aortic valve in 18 cases and of the tricuspid valve once. Double valve replacement was done in 33 and triple valve replacement in 2 cases. Starr Edwards prostheses were used in most instances. Operative mortality was 5.9% and late death occurred in 8.7%. The follow-up period ranged from 1-9 years with a mean of 3.6 years. The majority of children

A 25-year-old Senegalese woman with severe multivalvular disease presented with jaundice (unconjugated bilirubin) augmented by exertion without evidence of frank anemia. Following the exclusion of all other causes of hemolysis, the diagnosis of mechanical hemolysis secondary to deformed valves was established. The jaundice and laboratory evidence of hemolysis diminished markedly after placement of (Starr) aortic and mitral valve prostheses and a tricuspid commissurotomy. While chronic hemolysis due to ball or disc valve prostheses is usually clinically insignificant yet often described in the

M-mode and two-dimensional echocardiography were performed to investigate the motion of the interventricular septum (IVS) and posterior wall (PW) in cases with pure mitral (MI) or aortic (AI) insufficiency. Subjects were classified into four groups; 15 cases with chorda rupture or floppy mitral valve (acute MI), four with AI of acute onset (acute AI), 17 with chronic MI including rheumatic MI, mitral valve prolapse syndrome, and 11 with chronic AI. There wee no differences of left ventricular dimension (LVDI), stroke volume (SV), ejection fraction (EF) and cardiac output (CI) between acute and

Eighty Nigerian children (mean age = 8.8 years) with rheumatic heart disease (RHD) were studied prospectively. The initial acute rheumatic fever (ARF) was recognized in 34 of them (42%) while 18 patients (23%) had recurrent ARF. 76 patients (95%) had mitral incompetence, either alone or in combination with other lesions. Symptomatic mitral stenosis was present in 21 patients (26%), occurred early in life, and appeared to be causally related to recurrent ARF and intense host reactions. Aortic and tricuspid valve diseases were uncommon and pulmonary valve disease was rare. 94% of the patients pr

Three cases of endocarditis confirmed at autopsy were identified in an intensive care nursery during an eight-month period. Echocardiography demonstrated abnormal echoes in the region of the mitral valve in one infant and the tricuspid valve in the other two infants. Both the diverse clinical manifestations of bacterial and nonbacterial endocarditis and the value of echocardiography in the establishment of an antemortem diagnosis are discussed. This previously unusual entity seems to be occurring with greater frequency in neonatal intensive care units that provide vigorous efforts to support s
