Infective endocarditis in the narcotic addict.

As long as the illicit use of heroin and other drugs continues in our society, infective endocarditis will remain a significant medical problem in the drug-using population. The majority of infections are produced by S. aureus, and the tricuspid valve is most commonly involved. Addicts, unlike the general population, may also develop endocarditis with a variety of gram-negative bacilli and have a higher incidence of fungal infection. The outcome of each individual infection is dependent on the prompt recognition of the underlying valvular infection and the institution of antimicrobial therapy.

[Isolated rheumatic tricuspid insufficiency (author's transl)].

Insufficiency on the tricuspid valve alone due to cardioarticular rheumatism is a rare condition. A 12-year-old boy had had six episodes of rheumatic fever over the previous 5 years. The only damage done to the heart was insufficiency of the tricuspid valve; none of the other valves were affected. The phonocardiographic response to the amyl nitrate and methoxamine tests as well as the correct evaluation of the jugular phlebogram confirmed the diagnosis of this valve pathology. The study of this heart condition was completed with the diagnosis of rheumatic fever (Jones' criteria, modified) and

Adherence of glucan-positive and glucan-negative streptococcal strains to normal and damaged heart valves.

The adherence of 18 strains of streptococci to sections of normal canine and human aortic, mitral, and tricuspid valves and to canine interatrial septum was compared in an in vitro system. Quantitative measurements of adherence ratios were performed by two independent methods. Adherence ratios for Streptococcus mutans, S. sanguis, S. bovis, and Group D streptococci were higher (0.0058-0.0101) than for the other streptococcal strains studied (0.0025-0.0041). With the exception of Group D streptococci, adherence ratios for each bacterial strain were similar with the aortic, mitral, and tricuspid

[Cardiovascular tissue response to intracardiac pacemaking (author's transl)].

With transvenous intracardiac implantation of pacemakers after a few days already a fibrotic wall has developed around the impulse generator by a chronical proliferous process. Where the electrode has been advanced through the vein-wall and in the following veins the cable is wrapped cuff-like by increased collagenous fibres. Frequently these formed an adhesion with the inner vein-wall for quite some distance. Inside the heart, adhesion to the tricuspid valve may lead to insufficiency of this. The incorporation in the myocardium occurs as embedding in connective tissue. Here also chronical pro

[Three years experience with biological heart valve prostheses (author's transl)].

Increasing clinical experience has led to a world wide expansion of glutaraldehyde fixed aortic heterografts for valve replacement in patients. The distinct advantage of this valve is a very low rate of thrombembolic complications during the postoperative course. In our hospital 324 such prostheses of the types Hancock and Carpentier have been implanted in 290 patients. In 154 cases follow-up 6 months to 3 years after the operation have been obtained. Thrombembolic complications occured in 1,4%. Other postoperative complications were hepatitis (13,4%), and valvular endocarditis (1,5%). Postope

Occurrence and significance of echocardiographically demonstrated tricuspid valve prolapse.

Echocardiograms from 562 patients were examined for evidence of the pattern of tricuspid valve prolapse. Criteria for the diagnosis can be established similar to those applicable to mitral prolapse. In 500 consecutive patients without mitral valve prolapse, there were no cases of isolated tricuspid valve prolapse. Eleven of 53 (21%) patients with mitral valve prolapse also had tricuspid valve prolapse. Four of six (67%) patients with Marfan's syndrome and mitral valve prolapse also had tricuspid valve prolapse. The occurrence of this echocardiographic pattern as an isolated finding as well as

Rheumatic tricuspid stenosis. A clinical overview.

The clinical, hemodynamic findings and surgical results of forty patients with rheumatic tricuspid stenosis are presented. It is concluded that tricuspid stenosis is a frequent lesion. Its signs should be sought carefully and documented by simultaneous measurements of the right ventricular diastolic and right atrial pressures. Tricuspid stenosis is usually associated with other valvular lesions, whose clinical and hemodynamic findings might be altered. Surgical procedures on these patients involved tricuspid valve repair, commissurotomy or prosthetic valve replacement, in addition to the proce

Thirty normal subjects who did not have valvular disease showed no evidence of tricuspid regurgitation or premature ventricular contractions during injection except in one case. On the other hand, 24 patients with combined aortic and mitral valve disease showed mild to moderate tricuspid regurgitation in 11 patients and severe tricuspid regurgitation in eight patients. Only two patients developed frequent run of premature ventricular contraction during injection. Se

[Results of implanting aortic valve xenografts into the tricuspid position].

From 1968 to 1976 transplantation of the tricuspid valve was performed on 47 patients with combined form of rheumatic heart disease. Hospital mortality was 36%. The follow-up period in 30 cases ranged from one to 7 1/2 years. Ten patients died in the late postoperative periods, only in one of them the cause of death was associated with incompetence of the transplanted valve. According to the findings of dynamic clinical follow-up, the function of the transplant in the other patients was quite satisfactory; at the same time, peculiarities of the phonocardiographic picture of valves treated by d

Cardiac pathology of transvenous pacemakers in dogs.

Transvenous right ventricular pacemaker catheters were implanted in 18 mongrel dogs for periods of 2 to 18 months (average 4.9 months). Heart block was produced in 15 of these dogs by injection of 37 per cent formaldehyde into the interatrial septum. In the other three dogs which served as controls, no heart block was produced and no electrical stimulation was applied to the implanted catheters. After the animals had been put to death, gross and microscopic examination of the hearts revealed four categories of morphological changes: (1) connective tissue sheath formation around the catheters,

Reappraisal of jugular phlebogram in the diagnosis of tricuspid regurgitation. Relationship between echocardiographic interventricular septal motion and jugular phlebogram.

