Two cases of complex congenital heart disease are described in which systemic and pulmonary bloodstreams crossed at the atrioventricular level. Both patients were examined clinically, echocardiographically, and by cardiac catheterization, including angiography, and both underwent cardiac surgery and had intraoperative mapping of their conduction systems. Both patients were found to have levocardia and situs solitus of viscera and atria, large ventricular septal defect, and straddling right atrioventricular valve. One patient had atrioventricular discordance but with the left ventricle anterior

This is a case report of a 16-year-old male with moderate mitral disease, probably rheumatic, and with severe tricuspid insufficiency, which at surgery appeared to be a congenital malformation of the tricuspid valve (absence of the anterior leaflet). A tricuspid heterograft was successfully inserted. The symptoms of congenital tricuspid insufficiency are discussed, and this entity is differentiated from other causes of tricuspid insufficiency.

Fourteen term newborn infants have been recognized as having transient tricuspid insufficiency associated with significant perinatal stress. Five of these infants underwent cardiac catheterization for presumed congenital heart disease, but had only massive tricuspid valve insufficiency. The other nine infants were diagnosed on the basis of a murmur characteristic of tricuspid valve insufficiency and on other clinical grounds. All had a history of significant perinatal stress in the form of asphyxia with or without hypoglycemia. Frequently, congestive heart failure, persistent cyanosis, and ECG

The experience with tricuspid valve replacement (TVR) with seven different prostheses, alone or combined with replacement of other valves, in 73 patients (64 rheumatic and nine nonrheumatic) between 1964 and March, 1975, at the University of Alberta Hospital has been reviewed. Early and late mortality rates in rheumatic patients were 41 and 23 percent, respectively (36 percent being alive after a mean of 5.6 years), compared to 33 and 11 percent, respectively, in the nonrheumatic patients (56 percent being alive after a mean of 2 years). Of all survivors, 88 percent were functionally improved.

21 infants and children with proven bacterial endocarditis were observed at the Unviersity Children Hospital of Düsseldorf from January 1969 to December 1976. There was high incidence of cases in the infant group and again among the 6 to 8 years old children. Some important aspects of the disease were characteristic for the infant group (N=5): No congenital cardiac abnormality was present, but a surgical cerebro-atrial connection in two cases of hydrocephalus and a prolonged artifical respiration in a third patient could have been predisposing factors. Staphylococci were the pathologic organis

The aortic valve, as well as the other valves of the heart, has a cellular structure. Collagenous fibres form the skeleton of the cellular structures, and the plexus of thinner fibres--the "walls". At early periods following the implantation destruction of thin fibres distributed along the periphery of the alveolus takes place. In the course of prolonged functioning of the allo- and xenovalves in a mitral position the large collagenous fibres collapse, defibration thereof develops and slits-vacuums and "cysts" are formed. In the valves which functioned for a long time in the tricuspid position

A new mitral valve prosthesis (Cooley-Cutter) is described, which incorporated a low profile design, an excentric sewing cuff, a full flow orifice, a controlled regurgitation for self-washing and symmetrical peripheral flow. The new prosthesis has a titanium double-caged frame, a Pyrolite carbon poppet and a Teflon knit sewing cuff. We have attempted to incorporate in the design of this new prosthesis tre most favorable features of each of many types of prosthesis used at the Texas Heart Institute since 1962. Isolated prosthetic mitral valve replacement for severe congenital or acquired valvul

Tricuspid regurgitation developed in two patients after inferior wall myocardial infarction. Neither patient had preexisting valvular heart disease or evidence of endocarditis, and neither had suffered chest trauma. Because abnormalities in right ventricular function may occur after inferior infarction, and because other known causes of tricuspid incompetence were not present, we postulate that these patients developed valvular regurgitation from dysfunction of the papillary muscle complex controlling tricuspid valve function, a mechanism similar to that proposed to explain mitral regurgitatio

The sound-pressure correlates of the second high frequency component of a split first heart sound (S1) were investigated in 27 patients. An external phonocardiogram was recorded with high fidelity sound and pressure from the left and right atria in 21 patients, from the pulmonary artery in 14 of these, and from the central aorta in 11. In the remaining six patients, high fidelity recordings from the central aorta and right-sided chambers were obtained with an external phonocardiogram. The external component of S1 that coincided with a left atrial C wave and "internal sound" was defined as M1.

