The haemodynamic and angiocardiographic data of 19 patients with Starr-Edwards mitral valve prosthesis were analysed to determine the causes of secondary deterioration observed in 14 of them. This was shown to be due to: 1. Surgical complications or progression of pre-existing valvular lesions which were not, or only partially, corrected (3 cases) (adiastole due to pericardial effusion, aortic valve disease and severe tricuspid incompetence); average diastolic pressure gradient CPW-LV 4.7 +/- 1.5 mmHG, the functional valve surface area (FVA) 1.37 +/- 0.2 cm2, LVEDP 17.7 +/- 4 mmHg, LVEDV 57.5

Valvular prosthesis made a good reputation in the treatment of acquired and a series of congenital heart diseases. At the Research Institute of Cardiovascular Diseases, 288 subjects were implanted a total of 324 valves from 1965 to 1979. Only from January 1, 1977 to May 1, 1979--132 patients got valvular prostheses, 71 of them with mitral prostheses, 42--with aortic prostheses and 19--with double valvular prostheses, with a total and immediate postoperative lethality of 22.8%. In 18 of the patients valvular prosthetization was accompanied by some other heart interventions--valvular commissuroto

Standard autopsy specimens from 49 infants and children, most with congenital heart disease, were prepared by controlled pressure coronary artery perfusion fixation of the heart. Autopsy measurements of tricuspid (TV) and pulmonary valve (PV) circumferences (Circ), right ventricular (RV) outflow circumference, (Circ), RV inflow and outflow tract lengths, and RV volume (Vol) were correlated with corresponding in vivo end-diastolic (D) and end-systolic (S) measurements from biplane cineangiograms (angio) in the same patients. Correlation of the dimensions studied yielded the following r values:

A group of 44 patients with a total of 48 cardiac lesions ( 22 tricuspid incompetence - TI - , 12 ventricular septal defect - VSD - and 14 atrial septal defect - ASD - , and two control groups of 20 normals and 23 patients with other cardiac disease , were studied by pulsed Doppler echocardiography ( PDE ) . The flow patterns recorded in the normal right heart were identical to those recorded previously during catheterisation . TI was diagnosed by the presence of an abnormal negative systolic wave usually associated with widening of the time interval histogram with a specificity of 82 % and a

The Fontan procedure was initially proposed for tricuspid atresia (TA). We describe our experience with modifications of this operation and its application to previously uncorrectable congenital lesions. From 1975 to 1979, 26 patients have undergone a Fontan-like procedure, at a mean age of 14 years (range: 3 months to 23 years). Nineteen patients had TA, one patient had mitral atresia, four patients had a univentricular heart, one patient had a "criss-cross" heart, and one patient had pulmonary atresia. Previous procedures included eight Glenn shunts, 18 systemic to pulmonary artery (PA) shun

About one third of patients with Löffler's endomyocardial disease have abnormal electrocardiograms and some develop arrhythmias and die suddenly. To assess the significance of these findings, continuous ambulatory ECG monitoring was performed for 48 hr on 6 patients with acute or chronic forms of Löffler's endomyocardial disease, and the types and frequencies of arrhythmias were compared with recordings from 6 other patients with equally high blood eosinophil counts who did not have clinically evident cardiac disease. It was hoped that this would show whether arrhythmias were related to high b

The recent association of transient tricuspid insufficiency in newborn infants with ischemic papillary muscle necrosis prompted a detailed retrospective clinical and histologic analysis of autopsied infants who died within seven days of birth between 1973 and 1977. Infants with congenital heart disease, erythroblastosis fetalis, and known congenital viral infections were excluded. Thirty-one of 82 infants had at least one site of ischemic myocardial necrosis: 11 had lesions only in the right ventricle, 13 had lesions only in the left ventricle, and seven had bilateral lesions. The apical regio

UNLABELLED: Congenital tricuspid incompetence due to valvular dysplasia is a defect involving the leaflets (normally inserted on the ring) the cordae tendinae and papillary muscles of the tricuspid valve. It is a rare condition, usually diagnosed at open heart. A case of congenital tricuspid incompetence in a 40 year old adult is reported. Surgery indicated for resistant heart failure, confirmed the preoperative clinical, echocardiographic and haemodynamic diagnosis. The dysplasia comprised absence of cordae and papillary muscles on two thirds of the anterior leaflet. Valvular replacement with

Tricuspid insufficiency is not infrequent in combined valvular diseases. We used Carpentier's De Vega's or Bex's annuloplasty with our own modification in combined tricuspid insufficiency and found the results much more satisfactory compared with previous methods. Of 152 patients with acquired valvular disease, 24 with combined valvular disease underwent one of these procedures. During the same period, three tricuspid valve replacements, using a glutaraldehyde-treated xenograft, were performed in patients with severly damaged tricuspid valves. Simultaneous procedures for other valves were open

A total of 300 patients were operated upon for heart valve disease by the same surgeon. The affected valve was the aortic in 200 cases and the mitral in the other 100 patients. The tricuspid valve, coronary arteries, and myocardium were not involved. This report attempts to define the frequency progression, prognosis, and significance of conduction abnormalities which appear during the course of this operation. In 35 cases, of which 16 were post-operative deaths, a histological examination of the conduction pathways was possible. The conclusions to be drawn from these observations are: Conduct

