Ocular community state types reveal distinct microbial compositions among microbiomes with implications for trachoma control.

Trachoma, a chronic ocular disease caused by Chlamydia trachomatis (Ct), is the leading infectious cause of blindness worldwide. Despite WHO's SAFE (Surgery, Antibiotics, Facial cleanliness, Environmental improvement) strategy, ~100M are at risk of blindness. Using metagenomic shotgun sequencing, we characterized the ocular microbiome of 680 villagers in Amhara Ethiopia, identifying 10 Community State Types (CSTs) associated with different population characteristics. Children with the highest prevalence of inflammatory trachoma and Ct were in CST10, dominated by Haemophilus influenzae and four

Discrepancy Between Active Trachoma and Tests of Chlamydial Infection in Alto Amazonas, Peru.

BACKGROUND: Trachoma mapping in Latin America is incomplete, especially in indigenous communities of the Amazon thought to be at highest risk. The aim of this study was to determine the prevalence of clinical, microbiological, and serologic evidence of trachoma in a remote area of the Peruvian Amazon. METHODS: A population-based sample of households from rural communities in Alto Amazonas province was selected using 2-stage cluster sampling. In children aged 1-9 years, conjunctival photographs, conjunctival swabs, and dried blood spots were collected. Photographs were graded for trachomatous

Enhancing the diagnosis accuracy of Chlamydia psittaci infection via metagenomic next-generation sequencing.

Metagenomic next-generation sequencing (mNGS) is an innovative diagnostic technique that integrates high-throughput sequencing with bioinformatics analysis. Recently, its unbiased approach, broad pathogen coverage, and rapid turnaround have led to its expanded use in diagnosing infectious diseases. This is particularly true for atypical bacterial pathogens such as Chlamydia psittaci, Chlamydia pneumoniae, Mycoplasma pneumoniae, and Legionella spp., for which mNGS has demonstrated significant diagnostic utility. C. psittaci, a highly pathogenic obligate intracellular bacterium, can cause severe

Chlamydia psittaci pneumonia without cough: recovery, case report and literature review.

OBJECTIVES: We report a case of Chlamydia psittaci pneumonia in a patient presenting with high fever and generalized myalgia but no cough. As infections with Chlamydia psittaci are potentially misdiagnosed and non-responsive to empirical antibiotic treatment, this report aims to enhance the awareness among the healthcare providers. METHODS: A 34-year-old male patient developed high fever and fatigue after exposure to poultry. He presented to the hospital for treatment. Chest computed tomography (CT) showed a high-density shadow in the deep area of the left lower lung. Sputum culture returned

Two Cases of Severe Chlamydia psittaci Pneumonia with Respiratory Failure and Literature Review.

BACKGROUND: Chlamydia psittaci pneumonia is a zoonotic disease with non-specific clinical manifestations, often leading to delayed diagnosis. Metagenomic next-generation sequencing (mNGS) can help us identify pathogens in a timely manner and quickly adjust treatment strategies. METHODS: We reported two cases of severe Chlamydia psittaci pneumonia with respiratory failure and reviewed relevant literature. RESULTS: Both patients were diagnosed with Chlamydia psittaci infection through mNGS after routine pathogen testing failed. After using Omadacycline based treatment, the patients' clinical a

Co-Infection of Chlamydia psittaci and Methicillin-Resistant Staphylococcus aureus in Community-Acquired Pneumonia - A Case Report.

This study presents a rare case of community-acquired pneumonia (CAP) caused by a co-infection of Chlamydia psittaci and Methicillin-resistant Staphylococcus aureus (MRSA), confirmed by targeted next-generation sequencing (tNGS). The patient had a history of diabetes mellitus and hypertension. He developed fever, fatigue, myalgia and poor appetite two days before admission. He had watched someone slaughter a chicken two days prior to the onset of symptoms. The chest CT revealed consolidative changes. We suspected that the patient might have Chlamydia psittaci infection. Given the low sensitivi

Trends in Chlamydia Prescribing Practices After Changes to National Treatment Guidelines in the United States, 2016-2023.

BACKGROUND: In 2021, the US Centers for Disease Control and Prevention revised Chlamydia trachomatis (CT) treatment guidelines to recommend only doxycycline as first line-therapy, instead of recommending either azithromycin or doxycycline. The extent to which this recommended change has been translated into practice is unknown. METHODS: We used 2016-2023 MarketScan data (national convenience sample of insurance claims from individuals with employer-sponsored insurance and Medicaid beneficiaries from 8 to 12 states) to identify encounters of CT diagnoses for individuals aged 15 to 60 years who

Impact of reduced chlamydia testing on STI testing and provider preferences in the Netherlands: an experimental vignette study.