Little information is available concerning the relationship between the pattern of the jugular phlebogram and interventricular septal motion. We studied 250 patients with various conditons. Abnormal septal motion was observed in only 33% of the patients with tricuspid regurgitation. All the patients having abnormal septal motion and 66% of those having normal septal motion had the jugular pattern of tricuspid regurgitation. After cardiac operation with the incision of cardiac chambers and pericardiotomy, abnormal septal motion was seen from 40 to 89% at early postoperative period. Of the patie

Suspected superior vena cava syndrome: the role of the Swan-Ganz catheter.

The use of the Swan-Ganz flow-directed catheter in establishing the diagnosis of the superior vena cava syndrome in two patients (one with Hodgkin's disease and the other with carcinoma of the lung) is described. A pressure tracing showing elevated pressure above the obstruction without respiratory or cardiac fluctuations is characteristic of obstruction of the superior vena cava.

The amino acid composition of actin and myosin and Ca2+-activated myosin adenosine triphosphatase in chronic canine congestive heart failure.

The Ca2+-activated myosin ATPase and the amino acid compositions of actin and myosin were determined for preparations from chronically failing dog hearts. Hypertrophy and congestive heart failure were produced by combined tricuspid valve insufficiency and pulmonary artery stenosis. Control, shamoperated, and noncardiac circulatory failure (inferior vena cava constriction) dogs also were studied. All hearts were divided into right ventricle, septum and left ventricle and each sample was individually analyzed. Calcium-activated ATPase decreased in the failing hearts and showed a distinct gradien

Ebstein's anomaly: a report of 3 cases in the dog.

Three young unrelated dogs were diagnosed as having the canine counterpart of Ebstein's anomaly, an eponymic term for a cardiac malformation in man. All of the dogs had systolic murmurs in the tricuspid area, and all had radiographic evidence of cardiac enlargement, particularly of the right atrium. Electrocardiography indicated atrial disease or right atrial enlargement and right ventricular conduction defects. The defect was confirmed by means of cardiac catheterization in all 3 dogs. Open heart surgery was performed on 2 of the dogs--plastic reconstruction of the tricuspid annulus and place

Prosthetic valve surgery in tricuspid area.

Up to September 20, 1975, 1,020 cases of valvular heart diseases were surgically treated at the Sapporo Medical University Hospital. In these series, there were 33 cases done for the tricuspid valve which included 27 valve replacements (81.8%) and 6 annuloplasties (18.2%). We have demonstrated that TVR may improve hemodynamic condition for any type of TI, however, postoperative A-V block and sinus bradycardias seemed to be serious complications with relatively high incidence. On the other hand, long term hemodynamic results of TAP was demonstrated to be comparable to that with TVR. Therefore,

Chronic congestive heart failure after repair of tetralogy of Fallot.

The etiology of chronic congestive heart failure (CHF) after repair of tetralogy of Fallot was determined in 102 patients consecutively catheterized 1 to 12 years postoperatively. Chronic CHF was observed in 36/102 patients. The most prevalent abnormality leading to congestive failure (31/36) was a large residual ventricular septal defect alone or in combination with other lesions. All postoperative patients with pulmonary to systemic flow ratios greater than 2:1 (25/102) had congestive failure and evidence of biventricular dysfunction. Significant tricuspid regurgitation (N = 11) and persiste

[Surgical repair of Ebstein's disease (author's transl)].

In 1976 4 patients with Ebstein's disease underwent surgical repair. In one patient (22 yrs.) the tricuspid valve leaflets were suitable for reconstructive surgery using Hardys method in addition to De Vegas annuloplasty. In the other three cases (17, 29, 51 yrs. old) excision of the malformed tricuspid valve was necessary, and porcine heterografts of greatest possible diameter were implanted. Three patients improved considerably 9 months postoperatively. The fourth patient, a 51 years old woman, died 2 days postoperatively on cardiac failure. Endocarditis due to enteric bacilli other than Salmonellae: case reports and literature review.

The clinical and pathological findings in two recent patients with non-salmonella enterobacterial endocarditis are described, and those of 42 patients in the literature are summarized. Most of the patients acquired their endocarditis secondary to urinary tract infection and had an acute clinical course characterized by high fever and chills. Thirty-two of these patients died, and all except one had a postmortem examination. The most frequent pathological finding was the occurrence of very large vegetations which caused relatively little destruction of the underlying valve. Prompt diagnosis and

[Personal experience with 2,500 closed heart mitral commissurotomies].

Between 1953 and 1976 a total of 2527 mitral commissurotomies have been performed in the Clinic for Cardio-Vascular Surgery of the "Fundeni" Hospital. This group included cases with pure mitral stenosis (predominantly following rheumatic disease), but also cases of mitral stenosis associated with other cardiac lesions in which the major hemodynamic element was the mitral obstruction. Criteria are discussed for surgery, the preoperative preparation of the patient, incidents, accidents and complications, as well as technical difficulties of the digital and/or instrumental mitral commissurotomies

Pathology of chronic rheumatic heart disease with particular reference to tricuspid value involvement.

This study is based on a retrospective analysis of 144 autopsied cases of rheumatic heart disease. Majority of cases (79%) were chronic and 21% were acute in nature. The mortality was high and was maximum below the age of 30 years. The males predominated over females. A high incidence of organic tricuspid valve involvement was observed (45.8%). Tricuspid stenosis was observed in 3 cases. Latter was associated with aortic and mitral valve involvement. Mitral stenosis was severe in all the three cases. Criteria, both gross and microscopic for the involvement of the tricuspid valve have been desc