Starting in February 1978 52 St. Jude Medical Prostheses have been implanted in 47 patients (age 13--73 years) in aortic, mitral and tricuspid positions. Perioperative death occurred in 3 cases. Post mortem examination of these patients showed unimpaired valvular function. There were no other complications in an accumulated total valve time of approximately 300 months. Prior to surgery and post-operatively, several hemolytic parameters were analyzed. There was only a mild elevation in Serum LDH (285 +/- 10 iV/L). Fifteen patients underwent recatheterization 3 months postoperatively. The mitral

A forty-five year old man with longstanding rheumatic heart disease whose surface electrocardiogram (ECG) showed absent P waves and two alternating QRS complex rhythms, one regular and the other irregular, was submitted to electrophysiological studies. For regular QRS complex rhythm these studies revealed (1) presence of total and bilateral atrial standstill and (2) regular ventricular rhythm originating in the A-V junction; while for irregular QRS complex rhythm the studies showed (1) presence of atrial standstill in the area of the upper right atrium, and (2) a +/- 150/min atrial activity ra

Among 164 patients who underwent mitral valve replacement because of mitral stenosis (with or without mitral regurgitation) and had radiographs taken of their operatively excised mitral valves, 20 had absent or minimal calcific deposits in the excised valves and absent or minimal mitral regurgitation as determined, except for one patient, by left ventricular angiography preoperatively. This report focuses on these 20 patients to ask if mitral valve replacement was preferable to mitral valve commissurotomy. Although in the pre-valve replacement era, all 20 patients almost surely would have been

The results obtained in 20 patients with the use of continue suture technic for prosthetic implantation of the mitral valve were compared to those observed in other 20 patients in which the prothesis were implanted with isolated suture technic. With the first one the aortic clamp time was reduced in average about 50 minutes; the extracorporeal circulation time also diminished in the same proportion. One patient died in each group, meaning a 5% of hospital mortality. The going after time in the first group was 2.5 years and in none was found prosthetic dysfunction or perivalvular lickage. We co

Heterograft porcine valves have gained wide acceptance in replacement of diseased cardiac valves, and their clinical performance in adults has been very satisfactory over follow-up periods of up to 8 years. Valve replacement in children is relatively infrequent and experience with porcine xenografts is necessarily small. Our combined experience at three university hospitals has been with 25 children, 17 months to 16 years of age, who have been followed for 10 to 54 months (mean follow-up 33 months). Porcine valves were used to replace the aortic valve in nine, the mitral valve in seven, both v

A patient is described with tricuspid valve endocarditis in whom the vegetation interfered with valve closure resulting in marked wide splitting of the first heart sound. M-mode and two-dimensional echocardiographic studies detected the presence of a vegetation. Simultaneous phonocardiographic and echocardiographic studies documented the marked delay in tricuspid valve closure and differentiated wide splitting of the first heart sound from other causes of early and mid-systolic sounds. Following pulmonary embolization, the first heart sound returned to normal. Echocardiographic disappearance o

Between 1969 and 1976, 10 severely disabled patients with Ebstein's anomaly were operated on at the National Heart Hospital using replacement of the tricuspid valve with mounted aortic homografts [9] or dura mater valve [1], plication of the atrialized right ventricle [10], and closure of an associated interatrail communication. 7 survivors have been followed up from 2 to 9 yr. 5 are asymptomatic. Symptomatic tricuspid regurgitation developed in 2 patients; from a paravalvar leak of the tricuspid homograft in one, and in the other calcification with stenosis in an irradiated homograft, 2.5 and

Constrictive pericarditis is not considered a complication of cardiac surgery. However, three cases are presented in which equalization of diastolic pressures and the ventricular pressure pattern of early diastolic dip-late diastolic plateau, characteristic of restrictive disease, appeared after cardiac surgery. In one patients cardiac constriction developed less than 2 weeks after surgery, and loculated clotted and unclotted viscous blood was removed from the pericardial space. In the other two patients the pericardial space was obliterated by dense adhesions. Thus constrictive pericarditis s

In order to investigate the basis of ventricular dysfunction in patients with univentricular heart, cineangiograms of 20 patients aged between 3 weeks and 22 years were digitized frame by frame and compared with left ventriculograms of patients with other cardiac malformations. Ejection fraction was at least normal (58% to 95%) and peak rate of systolic minor axis shortening was normal in all. However in only 3 patients was the change in volume achieved by virtue of the normal change in cavity shape; in 17 the end-systolic outline was abnormally circular and shape index, 4 pi (area)/(perimeter

A case of tricuspid atresia in a premature foal associated with large atrial septal and ventricular septal defects is reported. This anomaly is compared with other cases in horses as reported in the literature. As tricuspid atresia in horses is usually not diagnosed so that relatively few cases have been reported, a classification similar to that of human anomalies is not possible. It is suggested that the present malformation is due to abnormal development of the atrioventricular canal. The factors involved in the pathogenesis of this abnormal development are obscure.