OBJECTIVES: The restrictive chlamydia testing guidelines for asymptomatic individuals at Dutch sexual health centres (SHCs) since January 2025 may influence perceptions and attitudes towards testing for sexually transmitted infections (STIs). This study aimed to assess the potential impact of this guideline change on STI testing preferences among young people and to examine determinants and underlying motivations of these preferences. METHODS: Heterosexuals aged 16-34 in the Netherlands, recruited via social media and SHCs, completed an online survey (April-June 2024). STI testing preferences

Infection and inflammation of the seminal tract: A review of its relationship to male fertility.

INTRODUCTION: In 1997, the WHO established the role of genital tract infections in human infertility, currently responsible for 15% of the causes of infertility in men. The WHO classifies urethritis, prostatitis, vesiculitis, orchitis, and epididymitis as infections of the male accessory gland. This paper represents the state of the art on inflammation and infection of seminal tract. Objective of the study is to revise literature and to discuss regarding association to fertility. METHODS: A literature search was performed on PubMed database (http://www.pubmed.gov) for peer-reviewed journal ar

Orlistat impairs Chlamydia trachomatis development by disrupting its developmental cycle.

Chlamydia trachomatis is a common sexually transmitted bacterial pathogen that often causes urogenital infection and may progress to chronic inflammation and fibrosis due to its clinically silent nature. Although azithromycin and doxycycline are widely used for treatment, therapeutic failure has been documented in clinical cases. In this study, we demonstrate that orlistat disrupts the chlamydial developmental cycle both in vitro and in vivo. Orlistat inhibits early stages of chlamydial infection, reticulate body replication, and reticulate body-to-elementary body differentiation. Transcriptom

Synergism on Cancer Development of Human Papillomavirus and Chlamydia Trachomatis Co-Infection.

Human papillomavirus (HPV) and Chlamydia trachomatis (CT) co-infection is increasingly recognized not as a mere coincidence, but as a synergistic partnership that accelerates oncogenic progression across multiple tissues. This review synthesizes existing evidence into a "central-peripheral" framework, positioning cervical cancer as the central, mechanistically well-established model of cooperation, while malignancies such as head and neck, ovarian, and breast cancers represent the emerging, though less substantiated, peripheral extensions. This synthesis delineates an emerging paradigm of bidi

Impact of Chlamydia trachomatis treatment with azithromycin or doxycycline on Mycoplasma genitalium in women.

OBJECTIVES: In women being screened for Chlamydia trachomatis (CT) infection, Mycoplasma genitalium (MG) testing is not recommended. Among women positive for CT, studies reported that 7% to 28% had concurrent urogenital MG. The aim of this study was to investigate the impact on MG of a single 1-g dose of azithromycin or a 7-day course of doxycycline regimens as part of CT treatment. METHODS: MG was tested in vaginal swabs from the Chlazidoxy cohort collected at baseline and the 6-week follow-up visit after CT treatment. MG genotype and antibiotic resistance were analysed by sequencing the mgp

A Dirichlet-Multinomial Gibbs Algorithm for Assessing the Accuracy of Binary Tests in the Absence of a Gold Standard.

Each patient is simultaneously given several binary tests for a disease. The tests are partitioned into disjoint groups, assumed to be conditionally independent between groups, but allowed to have arbitrary dependence within a group. The groups are intended to capture similar biological features of the tests. A Dirichlet-multinomial model is employed with a Gibbs Sampler to estimate the sensitivity and specificity of the tests. The model is exemplified by data on four tests for Chlamydia, both with complete data and with a random 10% of the data treated as missing.

Sex hormone-responsive human vaginal epithelial organoids: a novel in vitro platform for studying Chlamydia trachomatis infection.

The vaginal epithelium, a hormone-sensitive barrier critical for reproductive health, lacks robust in vitro models due to primary tissue scarcity and existing systems’ limitations. Here, we established the first long-term, genetically stable human vaginal epithelial organoids (VEOs) using a tailored Matrigel-based culture medium that preserves the native stratified squamous architecture. Through transcriptomic and functional profiling, we identified progesterone as a key suppressor of immune defense and keratinization. Modeling Chlamydia trachomatis (CT) infection in VEOs recapitulated infecti