During the past 12 years, 13 patients with atrial (10 left and 3 right) myxoma have been treated. The tumors of the left atrium produced signs and symptoms of mitral valve obstruction and/or subacute bacterial endocarditis and those of the right atrium manifestations of tricuspid valve disease or of pulmonary embolus or hypertension. The diagnosis was established by angiocardiography in 8 patients, at surgery performed for suspected mitral stenosis in 3 patients, and at autopsy in 2 patients. Resection of the atrial myxoma alone in 5 patients or with atrial septum where the atrial myxoma was a

We examined 129 standard M-mode echocardiograms obtained in 65 patients (16 to 73 years old) with bacterial endocarditis. Twenty of the 22 patients with vegetations recognizable by echocargiography died, or underwent cardiac operation (mean interval from admission 22 days, and range two to 120 days). Vegetations were seen on the echocardiograms in 22 (aortic 10, mitral nine and tricuspid three, with anatomic confirmation in 19). Of patients without vegetations on echocardiography none underwent emergency operation or died as a result of cardiac disease (mean follow-up period of 14 months, rang

Out of 350 cases of tricuspid regurgitation operated between 1968 and 1975, a serie of 53 prosthetic replacements (3 of them isolated) was analysed, with specification of operative indications, pre-operative condition, operative findings, procedure used and results. Operative death rate was high (28 per cent) and remote results (6 months to 7 years) were not all satisfactory. This is because functional "tricuspid regurgitation", secondary to advanced mitral lesions, expresses in fact considerable myocardial weakening with severe polyvisceral implications. This picture, where tricuspid patency

The best means of managing tricuspid regurgitation associated with mitral or mitral and aortic valve disease is still to be determined. During the period 1972 to 1974, we treated 76 patients who had tricuspid regurgitation along with associated valvular dysfunction. Patients with mold regurgitation were treated conservatively, those with moderate regurgation underwent annuloplasty, and those with severe regurgitation had tricuspid valve replacement. We found the results to be less satisfactory in the group treated by annuloplasty than in the other two groups. We still manage conservatively tho

In 25 patients aged 6 days to 9 years presenting as 'isolated' pulmonary valve stenosis, histology of the myocardium of right and left ventricles, coronary arteries, and ascending aorta has shown abnormality in one or all these areas. Myocardial necrosis, old and recent, unrelated to coronary occlusion was frequent. Myocardial 'dysplasia' involving both ventricles, and resembling hypertrophic cardiomyopathy (HOCM, ASH) was found in 10 and a relation of this to myocardial injury in the fetus is postulated. Varying degrees of coronary occlusion were frequently seen in both right and left coronar

Sixteen patients aged between one day to 18 years with Ebstein's malformation of the tricuspid valve were studied with ultrasound. The findings were compared with a group of 74 patients without Ebstein's malformation. Two features were considered specific for Ebstein's malformation: 1) ability to record the anterior tricuspid leaflet (ATL) farther to the left of the left sternal border than in the control group; and 2) abnormally prolonged interval between the "C" points of the tricuspid and the mitral valve echoes (McTcI). All other parameters measured were nonspecific for Ebstein's anomaly o

Reconstruction of the right ventricular outflow tract with an aortic homograft conduit was performed in 75 patients from 1966 to 1974. The types of congenital heart disease were as follows: pulmonary atresia, 35 cases; severe tetralogy of Fallot, 22 cases; truncus arteriosus, 6 cases; transposition of the great arteries (TGA), 3 cases; single ventricle, 2 cases; and tricuspid atresia, 7 cases. Ninety per cent of the patients had one or more previous shunts, and this was a factor affecting the mortality rate. Other factors included age, pulmonary vascular resistance, surgical anatomy, and techn

Between 1961 and 1972, 354 patients with atrial septal defect ware treated surgically. Of these 80 patients were aged over 40 years at the time of operation. The hospital mortality was 5%. 2 of these patients had a increased preoperative pulmonary artery systolic pressure (50-75 mmHg), the other 2 had a mitral or tricuspid incompetence. At late follow-up 82% of patients were symptomfree, 18% had slight clinical signs of breathlesness on exertion. The heart size decreased in 37 of 40 patients, the electrocardiographis signs of right ventricular hypertrophy in 23 patients. There was no influence

The study has been carried out on eight cases of pulmonary atresia with ventricular septal defect, this abnormality involves both valvular atresia and the stenosis of the outflow tract of the right ventricle; that means a pulmonary atresia both valvular and infundibular. The atresia is always combined with a dextroposition of the aorta, overriding the ventricular septum; often with an auricular septal defect or a patency of the foramen ovale. In two cases, other associative caridac malformations exist : either a single ventricle or an atresia of the tricuspid valve. The arterial supply to the

The paper deals with the diagnosis of defective development of the patent ductus atrioventricularis. Electrocardiographic changes in 81 patients with firmly established hemodynamic form of the patent ductus atrioventricularis, ascertained during surgery and catheterization, are summarized. Characteristic electrocardiographic signs of the defect distinguishing it from other congenital diseases and differential-diagnostic symptoms of diverse anatomo-hemodynamic forms of the patent ductus atrioventricularis were educed.
